Correlation between in vitro and in vivo response to androgens in patients with aplastic anemia.

In an attempt to predict the clinical response to androgens, the effects of added fluoxymesterone (FMT) on the in vitro growth of marrow erythroid progenitors (CFU-E) were examined in 15 patients with aplastic anemia and 6 control subjects with normal marrow morphology. In the control group, the addition of FMT enhanced the growth of CFU-E in vitro, the maximum absolute increase being dependent on the basal number of CFU-E. In 10 out of the 11 aplastic anemia patients who responded to androgens in vivo. FMT enhanced the growth of CFU-E in vitro. In this group, there was a positive correlation between the basal number of CFU-E and the maximum absolute increase of CFU-E induced by FMT. In patients who did not respond to androgens in vivo, all except one showed no increase in the growth of CFU-E in vitro by the addition of FMT. Furthermore, in four out of seven patients examined prior to androgen therapy, the addition of FMT enhanced the in vitro growth of CFU-E. Androgen therapy in vivo resulted in marked hematological improvements in all of these four patients. Two out of the remaining three patients in whom FMT did not enhance the in vitro growh of CFU-E failed to respond in androgen therapy. These results indicate that an in vitro CFU-E culture system to the presence of FMT may be helpful in predicting the response to androgens in vivo in patients with aplastic anemia.

Preparation of specific antisera to estrone-3-glucuronide and estriol-3-glucuronide.

The preparation and antigenic properties of estrone-3-glucuronide- and estriol-3-glucuronide-bovine serum albumin conjugates in which the hapten is linked to the carrier protein through an (O-carboxymethyl)oxime bridge at the C-6 position on the steroid nucleus, have been described. Antibodies raised against the two immunogens in the rabbit possessed high specificity to estrone-3-glucuronide and estriol-3-glucuronide, respectively, exhibiting little cross-reactivities with other estrogen conjugates and no cross-reactions with related steroids except for free estrogens, their 3-methyl ethers and 3-sulfates. The cross-reactive antibodies were eliminated by partial immunoadsorption on affinity chromatographic media using the estrone-3-methyl ether 17-(O-carboxymethyl)oxime- and estriol-3-methyl ether 16 (or 17)-hemisuccinate-aminohexyl Sepharose conjugates, respectively. The purified antisera exhibited no cross-reactivities with free estrogens and ring A conjugates of estrone and estriol.

Migration of bone marrow lymphocytes in mice: immunofluorescent staining using antiallotype serum.

Bone marrow cells from CSW (Igha) mice were injected intravenously into the congeneic CWB (Ighb) mice. The lymphoid tissues of the recipient CWB mice were examined for donor type surface immunoglobulin (SIg)-positive cells using fluorescein isothiocyanate-conjugated antiallotype serum Ighb anti-Igha. Donor type SIg-positive cells were rarely found in the recipients immediately after injection. However, the donor type B cells increased in the recipient's spleen from days 1-2 and reached a plateau thereafter. The present method provides a useful means for studying the migration patterns and differentiation of bone marrow lymphocytes.

Conjugated oestrogen during the menstrual cycle measured by a direct radioimmunoassay with an antiserum prepared against oestradiol-17-glucosiduronate-[C-6]-BSA conjugate.

Early morning and 24 h urine samples and serum were collected daily throughout the menstrual cycle in women. Urinary oestradiol-17-glucosiduronate (E2-17-G) was measured with a direct radioimmunoassay whose antiserum had been prepared against E2-17-G-[C-6]-bovine serum albumin conjugate and was very specific. On the average, E2-17-G in early morning and 24 h urine samples showed a prominent peak one day before the peak of urinary LH. The time relationship between these urinary E2-17-G and serum E2 levels and peaks was also investigated.

[Formalin-induced minor tremor response as an indicator of pain].

Formalin which was said to produce prolonged pain and inflammation was injected subcutaneously into the back of guinea pigs, and minor tremor pain response (MTP-response) was measured using the MT-pick up, integrator and digital volt meter. The MTP-response curve showed a biphasic pattern. Immediately after injection, the MTP-response curve showed a significant peak which lasted for about 2 min (the first phase) and subsequently dipped rapidly, and after 5 min, it began to rise slowly again and had a peak at 30 min (the second phase). Morphine (6 mg/kg, s.c.) inhibited completely the first and second phases. Levallorphan (1.2 mg/kg), however, reversed the inhibitory effect of morphine at the first phase, but not at the second phase. Aspirin (200 mg/kg, i.p.), aminopyrine (100 mg/kg, s.c.) and pentazocine (5 mg-10 mg/kg, s.c.) inhibited significantly the formalin-induced MTP-response at both phases. Pyridinol carbamate (200 mg/kg, i.p.) and hydrocortisone (25 mg/kg, i.p.) had no effect on the MTP-response at the first phase, but inhibited it at the second phase. There was a parallelism between the time course of the vascular permeability induced by formalin and that of the second phase of MTP-response. From these results, it is suggested that the first phase of MTP-response is derived from the direct effect of formalin on free nerve endings, while the second phase is derived from the inflammation. Since two kinds of pain features were differentiated in this method, the relationships with so-called "immediate pain" and "delayed pain" were discussed. Furthermore, this method can be utilized to assess pain and the action of analgesics objectively and quantitatively.

Antigenic properties of estriol 3-glucuronide-[C-6]-bovine serum albumin conjugates having oxime bridges.

The preparation and antigenic properties of estriol 3-glucuronide-bovine serum albumin (BSA) conjugate in which the hapten is linked to the carrier protein through an (O-3-carboxypropylcarbamoylmethyl)oxime bridge at the C-6 position on the steroid nucleus, has been described. 16,17-Di-O-acetyl-6-oxoestriol 6-(O-carboxymethyl)oxime 3-glucuronide acetate-methyl ester was condensed with gamma-amino-n-butyric acid by the mixed anhydride method. Subsequent coupling with BSA followed by removal of the protecting groups with alkali gave the desired hapten-BSA conjugate. The antisera elicited in the rabbit with the conjugate were highly specific to estriol 3-glucuronide, discriminating from other 3-substituted compounds. The specificity of antisera improved by elongation of the 6-(O-carboxymethyl)oxime bridge has been discussed.

Clinical significance of the right ventricular free wall appearance on stress Tl-201 myocardial images in ischemic heart disease.

In order to evaluate the clinical significance of thallium (Tl)-201 myocardial imaging for diagnosing the right ventricular (RV) ischemia, we studied the relationship of right ventricular free wall (RVFW) appearance on myocardial images to coronary arteriographic findings. Patients were divided into 3 groups as follows: 1) normal control (NC) group (19 cases) without angiographically documented coronary artery disease; 2) non-RCA group (18 cases) with significant coronary artery lesion restricted to the left coronary artery (LCA); and 3) RCA group (36 cases) with significant right coronary artery (RCA) stenosis regardless of underlying LCA disease. After the patients had exercised up to 80-85% of the predicted maximal heart rate, immediate and 3-4 hour delayed myocardial images were obtained. As for presence or absence of the RV ischemia on images, the RVFW appearance in 30 degrees and 60 degrees left anterior oblique (LAO) views were assessed carefully. On the immediate images, all patients except one in the NC group and all except 3 in the non-RCA group demonstrated 'continuous visualization' of the RVFW in both views. In the RCA group, 6 showed 'non-visualization' and 11 'defective visualization' of the RVFW in 30 degrees LAO view. In 60 degrees LAO view, 6 presented 'non-visualization' and other 6 'defective visualization' of the RVFW. On the delayed images, although none of the patients in the groups NC and non-RCA demonstrated redistribution phenomenon of the RVFW, 4 patients in the RCA group showed redistribution of Tl-201 into the RVFW. Non- and defective visualization of the RVFW on the immediate images were related to the proximally located RCA lesion, previous history of inferior myocardial infarction and high grade RCA stenosis. Collateral vessels seemed to protect the RVFW against the development of exercise induced ischemia and affect the occurrence of redistribution of Tl-201 into the RVFW. In conclusion, stress Tl-201 myocardial imaging enables us to estimate the myocardial blood flow of the RV and is a useful non-invasive method in the evaluation of RV ischemia.

[A successfully operated case of atrial septal defect with pheochromocytoma].

A 16 year-old male, who had marked hypertension and complained of palpitation, hyperhidrosis, headache and weight-loss, was diagnosed as atrial septal defect with pheochromocytoma. The first operation was performed for tumor on September 3, 1981. The hemodynamic change was recorded through the operation. The total systemic peripheral resistance and the total pulmonary resistance were improved after surgery. The systemic and pulmonary blood pressure were also decreased to normal level after surgery, although pulmonary arterial flow was increased. On one hundred and third day after this operation, radical correction for the cardiac malformation was performed. Operative and postoperative course were uneventful and the patient is free from all untoward complaints at this moment. Blood or urine catecholamine levels have also improved and are within normal limits.
