Cowden's disease--a report on the first case in Korea and literature review.

Cowden's disease, or multiple hamartoma syndrome, is an uncommon condition with characteristic mucocutaneous lesions associated with abnormalities of the breast, thyroid, and gastrointestinal tract. We describe a 32-year-old man with oral mucosal papillomatosis and plantar hyperkeratosis as a definite case of Cowden's disease according to the criteria proposed by Salem and Steck. The patient also had a thyroid mass and numerous gastrointestinal polyps endoscopically. Histologically the polyps were hamartomatous or hyperplastic polyps. The oral papillary lesions were fibroepithelial polyps and the thyroid mass was a follicular adenoma. We review the literature on this entity and summarize the pertinent findings. To the best of our knowledge, this is the first documented case of Cowden's disease in a Korean.

Kikuchi disease: CT and MR findings.

Two cases of Kikuchi disease showed variable nodal enhancing features, including homogeneous enhancement and focal or extensive nodal necrosis on contrast-enhanced CT scans. At MR imaging, the area of central necrosis was isointense or hypointense on T1-weighted images and had a lower signal than nonnecrotic areas on T2-weighted images. The CT appearance of Kikuchi disease can be variable and can mimic not only lymphoma but various nodal diseases with nodal necrosis, including metastasis and tuberculosis.

A three-dimensional analysis of mandibular arch changes following curve of Spee leveling in nonextraction orthodontic treatment.

The present study was conducted to determine the effects of curve of Spee leveling on three-dimensional arch length and specific components of arch form. The mandibular pretreatment and posttreatment dental casts of 33 patients treated orthodontically without the extraction of teeth were digitized with a three-dimensional positioning transducer integrated with a graphics calculator. Statistical analysis was carried out with general linear models and the calculation of Pearson correlation coefficients of the relationships between the dependent variable of the curve of Spee leveling and the independent variables of arch length, arch depth, and three arch widths. The regression and correlation analyses essentially showed no relationship between the independent variables (arch length, arch depth, and the three arch widths) and the dependent variable (curve of Spee leveling). Although the three-dimensional arch length was increased after leveling of the curve of Spee in 24 of the 33 dental casts evaluated, the results from the statistical analyses indicated that this increased arch length was not necessarily the result of the curve of Spee leveling but may have been caused by other variables.

Primary malignant melanoma of the sinonasal cavity: MR imaging evaluation.

To evaluate the magnetic resonance (MR) imaging characteristics of primary malignant melanoma of the sinonasal cavity, T1- and T2-weighted MR images of 12 patients with primary sinonasal melanoma were retrospectively reviewed. Gadolinium-enhanced imaging was performed in seven cases. The MR images were compared with histopathologic results. There were seven melanotic melanomas and five amelanotic melanomas; hemorrhage was present in three melanotic and two amelanotic melanomas. The seven melanotic melanomas were hyperintense to gray matter on T1-weighted images (whether hemorrhage was present or not), consistent with the paramagnetic effect of melanin. Four of the five amelanotic melanomas had intermediate signal intensity on T1-weighted images; one was not detected. On T2-weighted images, all of the melanomas detected had intermediate though variable signal intensity compared with that of gray matter. On gadolinium-enhanced images, all cases demonstrated mild to moderate enhancement. The signal intensity of sinonasal melanoma appears to vary according to the histopathologic components of the tumor. High signal intensity within the lesion on T1-weighted images suggests the presence of melanin.

HMPAO SPECT to assess neurologic deficits during balloon test occlusion.

UNLABELLED: The purpose of this study was to determine if one could objectively and preoperatively predict the safety of permanent occlusion of an internal carotid artery with 99mTc-HMPAO brain SPECT. METHODS: Twenty-four patients underwent balloon test occlusion of the internal carotid arteries because of neck and skull base tumors. We assessed the uptake of both middle cerebral artery territories before and during balloon test occlusion with 99mTc-HMPAO brain SPECT using the semiquantitative analysis. The results were compared with other factors, including neurologic examination, arterial stump pressure and electroencephalogram. RESULTS: Nineteen patients experienced no neurological deterioration or any problem during balloon test occlusion. The comparative uptake of their middle cerebral artery territories was 95%-101% of the pre-balloon test occlusion state. The remaining five patients showed severe neurologic symptoms, such as transient hemiplegia and unconsciousness. The comparative uptake of their middle cerebral artery territories was 77%-85% of the pre-balloon test occlusion state and was well matched with other factors. CONCLUSION: Technetium-99m-HMPAO brain SPECT before and during balloon test occlusion seems to be a simple and objective method for predicting permanent neurologic deficits when the comparative uptake of middle cerebral artery territories during balloon test occlusion is less than 85% of that before balloon test occlusion.

Increased contralateral cerebellar uptake of technetium-99m-HMPAO on ictal brain SPECT.

UNLABELLED: Crossed cerebellar diaschisis (CCD) is a well-known brain SPECT finding in stroke patients. Two reports, however, have described supratentorial and contralateral cerebellar hyperperfusion (crossed cerebellar hyperperfusion) on ictal brain SPECT in epileptic patients. The purpose of this study was to assess the usefulness of crossed cerebellar hyperperfusion (CCH) for the detection of epileptic foci on ictal scan. METHODS: Twelve patients with complex partial seizures having characteristic clinical, electroencephalographic (EEG) and brain SPECT findings were included. Fifteen to 20 mCi 99mTc-HMPAO were injected intravenously during the seizure period or the aura for the ictal SPECT study. The SPECT findings were visually assessed to determine whether the finding of CCH was valuable in the localization of ictal foci. RESULTS: Epileptic foci were found in the right temporal (n = 6), left temporal (n = 4), right occipital (n = 1) and left frontal (n = 1) areas. CCH was observed in 8 (75%) of the 12 patients. In two patients, contralateral cerebellar uptake was more obvious than that in the epileptic foci. In the interictal scans, cerebellar activity, which was increased in ictal period, was equalized in seven of eight patients, while perfusion was diminished in the remaining patient. CONCLUSION: CCH is a frequent finding of ictal brain SPECT and may aid in the lateralization of epileptic foci.

Detection of malignant melanoma by Tc-99m HMPAO.

A number of radiopharmaceutical agents have been evaluated for the detection of malignant melanoma. Because anamine is necessary for melanin synthesis, melanotic tumors may possess numerous amine receptors. Tc-99m HMPAO consists of macrocyclic amine radicals. The purpose of this study was to evaluate Tc-99m HMPAO as an effective agent for localizing malignant melanomas on the radioisotope imaging. Six patients, with malignant melanomas were studied. Tc-99m HMPAO SPECT images were obtained in all patients. SPECT was used for the localization of the primary focus, and whole body scans were used for the evaluation of unexpected metastatic lesions. The results were compared with MRI and CT. Tc-99m HMPAO SPECT revealed a high correlation with CT and MRI for the primary focus. Abnormally increased activity at the unexpected metastatic lymph nodes in two patients were also noticed. This was confirmed by CT and MRI for those sites. Tc-99m HMPAO appears to be a useful agent for the localization of both the primary focus and unexpected metastatic lesion in patients with malignant melanoma.

An unusual cause of back pain in a patient with an abdominal aortic aneurysm. A case report.

A case of a 50-year-old man with chronic back pain, an abdominal aortic aneurysm and bony erosion of the lumbar spine due to a chondrosarcoma is reported. This report shows that the lumbar erosion seen in association with an aneurysm is unlikely to be a result of pressure erosion of the spine by the aneurysm, and that an extensive metastatic and septic work-up is required in such patients.

MR of mandibular invasion in patients with oral and oropharyngeal malignant neoplasms.

PURPOSE: To investigate whether MR imaging is an accurate means of assessing mandibular invasion in patients with carcinoma. METHODS: We retrospectively studied the MR scans of 22 patients with pathologic or surgical confirmation of mandibular invasion from oral and oropharyngeal cancers. The MR images were blindly analyzed using primary criteria of: (a) cortical breakdown, (b) replacement of bone marrow fat, or (c) gadopentetate dimeglumine enhancement of a mass at the bone marrow defect. Secondary criteria of: (a) contiguous soft-tissue mass, and (b) mass on both sides of the mandibular cortex were also examined. Mandibular invasion was graded as periosteal/cortical, medullary, or no invasion. RESULTS: Primary positive findings of cortical breakdown and abnormal bone marrow signal were highly sensitive (100%) for periosteal/cortical invasion and medullary involvement, respectively. However, a high rate of false-positive studies hampered the MR accuracy, which fell into the 73% to 77% range. A negative MR study was highly predictive, but a positive study was less valuable. Gadolinium enhancement added little to the MR study's accuracy. False-positive studies mainly occurred in the setting of prior irradiation, osteoradionecrosis, and odontogenic infections. CONCLUSIONS: MR imaging is a sensitive method for detecting mandibular invasion but has a low positive predictive value. A negative study virtually excludes cortical/periosteal or bone marrow invasion.

Primary idiopathic chylopericardium: report of a case and review of the literature.

Primary or isolated chylopericardium of unknown etiology is considered a rare cause of pericardial effusion. Its etiology is obscure but certain communication between the lymphatic system and pericardial sac was suggested. Up to 1991, there was only one case report that successfully showed the direct communication by a lymphangiogram. We report a case of chylopericardium occurring in a nearly asymptomatic 22-year-old man with no apparent history of trauma, infection or mediastinal neoplasm, in which we succeeded in visualizing the communication between the thoracic duct and pericardial sac by lymphangiography and computed tomography of the chest. A review of the previous cases is described also.

Increased cerebral perfusion after detachable balloon embolization of carotid cavernous fistula on technetium-99m-HMPAO brain SPECT.

Most symptoms and signs associated with a carotid cavernous fistula (CCF) are thought to be related to regurgitation of flow into cortical veins and to venous congestion. Arteriovenous shunting and secondary perfusion insufficiency is regarded as less important in causing symptoms. We describe a 27-yr-old male patient who had improvement of neurologic symptoms and signs after detachable balloon embolization of traumatic CCF. The pre- and postocclusion 99mTc-HMPAO brain SPECT scan showed improved cerebral blood flow after occlusion. The CCF had shown marked arteriovenous shunting without significant venous congestion on pre-occlusion cerebral angiogram. The postocclusion cerebral angiogram revealed complete occlusion of the CCF with increased blood flow in the ipsilateral middle cerebral artery distribution. These findings suggest that cerebral dysfunction may be related to perfusion insufficiency from the CCF. Brain SPECT scanning can assess the functional status of cerebral perfusion and may be a useful, noninvasive adjunct to angiography.

Thalamic syndrome and cortical hypoperfusion on technetium-99m HM-PAO brain SPECT.

The six patients included in this study had painful dysesthesia, resulting from vascular lesions in or near the thalamus, confirmed by computerized tomography(CT) brain scan. Using hexamethyl propyleneamine oxime(HM-PAO) single photon emission computed tomography(SPECT) brain scanning, regional cerebral perfusion(rCP) was demonstrated. In contrast to three patients with lesions near the thalamus who showed symmetrical cortical radioactivity, the other three patients with thalamic lesions revealed decreased rCP in the ipsilateral cerebral cortex on HM-PAO brain SPECT. We thought that the loss of afferent activating stimuli from the thalamus led to decreased cortical neuronal activity and the following hypoperfusion. In patients with thalamic syndrome resulting from thalamic lesions, the role of the remote effect of the thalamic damage and consequent cortical deregulation in the development of thalamic pain and/or neuropsychological symptoms cannot be excluded completely.

CT findings of the nose and paranasal sinuses in chromium intoxication.

We performed computed tomography (CT) of the nose and paranasal sinuses in 21 patients with chromium induced septal perforation or thinning. Twenty patients showed various magnitudes of septal perforation. Twelve of 20 had perforation at the mid portion of the cartilaginous nasal septum. One patient had a sheet-like thinning of septal cartilage. Sixteen patients had mucosal thinning of the nasal conchas. In most cases, unilateral involvement of the inferior concha was seen. Eleven of 21 cases showed paranasal sinus mucosal thickening and one patient had a cyst or polyp in the sinus cavity. The main finding was nodular thickening of mucosa. Septal perforation by inhalation of chromic acid was located in the cartilaginous septum and there was no destruction of the bony septum and wall of the sinuses.

Carcinoma of the prostate with brain metastasis.

Brain metastasis from prostatic carcinoma is considered rare and usually is diagnosed at postmortem examination. The authors reviewed the tumor registry of 1314 patients with proven diagnosis of prostatic carcinoma seen at Hines VA Hospital from January 1963 to December 1978 and found that 8 of the 1314 patients (0.6%) had brain metastasis. Six of eight patients (75%) were diagnosed having brain metastasis at premortem and of these, two patients presented with brain metastasis before diagnosis of prostatic carcinoma was made. Two of four patients who received palliative whole brain irradiation survived more than 1.5 years. It is our belief that patients with prostatic carcinoma who have symptoms of brain metastasis should be worked up and given palliative whole brain irradiation.

Wilms tumor in adults.

Wilms tumor rarely occurs during adult life. We have found 197 such cases reported in the world literature. However, there are some controversies whether all of these are true Wilms tumors or rather a variety of adenocarcinomas or sarcomatoid renal cell carcinoma. We report here 2 additional cases of well-documented Wilms tumors in adults.
