Chiasmal apoplexy. Report of four cases.

Four patients with chiasmal syndromes were found to have intrachiasmal hematomas. Computerized tomography scans showed high-density suprasellar masses in all cases. Cryptic vascular anomalies were presumed to be responsible for spontaneous bleeding into the chiasm in three of the cases. In the remaining case, hemorrhage was due to an occult optic glioma. Surgical evacuation of clotted blood resulted in improved visual function in three of the four cases.

Cultured juxtaglomerular cells cause hypertension by secreting angiotensin.

Cultured JGC contain renin, angiotensin I, angiotensin I-converting enzyme, angiotensin II, and, by implication, the entire RAS. JGC, as transplants, appear to secrete angiotensin II/III directly into the bloodstream to cause hypertension when the renal mass is reduced. There are two main phases of the hypertensive state, an angiotensin-dependent developmental phase and a non-angiotensin-dependent maintenance phase. This model may be useful in attempts to evaluate pro-hypertensive actions of angiotensin other than those due to direct systemic vasoconstriction. Certain of these actions appear to be intrarenal and include the stimulation of sodium reabsorption, a decrease in renopapillary blood flow, the stimulation of prostaglandin synthesis, and a constraint on the antihypertensive function of the RIC.

[Tissue concentration of 5-FU following pre-operative administration of FT-207].

Forty patients with gastric cancer and 30 patients with colon cancer were administered FT-207 prior to the operation. The relation between a total dose (4-88g) and tissue concentration of 5-FU was investigated and the following results were obtained. (5-FU concentration was measured with the method of Gas chromatography mas fragmentography) 1) The relation between total doses of FT-207 (x) and concentration of 5-FU in the tissue (y) was demonstrated by the formula: y = 0.00317x + 0.025 (gastric cancer) r = 0.519, p = 0.0001 y = 0.0019x + 0.043 (colon cancer) r = 0.641, p = 0.0001 2) In most of the patients with rectal cancer who received radiation therapy prior to the operation, 5-FU concentration in the tissue was extremely low. 3) 5-FU concentration showed no difference between the normal and metastatic lymph nodes, or among the lymph node groups. Administration methods (oral intake or suppository) had no influence on the concentration of 5-FU. 4) Among the organs, 5-FU concentration was higher in the following orders: liver, normal mucosa, lymph node, tumor, normal serosa. 5) In five autopsy cases, 5-FU concentration of the tissue show a great difference among them, and some cases showed 10 times as high concentration as others in the tissue of every organ.

Injection sclerotherapy of esophageal varices for patients undergoing emergency and elective surgery.

From October 1977 to September 1981, 68 patients with esophageal varices (30 emergency cases of bleeding and 38 elective cases) were treated by injecting 5% ethanolamine oleate into varices, using an esophagofiberscope. Esophageal bleeding was successfully controlled in 29 of 30 patients who had emergency surgery. None of the 38 patients who underwent elective operation had bleeding after treatment. When recurrence occurred 1 or 2 years after treatment, the same procedure was repeated. Pleuritis occurred in one of the patients who had emergency surgery, and bleeding (300 to 400 ml) from the esophagocardial junction occurred in two patients who underwent elective operation. These patients were treatment conservatively.

[Computed tomography in brain metastases (author's transl)].

CT scan of the one hundred and sixteen metastatic brain tumors in 50 patients was reviewed and compared to the previous reports. The most common primary organ was lung (43%) followed by breast (18%), colon (10%) and kidney (6%). Solitary nodule was found in 20 patients (40%). On plain CT scan, tumor nodules were demonstrated as slightly high density in 38 (33%), isodensity in 50 (43%) and slightly low density in 28 (24%). Majority of tumor nodules were present in the corticomedullary junction (82%). Brain edema seen as a peritumoral low density was extensive in comparison with the edema in the glioma of the similar size. On contrast CT scan, ring like enhancement was seen in 41 (35%). Intra-tumoral bleeding was shown in 2 metastatic nodules from choriocarcinoma and in one metastatic nodule from renal cell carcinoma. All 3 cases were proved by surgery or autopsy.

Renin, angiotensins, and angiotensin-converting enzyme in neuroblastoma cells: evidence for intracellular formation of angiotensins.

The mechanism of formation of various peptide hormones in neuronal cells in the brain is not clear. The question of whether brain angiotensin II is formed by an extracellular mechanism as in the peripheral system or by an intracellular mechanism can be answered by using cloned cells in culture. We have screened several neuroblastoma cell lines of rat and mouse origin and found at least three cell lines that contain renin (EC 3.4.99.19), angiotensin-converting enzyme (dipeptidyl carboxypeptidase; peptidyldipeptide hydrolase, EC 3.4.15.1), and angiotensins I and II. This finding was interpreted to indicate that in these cells angiotensin formation takes place by an intracellular mechanism, in contrast to the extracellular mechanism well known to occur in plasma. This study also demonstrates the existence of viable and cloned cell lines that produce renin.

Five-year follow-up study of patients with persistent vegetative state.

One hundred and ten patients with persistent vegetative state, were followed up for a period of five years from 1973 to 1978. Seventy-three per cent of the patients died during the period while about 10% of them recovered partially from the vegetative state. Among the recovered patients, only three became able to communicate, but two of these are not independent for their daily activites. Reactivity and clinical symptoms of 30 survivors were examined, but no great changes occurred during the last two years of the study.

Standard blood pressure curves of schoolboys and schoolgirls in Akita prefecture.

Blood pressure of 251,742 schoolboys and schoolgirls aged 7 to 18 years was measured in the year 1970. Analyzing these data, the authors obtained the percentile values classified by age. The lowest hypertensive systolic blood pressures, which are represented by the 95th percentile, are 123, 126, 128, 128, 129, 134, 138, 144, 149, 152, 155, and 154 mmHg in boys from 7 to 18 years of age, respectively. The corresponding values are 123, 126, 128, 130, 134, 138, 139, 143, 144, 142, 142, 141 mmHg in girls of the same ages. Girls' systolic blood pressure is higher than that of boys between the ages of 10 and 13 years. After the 14th year of age, girls' systolic blood pressure does not increase any further, whereas boy's blood pressure continuously increases until 18 years. The present results differ in the following two points from the study in USA (1977): 1) The uniform rise of blood pressure with age is not observed. 2) Blood pressure, especially systolic pressure, is higher in girls than in boys between the ages of 10 and 13 years, although it is higher in boys thereafter until 18 years.
