[Diagnosis of myocardial ischemia in Kawasaki disease: thallium-201 myocardial imagings at rest, with exercise and with dipyridamole administration].

Thallium-201 myocardial imaging was performed at rest in 131 children with coronary arterial lesions due to Kawasaki disease. The coronary arterial lesions were assessed by selective coronary angiography within a few days of the isotope study. Twenty-one children had occlusive lesions, and segmental stenotic lesions were seen in 16 children. Perfusion defects of the myocardial images were detected in nine of the former and in three of the latter. The locations of the perfusion defects coincided with the perfusion areas of the affected vessels on coronary angiography. Twelve patients with initial perfusion defects at rest had a follow-up study and the defects disappeared in five. These patients had re-establishment of coronary blood flow in the initially affected areas by either development of collateral vessels or recanalization. Myocardial imaging with exercise was performed in 27 patients including four with coronary arterial occlusion and two with segmental stenosis on coronary angiography. All with coronary artery lesions showed perfusion defects on the imaging with exercise, while the resting study showed the defects only in one patient, in whom more extensive perfusion defects were observed after exercise. Myocardial imaging following intravenous injection of dipyridamole was carried out in 43 patients. Perfusion defects after the injection were noted in 15 of 17 patients with coronary occlusion and in nine of 13 patients with segmental stenosis. In four patients with perfusion defects at rest, additional or more extensive defects were revealed by this drug in the areas of additional coronary arterial involvements. In 20 patients with perfusion defects only after dipyridamole injection, the perfusion defects coincided with the angiographic findings very well. A perfusion defect was documented following dipyridamole injection in one exceptional patient who had no stenotic lesions, but had three giant coronary aneurysms of the right coronary artery. Thus the dilated coronary lesions seemed to give a perfusion defect. In some of the patients whose perfusion defects disappeared at rest on a follow-up study, the defects were disclosed by exercise and/or dipyridamole administration. Thus, thallium-201 myocardial imagings combining resting and exercise or dipyridamole studies were valuable for the detection and assessment of coronary arterial lesions of Kawasaki disease.

[Ectopic ureterocele: a case report].

A case of ectopic ureterocele is reported. A 2-year-old girl was admitted for recurrent episode of urinary tract infection associated with fever. IVP showed bilateral duplex kidney, although no excretion of dye was noted from the right upper kidney. IVP also revealed a large filling defect at the bladder neck which was diagnosed as ureterocele by cystoscopy. Cystography demonstrated VUR to the right lower kidney. A complete duplication of ureter with ectopic ureterocele on the right, and incomplete duplication of ureter on the left were found at operation. She underwent complete removal of the ectopic ureterocele and reimplantation of the right two ureters. Her postoperative course was uneventful, post-operative IVP revealed improvement of pyelography of the right lower kidney and cystography revealed no VUR.

[Thallium-201 myocardial imaging in patients with congenital heart disease following open-heart surgery].

Thallium-201 myocardial imaging was performed for 77 patients with congenital heart disease to evaluate myocardial damage following open-heart surgery. There were 23 patients with ventricular septal defects, 33 with tetralogy of Fallot, 12 with atrial septal defects, and nine with other diseases. The imaging results were compared with the angiographic and electrocardiographic findings. Patchy perfusion defects or images of poor thallium uptake were noted in five of 77 patients, among whom four had accompanying serious ventricular arrhythmias. This suggested a close correlation of the post-operative ventricular arrhythmias with myocardial damage. For 20 of 77 patients, vents were inserted through the left ventricle, and for the remaining 57 through the left atrium. Perfusion defects were found at the apex in three, and left ventriculography showed a paradoxical motion of the apex in five in patients whose vents were inserted through the left ventricle. Sensitivity and specificity in detecting myocardial damage of the apex were 60% and 98%, respectively. In one case, myocardial infarction was demonstrated immediately after the repair of the ventricular septal defect. Myocardial imaging was useful in evaluating the infarcted area and for follow-up study. Thus, thallium-201 myocardial imaging was regarded useful for assessing myocardial damage following open-heart surgery in congenital heart disease patients.

[Treatment of advanced prostatic carcinoma with cis-diamminedichloroplatinum].

Chemotherapeutic effects of CDDP used as the main drug were studied in 20 patients with progressive prostatic cancer in stage C or D. On the average 208 mg of CDDP was given to the patients receiving chemotherapy without antiandrogen therapy (13 patients who showed resistance to hormone and an untreated new patient) and both ADM and IFM were also given to 3 of them. According to the criterion proposed by Shida and his coworkers, the chemotherapy without antiandrogen therapy was effective in 2 cases, relatively effective in 7 cases, and ineffective in 5 cases. The chemotherapy was effective for metastatic tumors of the lung in 2 out of 2 cases, but had no effect on tumors of the lymph node (1 case) and primary lesion of the tumors (14 cases). The chemotherapy improved acid phosphatase values in 5 out of 10 cases, alkaline phosphatase values in 3 out of 10 cases, dysuria in 4 out of 8 cases, nocturia in 1 out of 12 cases, residual urine in 5 out of 6 cases, lumbago in 6 out of 8 cases, and constitutional symptom in 6 out of 12 cases. The effect of the chemotherapy in combination with antiandrogen therapy was excellent in 4 and good in 2 of the 6 patients treated with castration + diethylstilbestrol diphosphate + CDDP + ADM +/- IFM. The chemotherapy with antiandrogen therapy had no effect on metastatic tumors of the bone (2 cases), but decreased the hardness and size of primary lesion in 6 out of 6 cases. Urethrography showed better changes in 6 out of 6 cases.(ABSTRACT TRUNCATED AT 250 WORDS)

[A clinical case of secondary tumor of the penis from the rectum, with malignant priapism].

We report a clinical case of carcinoma of the rectum. A 45-year-old patient had undergone resection of the rectum and proctostomy 22 months ago. Carcinoma caused metastasis to the corpora cavernosa of the penis in this patient, and caused local recurrence of the carcinoma of the rectum, pulmonary metastasis and malignant priapism. A statistical analysis of 62 cases of secondary tumor of the penis in Japan was also made. The present clinical case was the 62nd case of secondary tumor of the penis in Japan, and the 4th case of secondary penile tumor from the rectum. The primary foci of the secondary tumor of the penis are mostly in the urinary bladder and the prostate, followed by the rectum, kidney, pelvis of the kidney and the ureter. Primary sites in the urogenital organs were found in 82.3% and in the neighboring organs in 85.2%. As the route of metastasis of the secondary tumor of the penis, arterial blood, retrovenous, retro-lymph and direct infiltrating metastasis may be possible. Secondary tumor of the penis is mostly found in aged persons, and the major symptoms may be penile nodule and mass, malignant priapism, penile pain and tenderness, and difficulty in urination and retention of urine. Regardless of the length to metastasis and difference in the treatment of the metastatic focus, the secondary tumor of the penis is poor in prognosis, and survival period may be up to 7 months. From the findings of post-mortem examination, secondary tumor of the penis should be regarded as a secondary sign due to recurrence of the primary tumor or presence of metastasis in other organs, and careless surgical operation should be avoided.

[A case report of retroperitoneal Schwannoma].

A 37-year-old man with complaints of lassitude and slight lumbal pain, who had been found to have a low abdominal median mass and was referred to our hospital by a practitioner, was admitted for further examination. IVP, examination of the gastrointestinal tract by the oral procedure and re-examination of the colon by a double contrast procedure revealed deviation of the right ureter, the urinary bladder and the alimentary tract. This mass was found to be a retroperitoneal tumor with central necrosis by low abdominal CT scanning and the low abdominal echography. The mass was removed easily, though it was slightly adherent to the anterior surface of the sacral bone. Pelvic lymphadenectomy was also done simultaneously. The removed mass was encapsulated by fibrous tissue, round in shape, 750 grams in weight (13 by 12 by 12 cm), evenly flat, elastic soft and contained 230 ml bloody exudate at the center. Histologically this mass contained areas where oval and spindle cells made palisading arrangement and areas where the tumor cells had no communication with each other and stroma was edematous. However, hyperchromatism of nuclei of tumor cells and high cellularity indicated this mass to be a malignant Schwannoma. Dissected lymph nodes had no metastatic involvement. Since the surgical margin was detected to be invaded by tumor cells, postoperative prophylactic irradiation of Linac (10 Me V-X, total doses 4,750 rads) was performed on the whole pelvis. This patient has been well and has had no signs of recurrence of tumor for 23 months after the operation. Ninety-four cases of benign retroperitoneal Schwannoma and thirty-six cases of malignant retroperitoneal Schwannoma reported in Japan are reviewed.

Modulation of action potential during the late slow excitatory postsynaptic potential in bullfrog sympathetic ganglia.

The spike peak and after-hyperpolarization of the action potential of bullfrog sympathetic ganglion cells were depressed during the late slow excitatory postsynaptic potential (EPSP). These changes in the action potential were mimicked by luteinizing hormone-releasing hormone (LH-RH), a neurotransmitter candidate for the late slow EPSP. LH-RH (5 microM) suppressed the voltage-dependent K+ currents, both the delayed rectifier K+ current (IK1) and the M current (IK2). It is suggested that the depression of the after-hyperpolarization of the action potential during the late slow EPSP may be due to suppression of IK1 and IK2.

Mode of action towards oligopeptides and proteins of hydrolase H, a high-molecular-weight aminoendopeptidase from rabbit skeletal muscle.

The mode of action towards oligopeptides and proteins of hydrolase H purified from rabbit skeletal muscle was studied. The presence of protamine or alpha-N-benzoylarginine p-nitroanilide (an endopeptidase substrate) changed both the Km and V values of the enzyme towards Leu-beta-naphthylamide (an aminopeptidase substrate). This indicates that the binding site for an endopeptidase substrate is different from that for an aminopeptidase substrate. Hydrolase H as an aminopeptidase displayed broad specificity. The enzyme hydrolyzed various dipeptides readily except the dipeptides containing Pro or an amino acid with a hydrophobic beta-branched chain at the NH2 terminus. Pro and Val at the NH2 terminus of tripeptides were also difficult to release, whereas Ile and Val of tetrapeptides were easily released in contrast with those of dipeptides. The longer the peptide chain of Glyn (n = 2, 3, 4), the more susceptible was it to hydrolase H. Hydrolase H behaved as an endopeptidase only towards protamine among the proteins tested. The other proteins, casein, bovine serum albumin, myofibrils, troponin, hemoglobin, sarcoplasmic proteins, and myoglobin were probably attacked only by the aminopeptidase activity of the enzyme.

Substance P inhibits the action potentials in bullfrog sympathetic ganglion cells.

Substance P (0.5-5 microM) depressed the spike peak and after-hyperpolarization of action potentials of bullfrog sympathetic ganglion cells. It also depressed the after-hyperpolarization and prolonged the falling phase in Ca2+ spikes. The voltage-dependent K+ currents, both the delayed rectifier K+ current (Ik1) and the M current (Ik2), were suppressed by substance P, suggesting that the depression of the after-hyperpolarization may be due to suppression of these K+ currents.

Decrease in mitochondrial levels of adenine nucleotides and concomitant mitochondrial dysfunction in ischemic rat liver.

The process of mitochondrial dysfunction in ischemic rat liver was studied. A close correlation was found between decrease in the mitochondrial adenine nucleotide content and deterioration of oxidative phosphorylation capacity. The level of total adenine nucleotides, which was 15--20 nmol/mg protein in mitochondria isolated from normal liver, fell to 1--2 nmol/mg protein with concomitant loss of oxidative phosphorylation capacity after anoxic incubation in vitro or in vivo for 120 min. However, neither the permeability barrier to adenine nucleotides nor matrix enzymes were affected under these conditions. The loss of adenine nucleotides was ascribed to degradation of AMP to adenosine and then leakage of the latter. Conventional procedures for maintenance of oxidative phosphorylation capacity of isolated mitochondria, preservation in the cold and addition of ATP or a respiratory substrate under aerobic conditions, were very effective in maintaining the intramitochondrial levels of adenine nucleotides. Of the three species of adenine nucleotides, only AMP was ineffective in maintaining mitochondrial function; mitochondria containing more than 5 nmol of ATP plus ADP/mg protein exhibited normal activity of oxidative phosphorylation, but with less than 2 nmol they showed no activity.
