OBJECTIVE: To determine whether injection of patients with Bicillin CR is less painful than injection with Bicillin LA. To discover if Bicillin CR with the addition of procaine, which doubles the volume, causes the injection to be less painful. DESIGN: An experimental, double-blinded crossover design was used for this study. SETTING: University children's and women's tertiary care emergency Department (ED) with an annual pediatric census of 22,000. PARTICIPANTS: A convenience sample was enrolled from the student body of a large university and house staff, and employees of a 152 bed children's and women's hospital in southern Alabama. The sample size was limited to 50 participants, all of whom completed the study. METHODS: Each participant received two penicillin injections, one Bicillin CR and one Bicillin LA, and rated the pain of the injection immediately after the injection, 1 hour after the injection and 12 hours after the injection. A visual analogue scale was the tool used for measuring the pain. The penicillin injections were randomized using a random number generator. RESULTS: For each of the three periods, comparisons of pain were made between the Bicillin CR versus Bicillin LA injections. Bicillin LA pain score values were consistently higher for all but the 12-hour comparison. These differences were statistically significant (P < 0.008-0.002). CONCLUSIONS: Injection of Bicillin CR with the addition of procaine to the benzathine penicillin G is statistically significantly less painful than the injection of Bicillin LA without the addition of procaine to the benzathine penicillin G, even though the Bicillin LA is one-half the volume of the Bicillin CR due to the addition of procaine to the Bicillin CR.
Rates of oxygen consumption and CO(2) production were measured in Virginia opossums (Didelphis virginiana) during fasting and prolonged exercise to quantify changes in total energy expenditure and oxidative fuel selection. We hypothesized that fasting would cause metabolic depression and a progressive shift towards lipid utilization to spare alternative substrates. It was also predicted that prolonged exercise would cause the same relative changes in fuel preference as fasting, but on a compressed time scale. The results show that hypometabolism is not used by the Virginia opossum to cope with food deprivation. However, a rapid exhaustion of limited carbohydrate reserves is prevented through a sixfold reduction in the percentage contribution of carbohydrates to total energy expenditure made possible by an increase in lipid utilization. No protein sparing is observed in this species. Prolonged low-intensity exercise elicits a potent mobilization of lipids that allows maximal running time to be extended by delaying the depletion of limited carbohydrate reserves. We conclude that fasting and prolonged low-intensity exercise cause similar changes in the relative use of lipids and carbohydrates, but on a different time scale, supporting the idea that endurance exercise is the metabolic equivalent of 'accelerated fasting'. The absence of metabolic depression and protein sparing during fasting shows that such physiological strategies have not been necessary for the rapid range expansion towards the North recently shown by this species.

The adoption of children reared in poor quality Romanian institutions provided a "natural experiment" to determine whether psychological privation is the cause of later deficits. Low IQ at six years was strongly related to the duration of institutional care, with malnutrition having a weaker effect. After taking both into account, there was an additional effect of a particularly marked lack of individualized care.
The authors measured gastric emptying in 13 patients undergoing radiation therapy to the chest, abdomen, or pelvis for nongastrointestinal cancer to investigate whether gastric emptying (GE) was altered by this therapy. Symptoms and weight were monitored at regular intervals. Patients served as their own controls and were compared to a group of healthy subjects. When studied prior to radiation therapy (baseline), cancer patients had a gastric emptying rate that was similar to a healthy control group (t1/2 mean+/-SEM 92.0+/-15.3 vs. 80.4+/-8.2 min). Irradiation did not change the emptying rate, either after the first dose (early) of 180 cGy (t1/2 99.5+/-17.9) or after 2 weeks of therapy (late) with 3000 cGy (t1/2 75.5+/-7.3). There was no correlation of radiation field or tumor type with gastric emptying rate. Two of the 13 patients experienced nausea and vomiting during their course of radiation, but their gastric emptying was unchanged from baseline.
