[The effect of static magnetic fields on osteoblast activity: an in-vitro study].

The aim of the present study was to investigate whether the influence of static magnetic fields can be demonstrated in an osteoblast cell culture. For this in-vitro study we used osteoblasts originating from the embryonal calvariae of rats. The total material (n = 41) was divided in four groups: a) cell cultures submitted to the fields of attracting magnets (n = 11), b) cell cultures in the fields of repelling magnets (n = 10), c) cell cultures under the influence of demagnetized magnets (sham group, n = 11), d) control cultures (n = 9). The cell cultures were subjected for 21 days to magnetic fields (intensity 178 ml) using Neodynium magnets. Osteoblast activity was histo-chemically determined by staining the alkaline phosphatase produced. The quantitative evaluation was performed by measuring the optical density of the cell cultures and the results were statistically analysed. The results of these in-vitro study are: 1. The optical density of the cell cultures was nearly the same in samples with or without the influence of magnetic fields. 2. No divergent reaction could be found between the application of homogeneous and heterogeneous magnetic fields. 3. The different orientation of homogeneous magnetic fields in vertical or horizontal direction did not effect the osteoblast cell culture. 4. The cellular distribution in the osteoblast cultures does not follow the orientation of the magnetic field lines. 5. The form of the osteoblasts was nearly the same among all four groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[The treatment of open bite using magnets].

Until recently, surgical correction of the jaws has been widely regarded as the most effective treatment of open bites in the permanent dentition. In the following case report an open bite was treated non-surgical by intrusion of the molars with a magnet appliance. The forces of repelling magnets, fixed opposite in the upper and lower jaw were used to intrude the first and second molars. This intrusion allowed an autorotation of the mandible upward and forward to close the open bite. By reduction of the open bite of about 3 mm a surgical intervention could be avoided.

Adolescent

Treatment of onychomycosis with terbinafine.

An open multicentre trial was conducted by 40 dermatologists in Switzerland involving 188 patients with onychomycosis of either the toenails or fingernails. Of these patients, 145 who had positive microscopy and culture of dermatophyte infection were evaluable: of the dermatophytes identified at the initial visit, 80% were Trichophyton rubrum and 12.4% were T. mentagrophytes. Only the most affected nail was evaluated during the observation period. Daily dosage was 250 mg of terbinafine (Lamisil) orally for up to 6 months. The cure rate (negative microscopy and culture) at the end of treatment was 77% for toenails and 100% for fingernails. A follow-up investigation was made 6 months after the end of treatment: of the 88 patients examined with onychomycosis of the toenail and the 14 with fingernail onychomycosis, 90.9% and 85.7%, respectively, remained free of recurrence. Of the 26 patients who had shown improvement, but not cure, by the end of the treatment period, 15 were clinically and mycologically cured at the time of the follow-up investigation. Terbinafine was generally well tolerated; the most frequent drug-related adverse events were mild-to-moderate gastrointestinal disturbances. Changes in liver or renal biochemical tests were not considered clinically relevant.

[Integrated and frequency spectra of the electromyograms in patients with temporomandibular symptoms].

With the aid of bipolar surface electrodes, the electromyographic activity of the anterior temporal and the superficial masseter muscles were recorded bilaterally and evaluated by counting the integrated potentials and mean power frequency using Fourier's analysis. A comparison between 17 patients with myofacial pain dysfunction (average age 21.3 years) and a group of 20 controls (average age 20.5 years) revealed a significantly greater activity in patients with pain syndrome, while no significant inter-group difference in mean power frequency was seen. The differences in the level of activity between the right and left sides in the control group were significantly smaller than in patients with pain dysfunction. An investigation of measures aimed at reducing the right/left asymmetry in a further 14 patients with pain dysfunction revealed the practice of "maximum occlusion with feedback" to be the most effective measure.

Molar distalization by intra-oral force application.

The molar distalizing bow (MDB) guarantees controlled distal movement of the molars. It is easy to handle, can be removed at any time and can be worn almost full time. Since there is no extra-oral force, there are no unphysiological effects on the cervical spine and neck muscles or on the molars to be moved. Furthermore, there is no risk of injury by wearing the appliance. Modifications of the basic appliance broaden the range of applications.

[The mandibular action of individual treatment remedies depending on facial morphology].

56 patients with Angle's Class II malocclusion were treated by three different orthodontic methods, depending on facial morphology. They were examined with regard to displacement and growth of the condyle and pogonion. The changes in the area of the condyle and pogonion were differentiated by "local remodeling" and "superimposition translocation". The results show that condylar growth can be influenced by different methods of functional appliances or cervical traction, in contrast to local remodeling of pogonion which is totally independent of the type of treatment.

Activator Appliances

[Apical root resorption during orthodontic treatment].

Pre- and posttreatment radiographs of 2050 orthodontically treated patients were examined and apical root resorptions of more than 2 mm were identified. 30 patients (211 teeth) were affected. Treatment characteristics and objectives of this group (with severe apical root resorption) were compared to a treated group of 30 patients (with no resorption) to determine clinical risk factors. Extrusion, but especially jiggling and a long treatment period were found to be significantly more frequent in the group exhibiting resorption than in the control group. Pathogenesis of treatment-induced root resorption is discussed in respect to histological results.

Adolescent

[Changes in clinical crown length during multiband treatment].

The clinical crown lengths of the incisors and first molars of 50 adolescents were measured on dental casts prior to and five to seven months following end of treatment and the differences were recorded. The patients were treated with fixed appliances for an average of 23 months. In addition, initial crown lengths were compared to those of ten children who were four years younger on average. Treatment time and objectives were recorded. Statistically significant differences were determined by means of the t-test. In order to test for correlations between treatment objectives, duration of treatment and change in clinical crown length, correlation analysis was used. The results were: clinical crown length increases physiologically from ten to 14 years of age by about 0.175 mm per year. During fixed appliance therapy in adolescents, the clinical crown length increases. This is accelerated when employing extrusive mechanics, whereas intrusive mechanics reduce this tendency. There is no evidence to suggest that the process is accelerated by the type of appliance.

[Treatment of nail mycoses].

Onychomycosis is defined as an infection of the nail unit by fungi. Four clinical types of onychomycoses are recognized by the manner in which the nail plate is invaded. A combined therapy with elimination of the nail by surgical extraction or a chemical onycholysis, a short systemic antifungal therapy and a topical treatment until the healthy nail has grown is recommended.

[Frequency spectrum and integral value of electromyographic recordings of the masseter and temporal muscles in correlation with muscle length].

In 15 children and 15 adults the influence of muscle length on the EMG-recordings of the anterior temporal and superficial masseter muscles was investigated. Myoelectric activity was measured by surface electromyography during maximal clenching in habitual occlusion. The quantitative EMG analysis was performed by means of a computer program which allowed calculation of a) the integrated value and b) the mean frequency of the power spectrum (MPF) from the raw signal after Fourier transformation. Muscle length was determined with the aid of lateral cephalometric films. Using a correlation analysis this investigation revealed the following results: Under standardized functional conditions short muscles work with a higher activity and a higher MPF than long muscles. In children as well as in adults muscle activity and MPF show close negative correlations with the parameter "muscle length". These relations are directly proportional.

[Bilateral non-congenital acanthokeratolytic epidermal nevus].

A patient with a noncongenital bilateral epidermolytic nevus is presented. The disease picture is compared with those of congenitial bullous ichthyosiform erythrodermia and bilateral epidermolytic nevi as reported in the literature. The genetic aspects are discussed.

Acantholysis

The neuromuscular reaction to continuous dynamic jaw extension in cases with restricted mouth opening.

20 patients with severe trismus resulting from either trauma or infection have been treated using the method of "continuous dynamic jaw extension". The clinical results and the electromyographic tests show that almost complete rehabilitation of mandibular movement is possible. The treatment is most effective during the healing phase of the abscess and as an early treatment for the traumatised patient. Extension exercises should not be undertaken during the exudative phase of abscess formation. The reaction ability of the masticatory musculature obtained from the electromyographic investigations is defined in the "reactive myotonus-index". Thus the jaw extension appliance used is helpful both in the diagnosis and treatment of severe trismus.
