Impairment of thermoregulatory cooling behavior by single cortical spreading depression in the rat.

Effects of single waves of unilateral cortical spreading depression (CSD) on skin-cooling operant behavior were studied in the rat with and without unilateral lesion in the preoptic and anterior hypothalamus (PO/AH). The skin-cooling behavior was severely suppressed for 6 min, with a resulting rise in rectal temperature (Tre), when CSD entered the frontal cortex contralateral to the PO/AH lesion. However, CSD ipsilateral to the PO/AH lesion slightly suppressed the skin-cooling behavior for 4 min with small rise in Tre. In the PO/AH intact rat, while 0.9% NaCl did not affect the skin-cooling behavior, unilateral CSD slightly suppressed it for 3 min almost in the same way as that of the ipsilateral CSD trial in the PO/AH lesioned rat. The results add further evidence to support the involvement of the frontal cortex in the central control of thermoregulation.

Hippocampal input to preoptic thermosensitive neurons in the rat.

The effects of electrical stimulation of ventral subiculum (VSB) of the hippocampus on the thermosensitive neurons in the preoptic area were studied in urethane-anesthetized rats. VSB stimulation affected thermosensitive neurons more frequently (92.1%, 58 of 63) than thermally insensitive neurons (71.4%, 55 of 77). The majority of the VSB-responsive thermosensitive neurons (33 of 44 warm-units and 11 of 14 cold-units) were initially inhibited following stimulation. The result provides further support for the involvement of hippocampus in the central control of thermoregulation.

Responses of preoptic thermosensitive neurons to mediobasal hypothalamic stimulation.

Single-unit responses of preoptic-anterior hypothalamic (PO/AH) thermosensitive neurons to electrical stimulation of the mediobasal hypothalamus (MBH: ventromedial and arcuate nuclei) have been studied in urethane-anesthetized male rats. About half of thermosensitive neurons (46.4% of 69 warm-units and 50% of 28 cold-units) were initially inhibited following MBH stimulation with latencies up to 51.2 msec, while only 29.7% of 91 thermally insensitive units showed the inhibitory response. Orthodromic excitation of spontaneous activity was observed in 17.6% of the units (13.0% of warm-units, 14.3% of cold-units and 21.9% of insensitive units) with the onset latencies between 1.0 and 41.3 msec. A small proportion of neurons (5.3%) were antidromically activated by MBH stimulation, and the estimated conduction velocities of these neurons ranged from 0.11 to 0.67 m/sec. The results suggest reciprocal connections between PO/AH thermosensitive neurons and MBH, which may play some roles in thermoregulation and, also, in functional interactions between thermoregulation and other vital functions controlled in the hypothalamus.

A review of the trend in facilities for health services in the community.

After the Second World War, the field of medicine has changed remarkably in Japan. A comprehensive health and medical care system has been organized to meet the increasing needs and demands for health and medical care services. Health centers have played an important role in promoting health care activities in the community. The authors describe the development of health centers and other health care facilities in Japan. The authors propose that it is necessary to build a new health facility specifically designed for public health nurse activities, termed a "public health nurse station". The authors also describe the status of the health care facilities in service and the activities of the stations and evaluated them. It is concluded that the stations have brought many changes in the field of health and medical care; moreover that the station should not become a substitute for a health center but should be a facility for public health nurse activities in a community. Health centers should also play important roles for comprehensive medical services in the future.
