[De novo malignancy following renal transplantation].

The characteristics and incidence of de novo malignancy was analyzed in 376 renal transplant recipients transplanted between April 1970 and December 1992. Malignancies developed in 21 recipients of living related donor renal allografts. The total number of malignancies was 23, with 2 patients developing 2 malignancies. The mean age at the time of diagnosis was 42 years. The average interval from transplantation to the time of diagnosis was 129 months. Malignancy developed in 12 of 21 patients who survived 10 years with a functioning graft. Nine patients died of their malignancy. The risk of developing malignancy is increased following renal transplantation. Compared with sex- and age-matched Japanese control, the incidence is 6.1 times greater for male, 10.5 times greater for female, and 7.3 times greater overall. The risk was increased at all sites except the stomach. The risk rate of acquiring malignancies at 5-year intervals following transplantation was 6.5 times greater from 0 to 5 years, 10.0 times greater from 5 to 10 years, and 9.3 times greater from 10 to 15 years. These results suggest that annual medical examinations should be performed as part of the routine log-term follow-up of renal transplant recipients to detect malignancies in the early stage.

[Expression of cytokine messenger RNA in human spleen].

The expression of the cytokine genes in human spleen was studied using reverse transcriptase-polymerase chain reaction (RT-PCR) method capable of detecting low levels of mRNA. Total RNA was prepared from human spleen by acid guanidinium thiocyanate-phenol-chloroform (AGPC) method. cDNA was synthesized by M-MLV RTase using oligo (dT)16 primer, and amplified using the oligonucleotide primers specific for IL-1 alpha, IL-1 beta, IL-2, IL-3, IL-4, IL-5, IL-6, IL-7, IL-8, IL-9, TNF-alpha, IFN-alpha, IFN-beta and IFN-gamma by PCR method. Although IL-1 beta, IL-4, IL-5, IL-6, IL-7, IL-8, TNF-alpha, IFN-alpha and IFN-gamma mRNA were detected in all the samples tested, IL-3 and IFN-beta mRNA was not detected at all. These results suggest that many kinds of cytokines may be produced constitutionally in human spleen, and its pattern of cytokine production was similar to that in mice.

[Myocardial preservation by continuous perfusion of Krebs-Henseleit solution--the temperature dependency of the optimal perfusion pressure].

The aim of this study was to determine the temperature dependency of the optimal pressure in myocardial preservation by continuous perfusion with Krebs-Henseleit bicarbonate buffer (KHBB) solution. Hearts from Wistar male rats were perfused with KHBB solution and cardiac function (aortic flow) was measured using an isolated working rat heart preparation. In the preliminary experiment, hearts were then preserved using Langendorff perfusion with KHBB solution of 37, 20 or 4 degrees C for 2 hours at a perfusion pressure of 100 cmH2O. This was followed by 15 min of Langendorff perfusion (37 degrees C, 100 cmH2O) and 20 min working perfusion. The 37 degrees C group and 20 degrees C group exhibited better functional recoveries of aortic flow (%AF) in the post-preservation period compared to the 4 degrees C group. In the test experiment, hearts were preserved using Langendorff perfusion for 4 hours at 37 degrees C or for 8 hours at 20 degrees C at various perfusion pressures. At 37 degrees C, %AF after 4 hours of the preservation were 64.7 +/- 2.6, 69.0 +/- 3.2, 81.9 +/- 3.1, 94.7 +/- 3.3 and 63.5 +/- 4.0% (p < 0.05 vs the 100 cmH2O group) at the perfusion pressure of 100, 60, 20, 15 and 10 cmH2O, respectively. %AF after 8 hours of the preservation at 15 cmH2O was 56.3 +/- 2.5%. At 20 degrees C, %AF after 8 hours of the preservatin was 78.9 +/- 3.3, 81.9 +/- 2.3, 67.4 +/- 1.9 and 65.9 +/- 2.2% (p < 0.05 vs the 60 cmH2O group) at the perfusion pressure of 60, 30, 10 and 15 cmH2O, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

A lysosomal arylamidase is a member of a larger family of peptidases in lysosomes.

One form (form VI) of lysosomal arylamidase, having a molecular weight of 135,000 and an isoelectric point of 7.8, was purified from rat liver to apparent electrophoretic homogeneity. The enzyme consisted of three identical polypeptides with a molecular weight of 45,000, as judged by disk electrophoresis in the presence of sodium dodecyl sulfate (SDS). The enzyme hydrolyzed di- and tripeptides. The enzyme released a N-terminal amino acid from Leu5-enkephalin and Met-Lys-bradykinin; these have a tyrosine and a methionine at the N-terminus, respectively. The enzyme did not hydrolyze bradykinin, angiotensin and melanocyte-stimulating hormone release-inhibitory factor (MIF); these have a basic amino acid, an acidic amino acid, and a proline at the N-terminus, respectively.

The effect of heat shock on immediate post-preservation lung function.

Exposure of animals to transient hyperthermia causes the induction of highly conserved proteins, heat shock proteins (HSPs), which are thought to by cytoprotective against a variety of injuries, including ischemia. We tested the hypothesis that heating donor animals prior to harvest would improve pulmonary preservation. Anaesthetized New Zealand White rabbits underwent radiant heating to 42.5-43.5 degrees C (rectal) 8 h prior to harvest of the lungs. The lungs were harvested without flush and stored for 18 h at 4 degrees C. The left lung were perfused ex vivo with fresh blood for 10 min. Blood gases, pulmonary artery (Ppa) and airway (P(aw) pressures, and wet/dry ratios (W/D) were measured. Control animals were treated identically except without heating. All heated animals had HSP72 at lung harvest and 18 h later, whereas no control had detectable levels of HSP72 at either time. In Experiment 1 (n = 12, VT 20 ml, F1O2 0.21, 30 bpm, PEEP 0.5 cm H2O), PO2 in the heated group was 57.6 +/- 7.3 mmHg (mean +/- SEM) vs. 51.6 +/- 5.7 in the controls (NS). In Experiment 2 (n = 8, VT 15 ml, F1O2 0.21, 35 bpm, PEEP 2 cm H2O), PO2 of the heated group was 63.5 +/- 6.5 vs. 83.1 +/- 9.5 in the controls (NS). Ppa after 10 min was not significantly different in the heated group in Experiment 1 (16.7 +/- 0.9 mmHg vs. 24.2 +/- 3.7 in controls) or in Experiment 2 (19.5 +/- 1.8 vs. 11.3 +/- 2.9 in controls). Wet/dry ratios were not different in either Experiment 1 (6.4 +/- 0.4 vs. 5.8 +/- 0.2 in controls) or Experiment 2 (5.0 +/- 0.2 vs. 5.0 +/- 0.5).(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of primary hyperparathyroidism associated with marked hypercalcemic crisis].

A 41-year-old woman was referred to our hospital with complaints of general fatigue, appetite loss, nausea and vomiting. Blood examination revealed high serum calcium level (21.6 mg/dl) and high serum parathyroid hormone level. Although enlarged parathyroid glands could not be clearly detected via ultrasonography, computed tomography and scintigraphy, we diagnosed her with hypercalcemic crisis due to primary hyperparathyroidism and performed parathyroidectomy after conservative therapy. A parathyroid tumor measuring 1.9 x 1.1 x 1.0 cm, and weighing 1,100 mg was found at the upper right pole of the thyroid gland, and three thyroid tumors were found in the bilateral lobes of the thyroid gland. Histological diagnosis was adenoma of the parathyroid gland and follicular adenomas of the thyroid gland. Following removal of the parathyroid tumor, the serum calcium level rapidly dropped and the symptoms rapidly improved. Hypercalcemic crisis has a high mortality, and the only treatment is surgical removal. Therefore, in patients with hypercalcemic crisis due to primary hyperparathyroidism, surgical removal should be done immediately, unless the serum calcium level has dropped and symptoms of crisis have disappeared after conservative treatment.

[Renal tubular dysfunction in open pediatric heart surgery--preventive effect of haptoglobin].

Preventive effect of haptoglobin (Hb) for renal tubular dysfunction was studied in 16 pediatric patients undergoing open heart surgery under extracorporeal circulation (ECC). The patients were divided into two groups; Hp(+) group consisting of 7 patients administered haptoglobin (100 U/kg) in priming of ECC, and Hp(-) group of 9 patients not administered haptoglobin. As parameters of renal tubular function, urinary alpha 1-microglobulin-index (U-alpha 1m.I), urinary beta 2-microglobulin-index (U-beta 2m.I), urinary N-acetyl-beta-D-glucosaminidase-index (U-NAG.I), fractional clearance of alpha 1m (Fc-alpha 1m) and beta 2m (Fc-beta 2m), as well as Free-Hb, were measured just before start of ECC and then at periodical time after ECC to the 7th day. The results obtained are as follows. (1) Maximum Free-Hb in Hp(+) group was significantly less (p < 0.05) than that in Hp(-) group. (2) Every parameter of renal tubular function were shown a peak at the end of ECC. Maximum values of U-alpha 1m.I, U-NAG.I and Fc-alpha 1m in Hp(+) group were significantly less (p < 0.05) than those in Hp(-) group. Maximum U-beta 2m.I and Fc-beta 2m in Hp(+) group were less than those in Hp(-) group, although there was no significant difference. These results indicate that administration of haptoglobin suppresses the increase of Free-Hb and is useful for prevention of renal tubular dysfunction during ECC.

[Left ventricular-right atrial shunt with bacterial and rheumatic endocarditis of tricuspid valve].

A 14-year-old boy who was operated on for left ventricular-right atrial shunt with bacterial and rheumatic endocarditis of the tricuspid valve (TV) is reported. The patient was pointed out perimembranous ventricular septal defect (VSD) at 4-month old and had complaint of occasional high fever for a half year. The echocardiography showed a vegetation at the TV. The vegetation did not disappear despite of administration of antibiotics for 2 weeks, and pulmonary embolism occurred. Then, the patient underwent resection of the vegetation and direct closure of VSD. The left ventricular-right atrial shunt was formed by the direct communication between VSD below TV annulus and the perforation of septal leaflet of TV adhered to the ventricular septum due to bacterial and rheumatic endocarditis. Earlier operation is needed for this type of case because antibiotics is not effective for the valvular disease.

[An autopsied case of pleuro-pulmonary cryptococcosis].

We report a case of pleuro-pulmonary cryptococcosis. An 86-year-old man was admitted to the hospital with dyspnea, a high fever and an oppressive sensation in the chest. Chest X-ray on admission revealed massive pleural effusions, interstitial shadows and consolidation with air bronchograms in both lung fields. The cause of pleural effusion was unknown. The patient died one month later. Autopsy revealed severe pleural cryptococcosis and organizing pneumonia with a small number of cryptococci but without granuloma. To our knowledge, there have been very few documented cases of pleuro-pulmonary cryptococcosis with such massive bilateral pleural effusions.

[Determination of serotonin in whole blood and platelet rich plasma by high-performance liquid chromatography with fluorometric detection].

An improved high-performance liquid chromatographic method with fluorometric detection (Ex 285nm, Em 345nm) for the assay of serotonin in whole blood and platelet rich plasma was developed for the routine clinical examination. Clinical sample was successfully deproteinized with perchloric acid in the presence of a reducing agent (ascorbic acid). Precision (CV < 5.0%), accuracy (recovery > 95%), lower assay limit (20ng/ml) and assay time (15min/sample) were allowable for the routine clinical examination. The level of serotonin (ng/ml, mean +/- SD) in whole blood and platelet rich plasma from normal volunteers (n = 53) was 143 +/- 47 and 180 +/- 60, respectively.

[Leiomyoma of the kidney: a case report].

A case of leiomyoma of the kidney is reported. A 57-year-old man with the chief complaint of right back pain was consulted to our hospital. Abdominal computed tomography (CT) demonstrated a right renal tumor. Right renal angiography revealed a hypovascular lesion. Under the diagnosis of right renal tumor, right radical nephrectomy was performed. Gross evaluation revealed a well encapsulated solid mass. The tumor was 48 x 43 x 42 mm in diameter and a hemorrhagic region was present. Microscopically, the tumor was composed of interlacing bundles of spindle cells. Diagnosis was leiomyoma of the kidney. We reviewed 42 cases of renal leiomyoma reported in Japanese literature.

[Development of coding system for respiratory diseases and its application to classifying cases reported in the Japanese Journal of Thoracic Diseases].

We developed a coding system for respiratory diseases and related conditions for computer-aided analyses of these diseases. It is a one-axis alphanumeric system with a list of three-character categories and additional digits separated by a decimal point for subcategories. Respiratory and related diseases are categorized into eight groups; diseases of (1) the lung and airway, (2) the pleura, (3) the mediastinum, (4) the diaphragm, (5) the chest wall, (6) the heart and cardiovascular system, (7) the esophagus, and (8) clinical conditions predisposing to pulmonary disease. The first character, a letter from the A to G, represents a major disease category of (1) to (7) and X that of (8); and subdivisions are shown in the second position using the numbers from 0 to 9. Further subdivisions are provided in the third position also using numbers. Actual coding was done on case reports which appeared in the Japanese Journal of Thoracic Diseases from 1973 to 1993, and all cases were coded properly. The system is expected to prove useful in computer aided analyses of respiratory diseases.

[AML1 gene rearrangements, but no 8;21 translocation in a child with acute myeloblastic leukemia (M2)].

A 3-year-old girl developed fever and bilateral exophthalmos. Her initial peripheral blood counts on admission were; Hb8.7 g/dl, Plt. 10.6 x 10(4)/microliters, and WBC 30,890/microliters with 31% peroxidase-positive blasts, some of which contained Auer rods. The bone marrow smears showed 22% blasts with a certain degree of maturation. She was diagnosed as having M2 type AML. Computed tomography revealed bilateral retoro-orbital tumor. The bone marrow karyotype showed no 8;21 translocation but a loss of one sex chromosome. Molecular analysis of the bone marrow cells disclosed rearrangements of the AML1 gene. Chimeric mRNA coded by the AML1-MTG8 gene was also detected by the RT-PCR method. We concluded that this patient had a masked 8;21 translocation in her leukemic cells.
