[Surgical treatment of incomplete endocardial cushion defect in adult patients].

Seven adult patients (> or = 40 years old) and six pediatric patients with incomplete endocardial cushion defect (ECD) underwent corrective surgery in our institution. Preoperative catheterization study showed that the left-to-right shunting rate and the pulmonary-to-systemic flow ratio were greater in adult patients than those in pediatric patients, but systolic pulmonary arterial pressure was not high and similar in the two groups. The degree of mitral regurgitation by left ventriculography was mild or moderate in both groups. However, in adult patients, a variety of arrhythmias were observed. Postoperatively, NYHA grades and arrhythmia improved markedly in the adult patients. Surgical correction for ECD is recommended even in adult patients before aggravation of cardiac failure and arrhythmia.

Stimulus effects of the medial pontine reticular formation and the mesencephalic locomotor region upon medullary reticulospinal neurons in acute decerebrate cats.

In acute decerebrate cats, medial pontine reticular formation (mPRF) and the mesencephalic locomotor region (MLR) were stimulated and their stimulus effects upon 250 medullary reticulospinal neurons (RSNs) were studied. One hundred and twenty-six RSNs were mono- and disynaptically activated. From the response patterns of the RSNs, they were divided into the mPRF-activated RSNs (n = 67) and the MLR-activated RSNs (n = 59). The former group of RSNs was located in the nucleus reticularis gigantocellularis (NRGc), while the latter group of RSNs was distributed in both the NRGc and the nucleus reticularis magnocellularis (NRMc). The activity of MLR-excited 12 RSNs was suppressed with the preceding mPRF stimulation. These RSNs were mainly located in the NRMc. Most mPRF-excited RSNs increased their discharge rates during mPRF-evoked suppression of postural muscle tone, and most MLR-excited RSNs increased their discharge rates during MLR-evoked locomotion. With mPRF stimulation, MLR-evoked locomotion was suppressed with cessation of MLR-excited RSNs activity. These results suggest that mPRF stimulation suppresses the activity of the locomotor rhythm generating system at the levels of not only the spinal cord but also the medullary output cells.

Multienzyme control serum (Seraclear-HE) containing human enzymes from established cell lines and other sources. 1: Preparation and properties.

We have developed a new multienzyme control serum, Seraclear-HE, which was designed to function not only as an accuracy and precision control serum but also as an intermethod calibrator for unifying interlaboratory clinical enzyme data in terms of reference method values. Seraclear-HE contains as analytes the following enzymes of human origin only: aspartate aminotransferase (AST, EC 2.6.1.1) and lactate dehydrogenase (LD, EC 1.1.1.27) from erythrocytes; alanine aminotransferase (ALT, EC 2.6.1.2) from a hepatoma cell line; alkaline phosphatase (ALP, EC 3.1.3.1) from an amnion cell line; creatine kinase (CK, EC 2.7.3.2) from an embryo kidney cell line; gamma-glutamyltransferase (GGT, EC 2.3.2.2) from a macrophage cell line; and amylase (AMY, EC 3.2.1.1) from urine and saliva. The seven partly purified enzymes were lyophilized in partially delipidated human serum containing sucrose (50 g/L), pyridoxal 5'-phosphate (30 mmol/L), and other stabilizers. The material is stable for at least 2 years at temperatures < or = 10 degrees C. For two concentrations of this preparation, reference method values (mainly International Federation of Clinical Chemistry and Japan Society of Clinical Chemistry) obtained at both 30 degrees C and 37 degrees C are assigned.

[The effects of glucose and insulin upon functional recovery in the rat heart preserved by continuous perfusion with St. Thomas' Hospital cardioplegic solution].

UNLABELLED: The aim of this study was to determine the effect of the continuous perfusion with Krebs-Henseleit bicarbonate buffer (KHBB) solution or St. Thomas' Hospital cardioplegic solution (ST solution) containing glucose and insulin upon the post-preservation recovery in the isolated rat heart. METHODS: Hearts from male Wistar rats (n = 6/group) were subjected to working perfusion with KHBB solution for measurement of cardiac function. They were continuously perfused (1) with KHBB solution or ST solution for 12 hours at 20 degrees C, (2) with ST solution for 12 hours at 37 degrees C, 20 degrees C or 4 degrees C, (3) with ST solution or ST solution containing glucose (9 mmol/L) for 12 hours at 20 degrees C or 4 degrees C, and (4) with ST solution containing glucose (9 mmol/L) or ST solution containing glucose (9 mmol/L) and insulin (10 U/L solution) for 20 hours at 20 degrees C. This was followed by 15 min of Langendorff perfusion and 20 min of working perfusion with KHBB solution. During the second working perfusion after preservation, cardiac function was measured again. RESULTS: (1) Percent recoveries of cardiac output (%CO) preserved with KHBB and ST solution were 44.9 +/- 4.0 and 57.7 +/- 3.8%, respectively (p < 0.05). (2) %CO after preservation with ST solution at 37, 20, 4 degrees C were 0*, 57.7 +/- 3.8 and 74.4 +/- 2.2*%, respectively (*p < 0.05 vs the 20 degrees C group). (3) At 20 degrees C, glucose addition to ST solution increased %CO to 76.5 +/- 2.4% from 57.7 +/- 3.8% in glucose-free ST solution group (p < 0.05). However, at 4 degrees C, glucose addition to ST solution was rather harmful and decreased %CO to 61.4 +/- 2.8% from 74.4 +/- 2.2 in glucose-free ST solution group (p < 0.05). (4) The addition of insulin to the glucose-containing ST solution resulted in a significant increase in %CO from 24.6 +/- 4.0% in the insulin-free solution to 69.2 +/- 2.0%. CONCLUSIONS: Our results indicate that 1) ST solution might be better than KHBB solution for a continuous perfusion medium of myocardial preservation, and 2) at 20 degrees C, the addition of glucose and insulin to the ST solution could be beneficial for the continuous infusion as a method of preservation, however, at 4 degrees C, these addition might be harmful, suggesting the temperature dependence in the utility of energy substrate.

[A successful usage of latissimus dorsi muscle flap in the surgery of synchronous gastric and pulmonary double cancer in an elderly patient--a case report].

A 77-year-old man, with both an advanced gastric cancer Borrman IV type (CME) S2N1P0 M0 stage II and lung cancer of left upper lobe pT3N1M0 stage IIIA, simultaneously received total gastrectomy with R2 lymph node dissection, and Roux-Y reconstruction, and left upper lobectomy with R2 lymph node dissection combined with partial pericardiectomy. A latissimus dorsi muscle flap with thoracodorsal artery and vein was used for covering the intrathoracic esophagojejunostomy, repairing of the pericardial defect and separating the thoracic cavity from the abdominal cavity. Postoperative esophagograms showed a leakage, which was localized by the muscle flap coverage without extensive spread. The leakage healed spontaneously after a left sub-diaphragmatic drainage. Our experience described here indicates that a latissimus dorsi muscle flap coverage is a useful operative method for double cancers of the lung and stomach requiring intrathoracic procedures.

[Usefulness of 123I-BMIPP myocardial imaging in patients with stable effort angina and unstable angina].

We evaluated the clinical significance of myocardial imaging using 123I-15-(p-iodophenyl)-3-methyl pentadecanoic acid (BMIPP) scintigraphy in patients with stable effort angina pectoris (SAP) and unstable angina pectoris (UAP). Thirty-three patients with SAP were studied using rest BMIPP and stress 201TlCl (Tl) myocardial scintigraphy, and 13 patients with worsening effort type of UAP were also examined using both rest BMIPP and Tl scintigraphy. We compared those BMIPP findings with myocardial perfusion images obtained with Tl and the regional wall motion determined by left ventriculography. In 45% of 282 segments of myocardial ischemia of SAP, the degree of myocardial uptake of BMIPP was concordant with that of stress Tl and the defect score of Tl was higher than that of BMIPP. On the other hand, in 32% of 62 segments of ischemia of UAP, the degree of myocardial BMIPP and Tl uptake was concordant and BMIPP defect score was higher than Tl score. In SAP, the decrease in regional wall motion agreed better with the decrease in myocardial uptake of BMIPP than that of Tl. These results suggest that myocardial ischemic regions decreased BMIPP uptake show the disturbance of fatty acid metabolism and lead to abnormal wall motions. Such ischemic regions may be clinically severe state in patients with angina pectoris.

[Pulmonary hamartoangiomyomatosis (lymphangiomyomatosis) in tuberous sclerosis--a case in which clinical course had been modified by previous surgical intervention].

A 42-year-old woman was referred to this institution because of persistent air leak associated with left spontaneous pneumothorax. She had undergone a radical hysterectomy with bilateral oophorectomy and left nephrectomy one year previously. The renal tumor was identified as an angiomyolipoma. This, along with the characteristic dermal lesions led to the diagnosis of tuberous sclerosis. Preoperative thin-slice chest CT showed multiple thin-walled, round and cystic air spaces in the pulmonary parenchyma bilaterally, as well as the left pneumothorax. Through a left thoracotomy, suture ligation and laser ablation of the pulmonary bullae was done. Examination of the biopsy specimen of the lung showed hamartoangiomyomatosis with multiple bullae. The proliferating smooth muscle in the walls of the bullae was positively stained by HMB45 monoclonal antibody. The patient recovered uneventfully, and was doing well three years later without medications. The prior surgical castration may have prevented progression of the disease.

[A case of pulmonary blastoma coexisting with well-differentiated adenocarcinoma].

A 66-year-old man came to our hospital because of hemoptysis and fever. Chest radiograph and CT scan showed a large circumscribed shadow in the posterior segment of the right upper lobe. Bronchoscopy revealed bronchial compression and stenosis. The histopathological specimen obtained by transbronchial biopsy showed total necrosis, but cytological examination of the bronchial lavage specimen showed class 5 adenocarcinoma. Right upper lobectomy was done, and regional lymph nodes were removed. The resected tumor was found to be a pulmonary blastoma by histopathological examination. The main tumor was composed of malignant epithelial and mesenchymal elements with rhabdoid differentiation and chondroid tissue, the former of which looked like fetal lung tissue. This case is interesting, because the fact that the adenocarcinoma and pulmonary blastoma were close together may be related to the histogenesis of pulmonary blastoma.

[A case of bilateral testicular metastases from carcinoma of the stomach].

A case of bilateral testicular metastases from carcinoma of the stomach is reported. A 39-year-old man was admitted to our hospital under the diagnosis of carcinoma of the stomach. During the hospitalization, the bilateral scrotal contents had become swollen and painful, but we could not resect this lesion because of his critical condition. Then he died of carcinoma of the stomach. The autopsy revealed that he had bilateral testicular metastases from carcinoma of the stomach. Including our case, 10 cases of testicular metastases from carcinoma of the stomach are reported in the Japanese literature. We mainly discuss the route of metastasis.

Histological change in cryopreserved rat aortic allograft.

In this study, we established an experimental cryopreserved aortic allograft model in rats and examined the long-term histological changes in the allograft. The thoracic aorta of Brown Norway rats (RT1n) was cryopreserved with 10% dimethylsulfoxide using a programmable freezer, and was allo-transplanted to the infrarenal abdominal aorta of Lewis rats (RT1l). Neither immunosuppressants nor anticoagulants were administered postoperatively. As a control, isografting was also performed between Lewis donor and recipient rats. In the allograft groups, cellular infiltration in the adventitia was massive at the acute phase after the operation and decreased gradually. Intimal thickening was predominantly observed from the early stage, followed by advancing thickening. In the media, decrease in cell number was detected after 1 month, and chondrocyte-like-cells were observed around which calcification was noted. Endothelial cells were observed in only one-third of the recipient investigated at 10 days and in over 80% of those at 12 months. In the isograft groups, a low grade of intimal thickening was detected over the experimental period. No decrease in cell number in the media was detected, and the degree of cellular infiltration in the adventitia was mild. After allotransplantation of the cryopreserved rat aorta, intimal thickening, medial necrosis and cellular infiltration in the adventitia, all the manifestations of rejection, occurred. Thus, as the cryopreserved tissue induces an immunological response, it is important to match the blood type and/or histocompatibility in clinical use.

[Solitary rectal carcinoma metastasis to the left hilar lymph nodes: a case report].

A 66-year-old woman was referred to this institution for treatment of hemoptysis, atelectasis of the left upper lobe, and marked hypoxia necessitating oxygen therapy. A low anterior resection of the rectum had been performed for rectal adenocarcinoma 6 years and 3 months before this admission, and was followed by another resection after a local recurrence 20 months later. Bronchoscopy revealed an endobronchial tumor obstructing the left upper lobe bronchus. Tissue from a transbronchial biopsy revealed metastatic rectal carcinoma of the endobronchial lumen. There was no evidence of local recurrence or metastasis to other organs. A left pneumonectomy and lymph node dissection were performed successfully. The postoperative course was uneventful, and the patient was discharged after marked improvement of the arterial blood gas results. The pathological diagnosis of a resected tissue specimen was metastatic adenocarcinoma of the left hilar lymph nodes with invasion of the left main bronchus and protrusion into the endobronchial lumen. The patient remained disease-free for 6 months. At that time, computed tomography of the chest disclosed small metastases in the right lung and chemotherapy was begun.

[Systemic necrotizing vasculitis with pulmonary hemorrhage and gastrointestinal bleeding].

A 61-year-old man was admitted to our hospital with pain and weakness in both lower legs. Eosinophil count was abnormally high (6800/mm3). Chest X-ray film revealed multiple infiltrates in both lung fields. A specimen obtained by transbronchial lung biopsy demonstrated pulmonary hemorrhage. Vasculitis syndrome was suspected and 1 g per day of methylprednisolone was administered for 3 days. Pulmonary infiltrates quickly disappeared, but 2 weeks later massive intestinal hemorrhage occurred. Emergency ilectomy was done, but the patient died of bleeding from other intestinal lesions. Microscopic examination of the resected ileum revealed mucosal ulceration and necrotizing vasculitis in small arteries. This case is important in that vascular occlusion and ischemia progressed soon after vascular inflammation was relieved by administration of corticosteroid hormone.

Xanthoma of the bladder.

We report a case of xanthoma of the bladder incidentally discovered during transurethral ureteral lithotripsy for a right ureteral stone and histopathologically diagnosed by cold cup biopsy. Xanthoma, a disorder of the lipid metabolism without tumorous or inflammatory changes, is rare in the bladder with only 7 cases reported previously. Interestingly, 5 of these 7 cases were reported in the Japanese literature, as were cases of xanthogranulomatous cystitis. We have no explanation for this occurrence. Xanthoma, caused by macrophages with the ability to phagocytose, is a benign lesion. It is necessary to continue careful followup by cystoscopy to determine if xanthoma increases or decreases in size.

[Identification of viable myocardium using 99mTc-tetrofosmin scintigraphy--comparison with 201Tl redistribution-reinjection images].

The purpose of this study was to clarify the diagnostic value of identifying viable myocardium using 99mTc-Tetrofosmin scintigraphy. Twenty-one patients with chronic coronary artery disease were studied using 201Tl exercise myocardial scintigraphy with reinjection and 99mTc-Tetrofosmin exercise myocardial scintigraphy. All patients had a history of old myocardial infarction. For 99mTc-Tetrofosmin scintigraphy, 222 MBq of 99mTc-Tetrofosmin was injected during exercise, and exercise images were obtained 20 min thereafter. There hours later, 666 MBq of 99mTc-Tetrofosmin was injected at rest, and images were obtained 40 min and 220 min later. Myocardial viability in the 99mTc-Tetrofosmin scintigraphy was estimated as fill-in findings (FF) or over 50% of %RI uptake (%TF) in the rest image. Myocardial viability in the 201Tl scintigraphy was estimated as redistribution (RD), fill-in findings in the reinjection image (FR) or over 50% of %RI uptake in the reinjection image (% TL). Sixteen of the 21 patients (76%) who underwent 201Tl scintigraphy (RD 10, FR 3, %TL 3 cases) and 15 of the 21 patients (71%) who underwent 99mTc-Tetrofosmin scintigraphy (FF 11, %TF 4 cases) had viable myocardium in the infarcted area. A comparison between the 99mTc-Tetrofosmin rest images obtained 40 min after the injection and that of 220 min revealed no redistribution findings. The %RI uptake of the infarcted area in the resting 99mTc-Tetrofosmin image (47 +/- 16%) was slightly lower than that in the 201Tl reinjection image (52 +/- 16%). In conclusion, viable myocardium was as clearly identified by 99mTc-Tetrofosmin, as by 201Tl scintigraphy.

Closure of leaks by fibrin gluing. Effects of various application techniques and temperatures.

This study was performed to determine the differences in covering power obtained with fibrin gluing using three different methods, layered, mixing, and spraying. The experimental system consisted of a 2 x 2 cm plastic plate with a hole 1 mm in diameter, a plastic syringe, and a digital manometer. The internal pressure of the syringe barrel was measured with the digital manometer. Five minutes after fibrin glue membranes (1 mm in thickness) had been prepared on the plastic plate with the layered, mixing, or spray method, the plunger of the syringe was pushed slowly, and the maximum internal pressure in the syringe barrel was measured, just before the breakage of the membrane. Experiments were performed five times at each four temperatures, 12 degrees C, 17 degrees C, 22 degrees C and 37 degrees C. Covering power of fibrin glue membranes (units: mmHg; atmospheric pressure 760 mmHg) were as follows: 1) Layered method (12 degrees C: 810.0 +/- 68.6, 17 degrees C: 769.0 +/- 10.2, 22 degrees C: 812.0 +/- 112.4, 37 degrees C: 773.6 +/- 24.4). 2) Mixing method (12 degrees C: 956.6 +/- 219.3, 17 degrees C: 972.4 +/- 243.5, 22 degrees C: 1045.2 +/- 233.0, 37 degrees C: 1059.0 +/- 220.2). 3) Spray method (12 degrees C: 1010.0 +/- 231.1, 17 degrees C: 1144.4 +/- 170.6, 22 degrees C: 1148.0 +/- 234.7, 37 degrees C: 1250.0 +/- 111.8).(ABSTRACT TRUNCATED AT 250 WORDS)
