Cefminox (CMNX, MT-141) was tried in children with various infection and the following results were obtained. Serum levels and urinary recovery of CMNX were studied in 2 patients aged with 9 and 11 years. After intravenous injection of 20 mg/kg, the mean serum concentrations at 15, 30 minutes, 1, 2, 4, and 6 hours after the administration were 178.7, 122.2, 73.9, 41.0, 14.0 and 5.3 micrograms/ml, respectively. The half-life in serum was 1.36 hours. The average urinary recovery rate of CMNX was 86.5% at 6 hours after the administration. The therapeutic efficacy was excellent in 18, good in 1 and poor in 1 patient, the efficacy rate being 95%. As for the side effects, slight elevation of S-GOT and drug fever were observed in 2 cases.
Penetration of aspoxicillin (ASPC), a new semisynthetic penicillin, to cerebrospinal fluid (CSF) and clinical studies against bacterial infections were carried out and the following results were obtained. The concentration of ASPC in CSF was below 1 microgram/ml at 1 hour after intravenous administration of about 50 mg/kg dose to 2 cases of aseptic meningitis on the acute stage. The concentration of ASPC in CSF was above 10 micrograms/ml at 1 hour after intravenous administration of about 80 mg/kg dose to 3 cases of purulent meningitis on the acute stage, and was above 2 micrograms/ml even on the recovering stage. On each stage, its concentration was more than minimum inhibitory concentration of H. influenzae (less than or equal to 0.05 microgram/ml; at inoculum size of 10(6) cells/ml). Clinical efficacy of ASPC was good in all 3 cases of purulent meningitis, excellent in 3 cases, good in 3 cases and poor in 1 case out of 7 cases of septicemia, good in 2 cases and poor in 1 case out of 3 cases of gastroenteritis, respectively. And clinical efficacy of other diseases were excellent or good, that were 2 cases of tonsillitis, 2 cases of soft tissue abscess, 1 case of purulent lymphadenitis and 1 case of urinary tract infection, respectively. Side effects were mild eosinophilia in only 2 cases out of 22 cases.
In vitro viable cell count studies of sustained release preparations of cefaclor (CCL) conclude that the mixture of nonenteric and enteric coated granules of CCL in the ratio of 4 to 6 is the most appropriate form (4:6 form) for the sustained release preparation of CCL. In order to clinically confirm the above conclusion, comparative double blind clinical studies of 3 mixtures forms (2:8, 4:6 and 6:4 forms) with a regular preparation (CCL form) were conducted in dental infections regarding efficacy, safety, and usefulness of the 4 forms. Evaluable cases for efficacy and usefulness were 364 in total (96 cases for the 2:8 form group, 89 cases for the 4:6 form group, 89 cases for the 6:4 form group, and 90 cases for the CCL form group). Evaluable cases for safety were 404 cases in total (102 for the 2:8 form, 100 for the 4:6 form, 102 for the 6:4 form, and 100 for the CCL form). Daily dose of the 3 forms of sustained release preparations was 375 mg b.i.d. after breakfast and dinner and that of the CCL form 250 mg t.i.d. after breakfast, lunch and dinner. Following are the results of the clinical studies: There were no significant differences among the 4 patient-groups (2:8 form, 4:6 form, 6:4 form, and CCL form) regarding background factors of the patients and findings of their subjective and objective symptoms before the initiation of the administration, and it was therefore confirmed that there were no problems in conducting the comparative double blind clinical studies. Overall clinical effective rate determined by the efficacy evaluation criteria of the Japanese society of oral surgery (JSOS) were 89.5% at day 3 and 94.8% at day 5 in the 2:8 form group, 87.4% at day 3 and 95.5% at day 5 in the 4:6 form group, 86.4% at day 3 and 91.0% at day 5 in the 6:4 form group, and 93.3% at day 3 and 96.7% at day 5 in the CCL form group. The effective rate determined by the physicians who actually treated the patients were 84.4% in the 2:8 form group, 87.6% in the 4:6 form group, 84.1% in the 6:4 form group, and 87.8% in the CCL form group. In both judgments by the efficacy evaluation criteria of JSOS and the physicians, there were no significant differences among the 4 forms regarding overall clinical efficacy.(ABSTRACT TRUNCATED AT 400 WORDS)

A clinical trial of ceftizoxime suppositories (CZX-S) was conducted in children whose chemotherapy was considered to be best performed in this dosage form at the physician's discretion. The subjects were 5 children with infection, consisting of 2 with pneumonia, 1 with tonsillitis, and 2 with UTI. The results were as follows. The clinical response to CZX-S was "markedly effective" in 3 and "effective" in 2, with the 100% effectiveness rate. Neither adverse drug reactions nor abnormal laboratory tests were detected. No unwanted expulsion of the suppository occurred. The serum concentration of CZX 30 minutes after the first insertion ranged from 8.38 to 11.4 micrograms/ml, and the urinary concentration of CZX in the 6-hour urine collections, from 23.6 to 290 micrograms/ml.
In children with asymptomatic proteinuria, a high proportion of low molecular weight (LMW) proteins is an indicator of tubular malfunction. In a routine screening program covering the last 15 years and involving 280,000 children, aged between 3 and 19 years, we have identified 5 boys with LMW proteinuria. In 4 of these, renal biopsy was histologically normal on the first presentation. Follow-up for 4-16 years showed normal growth curves, but further evidence of tubular dysfunction appeared: glycosuria and hypophosphatemia in 2 patients; one of them had also aminoaciduria and rising serum creatinine (greater than 1.2 mg/100 ml). Another patient had only increased serum creatinine. The other two, still less than 13 years old, show so far no other abnormality than persistent LMW proteinuria. It is suggested that early identification of LMW proteinuria may presage gradual development of progressive tubular dysfunction with age and that such patients should be followed up indefinitely.

This study regards the urinary protein of patients with renal diseases. Analysis was done by high-performance ion-exchange chromatography (HPIEC). In patients with steroid responsive nephrotic syndrome, the urinary proteins could be separated into about 10 peaks by HPIEC, whereas in the tubular dysfunction about 15 peaks were obtained. Main peaks obtained by HPIEC were identified by SDS-polyacrylamide gel electrophoresis (SDS-PAGE) and immunochemical methods. HPIEC is an easy, reliable and rapid method for the estimation of glomerular and tubular proteinurias patterns, and it seems to be a good indicator of the selectivity of glomerular proteinuria. So, we recommend HPIEC for routine use in the clinical investigation of proteinuria.
A case of cervico-arterial fibromuscular dysplasia (FMD) associated with an aneurysm of middle cerebral artery was reported in a women aged 54. On December 12, 1983, she was admitted to our hospital, who had been diagnosed as head injury (cerebral concussion) and a cerebral aneurysm. On admission, her general condition was favorable, her conscious was clear and neurological examination was normal. The four vessels cerebral angiograms showed right M1M2 aneurysm and FMD which was characterized as typical changes of form "string of beads" in the extracranial internal carotid and vertebral arteries of both sides. Clipping of aneurysm was performed and favorable post-surgical progress was observed. FMD of the cervico-cephalic arteries has been seen rarely in Japanese, but the detection frequency of FMD from cerebral angiography is 0.3-0.9% and the frequency of complication with aneurysm is high at 13-51% on literature. The histologic similarities between intracranial aneurysms and the aneurysmal dilatations of FMD are the absence of the muscular media and a defective interval elastic lamella. This paper suggested strong relationship between FMD and cerebral aneurysm.

The penetration of aztreonam (AZT), a new synthetic monobactam, into cerebrospinal fluid (CSF) and the clinical studies for bacterial infections were carried out. The following results were obtained. The concentrations of AZT in CSF were less than 0.31 microgram/ml and 0.42 microgram/ml, respectively, at 1 hour after intravenous administration of 34 mg/kg and 71 mg/kg in 2 cases of aseptic meningitis at the acute stage. The concentration of AZT in CSF was 6.9 micrograms/ml at 1 hour after intravenous administration of 100 mg/kg in 1 case of purulent meningitis at the acute stage and was 0.62-0.98 micrograms/ml even at the recovering stage. At each stage, its concentration was more than the minimum inhibitory concentration of E. coli (0.10, less than 0.05 microgram/ml; at inoculum size of 10(8), 10(6) cells/ml). Clinical efficacy of AZT was good in 2 cases of purulent meningitis, excellent in 1 case of septicemia, excellent in 5 cases of urinary tract infection, excellent in 1 case and good in 3 cases out of 4 cases of gastroenteritis, excellent in 4 cases and poor in 2 cases out of 6 cases of pneumonia and bronchitis, excellent in 2 cases and good in 1 case out of 3 cases of tonsillitis. No side effects and no abnormal laboratory findings were observed except 1 case of mild diarrhea out of 21 cases.

A new metabolite was produced from 18-hydroxydeoxycorticosterone by the cytochrome P-450(11) beta linked hydroxylase system purified from bovine adrenocortical mitochondria. It was identified as 18,19-dihydroxydeoxycorticosterone by chemical synthesis on the basis of high-performance liquid chromatography, gas chromatography-mass spectrometry, and proton nuclear magnetic resonance (1H NMR) spectroscopy, and detailed structural analysis of it was performed by 1H NMR spectroscopy. The methylene protons at the C-19 position of the steroid were nonequivalent and coupled with each other, having a coupling constant of 10.6 Hz. These protons had different coupling constants, 6.7 and 3.4 Hz, for the hydroxy proton at the C-19 position. Due to these couplings, the signals of the methylene protons were observed around 3.9 ppm as two double doublets. The methylene protons at the C-21 position were also nonequivalent, having a coupling constant of 11.1 Hz. Coupling constants between these methylene protons and the hydroxy proton at the C-21 position were 8.2 and 4.2 Hz, respectively. These results indicate that both hydroxymethyl groups at the C-19 and C-21 positions do not freely rotate in chloroform solution. The signals of hydroxy protons at the C-19 and C-21 positions were found at 1.25 and 1.87 ppm, respectively, by means of decoupling of the corresponding methylene protons. The hydroxy proton at the C-18 position was found to scarcely couple with any proton. This fact suggests that this hydroxy group is linked to the C-20 position, making a hemiketal bridge between the C-18 and the C-20.
Bilateral Wilms' tumour of the kidneys and the retrouretheral region in a 52-day-old boy is reported. These tumours are of the same histological appearance, similar to that of a malignant rhabdoid tumour of the kidney. We suggest that these tumours have arisen from the mesonephric duct or the mesonephros and Wilms' tumour may have originated from mesonephros or an even earlier developmental stage.
To evaluate the actual level of exposure of nonsmokers to tobacco smoke in their living environments, a convenient personal monitor of nicotine specific for tobacco smoke has been developed. The nicotine personal monitor consists of a sampler tube containing 450 mg of Uniport-S coated with silicon OV-17 and a portable sampling pump with a mechanical counter for obtaining total sampling volume. Using the personal monitor attached to a nonsmoker, ambient nicotine was collected in the sampler tube by drawing environmental air at a constant flow rate for a maximum period of 8 hr. The collected nicotine was desorbed by heating and directly transferred onto a GC column with a carrier gas. The amounts of nicotine inhaled by passive smoking in various living environments were estimated to be in the range of 0.9-40 micrograms/hr. These levels are equivalent to those from the active smoking of about 0.001-0.044 ordinary cigarettes in 1 hr.
