The role of nurses in improving emergency cardiac care.

Cardiovascular diseases exert a huge burden on individuals and society, with coronary heart disease (CHD) the single most common cause of death in the United Kingdom and other developed countries (British Heart Foundation 2004). Improved clinical care has been responsible for around two fifths of the decline in mortality from CHD in England and Wales over the past decade (Unal et al 2004). This article describes how developments in cardiac care, most of which have closely engaged nurses, have contributed to improvements in care for patients with acute myocardial infarction and other acute coronary syndromes.

Prevalence and patterns of condom use among patients attending a sexually transmitted disease clinic.

The purpose of this study is to evaluate prevalence and patterns of condom use among patients attending a sexually transmitted disease (STD) clinic, who are at increased risk of developing STDs. Interviews of 800 patients attending a Baltimore STD clinic were conducted. The prevalence of condom use was dependent on the measure of use employed. For example, 9 out of 10 patients reported ever having used condoms, slightly less than one-third of the patients reported having used condoms in the past 30 days, and only 17% of the patients reported using a condom at last sex. Use also varied somewhat by type of sexual partner, and within any given relationship, from first to last sexual encounter. The most significant factor corresponding to condom use in the 30 days prior to the interview was number of sexual partners (lifetime and in the last month). Multivariate analyses revealed number of lifetime partners to be the most significant predictor of condom use for men, whereas age was the most significant variable for women. These findings suggest that interventions should focus not only on the individual, but the couple.

Archiving requirements for electronic pharmaceutical manufacturing documents and associated executable software.

Many pharmaceutical manufacturers are currently evaluating the feasibility of electronic batch record (EBR) and electronic document management (EDM) systems. Considerable effort has been invested in the design of the batch record files and electronic signature devices and procedures. Much less consideration has been given to the potential need for pharmaceutical manufacturers to be able to re-create the operational software environment necessary to review archived documents at some future date. The paper discusses methods, policies, and equipment that can be used to fulfill this function.

Early experience of nurse-led elective DC cardioversion.

Direct current (DC) cardioversion is an established therapy for patients with atrial fibrillation or atrial flutter. In this observational study 88 patients (59 [67%] male) were admitted as day cases to a short-stay ward for elective cardioversion, of whom 74 (88%) were referred for cardioversion of atrial fibrillation, 12 (14%) for atrial flutter and two (2%) for atrial tachycardia. 40 (54%) of patients with atrial fibrillation were initially converted to sinus rhythm by DC shock, although 3 (7% of this group) reverted to atrial fibrillation during the recovery period. Nurse-led, day case elective cardioversion on an elective ward appears to offer a safe alternative to CCU admission.
