Improved pulmonary venous anastomosis using a single left atrial cuff in one-stage bilateral lung autotransplantation.

A technical modification in pulmonary venous anastomosis is presented for one-stage bilateral lung autotransplantation in dogs. This method nullified or minimized both narrowing and loss of vascular distensibility due to scar formation at anastomotic line which inevitably resulted in functional defects after the operation. In this new procedure, bilateral pulmonary arteries were anastomosed using angioplastic techniques and both right and left pulmonary veins were sutured orthotopically as a single atrial cuff. Of 11 dogs which underwent one-stage bilateral lung replantation, 6 survived after operation. Sites of vascular anastomoses in the long-term survivors showed no narrowing due to scar formation at autopsy after hemodynamic examination. In these animals rapid and continuing infusion of 2000 to 3000 ml of blood or plasma explander was performed within 15 min through an intraatrial catheter. Various hemodynamic values were measured simultaneously. In the dog with bilateral lung transplants pulmonary arterial pressure showed linear increase with an increase in cardiac output when compared with control animal studies. It could be interpreted from these results that some changes occurred in the mechanism of pulmonary circulation of the transplant.

[Prediction of labor onset and difficulty by the multivariate analysis (author's transl)].

Based on the multivariate analysis, we analysed the weekly change of the antepartum cervical ripe (consistency, effacement, dilatation, position) and the station, then predict the labor onset and difficulty retrospectively. 1) According to longterm observation of the internal examination of 210 primipara, in third trimester, the ripe type increased, and the unripe type decreased are found. Because of the intermediate type increased and unmovable in incidence relatively, so based on the former scoring method of mere addition of scores, it appears ineffective to predict the labor onset. 2) We studied primipara 385 and multipara 434 from 6 weeks before labor. Based on the multiple regression analysis, we could predict the labor onset. The predict value is higher in multipara than primipara, on the primipara, 78.2% antepartum 6 weeks, 82.2% 4 weeks, 92.6% 2 weeks, and on the multipara 84.6%, 82.2%, 96.2% respectively. 3) Based on the method of Mahalanobis' generalized distance, we could predict the labor difficulty in the primipara and multipara. Accuracy of abnormal prediction are 77.1% at 4 weeks formula, 77.2% at 2 weeks formula on primipara; 69.0%, 69.2% respectively on multipara. Accuracy of normal prediction 53.3%, 56.9% on primipara; 30.9% (4 weeks formula) on multipara are found. The abnormal prediction is better than the normal prediction. This method can early predict the labor onset and difficulty than the former scoring method of mere additional scores.

Granulocytosis and colony-stimulating activity (CSA) produced by a human squamous cell carcinoma.

A patient with a squamous cell carcinoma accompanied by a marked granulocytosis (100,000/mm3) of unknown origin was examined for Colony-Stimulating Activity (CSA). The pleural fluid and the tumor extract revealed high CSA. The floating cells in the pleural fluid were successfully transplanted into nude mice as a localized tumor with cyst formation. The tumor invariably caused a marked granulocytosis (100,000--300,000/mm3) with induction of a conspicuous splenic granulopoiesis in the transplanted mice. High CSA were demonstrated in their cystic fluid as well. Media conditioned by the primary cultures of these tumor cells revealed the same CSA, demonstrating the direct production of CSA by the tumor itself. These results indicate the presence of human CSA producing tumor and that such a tumor may in part account for a marked granulocytosis of unknown origin observed in some patients with cancer.

Comparison of some properties of polyether and polysulfide materials.

Based on the present measurements on working time, setting time, and compression set of polyether and polysulfide materials, the following observations were made: 1. The manufacturer recommends a working time for polyether that is approximately 40 seconds more than that indicated by the clinical or ADA test. 2. Clinically, if more working time is desired, polysulfide impression material should be used. 3. Polysulfide material is more sensitive to temperature change than the polyether material. 4. If the material is allowed to set longer, the percentage change in compression set decreases. 5. For polyether, an increase in setting time of 2 minutes will significantly reduce the distortion of the impression upon removal.

A study of high copper amalgams. IV. Formation of eta Cu-Sn (Cu6Sn5) crystals in a high copper dispersant amalgam matrix.

In an HCD amalgam, eta Cu-Sn crystals were found dispersed within gamma1 matrix areas. Previously, eta Cu-Sn phase was thought to form only as part of a reaction zone surrounding Ag-Cu dispersant particles. The eta Cu-Sn crystals found in matrix areas of this HCD amalgam are smaller and more widely scattered than eta Cu-Sn crystals dispersed in the gamma1 matrix of HCSC amalgams.

[The range of joint motions of the extremities in healthy Japanese people--the difference according to the age (author's transl)].

The passive range of motion was measured in the shoulder, elbow, forearm, wrist, hip, knee and ankle joints in 624 healthy Japanese persons ranging from the neonatal to 80 years of age, by the method formally decided by the Japanese Orthopaedic Association and the Japanese Association of Rehabilitation Medicine in 1974, followed by a statistical analysis. 1. The joint motions, the range of motion value of which being remarkably different by age, were found among the movements in external rotation and horizontal abduction of the shoulder, extension, abduction, adduction, external rotation, internal rotation of the hip and dorsiflexion of the ankle. 2. Elbow flexion, forearm pronation and supination, wrist dorsiflexion and knee flexion showed little change by age. 3. Extension, external rotation and horizontal abduction of the shoulder, dorsiflexion of the wrist, flexion of the hip, flexion of the knee and dorsiflexion and plantar flexion of the ankle gave values exceeding the normal range indicated by the associations. 4. The fluctuation in the range of joint motion was especially wide in the infant and the aged person.

Immune elimination of host fibroblasts for the cultivation of human tumors transplanted into nude mice.

We report here a useful method for the isolation and cultivation of human tumor cells in vitro from human tumors grown in nude mice. A rabbit was immunized with spleen cells obtained from adult nude mice. The rabbit antiserum in the presence of complement effectively killed cultured cells derived from various mouse tissues, but it was not cytotoxic to cultured cells from human tissues including tumors. When mixed cultures consisting of human tumor cells and nude mouse fibroblasts were treated with the antiserum and complement, the nude mouse fibroblasts were completely removed from the cultures, and the human tumor cells could be propagated without noticeable changes in morphological features. Primary cultures of heterotransplanted human tumors grown in nude mice were also successfully treated, resulting in the ultimate elimination of fibroblastic cells derived from the stroma of the tumor. The functional properties of the tumor cells (production of human chorionic gonadotropin by choriocarcinoma cells and production of carcinoembryonic antigens by pancreas carcinoma cells) were also maintained after the antiserum treatment.

[Clinical effects of NK 631, a new bleomycin derivative, in treatment of oral cancer (author's transl)].

We used only NK 631, a new bleomycin derivative, for 10 cases of primary oral cancer, and obtained following results. (1) Anti-cancer effects were immediate and as follow: remarkably good in 1 case, efficacious in 8 cases, and none in 1 case. (2) In clinical examination, peripheral blood, function of kidney, liver, etc. were normal. But attention must be payed to blood gas. (3) Loss of hair, stomatitis and exanthema were noticed as side effects more clearly than regular bleomycin, but no fever. As the result of the above, NK 631 is better than regular bleomycin in anti-cancer effect and activity, but more attention should be payed to the side effect.

Corrosion behavior of structural phases in high copper dental amalgam.

The effects of crevice corrosion on the major structural phases of a high copper dental amalgam were studied in vitro. Three kinds of samples were used: (1) amalgam prepared from a high copper, single composition alloy, (2) annealed ingots of the Ag3Sn and Cu3Sn phases, and (3) high copper, single composition alloy tablets electroplated with mercury. Specimens were examined in a scanning electron microscope before and after the exposure to a corrosion environment. The Ag3Sn and Cu3Sn phases were least affected by corrosion. The Ag-Hg-Sn phase (gamma1) slowly deteriorated, but the attack was nonuniform. The Cu6Sn5 phase exhibited good corrosion resistance, better than that of gamma1 phase. Particles of a tin rich phase of uncertain origin were observed on some of the samples after long exposures.

Hand orthosis for various finger impairments--the K U finger splint.

The K U (Kumamoto University) finger splint has been manufactured for the purpose of developing an effective hand orthosis for various finger impairments using simple parts and materials. By combining the W type orthosis for which piano wire is mostly used, and the S type for which fishing line is mainly used, it has been used to good effect on patients with several kinds of finger impairments. The fabrication and adjustment of the orthosis is simple and if the material is in kit form, a doctor or an occupational therapist can at once construct one and give it to a patient. For this reason, the patient will not lose the chance of timely treatment.

Establishment and characterization of a human cancer cell line that produces human colony-stimulating factor.

A human colony-stimulating factor (CSF) producing cell line, T3M-1, has been established from explant cultures of a human squamous cell carcinoma of the oral cavity that secretes human CSF. It has been continously propagated during the past 15 months. The cells grew in a monolayered sheet with about 17 hr of population-doubling time and showed a colony-forming capacity with about 5% plating efficiency. The cells exhibited an epithelioid morphology resembling the structure of the original tumor, and they showed "tumor takes" when inoculated into nude mice. Karyotypic analysis revealed the cell line to be a human aneuploid one with a hypotriploid mode, including the Y-chromosome(s) and at least 10 common markers. T3M-1 cells possess the characteristic function of human CSF production in vitro, and a marked neutrophilia was observed in nude mice bearing the tumors produced by inoculation with the T3M-1 represents a new human cell line that secretes human CSF.
