Self-mutilation: three cases and a review of the literature.

The self-mutilating patient is an unusual psychiatric presentation in the emergency department (ED). Nonetheless, serious underlying psychiatric pathology and drug abuse are important background risk factors. A careful stepwise approach in the ED is essential, although the prognosis, follow-up, and eventual rehabilitation are poor.

Thoracodorsal nerve conduction study.

There is no nerve conduction study for the thoracodorsal nerve in the literature. A conduction study for this nerve is described. Thirty healthy adults (16 males) with a mean age of 41.5 +/- 10.6 (range, 22-63) years were studied. The thoracodorsal nerve was stimulated at axilla and Erb's point with recording over the latissimus dorsi. The latency was 1.9 +/- 0.4 (range, 1.2-2.7) ms and 3.6 +/- 0.4 (range, 2.8-4.5) ms for the axillary and Erb's stimulations, respectively. The amplitude of the compound muscle action potential was 4.1 +/- 1.8 mv on the right and 3.9 +/- 1.4 mv on the left. The compound muscle action potential ratio was 0.8 +/- 0.12 (range, 0.55-0.99). This study may be useful to evaluate the integrity of the thoracodorsal nerve and to assist in the diagnosis and prognosis of brachial plexus injury.

Computed tomography of rib hemangiomas.

Hemangiomas of the rib may have confusing plain film findings. These tumors may be single or multiple and may remain stable or increase in size. The CT examinations reveal an expanded cortex and bone destruction with irregular trabeculation. An adjacent soft tissue mass may be present but pleural fluid is characteristically absent.

Orthodeoxia and platypnea secondary to a patent foramen ovale despite normal right-sided cardiac pressures.

Numerous cases of orthodeoxia and platypnea have been reported. Some have been found to be due to intracardiac shunts despite normal intracardiac pressures. In the case presented, a 79-year-old woman was noted to have orthodeoxia and platypnea secondary to a patent foramen ovale (PFO) despite normal intracardiac pressures. She recovered after closure of the PFO. The authors postulate that recent vertebral fractures and subsequent kyphosis, as well as a dilated thoracic aorta, altered intrathoracic relationships and may have led to a stream of venous return being directed across the PFO in the upright position, despite normal intracardiac pressures.

Suicide policies in secure environments.

A cohort of nurses and prison officers working in regional secure units, special hospitals and prisons were surveyed to establish their attitudes about observation policies designed to prevent patients/inmates from committing suicide. The results suggest that policies are rarely, if ever, reviewed, although there appear to be differences of emphasis in the three environments. Recommendations of the study include the need for improved staff communication, regular review of policies and better preparation of personnel.
