Femoropopliteal bypass for limb salvage.

A total of 148 femoropopliteal bypass procedures followed for a minimum of 2 years yielded a patency rate of 61.9 percent among survivors. Diabetes mellitus, cigarette smoking, quality of the outflow, or prior inflow procedures did not influence patency rate. Results were less satisfactory in women and when cloth grafts were used. The indications for femoropopliteal bypass require continuing careful assessment.

The influence of venous occlusion on xenon 133 muscle clearance.

Progressive degrees of venous occlusion in the canine hind limb were employed to evaluate the influence of venous disease on the xenon 133 muscle washout test. Increasing degrees of venous occlusion, from superficial venous occlusion alone to total occlusion induced with intravenously administered thrombin, decreased both resting muscle blood flow and maximal muscle blood flow stimulated with intra-arterially administered papaverine hydrochloride. This alteration lasted for varying lengths of time with all but the group with total venous occlusion eventually returning to a normal flow. The status of the venous circulation must be considered in order to accurately interpret the results of the 133Xe clearance test when used to evaluate arterial occlusive disease. The 133Xe washout test may be an accurate indicator of venous occlusion in a limb with normal arterial supply.

Long-term patency of vein grafts preserved in liquid nitrogen in dimethyl sulfoxide.

Autogenous canine jugular veins were stored in 15% dimethyl sulfoxide (DMSO) in liquid nitrogen vapor for one to 28 days and then implanted in the carotid artery as autografts. The patency rate at one year was 62.5-87.5%. The patency rate of fresh jugular vein autografts placed in the carotid artery for one year was 75%. Similar autografts stored in liquid nitrogen vapor for one to 28 days without the cryopreservative DMSO exhibited a zero to 12.5% patency rate at one year. Scanning electron microscope studies revealed preservation of theendothelium in DMSO protected veins and a damaged or sloughed endothelium in veins frozen without DMSO cryopreservation.

Focal infection of aortofemoral prostheses.

A canine model of unilateral aortofemoral bypass was utilized to study periprosthetic infections in woven, knitted, and velour Dacron vascular prostheses. These studies suggest that physical configuration, porosity, and time of inoculation all are important variables when managing an infected prosthesis, although size of inoculum seems less important.

The use of velour as a vascular prosthesis.

Knitted Dacron cloth fabricated into tubes has gained widespread acceptance as an arterial prosthesis. Continued research and development in the field of vascular prostheses is required because existing materials are not ideal. This paper relates general developments in vascular prosthesis research and focuses on specific results with Dacron velour materials. The nature of the velour surface offers several advantages that are important in designing a more satisfactory prosthesis.

The influence of acculturation on attitudes toward alcohol and alcohol use within the Punjabi community: an exploratory analysis.

A number of authors have found that acculturation and alcohol use are significantly related. In general, as a person becomes more acculturated within a new country, levels of use of alcohol and alcohol-related problems tend to become more similar to the host cultures. The purpose of this paper is to examine the relationship between the level of acculturation, attitudes toward alcohol, and the use of alcohol within the Punjabi community of Metropolitan Toronto, while taking a number of demographic variables into account. We found that increased levels of acculturation were associated with increased lifetime use of alcohol and more liberal attitudes toward the use of alcohol. In addition, those who had lower levels of acculturation were experiencing relatively more alcohol use-related problems. Finally, gender was also an important factor in predicting attitudes and behavior associated with alcohol, with women more likely to be abstainers and negative toward the use of alcohol.

Acute cholecystitis after spinal fusion and instrumentation in children.

Three children developed noncalculus cholecystitis after spinal fusion and instrumentation for their spinal deformity. Two children responded to conservative therapy, and one required cholecystectomy. If diagnosed early, this complication may be successfully treated by conservative means.
