Inhibitory effects of rabbit antisera on mitogenic activity of Actinobacillus actinomycetemcomitans lipopolysaccharide.

Lipopolysaccharides (LPSs) were isolated from Actinobacillus actinomycetemcomitans strains ATCC 29523 (serotype a), Y4 (b), and NCTC 9710 (c) by the hot phenol-water procedure. Y4 lipid A was obtained by the hydrolysis of Y4 LPS in 1% acetic acid. All the LPS preparations and Y4 lipid A were mitogenic for C3H/HeN mouse spleen cells, but not for C3H/HeJ mouse spleen cells. Immunoglobulin preparations partially purified by ammonium sulfate precipitation at 33% saturation from rabbit antisera against Y4 whole cells inhibited the mitogenic response of C3H/HeN mouse spleen cells to LPSs from all the strains of A. actinomycetemcomitans and Y4 lipid A. Anti-Y4 LPS immunoglobulin preparation inhibited the mitogenic activity of Y4 LPS and Y4 lipid A. Furthermore, anti-Y4 whole cell Fab fragments inhibited the mitogenic activity of both Y4 LPS and Y4 lipid A. These results suggest that antibodies against A. actinomycetemcomitans LPS may modify immune responses of lymphocytes to this organism at periodontal sites.

Immunochemical and structural characterization of a serotype-specific polysaccharide antigen from Actinobacillus actinomycetemcomitans Y4 (serotype b).

A serotype-specific polysaccharide antigen of Actinobacillus actinomycetemcomitans Y4 (serotype b) was extracted from whole cells by autoclaving. The extract was purified by chromatography on DEAE-Sephadex A-25 and Sephacryl S-300 columns. The purified polysaccharide antigen formed a single precipitin line with anti-type b serum but not with anti-type a serum and anti-type c serum. The antigen was composed of 43.9% L-rhamnose, 49.1% D-fucose, and a trace amount of fatty acid. Methylation analysis, Smith degradation, and optical rotation data showed that the antigen was a polymer consisting of a disaccharide repeating unit, ----3)-alpha-D-fucopyranosyl-(1----2)-beta-L-rhamnopyranosyl-(1----. In quantitative precipitin inhibition tests, D-fucose and L-rhamnose showed very low inhibition, but the partial hydrolysate of the purified antigen was an effective inhibitor, suggesting that the serotype b specific antiserum recognizes the larger oligosaccharide units.

Papillotubular adenoma of the ampullary region.

Papillotubular adenoma of the ampullary region in a rare tumor which has the potential for malignant transformation. Although reported more frequently nowadays, ampullary tumors are difficult to diagnose before operation unless they cause obstructive jaundice. Occasionally they are detected by ultrasonography, computed tomography or ERCP in patients who complain of nonspecific upper abdominal discomfort. We report a case of a periampullary papillotubular adenoma causing generalized pruritus and weight loss but no jaundice. The tumor was extirpated by pancreaticoduodenectomy, and the patient has remained healthy for over five years.

Morphology of lymphatics of the mammalian heart with special reference to the architecture and distribution of the subepicardial lymphatic system.

The subepicardial lymphatic system in the rat and dog heart has been investigated by means of scanning electron microscopy. Following application of hydrogen peroxide, the epicardium was removed with a forceps under a dissecting microscope. The subepicardial region contained a well-developed lymphatic system which consisted of the main lymphatic trunks and lymphatic capillaries. The lymphatic trunks of large diameters ran from the apex of the heart to its base. The subepicardial lymphatic capillaries were ramified and anastomosed with each other to form a relatively dense network which extended over the entire surface of both ventricles. These networks joined the main lymphatic trunks. Further, some similar networks were connected with the underlying myocardial lymphatic capillaries.

Studies on sustained-release suppositories. II. Evaluation of polymer electrolyte containing acidic groups for prolonged rectal absorption of bacampicillin in rabbits.

Rectal absorption of bacampicillin hydrochloride (BAPC) was found to show the best bioavailability with Witepsol H-15 as suppository base among various Witepsol bases. However, an effective plasma concentration of drug (above 0.5 micrograms/ml) was only maintained for 2 h, so sustained-release suppositories of BAPC were studied. Bacampicillin reacts with acidic polymer electrolytes such as pectic acid (Pc), chondroitin sulfate (Cd) and precipitates as its adduct with the polymer in an aqueous solution. The dissolution rate of BAPC from the adducts in a solution was slower than that of BAPC itself. The absorptions of BAPC from the suppositories containing the adducts were prolonged, but the bioavailabilities were decreased compared to that from the suppository containing BAPC alone. Similar prolonged absorption could be obtained simply by mixing Pc or Cd with BAPC in a base. Further, the absorption rate was found to be controlled by the amount of the polymer addition, and both a high plasma level and excellent bioavailability were obtained. This desirable outcome may be due to the simultaneous occurrence of rapid absorption of BAPC itself and formation of the adducts.

Studies on sustained-release suppositories. I. Effect of alginic acid addition on rectal absorption of bacampicillin in rabbits.

Sustained-release suppositories of bacampicillin (BAPC) were prepared by the use of the adduct which was precipitated from an aqueous solution containing BAPC and alginic acid (Alg). As the suppository base, Witepsol H-15 and macrogol were used. Absorptions of BAPC from the suppositories were prolonged in rabbits, but the bioavailabilities were decreased compared to that after administration of BAPC alone. However, these absorptions were improved enormously by the addition of surface-active agents, that is, an excellent prolonged absorption and high bioavailability were obtained. Interestingly, similar prolonged absorption could be obtained only by mixing Alg with BAPC in a suppository base. Further, this absorption rate was found to be controlled by the amount of Alg addition. The absorption profiles from a suppository containing the mixture differed from that containing the adduct in exhibiting both high plasma level and prolonged absorption. This may be due to simultaneous fast absorption of BAPC itself and formation of the adducts. Thus, it seemed that BAPC preparations containing Alg may be practically useful as a rectal preparation with prolonged action and giving a high plasma level.

Isolated ACTH deficiency associated with transient thyrotoxicosis and hyperprolactinemia.

A 43-year-old woman with isolated ACTH deficiency in association with transient thyrotoxicosis is reported. The initial evaluation revealed that plasma ACTH and cortisol did not respond to corticotropin-releasing hormone (CRH) in the presence of hyperthyroxinemia and hyperprolactinemia. During the replacement therapy with dexamethasone, she developed transient hypothyroxinemia with persistent hyperprolactinemia. Although thyroid open biopsy did not show any evidence of autoimmune thyroiditis or subacute thyroiditis, the data appear to provide other evidence of a possible relationship between acute adrenal insufficiency and transient thyroid dysfunction.

[Effect of Ca level on 65Zn absorption at the ligated duodenal loop of rat].

Effect of calcium concentration on Zn absorption was studied in rats. Administration of Zn solution with calcium through gastric tracts depressed markedly the levels of Zn in serum taken from portal vein. Dependency on calcium concentrations of the absorption and distribution of Ca in the body was determined using in situ administration of Zn into ligated duodenal loop of rats. Administration of Zn with high calcium solution decreased significantly the levels of Zn in serum and all the tissues compared with the administration with low and medium calcium solutions. A considerable disease in the level of Zn was observed in the liver and kidney compared with the rats administered with medium and low calcium solutions.

[Clinical application of tetracycline-containing strips (LSD) in periodontal disease].

The purpose of this study was to evaluate the clinical application of strips containing 10% Tetracycline (TC) in periodontal disease. In this double blind study, placebo strips, which did not contain TC, were also prepared. Ninety-two sites in 46 cases with greater than a 4 mm periodontal pocket were randomly assigned to experimental and control study groups. The following clinical parameters at each site were recorded at Weeks 0, 1, 2 and 3 after insertion of the LSD or placebo strip: plaque and calculus score, probing depth, gingival inflammation (redness, swelling, bleeding on probing, pus discharge), mobility of the tooth, pain on insertion of strip and pain after the procedure. The residual quantity of TC in the periodontal pocket was also determined at 1, 2 and 7 days after inserting the LSD. All clinical data was evaluated using 3 types of statistical test (X2 test, Mann-Whitney's U test and Wilcoxon's test). Thus, the following results were obtained: 1. The LSD group showed much improvement in clinical condition compared with the placebo group. 2. The conditions of bleeding, redness or swelling of gingival tissue were much improved in the LSD group, even after data evaluation with all 3 types of statistical test. 3. The residual quantity of TC was decreased day by day until finally it was not recognized in the 7 day samples. 4. This study suggested that the application of LSD in the periodontal pocket could be effective in periodontal treatment.

Histochemical studies of the vestibular nerve system in the developing chick embryo. Glycoconjugates in the vestibular end organs.

Glycoconjugates of the vestibular end organs in the developing chick embryos were well defined and localized using lectins-HRP (WGA, PNA, UEA-1) and HID-AB stains. In spite of the fact that no differentiation of the otic vesicle was seen on the 4-day-old chick embryos, the luminal surface of the otic vesicle was labeled with lectins. The cupula and otoconia were labeled most intensely with lectins on the 6-day-old chick embryos. The reactivity of lectins with the cupula and otoconia was as follows: WGA greater than UEA-1 greater than PNA. As the vestibular end organs had become mature, the epithelial cells of the semicircular duct, utricle and saccule were labeled with lectins. These findings suggest that glycoconjugates play an important role in the development and formation of the vestibular end organs.

[New approach for unresectable primary liver cancer--intra-arterial combination chemotherapy during and after operation with local injection of ethanol].

From 1976 to 1988, hepatic arterial infusion chemotherapy was performed in 52 patients with unresectable hepato-cellular carcinomas. Forty-nine (94%) out of 52 patients were stage III or stage IV. Thirty-five patients underwent continuous infusion of 5-FU and MMC employing chronometric infusion pumps. Mean survival of this group was 9 months. Six patients survived more than 12 months; the longest survivor died of different causes 40 months after surgery. Another patient lived for 30 months. In 10 patients, hepatic arterial ligation was performed in combination with infusion chemotherapy in the hepatic artery. Mean survival of these patients was 14 months. Four out of the 10 patients survived over 12 months. Recently, in 7 patients whose liver tumors were less than 4 cm in diameter and/or whose tumors were multiple and had liver disfunction, ethanol injection into the tumors during laparotomy was performed in combination with hepatic arterial infusion chemotherapy. In these 7 patients, an injection port system was implanted subcutaneously for intermittent infusion chemotherapy of MMC + 5-FU and ADM + lipiodol. Mean survival in this group was 14 months. One of 7 patients died 21 months after operation, but the others are alive at this writing. Five of these 6 patients have survived more than 12 months. AFP levels decreased in 3 patients after these treatments. In one case, AFP decreased to normal range for 18 months. These results suggested that intraoperative ethanol injection therapy into the tumors combining with hepatic arterial infusion chemotherapy may be of value for unresectable smaller liver carcinomas, since this method is technically easy and can assure injection of ethanol into the tumors during laparotomy.

[The significance of lipid peroxidation (free radical) on hepatocellular dysfunction following hepatectomy].

The possibility of extended hepatectomy was examined clinically and experimentally, with special reference to relation between remnant liver dysfunction and lipid peroxidation. Clinical study: When the functional reserve of the remnant liver prior to hepatectomy was below the critical level, both serum phospholipase A2 and plasma lipid peroxide increased and severe hepatic insufficiency occurred. Experimental study in dogs: The amount of lipid peroxide in the plasma and remnant liver increased more after 84% hepatectomy than after 70% hepatectomy, causing significant impairment of the remnant liver function. To prevent hepatic dysfunction after hepatectomy, Coenzyme Q10, an antioxidant and a membrane stabilizer, were administered. These substances reduced the increase of lipid in the plasma and remnant liver, and restored the liver function with subsequent good regeneration. The results suggest that the administration of Coenzyme Q10 may be useful for recovery of liver function and a more favorable prognosis following the extended hepatectomy.

An experimental study on reconstruction of the posterior cruciate ligament using the medial meniscus.

The posterior cruciate ligament (PCL) was experimentally reconstructed using the medial meniscus and preserving its posterior attachment in 70 rabbits. Time-course changes in the transferred meniscus were studied macroscopically, histologically and biomechanically to evaluate the validity of the meniscus as a substitute in ligament reconstruction. Observation of the transferred menisci for 144 weeks revealed that the fibrocartilage cells and the matrix of the transferred menisci were not completely replaced, especially in the central part. Blood circulation of the transferred meniscus was relatively abundant at the invasion site of the synovial tissue, particularly its attachments where the transferred meniscus was attached to the femur and tibia, whereas the central part was deficient in blood circulation. The mechanical strength of the transferred meniscus was about 26% of that of the PCL on the control side, and was not related to the period of time after the operation. Therefore, reconstruction of the PCL using the medial meniscus alone was considered questionable from the viewpoints of strength, slow absorption and replacement in clinical applications. This procedure, however, is still useful for augmentation of acute PCL tears.

Multiplication of the dictyosome during the formation of autospores in the green alga Chlorococcum infusionum.

Numerical changes in dictyosomes during the formation of autospores in a green alga, Chlorococcum infusionum, were investigated by electron microscopy. Two dictyosomes were seen near the nucleus in young vegetative cells. Four dictyosomes were seen in large mononucleate cells which appeared to enter mitosis soon. Binucleate cells contained 4 or 8 dictyosomes, the latter number being found in the large binucleate cells. Large tetranucleate cells contained 16-25 dictyosomes in each cell. Dictyosomes consisted of about 4 cisternae with a diameter of 0.4-0.6 micron in mono- to tetranucleate cells. Cytokinesis began with the formation of the septa during the third nuclear division, and 16 cells were finally formed. Dictyosomes did not increase in number in 8- and 16-nucleate cells. In septum-forming cells, dictyosomes were 0.6-1.0 microns in diameter, with 6-9 cisternae. A single dictyosome was included in each of the 16 resultant cells. These observations suggest that the dictyosomes multiply in association with the multiplication of the nuclei without correlation with formation of the cell wall or cytokinesis.

[Sepsis in patients with hematological diseases].

Sepsis is one of the important complications on the treatment of severe hematological diseases. In this report, we analyzed sepsis in 309 patients with hematological diseases who were admitted to the First Department of Internal Medicine of Yokohama City University Hospital from 1979 to 1986. Positive blood culture were found in 17.8% (55/309 cases) and total positive cases were 73 including recurrent patients. Positive rate by underlying diseases was 30.3% in acute leukemia, 20.8% in chronic myelocytic leukemia, 17.2% in aplastic anemia, 8.0% in multiple myeloma, 6.0% in malignant lymphoma and 6.5% in others. The organisms causing sepsis were as follows; gram negative bacilli 56.4%, gram positive organisms 34.6%, fungus 6.4% and anaerobic bacteria 2.6%. Pseudomonas aeruginosa was found in 19.2%. The mortality rate of patients with sepsis was 34.2% (25/73 cases). The significant prognostic factors in patients with sepsis were the degree of neutropenia, duration of neutropenia (500 less than microliters), the species of organisms, simultaneous complication with shock and the site of other infections.
