Unique mechanism of action of an antifungal antibiotic RI-331.

An amino acid antibiotic RI-331, (S)-2-amino-5-hydroxy-4-oxopentanoic acid, produced by a Streptomyces sp., is a novel antifungal agent. Biochemical studies on the mechanism of action of the antibiotic were carried out with a susceptible yeast Saccharomyces cerevisiae. In growing yeast cells, RI-331 inhibited protein synthesis to a greater extent than RNA or DNA synthesis. However, polypeptide synthesis by a cell-free preparation from yeasts was refractory to the antibiotic. The inhibition of protein synthesis by RI-331 was revealed to be due to the depletion of several amino acids such as threonine, methionine, isoleucine and serine in the cellular pool. These results suggest the possibility that some steps in amino acid metabolic pathways, especially those involved in biosynthesis of threonine, methionine and isoleucine, may constitute a primary target of RI-331 action.

[The influence of percutaneous nephrolithotomy on renal function evaluated by 99m-Tc-DTPA renoscintigram].

The influence of percutaneous nephrolithotomy (PNL) on renal function was evaluated by 99m-Tc-DTPA renoscintigraphy. The renoscintigram was recorded and regions of interest of both kidneys (R) and background (BKG) were settled. Renal uptake ratio was calculated by the following formula. Renal uptake ratio = (formula; see text). The renal uptake ratio of 50 patients were compared before and up to 6 months after PNL. Most cases showed an increase or no change in post-PNL renal uptake ratio in operated kidney. Five cases (10%) showed decreased renal uptake ratio of post-PNL. No risk factors for the renal damage caused by PNL such as age, sex, stone size, number of nephrostomy and course of PNL were detected when compared with increased or no change groups. In cases of complete staghorn calculi in the decreased renal uptake ratio group, compensatory hypertrophy of contralateral kidney PNL course might occur, and the function of the operated kidney might not have recovered. More during cases and longer follow-up is necessary for the recovery of renal function.

Properties of target molecule of murine lymphokine-activated killer (LAK) cells and the clones.

Properties of mouse lymphokine activated-killer (LAK) cells were examined by using polyclonal and monoclonal LAK cells. To identify cell types of LAK cells, LAK cells were induced from various mouse lymphoid tissues and examined for the surface phenotypes by means of negative selection and FACS analysis. Irrespective of the cell sources, LAK cells expressed Thy 1 and lymphocyte function-associated antigen 1 (LFA-1) as common markers but the expressions of asialoGM1 and Lyt antigens were different from their cell sources. The induction of LAK cells from spleen cells was more frequent in asialoGM1+, Lyt 2- (natural killer (NK) cell type) cells than in asialoGM1-, Lyt 2+ (T cell type). To further assess the properties of LAK cells, we established LAK cell clones from LAK cell lines induced from C57BL/6 mouse spleen cells. Although these clones expressed the similar phenotypes to the parent LAK cells, Lyt 2 was expressed in a limited portion of the clones. All clones were found to express T3 and T cell receptor (TcR)-alpha beta, and rearrangement patterns of TcR-beta were the same among the clones derived from the same parent cell line but different in clones derived from different cell lines as determined by using C beta 1 and J beta 2 probes. The molecules responsible for LAK-target cell binding were examined by using killer blocking antibody (KBA) (anti-LFA-1) monoclonal antibody (mAb) and anti-idiotypic polyclonal antibody (Id) to KBA.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical evaluation of aztreonam against pediatric purulent meningitis].

A monotherapy of aztreonam (AZT) was applied to 9 male and 3 female pediatric patients of ages ranging from 2 months to 8 years and 11 months with serious purulent meningitis. The results are summarized as follows: 1. Daily dosages of 134 to 400 mg/kg were given 3 to 4 times a day for 10 to 28 days. 2. The isolated pathogens were Neisseria meningitidis in 2 cases, Escherichia coli in 3 cases, Haemophilus influenzae in 6 cases, but the pathogen of the last case was unknown. In this case, the patient had myelomeningocele and suffered from recurrent purulent meningitis. 3. Clinical effect was "excellent" in 5 of 12 cases, "good" in 7 cases and efficacy rate was 100%. Microbiological tests were performed on 9 cases during the course of treatment and bacterial elimination rate was 100%. All the pathogens were eliminated in 72 hours. 4. One case of diarrhea was observed, but the diarrhea started before the administration of AZT and was stopped after the AZT treatment. A slight elevation of GOT value was observed in 1 case but the value returned to the initial level promptly after the treatment was completed. It is considered that AZT is highly useful for purulent meningitis caused by Gram-negative pathogens such as H. influenzae, E. coli and N. meningitidis.

Intracardiac malignant lymphoma detected by gallium-67 citrate and thallium-201 chloride.

An abnormal uptake of 67Ga and 201Tl in the right atrium was observed initially in a 77-yr-old man with superior vena cava syndrome. The pathological diagnosis of the surgically resected specimen was non-Hodgkin's malignant lymphoma in the right atrium. Thallium-201 is used as an imaging agent for malignant tumors. However, previous reports suggested that 201Tl scintigraphy may not be a useful method to detect cardiac involvement in patients with malignant neoplasms. In this case both 201Tl and 67Ga accumulation was observed in the intracardiac tumor by scintigraphy.

[A case of myelopathy caused by calcified nodules of cervical ligamentum flavum].

A case is reported of a 71-year-old female with cervical myelopathy caused by calcified nodules in the cervical ligamentum flavum and improved by en bloc laminectomy. The calcified nodules were formed by the deposition of two crystals, hydroxyapatite (HAP) and calcium pyrophosphate dihydrate (CPPD). Light microscopy, scanning electron microscopy, energy dispersive X-ray microanalysis and X-ray diffraction study disclosed that the larger nodule located on the right was composed mainly of HAP in the central part and CPPD in the circumference. On the other hand, the smaller nodule located on the left was composed mainly of CPPD crystal. These findings support the hypothesis that calcification of the ligamentum flavum and CPPD crystal deposition disease in the cervical region are the same entity and the both are merely different aspects of the same entity at different chronological stages.

[A case report of non-functioning adrenal carcinoma diagnosed by percutaneous needle biopsy and which responded to preoperative chemotherapy].

A 51-year-old female complained of right upper abdominal pain. Non-functioning adrenal carcinoma was suspected from the laboratory data of elevated lactate dehydrogenase level and total amount of urinary excretion of 17-ketosteroid. To deny the possibility of other diseases, such as retroperitoneal sarcoma, a percutaneous needle biopsy of the tumor was carried out. As the pathological diagnosis was adrenal carcinoma, pre-operative chemotherapy consisting of cis-dichlorodiammineplatinum (CDDP), adriamycin, 5-fluorouracil, cyclophosphamide and o,p'-DDD was carried out twice. The tumor size decreased after chemotherapy, and we could operate on the patient safely and relatively easily. Almost all of the tumor was replaced by a necrotic mass. However, cancer cells still remained in the peripheral lesion of the tumor, and the pathological diagnosis was the same as that of biopsy specimen. Although chemotherapy of adrenal carcinoma has not been established yet, we believe from the experience of this case that this disease should be treated by chemotherapy including CDDP.

[Laboratory and clinical studies of sultamicillin in pediatric field].

We have carried out laboratory and clinical studies on sultamicillin (SBTPC). The results are summarized as follows. SBTPC was given by oral administration to 2 children in a single dose at 5 mg/kg and to 3 children in a single dose at 10 mg/kg. After the oral administration, mean peak serum levels of ampicillin (ABPC) and sulbactam (SBT) obtained for the 2 dose levels were 1.91 +/- 1.34 and 2.06 +/- 1.06 micrograms/ml and 2.43 +/- 0.68 and 2.96 +/- 0.77 micrograms/ml at 1 hour, respectively, and mean half-lives were 0.80 +/- 0.10 and 0.98 +/- 0.46 hour and 1.57 +/- 0.57 and 2.01 +/- 0.70 hours, respectively. SBTPC was given to 2 children in a single dose at 15 mg/kg. After oral administration, the mean serum levels of ABPC and SBT at 30 minutes were 6.55 +/- 1.63 and 6.00 +/- 1.00 micrograms/ml, and the mean half-lives were 0.90 +/- 0.13 and 0.82 +/- 0.16 hour. SBTPC was given to 1 child at a single dose of 20 mg/kg. The peak serum levels of ABPC and SBT were 11.3 and 8.64 micrograms/ml, and the half-lives were 0.87 and 0.92 hour. Mean urinary excretion rates of ABPC and SBT were 38.4 +/- 2.7 and 34.6 +/- 4.7%, 43.0 +/- 3.6 and 41.6 +/- 5.8%, 47.7 +/- 5.2 and 51.6 +/- 3.5% in 6 hours and 66.1 and 59.2% in 8 hours after oral administration of 5 mg/kg, 10 mg/kg, 15 mg/kg and 20 mg/kg, respectively. Treatment with SBTPC was made in 34 cases of pediatric bacterial infections; 2 cases of pharyngitis, 19 cases of tonsillitis, 2 cases of bronchitis, 3 cases of impetigo, 2 cases of staphylococcal skalded skin syndrome, 4 cases of urinary tract infection and 1 case each of pneumonia and scarlet fever. Results obtained were excellent in 20 cases, good in 13 cases and poor in 1 case. No significant side effect due to the drug was observed in any cases.
