Peritoneal dissemination after a curative gastrectomy in patients with undifferentiated adenocarcinoma of the stomach.

Peritoneal dissemination is the major pattern of recurrence in patients with gastric cancer. Among the 1,337 patients with gastric cancer who underwent a curative resection, a multivariate analysis showed that the independent risk factors for the occurrence of peritoneal dissemination were an undifferentiated tissue type and serosal invasion. We focus our attention on 477 patients with undifferentiated type tumor, in which the postoperative prognosis and the pattern of recurrence after curative resection were evaluated with regard to the depth of the cancerous invasion in the gastric wall. Patients with tumors limiting to the subserosal layer had a satisfactory post-operative course, with five-year survival rates of 98.5%, 97.4%, and 82.5% in patients with tumors limiting to the submucosal layer, the muscularis propria and subserosal layer, respectively. In 179 patients with tumors invading to the subserosal layer, however, the five year survival rate was only 48.2%. Seventy-one died of peritoneal dissemination, while 24 and 15 were due to hematogenous and local recurrences, respectively. Only 50 (27.9%) survived over 5 years without any recurrence.

Prognosis of patients with gastric cancer and pyloric stenosis: histological differentiation.

The clinicopathology of gastric cancer with pyloric stenosis was examined with special reference to histological differentiation. One hundred sixteen patients treated in Department of Surgery II, Kyushu University Hospital, were classified into differentiated (DT: n = 47) and undifferentiated types (UT: n = 69). The UT group was significantly younger than the DT group (P < 0.05). There were no differences in the incidence of serosal invasion, liver metastasis, lymph node metastasis, and the clinical stage between the groups. The incidence of peritoneal dissemination of the UT group was 41% (28/69), a value significantly higher than the 23% (11/47) for the DT group (P < 0.05). The 5-year survival rates of the DT and UT groups were 27% and 16%, respectively, with survival time for the UT group being significantly less than that of the DT group (P < 0.05). In patients with pyloric stenosis and an undifferentiated adenocarcinoma with the risk of peritoneal dissemination, intensive adjuvant chemotherapy seems essential to improve the prognosis.

Continuous hyperthermic peritoneal perfusion with cisplatin in mice with peritoneal dissemination of B16 melanoma.

We designed a model of peritoneal dissemination of gastric cancer in mice. B16 melanoma cells were injected into the peritoneal cavity of C57 BL mice, the object being to search for an effective treatment for peritoneal dissemination, using hyperthermia combined with cisplatin. Following the intraperitoneal administration of tumor cells, each mouse was subjected to continuous hyperthermic peritoneal perfusion (CHPP) using the peritoneal cavity expander (PCE), with or without cisplatin in the perfusate under conditions of laparotomy. Effects of the therapy were evaluated, based on comparison of survival time between the control and treated groups. There was no prolongation of mean survival periods for the groups given CHPP, with or without cisplatin, compared to findings in the no treatment control group. Optimal timing and doses of hyperthermia and cisplatin, or adding biochemical modulators to enhance the effects of cisplatin, will need to be determined to increase the efficacy of CHPP, with or without the concomitant administration of cisplatin.

Analysis of 390 patients surviving 10 years or longer after curative resection for gastric cancer.

We analyzed clinicopathological features and prognosis of 390 Japanese patients with gastric cancer who survived 10 years or longer after curative resection. All of the patients were treated in our clinics. One hundred and ninety-one of these patients had early gastric cancer and the other 199 had advanced gastric cancer. The mean age was 53.5 years and the male to female ratio was 2:1. The upper one-third of the stomach was less commonly involved and a partial gastrectomy was most often done. Lymph node metastasis was present in 28.7% and extensive lymph node dissection (R2 or R3) was done in 92.5% of the cases. Nine patients died with a recurrence of gastric cancer and 20 patients died with a malignancy in other organs. Curative resection with extensive lymph node dissection and, when required, combined resection of the neighboring organs does improve the survival rate for patients with either early or advanced gastric cancer. An annual follow-up examination to rule out recurrences or malignancies in other organs is to be recommended.

Effect of altered duration of 41.5 degrees C whole body hyperthermia in combination with cis-diamminedichloroplatinum (II) on tumor and normal tissue apoptosis and tumor response in rats.

We investigated the correlation between antitumor efficacy and kinetics of tumor and normal tissue apoptosis when cis-diamminedichloroplatinum (II) (CDDP) was combined with two different durations of whole body hyperthermia [SH-WBH, at 41.5 degrees C for 1 h (1 h WBH) or 2 h (2 h WBH)]. Rats bearing a mammary adenocarcinoma (MTLn3) were treated with 1 or 2 h WBH CDDP and then assessed for tumor growth delay (TGD). A separate study examined the amount of induced apoptosis in tumor and normal tissue (thymus and ileum) over 96 h following the same treatments. 1 h WBH + CDDP increased the TGD to 10.5+/-0.5 days, which was not statistically different from the TGD of 12.3+/-0.5 days obtained with 2 h WBH + CDDP. The area under the curve (AUC) of percentage tumor apoptosis for 1 h WBH + CDDP was 50% of that of 2 h WBH + CDDP. The AUC of percentage thymus apoptosis for 1 h WBH + CDDP was 25% of that of 2 h WBH + CDDP, and the AUC of the score of ileal apoptosis for 1 h WBH + CDDP was 81% of that of 2 h WBH + CDDP. These data indicate that while 1 h WBH + CDDP induced less tumor apoptosis than 2 h WBH + CDDP, antitumor activity was enhanced to a similar degree by both 1 h and 2 h WBH + CDDP, and since 1 h WBH + CDDP caused less normal tissue apoptosis than 2 h WBH + CDDP, a 1 h duration of WBH + CDDP may be a therapy that is both, as effective as, and safer than a 2 h duration of WBH + CDDP.

Heterogeneity and clinical role of thymidine phosphorylase activity in gastric cancer.

We examined the dThdPase activity in primary gastric cancer and metastatic lymph nodes from human subjects. The dThdPase activities were significantly higher in primary tumors than in the normal gastric wall, particularly in the center of the tumor rather than in the periphery (P<0.01). Tumors with a high dThdPase activity often had venous invasion (P<0.01). The dThdPase activities were significantly higher in metastatic lymph nodes than in the nodes without metastasis (P<0.01). The intratumoral heterogeneity of dThdPase activity was identified and cancer cells with high dThdPase activity may indicate that metastasis will likely occur.

Multicystic cavernous hemangioma of the liver: report of a case including diagnostic imaging and pathologic correlation.

The case of a multicystic hemangioma in the liver of a 78-year-old woman is reported. The patient complained of upper abdominal pain and had been seen at a local hospital a few months prior to this admission. An endoscopic examination of the stomach revealed an active gastric ulcer, and ultrasonography (US) of the upper abdomen also incidentally detected a liver tumor. After treating the gastric ulcer, she was then referred to Ryukyu University Hospital in January 1997. US revealed a 3.5-cm, oval-shaped, echogenic tumor with multiple cystic areas in the right lobe of the liver. A CT scan demonstrated a hypodense tumor, that was not enhanced on dynamic CT. Angiography showed a hypovascular tumor that appeared to be a multicystic tumor of heterogeneous high intensity on T2-weighted MRI. The tumor measured 3.5 cm x 3.5 cm in size and was multicystic with a fibrous septum and serous fluid. Histologically the tumor was determined to be cavernous hemangioma of the liver. Atypical hemangiomas should be included in the differential diagnosis when hemangiomas show multicystic features.
