Review of 108 patients with hepatolithiasis showed a recent increase of primary intrahepatic gallstones. 55 per cent of cases with hepatolithiasis had their gallstones in the left intrahepatic bile ducts. Clinicopathological study on the resected hepatic specimens of 33 patients revealed numerous intrahepatic periductal glandular formations. Periductal glandular formations were classified into the intramural and extramural glands. The mucous substances which might had been released from the periductal glands seemed to play a role in the formation of stones in combination with bilirubin pigments, cholesterin, bacterial organisms, cellular debris and other bile component. Intrahepatic gallstones and extramural glands were seen in the intrahepatic segment and area ducts. The defunctionalized atrophic hepatic lobe or segment should be resected in order to remove the calculi completely and to prevent the recurrence.

Phosphorylation of purified yeast fructose-1,6-bisphosphatase was studied using purified preparations from yeast of two different cyclic AMP-independent protein kinases and a cyclic AMP-dependent protein kinase. Incorporation of 32P into fructose-1,6-bisphosphatase could be demonstrated only with the cyclic AMP-dependent protein kinase. Phosphorylation of fructose-1,6-bisphosphatase was stimulated by 3 microM fructose-2,6-bisphosphate and inhibited by 1 mM 5'-AMP.
Intracellular recordings and morphological identification of neurons by using intracellular HRP staining were performed in the cat motor cortex. By cerebellar stimulation, stellate cells in layers II-III, pyramidal cells in layer III and fast pyramidal tract neurons (PTNs) were activated with short latency and fast rising EPSPs, while pyramidal cells in layer II and slow PTNs showed a wide range of latency and slow rising EPSPs. This difference may be related to activation through the deep and superficial thalamocortical projections.
Extracellular single unit recording during high fixed ratio bar press behavior, guided by multimodal cue stimuli for food intake, revealed functional heterogeneity in the monkey dorsolateral prefrontal cortex. Cells were found significantly more often in the ventral arcuate concavity, the dorsal arcuate concavity, the principal sulcal area and the inferior convexity which responded, respectively, to visual events, auditory events, visual plus auditory events and bar press.
A psychological procedure which does not involve the application of physical stimulation was used to produce gastric ulcers experimentally. Ulceration was induced in rats by exposing the animals to the aggressive attacks of rats treated with 6-hydroxy-dopamine (6-OHDA). Gastric secretion and the effects of autonomic drugs on ulcer formation were investigated. Atropine methylbromide did not significantly inhibit the occurrence of erosions. Phentolamine or hexamethonium bromide significantly inhibited the production of erosions, and combined administration of an anticholinergic agents and alpha-blocking agent led to a complete inhibition, with no notable behavioral change. In case of pylorus ligation, gastric secretion during exposure to attack of 6-OHDA-treated rats was significantly less than that in the controls. We suggest that the sympathetic nervous system plays an important role in the production of gastric erosions, as induced by the methods reported in this study.
Seventy-four patients with unstable angina are reviewed. A rather high incidence (65%) of a changing pattern syndrome was observed in the study group. A history of prior myocardial infarction was obtained in 22 patients (30%). Fifty-seven patients (77%) were investigated using coronary angiography: 15 did not have any significant occlusive disease but are included in this study; single vessel disease was present in 20, double in 8 and triple in 14. Ten patients (13.5%) had myocardial infarcts which had occurred between 48 hours and 3 months after admission, and no patients died during this period. Patients who on admission had resting angina of new onset were more likely to have a worse short-term prognosis. Four patients underwent coronary bypass surgery in the first year of the follow-up period because of unsuccessful medical treatment. Six patients died within the first 4 years. Of the 6 deaths, 5 had triple vessel disease and one did not have any significant coronary stenosis. Mortality rate at 2 years was 2.7% and at 4 years 10.3%. This low mortality is probably attributable to a low incidence of 2- or 3-vessel disease.
An homologous radioimmunoassay for human carboxyl-terminal parathyroid hormone (PTH) was developed using the antiserum raised against synthetic human PTH (46-84) and its clinical evaluation was attempted. Antisera were produced in guinea pigs by immunization with synthetic human PTH (46-84) conjugated with BSA. One of the antisera was used at a final concentration of 1 : 10,000 in the assay, and synthetic human PTH (53-84) for a standard and 125I-Tyr45 human PTH (46-84) for a tracer were used. A standard curve was obtained at concentrations from 0.001 to 10 ng per tube of human PTH (53-84). The displacement curves for human PTH (51-84), human PTH (46-84), Tyr45 human PTH (46-84) and bovine PTH (1-84) were almost identical with the standard curve made by synthetic human PTH (53-84). This antiserum, however, did not recognize the human PTH (1-34) fragment at all. Using this assay system, circulating immunoreactive PTH was measured in normal subjects and patients with parathyroid disorders. Normal PTH values in serum ranged from 0.47 ng/ml to an undetectable level (undetectable in two out of 32 normal subjects) and serum PTH values in patients with primary or secondary hyperparathyroidism or idiopathic hypoparathyroidism were almost completely discriminated from normal values. These results suggest that this homologous radioimmunoassay is a carboxyl-terminal specific assay for human PTH and can be quite useful in clinical practice.

To evaluate the effect of pancreatic fragments transplantation on glucose and lipid metabolism in the apancreatic status, 22 healthy mongrel dogs were totally pancreatectomized and then divided into three groups, (1) 5 dogs without any supplement , (2) 5 dogs with insulin replacement, and (3) 12 dogs with additional pancreatic fragments transplantation. In these three groups, changes in fasting plasma sugar, triglyceride, total cholesterol, phospholipid and lipoprotein fractions were studied. Stability of serum glucose levels were compared between the dogs with insulin replacement and those with transplantation by using glucose infusion and insulin test. Without insulin replacement or transplantation, apancreatic dogs showed high serum glucose levels immediately after total pancreatectomy. With pancreatic transplantation, serum glucose levels and lipids were well controlled within normal limits for a long time period after the operation. However, in the dogs with insulin replacement, serum glucose levels and lipids were remarkably unstable in fasting period and also showed easily tendencies to become hypoglycemic status by insulin test and diabetic ketoacidosis by glucose infusion test as compared with those in the transplanted dogs.
A new fluoropyrimidine antitumor agent, carmofur (HCFU, Mifurol) was administered to patients with malignant ovarian tumor. Two of these patients revealed favorable results. The first patient was a 72-year-old female, who was diagnosed as having ovarian serous cystadenocarcinoma with metastatic omental tumor at exploratory laparotomy, its size was newborn child head size. She was started on with a combination chemotherapy of Mifurol (600 mg p.o. daily), Endoxan (4 mg/kg i.v. twice a week), Mitomycin C (0.04 mg/kg i.v. twice a week) and Toyomycin (0.01 mg/kg i.v. twice a week). After four weeks, this combination therapy brought her a complete response with disappearance of pleural effusion, ascites and metastatic tumor. The second case was a 39-year-old female, who underwent adnexectomy elsewhere which led to the discovery of Krukenberg tumor, and was referred to our hospital. After the first course of the same combination chemotherapy, second look operation was performed. Histological examination of the specimen obtained by metastatic tumor of uterosacral ligament showed the degeneration (grade II b-III, Oboshi and Shimozato) of cancer cell. It is suggested that this combination chemotherapy including Mifurol is effective and useful for the patients with ovarian carcinoma.
