Development of EEG background activity in children with benign partial epilepsy.

We investigated the developmental features of the EEG-background activity of the 30 patients with benign childhood epilepsy. We found significant spectral differences between the benign childhood epilepsy group (BECCT; 25 cases, CEOP; 5 cases) and the normal control group. Compared with the normal group, the epilepsy group showed a shift of the spectrum to slower frequencies, with an increase in Delta and Theta, and a decrease in Alpha 2. The decrease in the mean frequency (O1) was significant from 8 years old to 10 years old. But in both the following periods; before 8 years old and after 10 years old, the difference was not significant. In the patients with benign childhood epilepsy, the development of the EEG-background activity came to be stagnant from 6 years old or so, and after 10 years old it developed again and approached to that of the normal children. It is suspected that the EEG background activity of children with benign partial epilepsy shows a special development.

[Response to CO2 and autoregulation of cortical cerebral blood flow during isoflurane anesthesia].

Response to CO2 and autoregulation of cortical cerebral blood flow (CBF) during isoflurane anesthesia were studied in 10 patients undergoing neurosurgery. The patients were anesthetized with 0.5 to 1.2% end-tidal isoflurane and 66% nitrous oxide in oxygen. The CBF was measured by thermal diffusion using a flow probe with a Peltier stack. PaCO2 was controlled to produce hypocarbia, normocarbia and hypercarbia by changing tidal volume and respiratory rate. Arterial blood pressure was altered. Hypotension was achieved by intravenous infusion of trimetaphan and hypertension was induced by intravenous administration of metaraminol. During isoflurane anesthesia the response to CO2 of CBF was kept at PaCO2 between 27.8 and 53.9 mmHg. The following relationship was obtained. CBF = 2.54 x PaCO2-53.0, r = 0.59, n = 131 The autoregulation of CBF was evaluated in 7 patients, and in 2 patients, the autoregulation of CBF was abolished.

[A clinical study on occlusal plane in relation with orofacial morphology and stomatognathic function].

Occlusal plane is one of the important and fundamental factors that establish the mandibular position and the form of dental arch and prosthesis. The aim of this study is to investigate the morphological and functional importance of occlusal plane in relation with orofacial morphology and stomatognathic function. Cephalograms were taken and electromyograms of masseter, anterior and posterior temporal muscles were recorded during clenching, tapping and chewing movements in 30 normal young adult subjects. Mandibular movements were simultaneously recorded using Sirognathograph (Siemens, West Germany). A correlation analysis was performed to study the relationship between the occlusal plane and morphological and functional parameters. As a result, the occlusal plane angle showed; 1) close correlations with the mandibular plane angle, Camper's plane angle and palatal plane angle, 2) correlations with the duration of activity of anterior temporal muscles and the sagittal angle of chewing pathway. The results suggest that there is a close relationship between the occlusal plane and orofacial morphology. Furthermore, the morphological parameters closely related to the occlusal plane were found to affect stomatognathic function. It is concluded that the occlusal plane is the important factor which harmonizes morphology and function of the stomatognathic system.

[Percutaneous radiofrequency neurotomy of lumbar medial branch (facet rhizotomy)--a report of 6 cases].

We performed radiofrequency neurotomy of lumbar medial branch as a treatment of facet syndrome. Our six cases had no indication for surgical therapy, such as neuralgia, spinal canal stenosis without neurological deficit except for pain and deformed spines. Facet joint injection with regional anesthetics and steroids could not prevent their recurrent back pain. By facet rhizotomy, they could obtain relief of back pain for several months to a year. We conclude that facet rhizotomy is a more effective method for facet syndrome when a proper indication is present.

[Clinical evaluation of cefuzonam of severe infections in leukemia and related disorders].

Cefuzonam (CZON) which has a broad spectrum on both Gram-negative and Gram-positive bacteria including methicillin-resistant Staphylococcus aureus was evaluated in severe infections associated with hematological disorders. Sixty five patients were treated with CZON. Among them, 56 patients were evaluable for effectiveness. Nine patients were not evaluable because 3 patients were treated with combination of other antibiotics such as ceftizoxime, norfloxacin, ofloxacin, 1 patient was subjected to additional therapy of G-CSF and gamma-globulin, 4 were the patients with other disease than hematologic disorder (3 malignant mesotheliomas, 1 ovarian cancer), and the remaining one was prophylactically treated. Excellent responses were observed in 21 (37.5%) patients, good responses in 11 (19.6%) patients, with an overall efficacy rate of 57.1%. The efficacy rate in septic patients was 80% (4/5), and that in patient whose peripheral granulocytes were continuously below 100/microliters was 60% (3/5). Three patients who suffered from malignant mesothelioma, one patient who suffered from ovarian cancer, one patient who was treated prophylactically were included in the final evaluation of side effects. Side effects were observed in 2 patients (2/61, 3.3%). In a patient of 7 years, mild liver disfunction (GOT/GPT, 46/55) was found in 10 days after CZON treatment was started. In a patient of 65 years, mild appetite loss was identified in 2 days after CZON administration was begun. The liver disfunction was improved soon after the cessation of the treatment. The mild appetite loss disappeared while the treatment was continued. These results showed that CZON was an effective and safe antibiotic for the treatment of severe infections in patients with hematological disorders.

Elevation by muroctasin of the serum level of the complement subcomponent C1q in mice and of its biosynthesis by cultured mouse peritoneal macrophages.

Augmentative effects of muroctasin (N2-[(N-acetylmuramoyl)-L-alanyl-D-isoglutaminyl]-N6-stearoyl-L-lysine, (MDP-Lys(L18] on the production of a subcomponent of the first complement component, C1q, in mice were examined. The serum level of C1q in mice treated subcutaneously with muroctasin was found to be elevated significantly at 24 h (p less than 0.1) and 48 h (p less than 0.01). At 24 h, samples of peritoneal macrophages were derived from mice, and incubated with Dulbecco's Modified Eagle Medium (MEM) at 37 degrees C for 24 h. At 19 h of incubation, the C1q concentration in the supernatant of the culture also increased significantly (p less than 0.05). Moreover, when the peritoneal macrophages from non-treated mice were cultured in MEM containing muroctasin (0.001 microgram/ml), the amount of C1q in the culture supernatants also increased significantly 3 h (p less than 0.01) after cultivation. These results indicate that muroctasin activated C1q generation by macrophages in mice through direct and/or indirect mechanisms.

[Evaluation of tumor marker in ovarian dermoid cyst].

Since ovarian dermoid cyst originates in parthenogenesis of the oocyte, it contains various kinds of tissues. Therefore, some tumor markers are expected to be positive. Clinical features of 174 pathologically confirmed dermoid cysts between Jan., 1985 and Aug., 1989 were reviewed, especially concerning tumor markers. The age of the patients was 11 years to 71 years (Mean 35.0 +/- 0.9 years). The weight of the dermoid cyst was 5g to 2.540g (mean 225.9 +/- 26.3g), while most of them were below 200g. The site of the tumors was similar on both the right and left. CA19-9 showed the highest positive rate of 45.5% (40/80). CA125 was second 12.7% (16/126) in markers with more than 20 examined cases, while the rate for AFP, Ferritin, CEA, LDH and TPA was 2.7% (3/110), 2.6% (1/38), 1.6% (2/128), 0.6% (1/161), 0% (0/24), respectively. SCC, IAP and beta 2-MG were examined in less than 20 cases and the positive rate was 18.8% (3/16), 9.1% (1/11) and 8.3% (1/12), respectively. There was no significant relationship between tumor weight and the CA19-9 concentration. In some cases, the serum CA19-9 concentration was followed up after the operation. The half life of CA19-9 was clinically estimated at 4.9 +/- 1.1 days.

[Quantitative analysis of background activity using ambulatory EEG monitoring system].

We studied a possibility of the quantitative analysis of background activities using ambulatory EEG monitoring system (AEEG). (1) This recorder system had a stable frequency characteristics from 1.0 Hz to 20 Hz, in which range the background activities on EEG were organized, and also had a good lineality of amplitude until 150 microV. Therefore, this system had no hindrance in quantitative analysis of EEG background activities. (2) We selected the method of continuous spectrum array (CSA) to analyse the records using AEEG. The CSAs at awakening, light sleep and deep sleep respectively revealed the characteristic patterns of spectrum. And, in the long-term examination of CSA during sleep, the dynamic alternation of spectrum was well recognized depending on the change of sleep stages. This method was useful to the evaluation of sleep cycle as well as the judgement of consciousness level. The AGGE has many advantages compared with the other long-term EEG systems. Major advantages are as follows: 1) The patient can move freely with this equipment. 2) A whole day's record can be stored on a cassette. 3) The operative technique is easy. The clinical and research application of AEEG may be more expanded to combine with the various computer analysing methods.

Epidemiologic and clinical aspects of feline immunodeficiency virus infection in cats from the continental United States and Canada and possible mode of transmission.

The epidemiologic features of feline immunodeficiency virus (FIV) infection were evaluated in 2,765 cats from the United States and Canada. Of these cats, 2,254 were considered by veterinarians to be at high risk for the infection, and 511 were healthy cats considered to be at low or unknown risk. Of the cats in the high-risk group, 318 (14%) were found to be infected with FIV. The infection rate among low- or unknown-risk cats was 6 of 511 (1.2%). Male cats in the high-risk group were 3 times more likely to be infected than were females, similarly as were cats greater than 6 years old, compared with younger cats; domestic cats, compared with purebred cats; and free-roaming cats, compared with confined cats. Feline immunodeficiency virus and FeLV infections did not appear to be linked with each other; 16% of FeLV-infected cats in the high- and low-risk groups were coinfected with FIV. In contrast, there was a pronounced linkage between FIV and feline syncytium-forming virus (FeSFV) infections. Seventy-four percent of FeSFV-infected cats in the high-risk study group were coinfected with FIV, compared with a 38% FIV infection rate among cats that were not infected with FeSFV. The major clinical manifestations associated with FIV infection in cats that were surveyed included chronic oral cavity infections (56%), chronic upper respiratory tract disease (34%), chronic enteritis (19%), and chronic conjunctivitis (11%). Bacterial infections of the urinary tract (cystitis), skin, and ears were seen in a small proportion of cats.(ABSTRACT TRUNCATED AT 250 WORDS)

Distribution of breakpoint within the breakpoint cluster region (bcr) in chronic myelogenous leukemia with a complex Philadelphia chromosome translocation.

We analyzed, by Southern blot hybridization, the site of breakpoint within the breakpoint cluster region (bcr) in six patients with a complex Philadelphia chromosome (Ph) translocation and in 23 unselected patients with a standard Ph. The breakpoint was found within the 5.8 kb bcr in all 29 patients. When the bcr was subdivided into four parts, fragments I-IV, based on the restriction enzyme sites, among the six patients with a complex Ph, two had a breakpoint at fragment I, three at fragment II, and one at fragment III. This distribution of breakpoints in patients with a complex Ph did not differ significantly from that in patients with a standard Ph. A deletion of an allele within the bcr was found in three patients (50%) with a complex Ph and in three (13%) with a standard Ph. The internal bcr deletion may be more common in patients with a complex Ph.

Acute nonlymphocytic leukemia (M2) with chromosome abnormality trisomy 4 developing eight years after radiation therapy for breast cancer.

We report here the development, 8 years after radiation therapy for breast cancer, of acute nonlymphocytic leukemia (ANLL), type M2 of the FAB classification, in which trisomy 4 was detected as the only chromosomal abnormality. Simultaneous observation of cytologic and cytogenetic features of individual colonies derived from leukemic progenitor (L-CFU) and early progenitor (CFU mix) cultures in this patient revealed that all colonies examined had a normal karyotype, although the clone with trisomy 4 was predominant in the direct bone-marrow culture. These findings suggest that progenitor cells with trisomy 4 were less predominant in colony growth when stimulated by colony-stimulating factors (CSFs) than were stem cells with a normal karyotype.

Ellagitannins as active constituents of medicinal plants.

Isolation and structure determination, accompanied by measurement of various biological activities of each isolated tannin, particularly of ellagitannins, have brought about a marked change in the concept of tannins as active constituents of medicinal plants. Their biological activities should now be discussed on the basis of the structural differences among each tannin, in a way similar to that of the other types of natural organic compounds. The anti-tumor activity exclusively exhibited by several oligomeric ellagitannins, and their anti-HIV activities are examples of such biological activities. The inhibitory activity against lipid peroxidation, which is different in the strength among tannins of various structures, is exhibited in general more strongly by ellagitannins than by the other types of tannins of similar structures. The radical-scavenging activities of tannins as the mechanism of their inhibition, which is regarded to participate in several biological activities of tannins, have been supported by the ESR spectral measurements. Other biological activities, i.e., inhibition of mutagenicity of carcinogens, inhibition of tumor promotion, etc., have been found for tannins including ellagitannins.

Arterialized venous skin flaps for the injured finger.

Seven cases of arterialized venous skin flaps for the treatment of both skin defects in the fingers, and digital arterial defects, are presented. The method involves taking the skin flaps along with a subcutaneous vein from the flexor side of the distal forearm or the dorsal aspect of the foot, and interpositioning the vein in the flap between the missing digital artery, to cover the skin defect as well as to restore blood circulation. The size of the flaps ranged from 1.0 x 1.0 cm to 4.5 x 3.0 cm. Almost complete survival of flaps was achieved in all cases. This method does not sacrifice any portion of the digital artery, and is also an excellent technique for restoring blood circulation and reconstructing skin defects simultaneously.
