[Changes of thyroid hormone levels in the perioperative period in patients with various types of goiter].

The thyroid hormone level was studied in the perioperative period in patients with various types of goitre. The clinical material comprised 85 patients treated surgically for nodular neutral goitre, nodular hyperactive goitre, Graves-Basedow disease, and cholecystolithiasis. The determination of total and free thyroid hormone blood serum levels was carried out in three periods of time, that is 4-5 days before operation, immediately after operation, and on the fifth day after operation. The obtained results led us to reaching the following conclusions: 1. Achieving of clinical euthyroid state in patients with Graves-Basedow disease during preoperative preparation with thyrostatic drugs was connected with the decrease of total and free thyroid hormone levels below the lower range of normal values which persisted immediately after, and on the 4th-5th day after operation. 2. During operation no significant increase was found of serum thyroid hormone levels. 3. On the 4th-5th day after operation, especially in patients with nodular neutral goitre and cholecystolithiasis a tendency was observed for a drop of triiodothyronine level, especially of its free fraction level.

[Heart rhythm variation and electrophysiologic parameters of the sino-atrial node].

UNLABELLED: Sinoatrial node (SAN) function depends on its intrinsic properties and on regulatory influences of autonomic nervous system. Disturbances in these both factors are responsible for intrinsic and extrinsic SAN dysfunction (SAND), respectively. Heart rate variability (HRV) is a reliable noninvasive marker of cardiovascular autonomic control. We examined 120 patients (pts) with syncope, palpitations and/or bradycardia (< 40 bpm during daily activity). In these patients, after one-minute oesophageal-ECG recording for HRV assessment, rapid continuous and programmed premature transesophageal left atrial pacing (TAP) were performed. Regarding the results of TAP, pts were divided into two groups: group I included 57 subjects with negative TAP, and group II consisted of 63 pts with positive TAP (SAND). We analysed two HRV parameters: variation ratio (VR, %) and maximal difference of any two consecutive sinus cycle lengths (DSCL, ms). The following TAP variables were calculated: corrected sinus node recovery time (CSNRT), second pause time (SPT) and calculated sinoatrial conduction time after Strauss (SACT). RESULTS: Mean DSCL was significantly shorter in group I (106 +/- 96) than in gr. II (151 +/- 169) (p < 0.05). Similarly behaved VR values. In SAND-pts with abnormal CSNRT or SPT we observed significantly greater VR and DSCL. This correlation was not confirmed in respect to SACT. We corroborated the significant coincidence of high HRV (VR < 20.0 and DSCL > 100) with pathologic CSNRT or SPT, but not with SACT.(ABSTRACT TRUNCATED AT 250 WORDS)

[Long-term mexiletine treatment of ventricular arrhythmia in patients after myocardial infarction].

Eighty two patients with ventricular extrasystole (VES) after myocardial infarction had been treated with antiarrhythmic drugs in the half (41 patients) with mexiletine (M) and in the half (41 patients) with other ones. The mortality rate in the M-group was twice time lower than in the other, however this difference was statistically not significant. M was mostly effective in VES of III and IVb degree after Lown's classification and did not cause any deterioration of the left ventricle function.

Determination of immune complexes in patients with acquired valvular heart disease.

The authors investigated the incidence of circulating immune complexes (CIC) in 52 patients with acquired valvular heart disease (VHD) and in 50 control blood donors. There was a statistically significant relationship between the CIC occurrence in patients with VHD and the presence of an active process in endocardium. However, no relation was found between the presence of CIC and heart operations, enucleation of palatine tonsils and the past history of rheumatic disease. The authors conclude that CIC investigation is a valuable diagnostic tool in patients with VHD and in those suspected of active infective endocarditis (IE). The obtained results confirm the presence of an immune factor in the etiopathogenesis of IE leading to VHD. However, they do not explain the implantation of the inflammatory process in the heart valves.
