[Susceptibilities of bacteria isolated from patients with respiratory infectious diseases to antibiotics (1991)].

Isolated bacteria from respiratory tract infections were collected since 1981 in cooperation with institutions located throughout Japan, and have been investigated for their sensitivities to various antibacterial agents and antibiotics and reported by IKEMOTO, et al. Relationships between these isolates and backgrounds of the patients were also studied each year. These results are discussed in detail in this report. In 20 institutions around the entire Japan from October 1991 to September 1992, 631 strains of bacteria were isolated mainly from sputa of 529 patients with respiratory tract infections and tentatively determined to be etiological agents. MICs of various antibacterial agents and antibiotics against 96 strains of Staphylococcus aureus, 112 strains of Streptococcus pneumoniae, 111 strains of Haemophilus influenzae, 114 strains of Pseudomonas aeruginosa (non-mucoid), 41 strains of Moraxella subgenus Branhamella catarrhalis, 39 strains of Pseudomonas aeruginosa (mucoid), Klebsiella pneumoniae and some others, were determined, and the drug sensitivities of these strains were determined except for the strains that had been killed during transportation: 1. S. aureus. S. aureus strains for which MICs of methicillin were higher than 4 micrograms/ml (methicillin-resistant S. aureus) accounted for 58.3% and the frequency of the drug resistant bacteria increased over previous year's 42.5%. As shown by the MICs, arbekacin was active as vancomycin against all the strains on S. aureus. 2. S. pneumoniae: Benzylpenicillin among the penicillins showed a potent activity against S. pneumoniae. Cefuzonam, cefmenoxime, cefozopran and cefotaxime among the cephems showed excellent antimicrobial activities against S. pneumoniae. Imipenem; a penem antibiotic, showed the most potent activity with MIC80 of 0.03 micrograms/ml. 3. H. influenzae: Activities of all drugs were excellent against H. influenzae strains tested. Ampicillin showed MIC80 of 1 micrograms/ml against H. influenzae. Cefuzonam showed the most potent activity among cephems, it completely killed all bacteria at MIC 0.06 micrograms/ml. Cefotaxime and cefmenoxime showed next most potent activities with MIC80s of 0.06 micrograms/ml. The antimicrobial activity of ofloxacin was equivalent to those of cephems. 4. P. aeruginosa (mucoid). Ciprofloxacin and tobramycin showed the most potent activities against P. aeruginosa (mucoid), and their MIC80s were 4 micrograms/ml. 5. P. aeruginosa (non-mucoid): Similarly, ciprofloxacin and tobramycin showed the most potent activities against P. aeruginosa (non-mucoid) with MIC80 of 2 micrograms/ml. Comparing to activities against P. aeruginosa (mucoid), all the drugs tested showed lower activities against P. aeruginosa (non-mucoid). 6. K. pneumoniae: The activities of all drugs except for penicillins were very high against K. pneumoniae.(ABSTRACT TRUNCATED AT 400 WORDS)

[Susceptibilities of bacteria isolated from patients with respiratory infectious diseases to antibiotics (1990)].

These investigations have been continued since 1981. In this year the isolation frequencies and sensitivities to antibiotics were investigated for 654 bacterial strains isolated from respiratory tract infections in 20 institutions during the period of October 1990 to September 1991. Among Staphylococcus aureus isolated from inpatients, many were methicillin-resistant S. aureus (MRSA), and the frequency of their isolation was 56.3% for inpatients and 4.3% for outpatients. Of the relation to antibiotic administration, the isolation frequency of MRSA before administration of antibiotics was 19.6% (10/51), and after administration was 75.0% (27/36). The sensitivities of S. aureus to imipenem and clindamycin (MIC80) decreased from 0.2 microgram/ml to 64 micrograms/ml and from 0.2 micrograms/ml to 128 micrograms/ml, respectively. We investigated year to year changes in the backgrounds of patients with respiratory tract infections. Bacterial pneumonia was 31.6% among respiratory tract infections in the period of the study, this trend has increase from 1989. Frequencies of different etiological bacteria in respiratory tract infections did not change appreciably from year to year, and S. aureus, Streptococcus pneumoniae and Pseudomonas aeruginosa were the most showing some frequent pathogens.

[Prognostic evaluation of peritoneal lavage smears and gastric wall brushing smears in gastric cancer surgery].

We investigated the correlation of intraoperative peritoneal lavage smear and gastric wall brushing smear cytology with macroscopic and histologic findings and prognosis, in 216 patients with gastric cancer. Some 196 of these patients underwent gastric resection and 20 did not. The incidence of positive cancer cells determined by these methods was significantly correlated with gastric resection, tumor invasion, and peritoneal dissemination. Among patients without macroscopic peritoneal dissemination, four were found to have positive peritoneal lavage smears or gastric wall brushing smears, respectively. The prognosis of these patients did not differ from that of patients with metastases to the adjacent peritoneum but not the distant peritoneum. These results suggest that peritoneal lavage smears and gastric wall brushing smears are useful for predicting the peritoneal cancer dissemination. Therefore we intend to target patients with no peritoneal metastases (Po) showing positive peritoneal cytology for intensive treatment.

Assessment of production of endothelium-derived relaxing factor by cultured porcine cerebral microvessel endothelial cells.

To establish an experimental system to assess the production of endothelium-derived relaxing factor (EDRF) by cultured porcine cerebral microvessel endothelial cells (PCMEC), the effect of PCMEC on thrombin-induced platelet aggregation was studied. For the platelet aggregation cuvettes lined with PCMEC were used. The cuvettes were prepared by seeding PCMEC in gelatin-coated cuvettes at a cell density of 2 x 10(5) cells/ml and culturing for 48 hours. Thrombin-induced platelet aggregation was markedly inhibited in the presence of PCMEC. Indomethacin reduced the PCMEC-dependent anti-platelet aggregatory effect. This PCMEC-dependent anti-platelet aggregatory effect was increased by the addition of bradykinin which stimulates the production of EDRF. These findings suggest that this simple experimental system is useful in assessing the production of EDRF by PCMEC and in examining the effects of various chemicals (or agents) on EDRF production.

[Proton-magnetic resonance spectroscopy (1H-MRS) of human plasma obtained from patients with various cancer--new detection method for common cancer].

1H-Magnetic Resonance (MR) Spectra of plasma obtained from patients with various cancer were measured. Clinical usefulness of 1H-MRS assay for detection of common cancer was examined. The following results were obtained. 1. High lactate peak was detected significantly with 1H-MR Spectra of plasma in every cancer patients. The patterns of 1H-MR spectra were very similar in each case, except for the lactate peak. 2. Lactate values (area ratio of lactate peak to TSP) of cancer patients showed high correlation with tumor marker of AFP, but not significant with CEA and CA19-9. 3. 1H-MRS assay has many advantages in the clinical examination. From these results, it was concluded that lactate peak in 1H-MRS might be useful to detect common cancer as a new tumor marker and 1H-MRS assay was equipped with enough characters as a new clinical laboratory assay for detection of specific metabolic deficiency.

[Thermotolerance and heat shock protein (HSP 70) in vascular endothelial cells].

Recently, it was determined that the endothelial cells of blood vessel play a very important physiological role in the regulation of blood coagulation and selective permeability. To study the thermotolerance of vascular endothelial cells, human umbilical vein endothelial cells (HUVEC) were heated at 40, 43, 45 or 50 degrees C for various lengths of time with or without preheating at 40 or 43 degrees C for 30 min. The cell viability (CV) of HUVEC decreased gradually according to heating time. However, the CV of preheated HUVEC decreased slightly or not at all. Heat shock protein (HSP) in HUVEC heated at 37, 40, 43, or 45 degrees C was examined by immunoblotting. A new HSP 70 band was detected in HUVEC by heating at 40, 43 or 45 degrees C. HUVEC revealed thermotolerance with induction of HSP by heat stress.

Effect of cadmium (CdCl2) on cell proliferation and production of EDRF (endothelium-derived relaxing factor) by cultured human umbilical arterial endothelial cells.

The effect of cadmium chloride (CdCl2) on cell proliferation and EDRF (endothelium-derived relaxing factor) production by cultured human umbilical arterial endothelial cells (HUAECs) was investigated. The viability of HUAECs decreased dose-dependently after the addition of Cd (cadmium chloride). Morphologic examination by phase contrast microscopy revealed severe damaging effects of Cd at higher concentrations. The cytotoxic effect of Cd on DNA synthesis was also concentration-dependent. The effect of Cd on EDRF production by indomethacin-treated HUAECs was assessed by its anti-platelet aggregatory effect. Platelet aggregation studies were carried out in cuvettes lined with HUAECs using an aggregometer. The anti-platelet aggregatory effect was decreased dose-dependently by pretreatment with Cd. These findings suggest that HUAECs are susceptible to concentration-dependent Cd cytotoxicity, and that Cd can inhibit the production of EDRF by HUAECs.

Hepatitis C virus serotype II responds more favorably to interferon-alpha therapy.

To determine whether serological typing of hepatitis C virus correlated with the response to interferon-alpha therapy, hepatitis C virus serotypes were determined by subtype-specific antibody to NS4 polypeptide by enzyme-linked immunosorbent assay in 55 Japanese patients with chronic active hepatitis C who subsequently received recombinant interferon-alpha 2a therapy. Response to interferon-alpha was defined as complete and sustained (n = 12), complete response followed by relapse (n = 26), and no response (n = 17). There was no difference in the clinical biochemical parameters between these patients groups. However, a higher proportion (50.0%) of patients with hepatitis C virus serotype II showed complete and sustained response to interferon-alpha, compared to serotype I (11.1%, p < 0.01). These data indicate that this simple hepatitis C virus serotyping assay is a useful predictor of response to interferon-alpha therapy in patients with chronic hepatitis C virus infection.

[A case of Corynebacterium jeikeium septicemia].

We report a case of Corynebacterium jeikeium septicemia associated with malignant lymphoma. The patient is a 58-year-old male who was diagnosed as malignant lymphoma on August 1992. May 15, 1993, he was admitted to our hospital because of oliguria, abdominal flatulence and vomiting which developed a few days before admission. Anticancer regimen were started. In the middle of July, white blood cell (WBC) count dropped to 100/mm3 and body temperature rose to 39 degrees C. He was been treated with Ceftazidime and Piperacillin. C. jeikeium was recovered from blood culture. Antibiotics were switched to minocycline and vancomycin. He died of septic shock and pneumonia. Autopsy revealed the presence of the colonies of Rods. Which were morphologically compatible with C. jeikeium were observed in lung tissue and in the small pulmonary vessels.

Evaluation of minocycline and cefuzonam for antimicrobial activity against clinical isolates.

The Antibacterial activity of minocycline (MINO) and that of cefuzonam (CZON) were assessed with clinical isolates of 19 species, and compared with that of other antibiotics. MINO was highly active against methicilli-sensitive Staphylococcus aureus (MSSA), Neisseria gonorrhoeae, Moraxella (Branhamella) catarrhalis, Haemophilus influenzae, Helicobacter pylori, Flavobacterium meningosepticum, Acinetobacter calcoaceticus, Peptostreptococcus spp. and Propionibacterium acnes, but not as effective against methicillin-resistant Staphylococcus aureus (MRSA), Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Pseudomonas cepacia and Alcaligenes xylosoxidans. CZON was highly active against MSSA, Streptococcus pyogenes, Streptococcus agalactiae, Streptococcus pneumoniae, N. Gonorrhoeae, M(B). catarrhalis, H. influenzae, H. pylori, P. mirabilis, Peptostreptococcus spp. and P. acnes, but not effective against MRSA. It was minimally active against Gram-negative rods (E. coli, K. pneumoniae, etc.) and bacteria that do not ferment glucose.

[Evaluation of peritoneal lavage smears and gastric wall brushing smears in gastric cancer surgery].

We investigated the utility of intraoperative peritoneal lavage smears and gastric wall brushing smears for diagnosis of peritoneal dissemination and serosal involvement during gastric cancer surgery. One hundred fifty-three patients with gastric cancer were examined; 138 who underwent gastric resection and 15 who did not. The incidence of cancer cell positivity determined by these methods was 8.0% in resected cases, and 80.0% in unresected cases. According to macroscopic serosal invasion, the incidence of positive cytology was 0% for S0, 15.4% for S1, 13.0% for S2, and 38.7% for S3. According to microscopic wall invasion, the incidence of positivity was 0% for m and sm, 6.3% for mp, 0% for ss, and 22.2% for se and si. In terms of macroscopic peritoneal dissemination, the incidence of positivity was 2.6% for P0, 28.6% for P1, 42.9% for P2, and 90.0% for P3. These results suggest that peritoneal lavage smears and gastric wall brushing smears are useful for predicting the spread of peritoneal dissemination. Thus, further study of intraoperative fast-smear cytology seems warranted.

[Treatment for peritoneal dissemination of gastric cancer by intraperitoneal administration of CDDP through Infuse-a-Port].

This report describes the effect of intraperitoneal administration of CDDP for the treatment of peritoneal dissemination (P) in 20 patients with gastric cancer. Eight patients after palliative resection, 4 after non-resection and 8 with recurrent cancer were given 25 mg of CDDP through a reservoir (Infuse-A-Port) implanted in the abdominal wall once a week. Simultaneously, 400 mg/body/day of UFT and 2 mg/body/week of Lentinan were administered. Twelve of the patients died of disease at 6-19 months (median 7 months), but 8 have survived 3 to 15 months (median 6 months). Complete and partial responses were obtained in 7 of the 8 patients with recurrence (88%) for malignant ascites. No patient showed any toxicity from treatment. All patients receive the treatment in the outpatient clinic after discharge, and 17 patients have shown a long hospital-free survival.

[Multi-center evaluation to discriminate between the strains of methicillin-resistant Staphylococcus aureus (MRSA) and those susceptible (MSSA) by Showa oxacillin and methicillin disk susceptibility tests].

The Showa disk susceptibility test using two penicillinase-resistant penicillins, oxacillin and methicillin, was evaluated to discriminate between the strains of Staphylococcus aureus resistant to methicillin (MRSA) and those susceptible (MSSA) in the multi-center trials. The study included 651 clinical isolates of S. aureus, comprising of 329 MRSA and 322 MSSA isolates. The inhibitory zone diameters by Showa disks to oxacillin and methicillin highly correlated with minimum inhibitory concentrations (MICs) determined by standard agar dilutions with 0.961 and 0.930 correlation coefficients, respectively. Of 651 duplicate MIC determinations, 79.9% (oxacillin) and 80.3% (methicillin) were within +/- 1 log2 dilutions with each other. When Showa oxacillin and methicillin disks were incubated at 35 degrees C, sensitivity and specificity of oxacillin to detect MRSA were 95.4% and 98.1%, and those of methicillin were 94.8% and 95.2%. When tested on agar plates supplemented with 5% NaCl, sensitivity and specificity markedly improved to > 97%. Also, when incubated at 30 degrees C, sensitivity and specificity became to nearly 100%. Of 329 MRSA isolates, the interpretive criteria combined with incubation at 30 degrees C and testing onto 5% NaCl supplemented agar plates could correctly identify 324 (98.5%) and 329 (100%) isolates, respectively. In conclusion, when the Showa oxacillin and methicillin disk susceptibility tests were employed exactly according to the manufacturer's instruction, the test performances to detect MRSA were enough reliable to screen MRSA isolates in clinical microbiology laboratories.

[Recent advances in immunological diagnosis for tuberculosis].

Recent advances in immunological diagnosis for tuberculosis including tuberculin Mantoux test and serodiagnosis were reviewed. New tuberculins (T1327, T1456) were reported to be more specific than the conventional one (RT23). Tuberculins from atypical mycobacteria (PPD-B, PPD-Y, PPD-F) were reported as a useful tool for the diagnosis of atypical mycobacteriosis. Optical density index method is a most appropriate method for a clinical use among several methods for the expression of antigen titer in serodiagnosis using ELISA. The duration of disease is important for understanding the results. IgM antibody rises in the early stage of the disease and comes down in 10 weeks after onset, while IgG antibody rises lately. Anti-cord factor antibody is a highly specific antibody for tuberculosis and may be a potent candidate for a clinical diagnostic tool. Anti-PPD-B antibody was elevated in atypical mycobacteriosis and pulmonary tuberculosis also. The ratio of anti-PPD-B antibody to anti-PPDs antibody was elevated in atypical mycobacteriosis while it was lowered in pulmonary tuberculosis. It may be helpful for suggesting the causative organism when the sputum smear is positive. The opposite results have been reported on the serodiagnosis in HIV infection. The usefulness of serodiagnosis for tuberculosis in HIV infected patients remained controversial.

[Multi-center evaluation of Showa ceftizoxime disk susceptibility test to discriminate between the strains of methicillin-resistant Staphylococcus aureus (MRSA) and those susceptible (MSSA)].

An increasing prevalence of methicillin-resistant Staphylococcus (S.) aureus (MRSA) has a serious therapeutic problem, and accurate methods to detect such strains are needed. We studied the antimicrobial susceptibility of S. aureus to ceftizoxime, in comparison with those to four other cephems (cefazolin, cefoxitin, latamoxef and cefmenoxime), by broth microdilutions and disk susceptibility tests, and also evaluated whether the reagents, in replace of penicillinase-resistant penicillins (PRPs), could discriminate between the strains of MRSA and those susceptible to PRPs (MSSA). A total of 651 clinical isolates of S. aureus were collected from six geographically different hospitals. All the strains collected were first classified into either MRSA (n = 329) or MSSA (n = 322) according to the interpretations of MRSA screening agar, minimum inhibitory concentrations (MICs) to oxacillin and methicillin (NCCLS M7-A2), and the presence or absence of mecA gene by polymerase chain reaction. In broth microdilution tests, the MICs of MRSA to ceftizoxime ranged > or = 64 micrograms/ml, whereas all the MSSA were at the concentration of < or = 16 micrograms/ml. The results of Showa disk diffusion tests highly correlated with those of MIC determinations. The distribution of inhibitory zone diameters to ceftizoxime were clearly divided into two groups; 99.2% (sensitivity) of MRSA had inhibitory zones of < or = 20 mm and 98.9% (specificity) of MSSA produced > or = 21 mm. It was concluded that the Showa ceftizoxime disk susceptibility test was useful and enough reliable to screen MRSA isolates in clinical laboratories.

Clonal analysis of multiple point mutations in the N-ras gene in patients with acute myeloid leukemia.

We have screened mutations of the N-ras gene at codons 12, 13, and 61 in leukemia cells obtained from 100 patients with acute myeloid leukemia (AML), and found mutated N-ras alleles in 9 patients. We further analyzed the polyclonality of multiple N-ras gene mutations in 4 AML patients. One patient, who had the monoclonal karyotype, t(11;17), had two types of double missense mutations at codons 13 and 61 in the same allele. Each of the remaining three patients, one of whom had t(15;17) with a monoclonal rearrangement of the retinoic acid receptor alpha and PML genes, carried two missense mutations in a relatively small population of leukemia cells. We have demonstrated that multiple clonality of the N-ras gene is occasionally observed in leukemia with a monoclonal karyotype. These findings indicate that the N-ras mutations may not always be characterized simply by an accumulative process and that the activated N-ras gene alone is not sufficient to cause leukemia.

Lung abscess: analysis of 66 cases from 1979 to 1991.

Sixty-six patients with bacterial lung abscess were treated between 1979 and 1991 in our hospital. Among these patients, death occurred in one of the 42 cases of community-acquired infection (mortality rate: 2.4%) and in 16 of the 24 cases of nosocomial infection (mortality rate: 66.7%). Of all 66 cases, 55 were culture-positive, and the etiologic agents isolated from 24 of the culture-positive cases were found to be anaerobic bacteria. The most common aerobes isolated from the foci were identified as Staphylococcus aureus, Klebsiella spp. and Pseudomonas aeruginosa, while the most common anaerobes were Bacteroides spp., Peptostreptococcus, Fusobacterium spp., microaerophilic Streptococcus and Veillonella. The mortality was higher in the cases with P. aeruginosa, Klebsiella spp. and Candida spp. than in those with other bacteria. The prognosis of lung abscess patients proved to depend on the presence of underlying diseases and on superinfection with aerobes.
