[The effect of portal venous drainage on graft survival and function in rat whole pancreaticoduodenal transplantation].

A whole pancreaticoduodenal transplant model with portal venous drainage was achieved in the rat, and effect of venous drainage from pancreas grafts into the portal vein on the functional graft survival and long-term glucose metabolism was investigated. In allogeneic series between ACI (RT1a) donors and streptozotocin-induced diabetic Lewis (RT1l) recipients (nonfasting plasma glucose greater than or equal to 400 mg/dl), the mean survival time of pancreas transplants determined by recurrent hyperglycemia (greater than or equal to 200 mg/gl) in rats with portal venous drainage (PV-group: 8.9 +/- 1.3 days) was slightly longer than that in rats with systemic venous drainage (SV-group: 8.3 +/- 0.9 days), but statistically insignificant. In the syngeneic series using Lewis rats, K-values (%/min) in IV-GTT at 1, 2 and 3 months after pancreas transplantation were, respectively, 1.5 +/- 1.0, 2.0 +/- 0.6 and 2.3 +/- 0.7 in PV-group, and 1.2 +/- 0.3, 1.2 +/- 0.4 and 1.8 +/- 0.5 in SV-group. Peripheral IRI (microU/ml) levels before glucose load were higher in both groups (PV-group, 15.1 +/- 10.5: SV-group, 22.8 +/- 16.2) at 3 months than in normal control (7.9 +/- 1.6: p greater than 0.05). These results indicate that the portal venous drainage in pancreas transplants has no remarkable profit in graft survival and that it can provide more physiological glucose control but cannot normalize insulin concentration.

[Levofloxacin in obstetrics and gynecology].

We investigated the clinical efficacy and the safety of levofloxacin (LVFX, DR-3355) in obstetrical and gynecological infections and the following results were obtained. 1. Clinical efficacies were evaluated as excellent in 2 and good in 9 cases, hence the efficacy rate was 100%. 2. Bacteriologically, the eradication rate was 16/17 (94.1%). 3. No Side effects nor abnormal changes in laboratory test results were observed. These results indicated that LVFX was highly useful in the treatment of obstetrical and gynecological infections.

[Surgical treatment of myocardial ischemia with cardiogenic shock].

We evaluated the results of surgical treatment of myocardial ischemia with cardiogenic shock in ten patients. Seven of 10 patients survived after surgery. Other three patients died of multiple organ failure (two patients) or severe left ventricular power failure (one patient). Two patients with previous infarction survived. Lesions in the group of survivors after surgery were left main trunk (LMT) disease in one patient, three vessel disease (3VD) in two, 2VD in two and 1VD in two patients. All nonsurvivors had LMT disease. Two patients died in whom preoperative electrocardiography (ECG) had shown Q wave. No significant difference in periods from onset to start of cardiopulmonary bypass (CPB) and from onset to recanalization was found between survivors and nonsurvivors. CPB time and aorta cross-clamp time in survivors were similar to those in nonsurvivors. At 24-hour after operation, pulmonary capillary wedge pressure in survivors showed significantly low value compared to that in nonsurvivors (12.8 +/- 3.9, 19.3 +/- 1.5 mmHg, respectively, p less than 0.05). Four patients out of seven survivors were in 1 of Forrester's classification, two in 2 and one in 3. Of three nonsurvivors, one were in 3 and two in 4. We conclude that mortality related to three risk factors: (1) LMT disease, (2) Q wave in preoperative ECG, and (3) hemodynamic state at 24-hour after operation.

[The intraocular pressure lowering effects of UF-021, a novel prostaglandin related compound, in animals].

The ophthalmic solution of UF-021, a novel prostaglandin (PG) related compound, was investigated for its intraocular pressure (IOP) reducing activity and local ocular side effects in different species of animals. UF-021 ophthalmic solution (0.03 to 0.24%), when topically applied to the eyes of rabbits, caused dose-dependent IOP reduction (2.8 to 5.2 mmHg), without transient IOP rise. Both in cats and monkeys, UF-021 ophthalmic solution (0.12%) elicited rapid, significant IOP reduction (ca. 9 mmHg and 2 mmHg, respectively), without any controversial, local ocular side effects being revealed. On the other hand, PGE2, PGF2 alpha-isopropyl ester all brought about marked increases in IOP prior to development of their IOP reducing activities. In addition, these primary PGs showed intense local ocular irritation, which presented a striking contrast with UF-021. Enhancement of IOP reducing activity, coupled with freedom from any significant ocular side effects, as described above, suggests that UF-021 ophthalmic solution could be promising as a new anti-glaucoma agent.

Basic fibroblast growth factor promotes proliferation of rat glomerular visceral epithelial cells in vitro.

Glomerular visceral epithelial cells (vGEC) play an important role in the synthesis of the glomerular basement membrane (GBM), and together with glomerular endothelial cells and the GBM, in glomerular ultrafiltration. Therefore clarification of the properties of vGEC is essential to investigations of glomerular morphology and function in both physiologic and pathologic conditions. This article demonstrates that basic fibroblast growth factor (bFGF) is mitogenic to vGEC in vitro. Its effect was found at concentrations as low as 1.25 ng/ml, and was synergistic with epidermal growth factor (EGF). In contrast, EGF by itself had no demonstrable mitogenic effect at concentrations of 1.25-100 ng/ml. In addition, mRNA for bFGF was identified in cultured vGEC by the method of reverse transcriptase polymerase chain reaction and the immunoreactivity of bFGF was found in GEC of the Sprague-Dawley rat kidney. These results suggest that bFGF stimulates the proliferation of vGEC in an autocrine manner in vivo. A unique relationship similar to that observed in endothelial cells may also exist among bFGF, vGEC, and the extracellular matrix (ECM). In a word, bFGF may be produced by vGEC and stored in the ECM, that is the GBM, and may be one factor that stimulates vGEC to proliferate when vGEC are injured and lost in vivo.

Brain corticotropin-releasing hormone increases arousal in stress.

The effect of restraint stress on pentobarbital-induced sleeping time was examined in rats. Restraint for 60 and 75 min significantly shortened pentobarbital-induced sleeping time. The shortening of sleeping time by restraint was completely reversed by intracerebroventricular (i.c.v.) administration of alpha-helical CRH(9-41), a corticotropin-releasing hormone (CRH) receptor antagonist. In conjunction with our previous finding that i.c.v. administration of CRH shortens pentobarbital-induced sleeping time, the results suggest that restraint stress increases arousal through brain CRH.

Expression of a novel immediate early gene during 12-O-tetradecanoylphorbol-13-acetate-induced macrophagic differentiation of HL-60 cells.

Human promyelocytic leukemia cell line, HL-60, undergoes macrophagic differentiation when it is stimulated with TPA (12-O-tetradecanoylphorbol-13-acetate). We have cloned ETR101 cDNA whose mRNA was induced immediate early (30 min) and transiently by TPA. The mRNA is superinduced by addition of the protein synthesis inhibitor cycloheximide. The sequence of ETR101 cDNA (1826 base pairs) reveals that (i) it will encode a protein of 223 amino acids with a formula molecular weight of 24,200, (ii) the amino acid sequence is highly homologous to mouse chx1 protein whose mRNA was found recently to be enhanced in activated T lymphocytes in response to cycloheximide, (iii) the amino acid sequence is also weakly homologous to jun family gene products, and (iv) in the mRNA 3'-flanking region, there is a unique GUUUG sequence which is complementary to a part of B1 repetitive sequence and may be involved in mRNA degradation. ETR101 mRNA is induced by TPA in a wide variety of leukemia cells including myeloid, T-lymphoid, and B-lymphoid lineages. We have found that this mRNA is also induced by okadaic acid, a protein phosphatase inhibitor, and that TPA or cycloheximide act synergistically with okadaic acid. In addition, the induction is inhibited by protein kinase C inhibitors. Therefore, ETR101 mRNA level is controlled, either directly or indirectly, by protein phosphorylation.

[Experience of coronary artery bypass grafting with internal thoracic artery].

Between March 1990 and May 1991, twenty-nine patients underwent coronary artery bypass grafting. The mean number of grafts was 2.7 +/- 0.9 grafts per patient. 28 patients (96.6% of all cases) underwent CABG with the internal thoracic arteries. The perioperative mortality was 3.4% (1/29 cases). The postoperative coronary angiography at one month revealed that the early patency of total grafts, of internal thoracic artery grafts and of saphenous vein grafts were 96.7% (58/60 grafts), 100% (29/29 grafts), 93.5% (29/31 grafts) respectively. The early patency of internal thoracic artery graft was better than of saphenous vein graft.

Protein kinase C subspecies in adult rat hippocampal synaptosomes. Activation by diacylglycerol and arachidonic acid.

Synaptosomes isolated from the adult rat hippocampus contain the alpha- and beta-subspecies of protein kinase C (PKC), but not the gamma-subspecies which is abundantly expressed in the pyramidal cells in this brain region. Although the gamma-subspecies is known to respond significantly to free arachidonic acid, it is found that both the alpha- and beta-subspecies are also activated dramatically by arachidonic acid in synergistic action with diacylglycerol. Oleic, linoleic, and linolenic acids are all active. It is possible that unsaturated fatty acids may take part in the activation of alpha- and beta-subspecies of PKC which are present in the presynaptic nerve endings terminating at the hippocampal pyramidal cells.

Amino acid sequences of nerve growth factors derived from cobra venoms.

Amino acid sequences of nerve growth factors (NGFs), purified from the venoms of Indian cobra (Naja naja) and Thailand cobra (Naja naja siamensis) were determined. The sequence of N. naja NGF differed from that reported previously by Hogue-Angeletti et al. [(1976) Biochemistry 15, 26-34]. The sequence of N. naja siamensis NGF was identical to that of Formosan cobra Naja naja atra NGF, determined previously by Oda et al. [(1989) Biochem. Int. 19, 909-917] and to that deduced from the nucleotide sequence of an NGF cDNA from the venom gland of N. naja siamensis, as reported by Selby et al. [(1987) J. Neurosci. Res., 18, 293-298].

Effects of alar ligament transection on upper cervical spine rotation.

Fresh human cadaveric specimens of occiput (C0) to C3 were subjected to axial torque. The resulting physiological motions were studied in an unconstrained three-dimensional manner. Effects of sequential transections of the left and right alar ligaments on the relative motions of C0-C1 and C1-C2 were studied. After transection of the left alar ligament, ranges of motion--due to 1.5 Nm torque--increased at both the C0-C1 and C1-C2 joints. Increases were small, on average 1.9 degrees to each side and at each level. Increases due to subsequent cutting of the right alar ligament were, on average, only 0.5 degrees and statistically not significant. In general, neutral zones showed greater increases, e.g., 3.9 degrees to each side at the C1-C2 joint. Comparing right and left axial rotations, after transection of the left alar ligament, showed greater percentage increases for the right, as compared to the left, axial rotation, at both C0-C1 and C1-C2 joints. Functional loss of the alar ligaments indicates a potential for rotatory instability, which, however, must be determined in conjunction with other clinical findings, such as neurological dysfunction, pain, and deformity.

Bone graft translation of four upper cervical spine fixation techniques in a cadaveric model.

The goal of spinal fixation is to promote bony fusion by restricting motion at the site of the bone graft. Therefore, in order to evaluate the efficiency of various cervical fixation techniques, we determined the translations at the posterior arch of C1 for four C1-C2 posterior techniques: Gallie, Brooks, Magerl, and Halifax. Our model was the cadaveric specimen, with extensive soft tissue injury: transection of the alar, transverse, and capsular ligaments. Under three-dimensional physiological loading, we recorded the motion of C1 relative to C2, and calculated the translations at the surface of the graft in three dimensions, 10 specimens being tested intact, injured, and instrumented with each of the techniques. We assumed that translational laxity or neutral zone was the critical motion parameter and evaluated it, quantified herein as the neutral zone, at seven points at the graft site. The three-dimensional neutral zone translations were analyzed by their axial and shear components. We found that there was no significant difference with the fixation techniques in the average axial translation (Brooks: 1.1 mm; Magerl: 1.3 mm; Gallie: 1.5 mm; and Halifax: 0.5 mm). In shear, the Magerl averaged 1 mm, which was significantly less than the Gallie (2.1 mm). The Brooks (1.6 mm) and Halifax (1 mm) were not different from each other, Magerl, or Gallie. We propose that evaluation by translational laxity (neutral zone) at the graft site is a noteworthy concept in biomechanical analysis.

Interleukin-1 inhibits stress-induced gastric erosion in rats.

The effect of interleukin (IL)-1 on the occurrence of stress-induced gastric erosions was examined in rats. The intracerebroventricular (icv) administration of IL-1 beta significantly inhibited the occurrence of water-immersion restraint stress-induced gastric erosion at doses of 200 ng, 500 ng and 1 microgram, whereas the intravenous (iv) administration of IL-1 beta altered the occurrence of gastric erosion only at a dose of 1 microgram. The inhibitory effect of IL-1 alpha icv administered on the occurrence of gastric erosion was found only at a dose of 1 microgram. The inhibitory effect of IL-1 beta icv administered on the occurrence of stress-induced gastric erosion was not influenced by icv administration of alpha-helical CRF(9-41), a corticotropin-releasing factor (CRF) receptor antagonist. Indomethacin completely blocked the inhibitory action of IL-1 beta icv administered on stress-induced gastric erosion. It is concluded from these results that IL-1 acts mainly in the central nervous system to inhibit the occurrence of stress-induced gastric erosion and that the IL-1 beta-induced inhibition of gastric erosion is mediated by prostaglandin in a manner that is independent of brain CRF.

Effects of diethylstilbestrol and its methyl ethers on aneuploidy induction and microtubule distribution in Chinese hamster V79 cells.

We previously reported that diethylstibestrol (DES) and its derivatives inhibit the in vitro polymerization of microtubule proteins isolated from porcine brain (Sato et al., 1987). We found that the presence of the free hydroxy group of DES was indispensable for the inhibition of microtubule assembly. In the present investigation, this structure-activity relationship was confirmed by the effects of DES and its methyl ethers on chromosome number and the cellular microtubule architecture of Chinese hamster V79 cells, revealed by fluorescent anti-tubulin antibody. DES induced tetra- and octa-ploidy and DES monomethyl ether induced only tetraploidy at a much slower rate, whereas DES dimethyl ether was found to be completely inactive. Furthermore, DES was more active than its monomethyl ether in disturbing microtubule formation within cells. These results support the initial assumption that polyploidy is largely a consequence of the disturbed assembly of microtubules.

Androstenedione and progesterone concentrations in preovulatory follicular fluid correlate with successful fertilization and cleavage of human oocytes in vitro.

OBJECTIVE: To determine if androstenedione (A) and progesterone (P) concentrations in preovulatory follicular fluid (FF) correlate with successful fertilization and cleavage of human oocytes in vitro. DESIGN: A retrospective randomized trial. SETTING: Hospital department of obstetrics and gynecology. PATIENTS: Fifty-five patients, ages 24 to 39 years, with normal menstrual cycles undergoing in vitro fertilization (IVF) for tubal infertility. INTERVENTIONS: Multiple follicular development was induced with clomiphene citrate and human menopausal gonadotropin. MAIN OUTCOME MEASURES: Relationships among FF steroid hormone, morphological maturity of oocyte-corona-cumulus complexes, and fertilization and cleavage of oocytes. RESULTS: Follicles with mature oocyte-corona-cumulus complexes and unfertilized oocytes contained significantly greater amounts of A (P less than 0.05) than those with mature oocyte-corona-cumulus complexes and fertilized ova, indicating the occurrence of atretic changes. Follicles yielding successfully fertilized and cleaved ova had significantly greater amounts of P (P less than 0.05) and A (P less than 0.01) but similar levels of 17 beta-estradiol compared with follicles yielding fertilized ova that failed to cleave. CONCLUSIONS: Follicles yielding oocytes that cleaved as a result of IVF have both a shift in steroidogenesis from estrogen to progestin accumulation and declining aromatase activity, thus reflecting progressive luteinization of the follicles.

Flexion, extension, and lateral bending of the upper cervical spine in response to alar ligament transections.

The purpose of this in vitro experimental study was to determine the role of alar ligaments in providing flexion, extension, and lateral bending stability to the upper cervical spine. Ten fresh human cadaver specimens occiput-C3 were studied in a complete unconstrained and three-dimensional manner, first intact and then after sequential cutting of the left and right alar ligaments. At the C0-C1 joint, there were increases in flexion motion with sequential cutting of the alar ligaments but none in extension. For the same joint, cutting of the left alar ligament resulted in a significant increase in neutral zone in right lateral bending but not in left lateral bending, whereas there were no significant increases in the ranges of motion. At the C1-C2 joint, there were significant increases both in flexion and extension due to cutting of the left alar ligament, but subsequent cutting of the right alar ligament resulted in a small increase for flexion only. At this joint, right lateral bending increased due to cutting of the left alar ligament, but the same was not true for the left lateral bending. Subsequent cutting of the right alar ligament resulted in significant increases for both the right and left lateral bending.
