Detection of GBV-C/HGV RNA in saliva and serum, but not in urine of infected patients.

Saliva and urine samples from six GB virus C (GBV-C)/hepatitis G virus (HGV)-infected renal transplant patients were tested by RT-PCR. Viral RNA was detected in all saliva samples, but the viral RNA titers in saliva were 100 to 10,000 lower than those in the corresponding sera. Comparative sequence analysis of the amplified 354 bp DNA from one patient revealed full identity of GBV-C/HGV variants present in serum and saliva. None of the urine samples from the six patients was found to contain GBV-C/HGV RNA. High prevalence of GBV-C/HGV RNA in saliva of infected individuals may contribute to a wide spread of GBV-C/HGV infection, at least in some settings.

Endothelin in cardiovascular control: the role of endothelin antagonists.

Endothelin (ET) is a potent vasoconstrictor associated with various cardiovascular diseases. ET mediates its effects through ET receptors on vascular smooth muscle cells as well as on the vascular endothelium. Furthermore, a neurotransmitter role for ET has been suggested on the basis of experimental and human in vivo studies. ET antagonists are potent tools for studying the effects of ET and its receptors. They have been widely used in vitro and in experimental models of cardiovascular disease, where ET levels are elevated and reactivity to ET is altered. Promising clinical trials in hypertension, coronary artery disease, and congestive heart failure are discussed in this review. Different forms of renal failure are associated with markedly increased ET levels, and ET antagonists experimentally improve renal function in these models. Extrapolating from experimental and first clinical experience, ET antagonists could be useful in the treatment of hypertension, coronary artery disease, congestive heart failure, and renal failure, especially in combination with other drugs, ie, angiotensin converting enzyme inhibitors. The inhibition of ET-induced stimulation of nociception allows for speculation that ET antagonists might even have analgesic properties.

Cystatin C: efficacy as screening test for reduced glomerular filtration rate.

Serum cystatin C, a cysteine proteinase inhibitor, has been proposed as a marker of glomerular filtration rate (GFR). Serum cystatin C, serum creatinine and creatinine clearance were measured in 226 patients with various nephropathies, covering the entire range of renal function, to evaluate the efficacy of cystatin C as a screening test to detect reduced creatinine clearance in comparison to creatinine. Subgroups of 53 patients with glomerular and 26 patients with tubular impairment were compared to assess whether cystatin C performed differently in either glomerular or tubular impairment. Cystatin C detected reduced creatinine clearance with higher sensitivity (97 vs. 83%), and higher negative predictive value (96 vs. 87%) compared to creatinine. In parallel, 95% sensitivity of cystatin C as derived from receiver-operating characteristic plot was significantly higher (p < 0.05). In the subgroups with glomerular or tubular impairment, cystatin C and creatinine did not significantly differ with regard to efficacy. Serum cystatin C is as efficacious as serum creatinine to detect reduced GFR as measured by creatinine clearance. The efficacy of cystatin C as a screening test may even be superior compared to creatinine. In addition, the efficacy of cystatin C is independent of either glomerular or tubular impairment.

Recurrent neurovascular hypertension: MR findings before and after surgical treatments.

A case of recurrent brain stem edema after surgical vascular decompression in a patient with neurogenic hypertension is presented. The surgical treatment resulted in occlusion of the left vertebral artery, stable blood pressure values, and no recurrence of the brain stem edema. MR imaging and MR angiography are excellent methods with which to assess patients suspected of having neurovascular hypertension, both before and after surgical treatment.

[Musculoskeletal manifestations of diabetes mellitus].

The article outlines changes of locomotor system in patients with diabetes mellitus. Classification of individual diseases is based on the requirements of clinical practice mainly. The following diseases are described here: diabetic cheiroarthropathy, shoulder impingement syndrome, Charcots joint, diffuse idiopathic skeletal hyperostosis, diabetic osteopathy and other pathologies associated with diabetes.

Morphologic cerebral changes in patients undergoing dialysis for renal failure.

Cranial computed tomographic examinations were performed on 22 patients before and after hemodialysis, and on 11 of those patients 8 months later. In 11 cases widening of the cerebrospinal fluid spaces due to brain atrophy was recognizable. Density measurements disclosed decreased absorption of cerebral parenchyma after dialysis in 73% of the cases. This finding is partly explained by cerebral edema or hydration, although the necessary contemporaneous narrowing of cerebrospinal fluid spaces existed in only six patients (27%). Long-term follow-up studies have not displayed any realizable changes in results. Finally, despite striking computed tomographic findings and alterations after dialysis, only mild neurologic symptoms or disequilibrium syndromes have been registered.
