Studies on subcellular fractions of human platelets by the lactoperoxidase-iodination technique.

Lactoperoxidase-catalyzed 125I iodination and sodium dodecyl sulphate-polyacrylamide gel electrophoresis have been performed on whole, washed platelets as well as on isolated platelet membranes and granules. Electrophoresis of the whole platelets demonstrated two major radioactive peaks, corresponding to glycopolypeptides of estimated molecular weights of 120 000 and 100 000. A small, but consistent amount of radioactivity was also associated with a 147 000 dalton glycopolypeptide. The membranes showed the same pattern of radioactivity as the whole platelets, whereas only negligible amounts of labeled material was found in the soluble and granule fractions. Practically all the polypeptides were labeled in membranes iodinated after their isolation. A glycopolypeptide of 147 000 molecular weight was observed also in the soluble and the granule fractions, but no radioactivity was associated with these substances. In unreduced form, the granule glycopolypeptide penetrated only slightly into the polyacrylamide gel. Thrombin induced the relase of this granule-located substance from whole platelets, as observed by gel electrophoresis of the supernatant after release reaction (secretion). The granule glycoproteins were only partly exposed on the granule membrane since about 50% of the acid-hydrolyzable sialic acid could be liberated by neuraminidase treatment of isolated granules. In whole, iodinated granules the bulk of the radioactivity was associated with a polypeptide of estimated molecular weight 46 000 (possibly actin). This polypeptide was not seen in the supernatant after removal of the thrombin-degranulated platelets by centrifugation, which indicates that the granule membrane is retained with the platelets during the secretion process.

Predictive value of liver scintiphotography for demonstration of metastases in malignant gastrointestinal disorders.

In a prospective investigation, the predictive value of liver scintiphotography was evaluated in 107 consecutive patients operated on suspicion of intra-abdominal cancer. Scintiphotography was performed by means of a Pho/Gamma HP scintillation camera after intravenous administration of 99mTc sulfur colloid. The final diagnoses were obtained within 10 days after scintiphotography by laparotomy or autopsy. The predictive value of an abnormal scintiphoto was found to be 57%, and the predictive value of a normal scintiphoto was 91%. It is concluded that scintophotography is helpful in the preoperative evaluation of patients with suspected intra-abdominal cancer, especially in order to rule out the presence of liver metastases.

Occurrence of goitre during lithium treatment.

Of 330 patients given lithium for recurrent manic-depressive disorder 12 developed goitre after treatment periods of five months to two years. All the patients remained clinically euthyroid. Pressure symptoms necessitated subtotal thyroidectomy in two patients. In 9 out of 10 patients with goitre, and in two out of seven without goitre study with radioactive iodine showed abnormal findings in iodine metabolism. Discontinuance of lithium led to disappearance of goitres, while thyroid metabolism returned to normal. Thyroxine or desiccated thyroid produced shrinkage of the gland in spite of continued lithium medication.

The influence of acromioclavicular joint morphology on rotator cuff tears.

A detailed anatomic study of 123 shoulders was performed to define the location of the acromioclavicular (AC) joint within the supraspinatus outlet and to determine the correlation of AC joint morphologic characteristics with the presence of full-thickness rotator cuff tears (RCTs). The presence, location, and extent of RCTs were first documented for each shoulder, and 2 anatomic landmarks consisting of the anterolateral acromial corner and the inferior midpoint of the AC joint were identified and their positions digitized in 3 dimensions to calculate outlet space and clearance and AC joint locations. High resolution x-ray films were then obtained to determine the extent of degenerative changes including the location and size of inferior osteophytes and joint space. These specimens were cleaned of all soft tissues, and additional measurements were made. All data were entered into a computerized database and analyzed with respect to age, sex, and the presence of RCT. Comparison of age-matched RCT versus non-RCT groups revealed no significant differences in the position of the AC joint within the supraspinatus outlet. When AC joint morphologic characteristics were compared with those of a nontear, age-matched group, no significant differences with regard to joint space narrowing were found, but significantly larger and greater number of osteophytes were identified on both sides of the joint in the RCT group. In conclusion, the position of the AC joint is fairly constant within the supraspinatus outlet and does not appear to predispose to RCTs. The morphology of the AC joint contribution of the tears seems to be acquired in nature rather than inherent in its location within the outlet.

Effect of insulin pump treatment for one year on renal function and retinal morphology in patients with IDDM.

The effect of strict metabolic control for 1 yr on renal function and retinal morphology was estimated in 24 insulin-dependent diabetic individuals (age 29 +/- 8 yr, diabetes duration 10 +/- 6 yr) with Albustix negative urine and minimal or no background retinopathy before the study. They were randomized to conventional insulin treatment (CIT) or continuous subcutaneous insulin infusion (CSII) with a portable pump. During CSII treatment the metabolic status was significantly improved and glomerular filtration rates (GFR) were found to decline (from 130 +/- 18 to 116 +/- 15 ml/min/1.73 m2, P less than 0.01). The mean value of urinary albumin excretion (UAE) was not statistically significantly reduced (from 15.3 +/- 24.1 to 6.8 +/- 1.5 mg/24 h, P greater than 0.1). In the CIT group both GFR and UAE values were unchanged. In all subjects before treatment we found a positive correlation between the metabolic status, estimated by the HbA1c values, and GFR as well as UAE values, respectively (r = 0.64, P less than 0.01 and r = 0.42, P less than 0.05). We conclude that elevated GFR values can be reduced toward normal level by insulin pump treatment for 1 yr. Retinal morphology was found to deteriorate in four of 12 CIT subjects and in three of 12 CSII subjects. However, the progression was mild and in most individuals only due to a slight increase in the number of microaneurysms. Pump treatment did not induce proliferation and "cotton-wool" exudates were not detectable in any pump subject after 1 yr of treatment. We conclude that CSII treatment for 1 yr was unable to stop the progression of retinopathy in individuals with minimal or no retinopathy on entering the study. On the other hand, pump treatment did not induce cotton-wool exudates or proliferation in these subjects as previously reported in subjects with more severe retinopathy before CSII treatment.
