[Long-term phenytoin (Diphedan) therapy leading to IgA nephropathy].

A single case of IgA nephropathy with a concomitant tubulointerstitial nephritis, developing during a long-term phenytoin therapy is reported. The prolonged phenytoin therapy did neither prevent the transitional increases of serum IgA level nor ward off deterioration of the histological alterations. This case suggests that acute tubulointerstitial nephritis may have played a role in the mesangial trapping of IgA immunocomplexes.

[Hemorrhage in the abdominal cavity caused by ruptured varices].

The authors report on the successful treatment of an unusual varix rupture causing intraabdominal hemorrhage in a patient who suffered from alcoholic hepatic cirrhosis. Simultaneously with the hypogastric pain, hypotension, growing circumference of the abdomen and increasing anaemia--without hematemesis or melena--referred to intraabdominal hemorrhage, which was confirmed by diagnostic paracentesis. Urgent exploratory laparotomy revealed the source of the bleeding and made the adequate surgical treatment possible. The authors are of the opinion that in case of a suspected intraabdominal hemorrhage the exploratory puncture is of diagnostic value and an operation performed without delay may ensure the improvement of the prognosis of this severe complication.

IgA glomerulonephritis: light microscopic and immunohistological studies.

Renal biopsy material of 50 patients suffering from IgA glomerulonephritis was studied by light microscopy, immunofluorescence and occasionally electron microscopy. Associated with a uniform immunohistological pattern mainly focal or diffuse mesangial proliferative glomerulonephritis was found. In addition to the glomerular changes the importance of frequent extraglomerular, mainly vascular and interstitial, alterations is stressed. In arterioles and arteries, deposition of a hyalinous substance was observed. Immunohistology revealed the presence of immunoglobulins and/or C3 in the vessels. In the interstitium fibrinogen/fibrin was often seen, accompanied by chronic inflammatory infiltration and fibrosis.
