Different effects of metoprolol and chlorthalidone on serum lipoprotein levels in mild hypertension. Possible implications for coronary heart disease risk status.

The effects of chlorthalidone and metoprolol on fasting plasma lipids and lipoprotein levels were compared in two similar nonrandomized groups of patients with mild hypertension. Chlorthalidone therapy was associated with an increase in serum cholesterol of 8.1% (17 mg/dl), mainly reflecting an increase in low-density lipoprotein (LDL)-cholesterol. High-density lipoproteins (HDL)-cholesterol decreased, but the difference between pre- and posttreatment levels did not reach statistical significance. Serum triglyceride (TG) concentration increased by 16% (20 mg/dl). Metoprolol therapy was not associated with changes in total, very low-density lipoprotein (VLDL)-, LDL- and HDL-cholesterol levels. Serum TG concentration increased by 22% (28 mg/dl), mainly due to an increase in VLDL-TG. Application of the Israel Ischemic Heart Disease Study data to these findings could predict only a very slight decrease in the 5-year estimated probability of myocardial infarction in the chlorthalidone-treated group. Metoprolol therapy has, theoretically, a more favorable influence on coronary heart disease risk status. These data suggest that the different forms of therapy for mild hypertension have a different effect on the theoretical coronary heart disease risk status, a fact that should be taken into consideration in the choice of medication.

Computerized tomography in the diagnosis of congenital hepatic fibrosis.

A case of congenital hepatic fibrosis (CHF), a relatively rare disease, was diagnosed preoperatively on the basis of clinical and radiological findings. The diagnosis was confirmed by computerized tomography and later proven by a liver biopsy performed during cholecystectomy. Increased awareness of this entity may reveal heretofore unidentified cases of CHF.

The relationship between posture and plasma catecholamines in the pregnant woman.

The response of endogenous norepinephrine levels (NE) to changes in body posture, lateral to supine, was investigated in a prospective study of 46 primigravid patients during the last half of pregnancy. Blood samples were obtained in the lateral and supine positions. The mean supine NE levels were not significantly higher in those patients destined to develop pregnancy-induced hypertension (PIH) than in those who remained normotensive. The mean percent relative change of NE in the patients destined to develop PIH and in those who remained normotensive was not significantly different.

Use of oral converting enzyme inhibitor, captopril for lateralizing renal venous renin activity.

Captopril was administered prior to renal vein renin sampling in an attempt to select patients amenable to surgical treatment for renin dependent hypertension. Renal venous blood for plasma renin activity was taken only after captopril stimulation. Sampling from the antecubital vein before and after this provocation showed a marked rise in renin, thereby confirming the efficacy of the test. Elimination of the initial selective renal vein sampling shortens the catheterization period without affecting the accuracy and dependability of the procedure.

Prolactin secretion in essential hypertension.

This study was designed to evaluate prolactin (PRL) secretion in patients with essential hypertension. PRL secretory pattern was assessed by hourly blood sampling between 2200 and 0800 hours. Additional samples were collected at 0810 and 1000 hours (10 and 120 minutes after assumption of upright posture), as well as at 1200, 1400, and 1800 hours under normal simulated activities. No difference could be detected between the two study groups at any of the sampling times, and the number of secretory episodes were similar. Basal plasma renin activity levels were inversely related to simultaneous PRL levels in the hypertensive patients (r= -0.60, p less than 0.05). In the normal subjects mean overnight PRL levels were inversely related (r= -0.78, p less than 0.05) to the overnight urinary Na/K excretion. There was no PRL response to posture in either group. Hypertensive patients had a greater early response to the dopamine antagonist, metoclopramide than did normal subjects. Our data do not support the previously introduced concept of enhanced PRL responses to normal physiologic stimuli in essential hypertensives. However, it appears that dopaminergic control of PRL secretion may be altered in this disease state.

Hypertension in middle-aged men. Associated factors and mortality experience.

Seven hundred and seventeen hypertensive middle-aged men (HTs) were compared with 4,438 normotensive men (NTs). The association between the prevalence of hypertension and a number of demographic, physical, biochemical and electrocardiographic characteristics was examined by multiple logistic analysis. HTs were characterized by significant elevations of pulse rate, relative weight, serum uric acid, and high-density lipoprotein-cholesterol. HTs had a higher percent of major electrocardiographic findings, such as ischemic T wave changes, ST depression and, most significantly, left-ventricular hypertrophy. They were 4.7 yr older than the NTs and were more often of Central European than of Middle Eastern descent. Mortality in HTs over a 4.5-yr period was dose-response related to casual systolic and diastolic blood pressure readings at baseline. The age-adjusted HT/NT mortality ratio was approximately 2.5:1. Cardiovascular and cerebrovascular disease accounted for 69% of the total mortality among HTs as compared with 48% among NTs. The estimated mortality fraction attributable to hypertension was 23%. This figure provides an estimate for the goal of hypertension control in the community.

The treatment of hypertension by labetalol--a new alpha- and beta-adrenoreceptor blocking agent.

A new alpha- and beta-blocking agent, labetalol, was used to treat 50 hypertensive patients, 34 of them with refractory hypertension. A significant reduction in hypertension was achieved with relatively few adverse reactions. The combination of alpha and beta blockers in labetalol with their mutually opposing characteristics offers an interesting and promising approach to the control of hypertension.

Angiotensin-I converting enzyme activity in the sera of captopril-treated hypertensive patients.

The blood pressure and serum angiotensin-I converting enzyme (SACE) activity were measured in captopril-treated hypertensive patients at frequent intervals. Inhibition of SACE was observed in patients responding to the treatment with lowering of blood pressure as well as in nonresponding cases. It was therefore concluded that a mechanism not depending on the formation of angiotensin-II by ACE is responsible for the high blood pressure persisting in the captopril treated nonresponding hypertensive patients. SACE activity in sera of captopril-treated patients recovers from inhibition when stored in frozen state at -20 degrees c. An apparent dissociation was therefore observed when regeneration of SACE activity during storage of sera was not taken into account. Meaningful activities are therefore obtained only if the assay is performed without prolonged storage.

Captopril, an orally active angiotensin I converting enzyme inhibitor in the treatment of renovascular and essential hypertension.

The hypotensive response to captopril is described for 12 hypertensive patients, 7 of whom had renovascular hypertension. The drug was effective in lowering blood pressure. The few reversible adverse reactions that occurred included loss of the sense of taste in one patient and rash and fever in another. Three patients with renal failure showed deterioration of renal function during treatment, suggesting the advisability of treating such cases with lower dosages.
