Insecticidal flavaglines and other compounds from Fijian Aglaia species.

Bioassays with lipophilic crude extracts of four Fijian Aglaia species against Spodoptera littoralis displayed strong insecticidal activity for A. basiphylla and A. gracilis, whereas A. archboldiana and A. vitiensis did not have any significant effects. The insect toxicity of A. basiphylla was caused by the well known benzofuran flavaglines rocaglamide, desmethylrocaglamide and aglafoline. In contrast, A. gracilis contained four related pyrimidinone derivatives in the root and stem bark, including two new congeners named marikarin and 3'-hydroxymarikarin. Moreover, two new putrescine bisamides, secoodorine and secopiriferine, a new benzopyran flavagline. desacetylaglain A. and a new norsesquiterpene were isolated from the leaves together with three known bisamides and 3-hydroxy-5,7,4'-trimethoxyflavone. The structures of the new compounds were elucidated by spectroscopic methods. Comparative feeding assays within the active pyrimidinone flavaglines showed that the free hydroxy group in aromatic ring A of marikarin diminishes insecticidal activity.

A 3-year study of prevention of postmenopausal bone loss: conjugated equine estrogens plus medroxyprogesterone acetate versus tibolone.

The aim of this study was to investigate the effects of tibolone in the prevention of postmenopausal bone loss over 3 years, and to compare these with the effects of sequential hormone replacement therapy. Forty early postmenopausal women were randomized to a 21-day regimen of conjugated equine estrogens (CEE, Premarin) plus sequential medroxyprogesterone acetate (MPA, Prodafem), or tibolone (Livial) daily. In total, 36 women completed 12 months and were considered for the intent-to-treat analysis, 34 completed 24 months and 23 completed 36 months. Main drop-out reasons were: lost to follow-up (n = 9) and minor side-effects (n = 4). Bone mineral density was measured at baseline and after 6, 12, 24 and 36 months, using dual-energy X-ray absorptiometry at the lumbar spine and the upper femur (neck, trochanter, total hip). In both groups, bone loss was prevented. Treatment with tibolone demonstrated significant increases in bone density at the spine (+4.6%; p < 0.01), at the total hip (+3.2%; p < 0.01) and at the trochanter (+4.5%; p < 0.01), whereas the CEE/MPA group showed a non-significant increase of bone mineral density at the lumbar spine (+2.6%) but no increases at the hip. Between-group differences in bone mineral density changes were significant (p < 0.05) for the total hip and the trochanter at 36 months. This increase of bone mineral density was not accompanied by changes in insulin-like growth factor-I (IGF-I) or insulin-like growth factor binding protein-3 (IGFBP-3) in either group. Osteocalcin, alkaline phosphatase and urinary ratios of hydroxyproline/creatinine and calcium/creatinine significantly decreased in both groups. In conclusion, sequential CEE/MPA prevented cortical and trabecular bone loss, with a transient increase of bone mineral density only during the first 6 months. Tibolone not only prevented cortical and trabecular bone loss, but further increased bone mineral density at the lumbar spine and at the hip throughout the 3 years of treatment, suggesting a sustained positive effect on bone mass.

Intrauterine insemination: evaluation of the results according to the woman's age, sperm quality, total sperm count per insemination and life table analysis.

We report on 332 infertile couples who underwent 1115 cycles of intrauterine insemination (IUI) with washed husband's semen. The indication for IUI was an abnormal post-coital test due to either a male or cervical infertility factor. The mean number of IUI cycles per patients was 3.4, the overall pregnancy rate 18.7%, and the pregnancy rate per cycle 5.6%. The cumulative pregnancy rate calculated by life table analysis showed that 16.0% of pregnancies occurred in the first three treatment cycles, while the cumulative pregnancy rate was 26.9% by the sixth cycle. The outcome of the therapy was adversely affected if the woman's age was > 39 years and/or total motile sperm count per insemination was < 1 x 10(6). No pregnancy occurred in women older than 44 years or in cases with a total motile sperm count before semen preparation of < 1 x 10(6).

On the stability of key enzymes of energy metabolism in muscle biopsies.

Enzymes of energy metabolism were tested for stability depending on different storage conditions (-20, -80 degrees C). To avoid problems due to the different fiber type composition of human muscle, we selected two muscles from rabbit. The m. psoas consists almost exclusively of type 2B fibers, and the m. soleus consists almost exclusively of type 1 fibers. Enzyme activities were measured from small aliquots of these muscles at various time points up to 1 year after sacrificing the animal. Enzymes from anaerobic metabolism were stable for more than 1 year, independent of whether the muscle was stored at -20 or -80 degrees C. Oxidative enzymes, such as succinate dehydrogenase, citrate synthetase, or cytochrome c oxidase (COX) decrease in activity at -20 degrees C and, to a lesser degree, at -80 degrees C. In addition, mitochondria were isolated from freshly taken muscle and stored at -80 degrees C. Oxidative enzymes were surprisingly stable for more than 1 year, with the exception of COX which decreased by 60% of its original activity in mitochondria from m. soleus.

[Herniated cervical disk, with or without arthrosis--surgical management by simple discectomy or associated with spondylodesis?].

58 patients underwent surgery by an anterior approach for nerve root or spinal cord compression at one or more levels. The cause was a soft disc herniation, an osteophyte or the two combined. 44 underwent simple discectomy with 70.5% good or very good results. 14 also had interbody fusion by the method of Smith and Robinson, with good and very good results in 57.1%. In most cases the latter group had major cervical spondylosis and myelopathy. The bad results were due to severe neurologic lesions (myelopathy), duration of symptoms and associated illness. The anterior approach with use of the microscope provides good vision and involves few complications. Neither recurrence nor secondary posture disorders were observed.

[Transplantation of the anterior tibial tubercle by the Elmslie-Trillat technic. Indications as a function of morphotype].

The authors note that medial transplantation of the tibial tubercle diminishes the control of lateral rotation of the tibia by the quadriceps and increases stresses on the knee in varus. They have reviewed a hundred knees operated on between 1973 and 1981. Before operation, the morphology of the patients compared with a normal population was characterised by a moderate medial torsion of the femur, a marked lateral torsion of the tibia and some genu varum. After the transplant the varus was identical but in about half of the cases, lateral laxity was noted. Passive rotation of the tibia was increased and this sign was even greater in cases of marked tibial torsion. The authors conclude that this procedure is not indicated in cases of genu varum, excessive lateral tibial torsion and lateral laxity. The best indications are cases of recurrent dislocation of the patella in knees with normal rotary alignment and painful patello-femoral syndromes. They emphasise the importance of an extensive release of the lateral patellar retinaculum, of distal transplantation of the tubercle in cases of patella alta but not too much and of some degree of anterior displacement of the tubercle.
