[Study on the effectiveness and safety of cefmenoxime in the treatment of respiratory tract infections. Tendency toward clinical hemorrhage related to the treatment].

The effectiveness and safety of cefmenoxime (CMX) in the treatment of respiratory tract infections were evaluated, mainly in relation to the appearance of hemorrhagic tendency. Out of 80 patients treated in this study, 57 were treated with CMX alone, and an effective rate was determined to be 75.4%. As for a tendency toward hemorrhage, none of the patients clinically exhibited a hemorrhagic tendency, while the prolongation of prothrombin time (PT) and/or activated partial thromboplastin time (APTT) was seen in 3 patients (3.8%), who were all at ages over 63. The prolongation of APTT was clearly related to CMX in 1 patient. Although the causal relation between CMX and the prolongation of PT or APTT was unclear in the remaining 2 patients because they had severe primary diseases (lung cancer and respiratory failure due to chronic obstructive lung disease in addition to severe hepatic disorder and received combination treatment with anticancer agents or other antibiotics), CMX might have a role in causing these phenomena. Even though none of the patients showed a hemorrhagic tendency clinically arousing problems, we should be careful in using CMX in the treatment of the aged and patients in poor general conditions because PT and/or APTT was observed to prolong with the use of CMX in some patients.

[Pulmonary manifestations of alveolar soft part sarcoma; bronchial arterial embolization for hemoptysis].

Alveolar soft part sarcoma is a rare soft tissue tumor arising in extremities. The universal tendency for late widespread metastasis to lung, bone and brain, make the treatment of this tumor a particular challenge. This time, we performed the intra-arterial infusion of MMC and bronchial arterial embolization by spongel for hemoptysis of this metastatic lung tumor. Rosenbaum has reported the characteristics of angiogram of the metastatic brain tumor. In our case, bronchial arteriography showed the manifest hypervascular pattern "rosary form," "pooling" and "B-P shunt." These findings should yield benefits for the diagnosis and treatment of this metastatic lung tumor.

Vitamin K effect in low birth weight infants.

Factor II coagulant antigen (FII-AG), the protein induced by vitamin K absence or antagonist II (PIVKA-II), and coagulant activity (normotest) were measured in low birth weight infants. The factor II coagulant antigen and normotest levels in one-day-old babies were lower than those of full-term infants (P less than .005, P less than .01, respectively). Infants whose normotest levels were less than 30% at one day (group A) received vitamin K2, and the others whose normotest levels were greater than 30% at one day (group B) were not treated. At this time, the mean factor II coagulant antigen level was significantly lower in group A than in group B (P less than .05). During the first seven days of life, in 65.2% of the infants in group B the PIVKA-II level became positive, but this did not occur in any infant in group A. After vitamin K treatment, there was greater improvement in the normotest level in infants with positive PIVKA-II levels than in those with negative PIVKA-II levels. This observation indicates that the hypoprothrombinemia found in low birth weight infants at one day of age is attributable to reduced synthesis of factor II coagulant antigen in the liver at this stage, but the prophylactic administration of vitamin K seemed to be effective even in such infants, probably because of the increase in factor II coagulant antigen synthesis (P less than .001) during the first seven days of life.

Radiolocalization of xenografted human lung cancer with monoclonal antibody 8 in nude mice.

A monoclonal antibody (MAb) 8 [immunoglobulin G3 (IgG3)], directed against a Mr 48,000 human lung cancer-associated antigen, was radiolabeled with either 125I or 131I, and its biodistribution was studied in nude mice bearing human lung cancer (TKB-2) over a 7-day period. 125I-labeled MAb 8 increased its binding to the tumor during the period, while the binding of 125I-labeled control IgG3 declined after initial uptake. At Day 7, percentages of injected dose of 125I-labeled MAb 8 bound to the tumor rose to 7.4%, which was a 4.4-fold increase from Day 1 and 16-fold binding of 125I-labeled control IgG3 at the same day. Tumor:blood ratios became 2.7:1 at Day 7, and tumor:liver, tumor:spleen, and tumor:kidney ratios were more than 9:1. Normal organs showed no significant uptake of 125I-labeled MAb 8, compared with those of 125I-labeled control IgG3. A clear image of the xenografted tumor was obtained at Day 5, and it further improved at Day 7, when 60% of whole body radioactivity was localized to the tumor. Autoradiography of the mouse with tumor confirmed the excellent localization of 125I-labeled MAb 8 to the tumor, although the radioactivity of the tumor was not uniformly distributed. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis showed that most of the radioactivity was present at the tumor in the form of degraded immunoglobulin. MAb 8 has a potential usefulness in the diagnosis and treatment of lung cancer.

Clinical features of a newly recognized type of lattice corneal dystrophy.

We examined five patients with an undescribed type of lattice corneal dystrophy. All patients were in the seventh to ninth decades of life and had developed decreasing vision late in life. None of the patients had suffered from recurrent epithelial erosions, there was no overt evidence of systemic amyloidosis, and the lattice lines were much thicker than those usually observed in lattice corneal dystrophy types I and II. Available pedigree data from two families of three patients indicated that the corneal disorder affected several siblings but not the parents or offspring. Two patients had no affected family members. There was no known consanguinity in any of the four families.

Bronchial carcinoid with multiple aerogenous implanted foci.

A case of bronchial carcinoid with intrapulmonary dissemination was demonstrated. A right upper lobectomy was performed because of an abnormal shadow in the lung detected by an X-ray mass survey. A carcinoid tumor originated from the spur region of the right upper lobe bronchus. The tumor protruded into the bronchial lumen and also extensively expanded into the surrounding alveolar tissue. The tumor showed typical histological features of bronchial carcinoid with a mixed insular, trabecular, and glandular pattern. Multiple implanted foci of tumor cells were found in the periphery of the lobe. The implanted tumor cells replaced the alveolar epithelium without destruction of the alveolar structure, which suggests the possibility of aergenous spread of bronchial carcinoid. This is the first report on bronchial carcinoid with aerogenous spread.

High-resolution scanning electron-microscopic studies on the three-dimensional structure of mitochondria and sarcoplasmic reticulum in the different twitch muscle fibers of the frog.

The three-dimensional structure of the mitochondria and sarcoplasmic reticulum (SR) in the three types of twitch fibers, i.e., the red, white and intermediate skeletal muscle fibers, of the vastus lateralis muscle of the Japanese meadow frog (Rana nigromaculata nigromaculata Hallowell) was examined by high resolution scanning electron microscopy, after removal of the cytoplasmic matrices. The small red fibers have numerous mitochondrial columns of large diameter, while the large white fibers have a small number of mitochondrial columns of small diameter. In the medium-size intermediate fibers, the number and diameter of the mitochondrial columns are intermediate between those of the red and white fibers. In all three types of fibers, the terminal cisternae and transverse tubules form triads at the level of each Z-line. The thick terminal cisternae continue into much thinner flat intermediate cisternae, through a transitional part where a row of tiny indentations can be observed. Numerous slender longitudinal tubules originating from the intermediate cisternae, extend longitudinally or obliquely and form elongated oval networks of various sizes in front of the A-band, then fuse to form the H-band collar (fenestrated collar) around the myofibrils. On the surface of the H-band collar, small fenestrations as well as tiny hollows are seen. The three-dimensional structure of SR is basically the same in all three muscle fiber-types. However, the SR is sparse on the surface of mitochondria, so the mitochondria-rich red fiber has a smaller total volume of SR than the mitochondria-poor white fiber. The volume of SR of the intermediate fiber is intermediate between other the two.

Lung fibrosis induced by diquat after intratracheal administration.

Diquat toxicity to lung was evaluated at various intervals after intratracheal administration to rats. Twelve hours after the injection, both type I and type II pneumocytes developed degenerative changes which were similar in nature to those induced by paraquat. A much larger dose of diquat, however, was needed for the changes compared to paraquat, which is known to be actively taken up by the lung. This morphological evidence suggests that diquat may cause alveolar damage by the same mechanism as paraquat, although it is not actively taken up by the lung. Moreover, initial alveolar damage induced by diquat was followed by lung fibrosis, as with paraquat. Since paraquat is a well known fibrogenic agent, the common property of these two agents in alveolar damage may be a key to interpreting the development of lung fibrosis.

The hypertrophic uncinate process of the pancreas wrapping the superior mesenteric vein and artery--a case report.

The hypertrophic uncinate process of the pancreas wrapping the superior mesenteric vein and artery was discovered during surgery on a patient with nodular chronic pancreatitis. Such a rare anomaly has never been noted in the English literature so far as we have been able to find. This malformation of the pancreatic uncinate process was considered to be due to excess fusion between the ventral and dorsal pancreatic anlages during embryonic stage.

Toxin production in the dinoflagellate Protogonyaulax tamarensis.

Many clones of Protogonyaulax tamarensis were collected from Ofunato Bay in the same season and cultured under the same conditions. The toxicity of the cultured cells of these clones was remarkably different from each other (maximum of 100-fold). Significant differences (maximum of 20-fold) in toxicity of cultured cells were also observed among subclones isolated from a single clonal culture. It is unlikely that this variation in toxicity among subclones is due to mutation during culture. From these observations we suggest that toxin production in P. tamarensis is not a hereditary characteristic.

Retinal vascular loss in idiopathic central serous chorioretinopathy with bullous retinal detachment.

Three patients with idiopathic central serous chorioretinopathy (ICSC) and bullous retinal detachment (RD) were examined at least 2 years after the onset of the disease in one patient and over 10 years in the other two patients. There were large areas of retinal pigment epithelium (RPE) degeneration in each patient. Interestingly, there was loss of retinal vascular trees in the inferior peripheral retina where the subretinal fluid presumably persisted for a long time. In one patient, there was a retinal neovascularization with sea-fan formation along the border of the vascular loss. To the authors' knowledge, this finding has not been described in the literature.
