[A multi-center trial of MY-1, a new biological response modifier, on the immunological parameters of postoperative gastric cancer patients].

A multi-center study was performed to clarify the immunological effects of MY-1, a new biological response modifier, on postoperative gastric cancer patients. A total of 90 patients were randomly allocated either to an MY-1 treated or an untreated group. MY-1 was given 3 times a week from the 4th postoperative day for a period of 3 weeks. Immunological parameters of the patients, such as lymphocyte subsets measured with flow cytometry using various monoclonal antibodies, blast transformation responses against PHA, ConA, and PPD, natural killer activity of peripheral lymphocytes, and skin test for MY-1 and PPD, were evaluated preoperatively, before and after MY-1 treatment. The results of the two color staining method with flow cytometry showed that the population of lymphocytes positive for both OKT8 and Leu15 antigens (suppressor cells) was statistically less, and that the population of lymphocytes positive for OKT8 and negative for Leu15 antigens (killer cells) was statistically increased in patients treated with MY-1 compared with the control group.

Evaluation of treatment for gastric cancer with liver metastasis.

In 161 cases of gastric cancer with liver metastasis but without peritoneal dissemination, evaluations were executed to find effective treatment. The most favorable results with best prognosis were obtained in the group receiving gastrectomy + hepatectomy + chemotherapy, followed by gastrectomy + chemotherapy, and gastrectomy alone. The most unfavorable outcome was in nonresected cases. Chief chemotherapy to be used after gastrectomy was FML (5-fluorouracil (5-FU) + mitomycin C [MMC] + lentinan) continuous intra-arterial infusion. Hepatectomy was found to be effective as an active measure for tumor reduction in cases of liver metastasis so far as the metastatic lesions are only a few scattered ones in both lobes.

Suppressive action of ACTH on growth hormone secretion in patients with infantile spasms.

The changes in insulin-induced growth hormone secretion and in serum cortisol level were studied in 3 cases of West syndrome. The ACTH therapy consisted of an eight weeks course with gradual tapering every two weeks. Daily administration of 12.5 or 25.0 micrograms per kg ACTH for two weeks suppressed an insulin-induced rise in serum GH. The patients who showed sharply suppressed responses as to serum GH had been exposed to high cortisol levels of over 50 micrograms per dl serum. When they were examined before starting the therapy and 72 or 96 hours after the last ACTH injection, all the subjects showed a normal rise in the level of serum GH. The clinical implications of the findings were discussed in terms of the possible adverse effect on the developing brain.

Relationship between beta-lactamase activity and resistance to beta-lactam antibiotics in Mycobacterium smegmatis.

Penicillin-susceptible mutants and beta-lactamase-negative mutants were isolated from Mycobacterium smegmatis after nitrosoguanidine mutagenesis. Both the mutants were found to be susceptible to low levels of penicillin and cephalosporins by twofold dilution testing. Clavulanic acid reduced the minimal inhibitory concentrations of beta-lactamase-labile beta-lactams for the penicillin-susceptible mutants and the parent strain, but had no effect on the susceptibility of the beta-lactamase-negative mutants. Comparison of the beta-lactamase activities found in these mutants and the parent strain indicated that there was a rough correlation between the beta-lactamase level in these organisms and their susceptibility to beta-lactams.

[Local irritation of isotonic acetic acid solutions of the rabbit M. vastus lateralis and confirmation of the injected sites].

One ml of 0.106%, 0.425% or 1.7% acetic acid (AA) solution, isotonic to physiological saline solution, was injected into the M. vastus lateralis of male Japanese white rabbits. Two, 7, 14 and 21 days after the injections, the rabbits were autopsied, and the removed muscles were observed macroscopically. After fixation with 10% neutral formalin, they were examined histologically. Hemorrhage and white degeneration of the muscles were grossly observed in 0.106%, 0.425% and 1.7% AA at 2 days. These lesions disappeared at 7 days and thereafter in 0.106% AA, and at 14 days and thereafter, except for one animal, in 0.425% AA. In 1.7% AA, these lesions were observed until 21 days, but gross hemorrhage began to recover at 21 days. The size of the lesion, which was expressed as a product of the major axis (mm) and minor axis (mm) of the lesion, showed reduction over time, especially in 1.7% AA. Histologically, swelling, necrosis and calcification of the muscle fibers and cell infiltration, hemorrhage and dilatation in the interstitium were observed after the injections of 0.106%, 0.425%, and 1.7% AA. At 7, 14 and 21 days, regeneration of the muscle fiber and fibrosis in the interstitium were observed. The injected sites were examined by injecting a chip of test-tube brush as a marker. In 20 (83%) of 24 cases, it was ascertained that the markers were injected into the M. vastus lateralis. The markers were found in the M. vastus intermedius in 3 out of 4 other cases and in the intermuscular tissue between the M. vastus lateralis and M. tensor fasciae latae in one case.

[Studies on antigenicity of difluprednate--antigenicity of difluprednate, phototoxicity and photocontact sensitivity of difluprednate ointment and cream].

The antigenicity of difluprednate, phototoxicity and photocontact sensitivity of difluprednate ointment and cream were studied in guinea pigs and mice. The results in this study were as follows. Guinea pigs immunized with difluprednate emulsified with Freund's complete adjuvant did not exhibit the systemic anaphylaxis and the delayed type hypersensitivity by the elicitation with either difluprednate alone or difluprednate-BSA conjugate. The antisera obtained from these guinea pigs did not show the positive response in the homologous 4-hour PCA reaction and the passive hemagglutination test against the challenge of the same antigens described above. In the maximization test, guinea pigs immunized with difluprednate did not show the contact sensitivity when elicited with difluprednate suspended in saline. Guinea pigs applied difluprednate ointment did not show positive contact sensitivity by the elicitation with difluprednate ointment and ointment base. The antisera obtained from mice immunized with difluprednate and difluprednate-OVA conjugate absorbed to Al (OH)3 gel did not give the positive response in the 72-hour PCA reaction against the challenge of either difluprednte alone or difluprednate-BSA conjugate. In the phototoxicity and photocontact sensitivity test, 0.05% difluprednate ointment and 0.05% difluprednate cream did not show positive reactions in guinea pigs, although very slight erythema was caused by the primary irritancy of cream base, with or without irradiation of ultraviolet ray (320-400 nm).

[Artificial renal supports in surgery--present status and problems].

Clinical and surgical situation and problems of the blood purification as an artificial renal supports, including hemodialysis, hemofiltration and hemoadsorption, were studied especially fields related with treatment of acute renal failure (ARF), surgeries in the patients treated with chronic hemodialysis and supportive care for the cadaveric renal transplantation. ARF: Our experimental data using septic rats showed that hemoadsorption activated host's reticuloendothelial function and consequently increased survival rate of septic rats. Clinically, similar results were observed by the combination of hemodialysis and hemoadsorption, and the survival rate of ARF with multiple organ failure increased to 68% (21 out of 31 cases) from 30% (8 out of 27) in the patients treated with only hemodialysis. Surgery in the chronic hemodialysis patients: One hundred twenty two operations have been done among the patients treated with chronic hemodialysis in our clinic. However, 15 in emergency cases were died within post operative period. There is no operative death in scheduled operations including total esophagectomy and simultanous radical operation of gall bladder cancer and colon cancer. Pre- and post operative supportive management with immunopotentiator, opsonic protein, coagulative factors and intravenous hyperalimentation with branched chain rich amino acid solution should be added routinely to artificial renal supports in pre- and post operative period. Cadaveric renal transplantation: Eighty-seven percent of cadaveric renal transplantation in our clinic were needed artificial renal support in early phase of post transplantation period because of insufficient renal function by acute tubular necrosis. Necessity of the acceptance of brain death category in Japan is strongly pleaded.

Dissociation and oxygen equilibrium properties of the extracellular hemoglobin of Eisenia foetida.

The dissociation and oxygen equilibrium properties of whole blood and the purified hemoglobin from Eisenia foetida were compared. Oxygen affinities agreed approximately with each other in the range of pH 6.0 to 9.5. The values of n1/2 were higher in whole blood than in the purified hemoglobin between pH 7.0 and 9.5. The maximum values, obtained near pH 8, were about 6 in whole blood and 3.5 in the purified hemoglobin. In the purified hemoglobin, alkaline dissociation started at pH 7.8, and the approximately 60 S whole molecule dissociated completely into approximately 10 S and 5-6 S components at pH 9.1. In whole blood, however, the dissociation started at pH 8.2 and the complete disappearance of the approximately 60 S molecule occurred at pH 9.6. The values of n1/2 for the dissociation products were lower than those of the purified hemoglobin between pH 7.0 and 9.0. The value of n1/2 decreased with increasing dissociation of the approximately 60 S whole molecule with a pH rise in both whole blood and the purified hemoglobin. Addition of CaCl2 or MgCl2 up to 10 mM to the purified hemoglobin at pH 8.0-8.1 induced increases in oxygen affinity and cooperativity and in the stability of the approximately 60 S whole molecule. The effect on the oxygenation properties was greater with CaCl2 than MgCl2 at the same molar concentration. The stabilizing effect on the approximately 60 S molecule was almost the same with both CaCl2 and MgCl2. These results suggest that the dissociation of property of the hemoglobin in whole blood is controlled by both Ca2+ and Mg2+, and that its oxygenation property is controlled by Ca2+.

Enforcement of specific unresponsiveness by the long-term presence of allogeneic heart grafts.

Mechanisms underlying the maintenance of long-term heart allografts were analyzed in rats treated with Cyclosporine. It was shown that acceptance of allografts after cyclosporine did not involve the attenuation of immunogenicity of the grafts. This conclusion was drawn from two observations: (1) Pretreatment of the donors with cyclosporine did not cause prolongation of graft survival time; and (2) cyclosporine stabilized allografts were normally rejected by secondary recipients when retransplanted on day 30. Studies of the acceptance of skin grafts from the heart donor strain indicated the existence of a mechanism to maintain donor-specific unresponsiveness in the presence of a stable allograft in a time-dependent manner. Thus, the mean rejection time of F344 skin grafts on WKA rats bearing F344 hearts was more than 80 days when transplanted on day 210, but it was 32 days when they were transplanted on day 30. Active participation of specific suppressor cells in the maintenance of unresponsiveness was suggested because data obtained in the cell transfer experiments was statistically significant. Transfer of lymphocytes from rats bearing long-term grafts showed a tendency to delay rejection of skin grafts by sublethally irradiated hosts, but it failed to delay the rapid rejection by normal lymphocytes cotransferred to the same recipients.

Role of IA-positive cells in the beneficial effect of donor blood transfusion and induction of suppressor cells in cardiac allotransplantation of rats.

An attempt was made to reveal the role of Ia-positive cells in the beneficial effect of pretransplant donor-specific blood transfusion (DST) and induction of suppressor cells in the cardiac allotransplantation of rats. Mean graft survival time (MST) of F344 (RT1 1v1) hearts transplanted in WKA (RT1k) rats was 6.5 +/- 0.8 days. Pretreatment of recipients with 1 ml DST 10 days before grafting prolonged it to 56.5 +/- 38.1 days. Plasma or erythrocytes did not prolong MST at all, whereas pretreatment with 1 X 10(7) lymphocytes prolonged MST significantly. B enriched cells separated on a nylon-wool column prolonged MST, but T-enriched cells did not. Treatment of lymphocytes with interspecies crossreactive monoclonal anti-Ia antibody and complement eliminated the ability of lymphocytes to prolong MST, suggesting an important role of Ia-positive cells in the beneficial effect of DST. The role of Ia-positive cells in prolonging graft survival, was analyzed by a cell and serum transfer experiment. MST of F344 heart allografts transplanted in sublethally irradiated (500 rads) WKA rats was 17.4 +/- 4.0 days. MSTs of allografts in rats irradiated and treated with serum or spleen cells of blood-conditioned rats were 23.2 +/- 1.6 (P less than 0.05) and 5.2 +/- 0.7 (NS) days, respectively. On the other hand, MSTs of the grafts in rats irradiated and treated with serum or spleen cells harvested from rats that had accepted grafts for 30 days were 18.7 +/- 1.9 (NS) and 63.8 +/- 29.9 (P less than 0.01) days, respectively. The results indicate that the initial role of a serum factor in prolonging graft survival induced by DST containing Ia-positive cells was followed by activating suppressor cells by the presence of the allograft.
