Morphological and cytogenetic studies of a human synovial sarcoma xenotransplanted into nude mice.

Specimens of a synovial sarcoma from the left thigh of a 60-year-old woman and cells from the tumor transplanted into nude mice were examined morphologically and immunohistochemically, and the karyotype was analyzed. Immunohistochemically, cells of both the parent tumor and the transplanted tumor were positive for vimentin and cytokeratin. The multicystic features of the parent tumor were not reproduced in the mice. On ultrastructural examination, the parent tumor cells were found to have dilated channels of endoplasmic reticulum as well as a junctional apparatus, and they formed extracellular spaces. The transplanted tumor cells, in contrast, had a poorly developed endoplasmic reticulum and were devoid of extracellular spaces. Chromosome analysis of the tumor cells revealed a translocation, t (X; 18) (p11; q11), as reported previously for synovial sarcoma, thus suggesting the diagnostic utility of the chromosome pattern for identification of synovial sarcoma.

[A case of non-functioning adrenocortical tumor].

A resected case of a non-functioning adrenocortical tumor is reported. The patient's chief complaint was fever and general fatigue. Computed tomography and ultrasonography revealed the suspected presence of a tumor on the right lobe of the liver, though a definite diagnosis of a right adrenal tumor was reached after hepatic angiography. An endocrinal examination, however, showed no abnormality. The tumor was dissected sharply from the right hepatic lobe and completely removed with a part of the diaphragm. It measured 10 x 12 x 18 cm and weighed 1,280 g. Hemorrhagic and necrotic changes were noted in the cut section, and a subsequent pathological examination revealed an adenocarcinoma of the adrenal cortex. An administration of 5-FU was given orally and the patient's progress has gone well without a recurrence or a metastasis for 4 years and a month since the surgery.

Osteochondritis dissecans of the distal radioulnar joint.

To our knowledge there has been no report of osteochondritis dissecans of the distal radioulnar joint. This patient was seen with episodes of painful motion in the left wrist. At operation two free bodies were removed from the distal radioulnar joint. From the clinical, radiographic, operative, and histologic findings, the diagnosis was osteochondritis dissecans of the distal radioulnar joint.

CD8+ cell-dominant alveolitis in pulmonary sarcoidosis.

A 55-year-old woman had bilateral hilar lymphadenopathy and retinouveitis. Microscopic examination of a biopsy specimen from a mediastinal lymph node revealed noncaseating epithelioid cell granulomas. Since granulomatous diseases of infectious origin and sarcoid reaction were reasonably excluded, she was diagnosed as having pulmonary sarcoidosis. Bronchoalveolar lavage findings were atypical in that the ratio of helper T cells to suppressor T cells (CD4/8 ratio) was 0.5 in two separate examinations, despite a moderately increased proportion of lymphocytes. Immunohistochemical examination of the lymph node also showed a predominancy of suppressor T cells. The inversion of the CD4/8 ratio suggests the presence of heterogeneity in the immunoregulatory mechanism in pulmonary sarcoidosis and may have a prognostic significance.

[A case of pulmonary sarcoidosis with CD8 positive cell dominant alveolitis].

A 55-year-old woman was admitted for examination of bilateral hilar lymphadenopathy. Serum angiotensin converting enzyme was not elevated, but gallium-scan revealed an increased uptake in both lung hilus. The percentage of lymphocytes in the bronchoalveolar lavage fluid (BALF) was elevated (28.9%), and the proportion of OKIa1 positive cells was also elevated (50.5%). However, the ratio of cluster differentiation (CD) 4/8 was inverted at 0.5. Microscopic examination of the mediastinal lymph nodes revealed epithelioid cell granulomas without ceasation, and a diagnosis of sarcoidosis was made. Immunohistochemical examination of the lymph nodes showed predominancy of CD8 positive cells. Patients with pulmonary sarcoidosis having a low CD4/8 ratio in BALF are extremely rare in the early stage of sarcoidosis. The inverted CD4/8 ratio was of interest in regard to the pathogenesis of sarcoidosis.

[Clinicopathologic investigation of 63 cases of pituitary adenoma--special reference to immunohistochemical study of hormone production].

We have referred 63 cases of pituitary adenoma during past 10 years at department pathology of Hokkaido University Hospital. The mean age at diagnosis was 38 years, with a range from 13 to 73 years. 35 patients complained of non-hormonal sign (head ache, disturbance of vision, disturbance of consciousness, fatigability), 32 patients presented with hormonal sign (acromegaly, troubles of menstruation, galactorrhoea, cushing syndrome) and 7 patients had both signs. Immunohistochemically, GH cell adenoma was 16 cases, Prolactine cell adenoma was 11 cases, ACTH cell adenoma was 4 cases, Non-functioning adenoma was 19 cases and Mixed type was 10 cases.

Diabetes insipidus caused by lymphocytic infundibuloneurohypophysitis.

Pathologic examination at autopsy of a 74-year-old man with central diabetes insipidus revealed that he had a chronic lymphocytic inflammation limited to the infundibulum, stalk, and posterior lobe of the pituitary gland. No meningitis, sarcoidosis, or granulomas were detected, though there was evidence of chronic pancreatitis. In addition, neuronal loss with gliosis was observed bilaterally in the supraoptic and paraventricular nuclei. The unusual localized inflammatory brain lesion was considered to be responsible for the diabetes insipidus.

[Cystic thyroid masses: ultrasonographic and pathologic correlation].

Forty-nine patients with 63 cystic thyroid masses who had undergone preoperative sonography were retrospectively reviewed. 52 lesions (83%) were benign and others (17%) were malignant. Among various sonographic findings of cystic thyroid masses, oval cystic lesions with polyp or dome like solid component projecting into the lumen were all diagnosed adenomatous goiter. Irregular cystic structures with more than 2 cm finger like pedunculated mass extended into and/or out of the lumen were all diagnosed papillary carcinoma. Small oval cysts (less than or equal to 1 cm) with strong echo were all diagnosed colloid goiter. The other sonographic type of cystic thyroid masses had somewhat malignancy (12-30%), not having characteristics which differentiate benign from malignant lesions. Pathologic findings of malignant lesions showed that cancer cells existed rather in pericystic portion than in cyst wall except for finger like solid component projecting into the lumen which was papillary carcinoma itself. Ultrasonically guided needle biopsy should be performed to get samples of above portions.

The alteration in the pattern of pulmonary metastasis with adjuvant chemotherapy in osteosarcoma.

Alterations in the pattern of pulmonary metastasis of patients with osteosarcoma treated with adjuvant chemotherapy were studied. Thirty two patients who were treated with both radical surgery and adjuvant chemotherapy were observed (chemotherapy group). As a control, sixty two patients treated with radical surgery alone were also assessed (non-chemotherapy group). The chemotherapy group demonstrated a reduction in the number and a delay in the appearance of metastases. The tumour doubling time did not differ between the chemotherapy and non-chemotherapy groups. The initial site of metastasis among the chemotherapy group was most commonly in the lower lung field, whereas among the non-chemotherapy group it was usually to the middle lung field. The chemotherapy group survived longer than the non-chemotherapy group after developing pulmonary metastases. Histological examination showed that the metastatic foci stimulated the primary sites.

High-grade surface osteosarcoma of the left ilium. A case report and review of the literature.

A rare case of high-grade surface osteosarcoma of the left ilium is reported. Trephine biopsy performed on a 31-year-old woman suffering from a huge tumor of the left buttock revealed high-grade osteosarcoma, and hemipelvectomy was undertaken. The surgical specimen showed that the tumor was present on the surface of the left ilium. Ten months after the operation, the patient died of brain metastasis. From these results, we considered this case to be one of high-grade surface osteosarcoma.

Steroid-induced osteonecrosis of the patella.

Two cases of previously unreported steroid-induced osteonecrosis of the patella were investigated. The first case was a 46-year-old man who had been receiving steroid therapy for three years for treatment of bronchial asthma. The second was a 32-year-old woman who had been receiving steroid therapy for nine years as treatment of systemic lupus erythematosus. Both reported slight knee pain. The roentgenographic findings revealed increased radiodensity with radiolucency of the upper one-half of the patella. A trephine biopsy revealed focal areas of dead trabecular bone.

Cytomorphologic features of antiallergic drug-induced eosinophilic cystitis with bronchial asthma.

The cytomorphologic features of voided urine specimens from two patients with antiallergic drug (Tranilast)-induced eosinophilic cystitis are described. The urothelial cells tend to occur in clusters and are of variable size. The remarkable vacuolization is observed in the cytoplasm. The markedly vacuolated cytoplasm which may be infiltrated by polymorpho-nuclear leucocytes is unremarkable. Some nuclei usually show transparent center surrounded by a rim of chromatin. The nucleoli are prominent. Marked vacuoles in the nucleus is observed. These findings are very important cytomorphologic characteristics for differential diagnosis between eosinophilic cystitis and malignant bladder tumor and cystitis due to bacterial infection.
