Atrial natriuretic peptide in diabetic patients before and after control of blood sugar level.

Atrial natriuretic peptide (ANP) was examined in 20 diabetic patients: 10 patients referred from the emergency room with severe hyperglycemia, (Group A), and 10 patients with uncontrolled diabetes referred by the outpatient clinic (Group B). Seven patients from Group A reached the nadir of less than 10 pg/ml, and three reached 12-16 pg/ml; following equilibration of sugar level, mean ANP level rose to 51.4 pg/ml (SD +/- 5.6). In Group B mean ANP level before treatment was 19.2 pg/ml +/- 11.4, and after diabetes control reached 40.4 pg/ml (SD +/- 10.04). Findings demonstrate a significant decrease in ANP in the acute hypoglycemic state, and a return to normal levels when sugar is controlled and hypovolemia corrected. Patients with chronic hyperglycemia exhibit compensation of intravascular volume. It seems that the equilibration system functioning via ANP is highly sensitive as a result of acute changes in total body fluid, and becomes desensitized during chronic disequilibrium.

Amplification of mineralocorticoid activity of aldosterone by 18-hydroxy-corticosterone and 18-hydroxy-19-nor-corticosterone in adrenalectomized rats.

A combination of aldosterone (1 micrograms) with either 18-OH-corticosterone (1 micrograms) or 18-OH-19-norcorticosterone (1 micrograms) injected to adrenalectomized rats indicated an amplification of mineralocorticoid activity as expressed by Na/K ratio in urine. Without aldosterone their mineralocorticoid potency was negligible.

Multiple Salmonella enteritidis leg abscesses in a patient with systemic lupus erythematosus.

We describe a 19 year old woman with systemic lupus erythematosus on corticosteroid therapy, who developed bilateral, multiple, gas-forming Salmonella enteritidis leg abscesses and osteomyelitis mimicking deep vein thrombosis. The infection was treated successfully by a combination of surgical drainage and intravenous ceftriaxone, followed by prolonged oral pefloxacin. This rare case of gas-producing S. enteritidis emphasizes the difficulty in diagnosing such complications in active systemic lupus erythematosus.

Pleural fluid to serum bilirubin concentration ratio for the separation of transudates from exudates.

Whether fluid accumulating in the pleural space is a transudate or an exudate is determined by the widely used criteria of the pleural fluid to serum LDH and protein concentration ratios. Such a distinction is important for limiting the extent of the differential diagnosis of possible causes for this condition. We have found that a pleural fluid to serum total bilirubin ratio can serve the same purpose. The correlation of a bilirubin concentration ratio of 0.6 or more with the presence of an exudate as determined by established criteria is statistically highly significant; and its sensitivity, specificity, positive predictive accuracy, and overall accuracy in relation to etiology and LDH or protein criteria (Light's criteria) are about 90 percent. Hence, the bilirubin criterion is statistically equivalent to the widely accepted LDH and protein criteria.

Preliminary report of the Cardiovascular Diseases and Alimentary Comparison Study: the Israeli experience.

The relationship between blood pressure and dietary constituents including potassium, sodium, calcium, and protein was studied in 183 randomly selected men and women, 50-54 years of age. Twenty-five of the subjects were drug-controlled hypertensive patients. Subjects were investigated by automated BP measurements, 24-h urine collection, and blood sampling. Mean systolic BP (SBP) was 119 +/- 17 mm Hg, placing the sample between Shanghai and Sweden on the distribution chart of the Cardiovascular Diseases and Alimentary Comparison (CARDIAC) Study. Salt intake estimated by 24-h sodium excretion was 8.5 +/- 3.4 g/day, between Australia and Okinawa on the distribution chart. Potassium was 48.55 +/- 20 20 mEq/day, between Urumqi (China), and Beppu and Ohda (Japan). Calcium was 162.3 +/- 89.9 mg/day. Urea nitrogen, which might reflect protein intake, was 9.5 +/- 3.1 g/day. Cholesterol was 195.1 +/- 38.1 mg/dl, between Brazil and Hirosaki. The Israeli results, as well as the data on other countries participating in the CARDIAC Study, show wide variability in the profiles generated by the investigated parameters. Each parameter placed Israel with a different CARDIAC Study group.

Treatment of hypertension by enalapril and hydrochlorothiazide separately and together: a multicenter study.

The efficacy and safety of enalapril and hydrochlorothiazide was investigated in a multicenter study of 81 patients with mild to moderate essential hypertension. The subjects were randomly assigned to one of three groups and, following a placebo period, given enalapril maleate (20 mg), hydrochlorothiazide (12.5 mg), or a combination of the two drugs (32.5 mg). A significant decrease in blood pressure was observed after only 2 weeks in the enalapril and enalapril-hydrochlorothiazide groups. A double dose was required to achieve a satisfactory response in one-third of the patients in both the enalapril (9 of 27) and the enalapril-hydrochlorothiazide groups (8 of 27). Adverse reactions included cough in one patient and mild hyperkalemia in another, both of whom received enalapril. Two patients on the drug combination developed side effects--symptomatic orthostatic hypotension in one and impotence in the second. Enalapril alone and in combination with hydrochlorothiazide appears to be effective and well tolerated.

Levels and correlates of blood pressure in recent and earlier Ethiopian immigrants to Israel.

Blood pressure (BP) and anthropometric data were collected on 483 recent immigrants from Ethiopia to Israel, and on 171 adults and 100 adolescent boarding school students who had immigrated two to three years earlier. Comparison of samples within the Ethiopian groups, and between new and early Ethiopian immigrants with Israel and resident Israelis revealed the following: Systolic and diastolic BP were considerably lower in recent immigrants of both sexes than among counterparts residing in Israel for two to three years. The latter group significantly approached, but did not reach, the BP levels of veteran Israelis. A similar comparison of 100 immigrant Ethiopian boarding school children with an Israeli high school population revealed apparently larger differences among the boys than the girls, whose BP was 'adapted' to a greater degree. Differences in relative weight paralleled those identified for BP, with the Quetelet index (QI) progressively higher when comparing new immigrants with earlier ones, and with persons who have lived in Israel for a longer period. Whether the weight differences provide a complete explanation for BP changes cannot be definitely stated, pending prospective data on weight and BP changes. Correlations of BP with anthropometric parameters and pulse rate indicated that weight rather than QI was the stronger correlate of BP in adults, whereas weight and height, but not QI, correlated with BP in adolescent boys. QI correlated with BP in adolescent, apparently sexually mature girls, but not in boys.

The use of aspirin to prevent pregnancy-induced hypertension and lower the ratio of thromboxane A2 to prostacyclin in relatively high risk pregnancies.

We carried out a prospective, randomized, double-blind, placebo-controlled study to investigate the capacity of aspirin to prevent pregnancy-induced hypertension and to alter prostaglandin metabolism. A total of 791 pregnant women with various risk factors for pre-eclamptic toxemia were screened with use of the rollover test (a comparison of blood pressure before and after the woman rolls from her left side to her back) during week 28 or 29 of gestation. Of 69 women with abnormal results (an increase in blood pressure during the rollover test), 65 entered the study and were treated with a daily dose of either aspirin (100 mg; 34 women) or placebo (31 women) during the third trimester of pregnancy. The number of women in whom pregnancy-induced hypertension developed was significantly lower among the aspirin-treated than among the placebo-treated women (4 [11.8 percent] vs. 11 [35.5 percent]; P = 0.024); the same was true for the incidence of preeclamptic toxemia (1 [2.9 percent] vs 7 [22.6 percent]; P = 0.019). The mean ratio of serum levels of thromboxane A2 to serum levels of prostacyclin metabolites after three weeks of treatment decreased by 34.7 percent in the aspirin-treated group but increased by 51.2 percent in the placebo-treated group. No serious maternal or neonatal side effects of treatment occurred in either group. We conclude that low daily doses of aspirin taken during the third trimester of pregnancy significantly reduce the incidence of pregnancy-induced hypertension and pre-eclamptic toxemia in women at high risk for these disorders, possibly through the correction of an imbalance between levels of thromboxane and prostacyclin.

Urinary output of urodiolenone in essential hypertension.

Increased amounts of urodiolenone, an isomer of a phytoalexin, were found in the urine of 34% of patients with essential hypertension. Urodiolenone was also excreted in 70% of patients with neurocirculatory asthenia (hyperkinetic syndrome) and labile hypertension; very small quantities were found in normal subjects. When grapefruit juice, which is rich in urodiolenone, was offered to hypertensive subjects, urodiolenone excretion was much higher than the amount ingested, indicating that output was not due to precursors present in the diet.

Blood pressure in Ethiopian immigrants in Israel: comparison with resident Israelis.

Blood pressure was examined in 483 Ethiopian immigrants a few weeks after their arrival in Israel. The results were compared with those from a group of 171 Ethiopian immigrants who came to Israel 2-3 years earlier. Mean blood pressure was significantly higher in the veteran immigrants, whose weight was also greater. Both groups had lower blood pressure than the resident Israeli population.
