Immunoglobulin and complement deposits in the skin and circulating immune complexes in scabies.

Sixteen patients with papulovesicular, 6 with nodular and one with a Norwegian scabies were studied. Direct immunofluorescence (IF) examination revealed C3 deposits in the skin lesions of 13 of the 18 patients. Among them were all 6 cases with nodular scabies. C3 was found mostly in dermal vessel walls and 3 of the patients also showed IgM and 2 IgA deposits at the same site. No circulating immune complexes were found, with a solid-phase C1q radioimmunoassay (RIA), but HSV- and RSV-RIA methods detected IgM antibodies of rheumatoid factor type in 5 of the 15 sera examined. These results suggest that local complement activation and perhaps also immune complex deposition may by important in the pathogenesis of the papular and nodular skin lesions of human scabies.

Persistent pruritic papules from deer ked bites.

Deer ked (Lipoptena cervi L.), a haematophagous louse fly of deer, also attacks man and can cause persistent pruritic papules. Nineteen patients with this conditions were examined. They suffered attacks while carrying out their work, or merely walking, in forested areas. The bite papules appeared mostly on the head and back. They were very itchy and resistant to treatment and persisted from 2 weeks up to 12 months. Histologically, a typical insect bite reaction was found without any signs of retained deer ked mouth parts. Direct immunofluorescence showed deposits of C3 in dermal vessel walls in 7 of the 11 papules examined. Skin tests with a deer ked whole body extract were positive in all patients tested, showing both immediate and delayed reactions. Moreover, 57% of the patients tested had elevated serum IgE levels. All these findings suggest that IgE, complement and cell-mediated immune mechanisms are involved in the pathogenesis of deer ked bite reactions.

Fabry's disease and cornea verticillata. A report of 3 cases.

Fabry's disease is a rare familial disorder of glycolipid metabolism which is caused by a deficiency of a lysosomal enzyme alpha-galactosidase. A Finnish family is described in which cornea verticillata was found in the father and 2 daughters. In all cases, there were symptoms suggesting Fabry's disease: febrile episodes the origin of which was not clear, limb pains and, in the case of the father, 20 years of proteinuria with elevated ESR, and hemiplegia and aphasia following a cerebral thrombosis at the age of 43. The diagnosis was confirmed by demonstration of an alpha-galactosidase deficit in the serum and urine of all patients. Deficiency of this enzyme leads to abnormally high urinary tri- and dihexosyl ceramide levels, and this was observed in the father and the elder daughter. At the age of 12, the daughter had loss of vision in her right eye as a result of occlusion of the central retinal artery. Electron microscopic (EM) examination of the father's dermal angioma suggested Fabry's disease. Computerized cranial tomography of the father revealed not only the cerebrovascular condition but also a disease affecting the white matter of the brain.

Effect of instruction and motivation on dental knowledge and behavior among wearers of partial dentures.

Patients receiving partial dentures were divided into three groups. One "maximum" group was taught and motivated, both individually and as a group, as thoroughly as possible. The second "medium" group was given instruction sheets describing the essential facts related to the cleaning and maintenance of teeth and dentures., but no individual instruction and motivation were provided. In the third "minimum" group the obtaining of information was left entirely to the initiative of the patients themselves. By the end of the period of treatment, the patients' knowledge was in proportion to the amount of teaching they had received. As regards dental health behavior, the groups did not differ significantly from one another, although there were some indications that increased knowledge and motivation had promoted positive oral hygiene habits. A year after the completion of treatment the positive behavior habits were markedly fewer in all groups, and no longer bore any relation to the amount or type of instruction or motivation given.

Solar urticaria: a case with increased skin mast cells and good therapeutic response to an antihistamine.

A 45-year-old woman with solar urticaria is described. She was found to have: (a) increase of skin mast cells; (b) a good therapeutic response to hydroxyzine hydrochloride; (c) frequent peptic ulcers of the stomach; (d) antinuclear IgM antibodies in a titre of 1 : 1 000; (e) cold urticaria and dermographism; (f) anamnestically, suppression of the solar urticaria by a sequential oral contraceptive.

A discrete multichannel procedure for the determination of acid phosphatase activities in serum.

The purpose of this study was to increase the capacity of the laboratory to perform analyses for the acid phosphatase activities by adapting the previous manual method to a discrete multichannel, computer controlled photometer. The modified system was far superior to the manual method, showing an improvement in the accuracy and precision of the method as well as a three times higher efficiency of the method for analyses. The reference values for serum acid phosphatase and its tartrate stable fraction were determined from apparently healthy women and men. The clinical significance of the method was discussed on the basis of the year's experience.

Functional disorders of the masticatory system in southwest Finland.

Functional disorders of the masticatory system in a working age population in Southwest Finland were investigated both by interview (subjective symptoms) and by clinical examination (objective symptoms). The randomized material consisted of 583 subjects. 58% had subjective symptoms of dysfunction. With certain exceptions, the symptoms did not correlate with age, sex or use of removable dentures. The number of subjects with symptoms was highest in the group with the highest number of occluding teeth. 5% of subjects with subjective symptoms had received treatment. Clinical examination revealed symptoms in 41%. Sex did not correlate with the findings. The percentage of subjects with clinical symptoms was higher among those over 35 than in the younger age groups; this correlation was not, however, observed in the full denture wearers. Full denture wearers had symptoms more often than the others. Although young persons with relatively good natural dentition showed objective symptoms less often than the elderly and those wearing full dentures or with incomplete dentition, they more frequently had subjective experiences. This perhaps explains why the findings based on patient materials differ from those reported from pure population studies.

Dentists and dental technicians as competing suppliers of complete dentures in Southwest Finland.

In Finland, not only dentists but also dental technicians with special training have the right to supply clients with complete dentures. The present study investigated how the demand for their services is distributed between these two professional groups, and what are the factors affecting the distribution. The material consisted of 220 randomly selected subjects who had lost all their teeth in one or both jaws. Of the full dentures worn, 50% had been made by a dentist and 41% by a dental technician, while in 10% one denture, either maxillary or mandibular, was made by a dentist and the other by a dental technician. 56% of the subjects said they would choose a dental technician the next time health center services become more general and patients are more able to get treatment by dentists at a reduced cost, it seems probable that the majority of the edentulous will again resort to the dentists. However, judging by the current attitudes, a considerable number of the edentulous will continue to resort directly to the dental technician for the supply of dentures.

Diphenylpyraline (Lergobine) in the treatment of patients suffering from allergic and vasomotor rhinitis.

In a double-blind study, diphenylpyraline (Lergobine) was given to 63 patients whose main symptoms were stuffiness of the nose, increased secretion of mucus, snuffling, sneezing and redness of the eyes. Fifty-seven patients were given placebo for identical symptoms. Diphenylpyraline was found to have a better effect on all the symptoms than placebo. The difference was statistically significant in respect of the discharge of mucus and redness of the eyes, and when the total symptoms were considered as a whole. In atopic patients the better effect of diphenylpyraline was highly significant.

A discrete multichanneal procedure for the chemical determination of urinary estrogens during pregnancy.

A colorimetric method, based on the Kober reaction, was adapted for the determination of urinary estrogens during pregnancy. The method yields final volumes for direct measurements by a multichannel computer-controlled photometer. The speed of the analysis was increased so that 100 samples can easily be completed by one laboratory technician as triplicate analyses during one working day. A satisfactory accuracy and precision of the method were also achieved. The values were determined using urine samples from 1097 apparently healthy pregnant women.

Testings of the ostial patency and penetrance.

In the present study, consisting of 118 cases of maxillary sinusitis with retention of secretion, ostial patency and penetrance were examined three times during each examination, natively, postsuctionally and postirrigationally, by using consecutive measuring technique. Results indicate that, when both ostial respiratory patency and sniff/blow penetrance were tested in these afore-mentioned three phases at each examination that the information gained about ostial patency and penetrance is most representative when the most normal of these results, i.e. the optimal result, is taken as the finding. This is particularly important when testing the ostial penetrance. Observed inconsistencies between the results of the patency and penetrance tests are apparently due to the different pressures used in testing ostial patency and penetrance.
