[Laboratory and clinical studies of cefdinir 5% and 10% fine granules in pediatric field].

We have carried out laboratory and clinical studies on cefdinir (CFDN) 5% and 10% fine granule preparations. The results are summarized as follows. CFDN 5% fine granule preparation was given via oral route to each of 2 children in the fasting state at a single dose of 3 mg/kg. After administration, the mean peak plasma level of CFDN was 0.76 micrograms/ml at 4 hours and the mean half-life was 1.77 hours. The mean urinary excretion rate of CFDN was 31.5% in the first 12 hours after oral administration. CFDN 10% fine granule preparation and CFDN 100 mg capsule were given via oral route 3 children and to another child in the fasting state at single doses of 3 mg/kg and 2.63 mg/kg, respectively. After administration of 10% granules the mean peak plasma level of CFDN was 0.73 micrograms/ml at 2 hours and the mean half-life was 1.62 hours. The peak serum level obtained after administration of CFDN 100 mg capsule was 0.91 micrograms/ml at 2 hours and the half-life was 1.08 hours. The mean urinary excretion rate obtained with CFDN 10% fine granules was 26.2% in the first 8 hours after oral administration and the urinary excretion rate obtained with CFDN 100 mg capsule was 19.7% in the first 12 hours after oral administration. Treatment with CFDN 5% fine granules was made for a total of 48 cases of pediatric bacterial infections including 21 cases of tonsillitis, 12 cases of scarlet fever, 3 cases of pharyngitis, 5 cases of impetigo, 1 case of subcutaneous abscess, 1 case of furuncle, 5 cases of UTI. Results obtained were excellent in 30 cases, good in 18 cases. Treatment with CFDN 10% fine granules was made for a total of 16 cases of pediatric bacterial infections including 6 cases of tonsillitis, 3 cases of pneumonia, 4 cases of scarlet fever, 2 cases of impetigo, 1 case of UTI. Results obtained were excellent in 8 cases, good in 7 cases, poor in 1 case. No significant side effects due to the drugs were observed except 2 cases (1 case with 5% preparation and another with 10%) with eosinophilia, 3 cases (all with 5%) with diarrhea and 1 case each of elevated GOT & GPT (with 5%) and elevated GOT, GPT & Al-P (with 10%).

[67Ga imaging in the patients with infective endocarditis after surgery for congenital heart disease].

67Ga imaging was performed in sixteen patients (age: 8 m.-18 y.) who had persistent fever and positive acute phase reactants after surgery for congenital heart disease. Abnormal uptake of 67Ga over the heart and the lungs was evaluated with a computer. Abnormal uptake of 67Ga was observed in seven patients, three of them showed it in the area of peripheral pulmonary artery and another four showed it in the area of artificial vessels for pulmonary artery reconstruction. In six patients with positive blood cultures, five showed abnormal uptake of 67Ga and in ten patients with negative blood cultures, two showed it. Vegetation was detected with 2D-echocardiography in four patients and all of them showed abnormal uptake of 67Ga, while in 12 patients without vegetation three showed it. In conclusion, 67Ga imaging was useful to detect the foci of infective endoarteritis or pulmonary embolism caused by the vegetation in infective endocarditis in the patients after surgery for congenital heart disease, especially in the peripheral pulmonary arteries and artificial vessels which could not be detected with 2D-echo.

[Detection of human anti-mouse antibody in patients receiving 111In-antimyosin Fab: multicenter clinical study in Japan].

In the multicenter clinical study of 111In-Antimyosin Fab (74 MBq, 0.5 mg), human anti-mouse antibody (HAMA) titers have been evaluated in serum from 456 patients using the sensitive enzyme-linked immunosorbent assay. Presence of HAMA was confirmed by the neutralization test or using the size exclusion high performance liquid chromatography (HPLC) analysis in 11 of 393 patients (2.8%). HAMA was still detectable in 2 patients even 40 weeks after injection. None of 7 patients judged as positive for the intradermal test, a patient with vascular pain or 3 patients with fever or flare and itching with fever, developed detectable levels HAMA in their serum. These results suggest that patients may develop HAMA response after infusion of 111In-Antimyosin Fab, but pathogenic role of HAMA in these patients remains to be studied.

[Prognostic significance of radionuclide-assessed right ventricular function in dilated cardiomyopathy].

To assess the prognostic significance of right ventricular function in dilated cardiomyopathy (DCM), we studied consecutive 57 DCM patients. There were 41 men and 16 women, whose mean age was 48 ys. (range 3-68 ys.). The mean LVEF in all patients was 29 +/- 11%, and the mean interval from the onset of symptom of cardiac failure (CHF history) was 4 ys. (range 0-33 ys.). With follow-up of 3.8 ys., five patients had died until the first year, and 14 had died until the third year. By using multivariate regression analysis, there were no prognostic significance in clinical parameters such as age, CHF history, sex, atrial fibrillation, except for NYHA class, and medication at the third year. In survival curves according to Kaplan-Meier method, RVEF and mean PA had predictive value (p less than 0.05), while LVEF did not. The patients with RVEF less than 45% had poor survival rate compared to those with RVEF greater than or equal to 45%. The patients with RVEF less than 45% showed lower LVEF and LVESVI. RVEF may offer prognostic predictive value through the effect of not only mean PA but also left ventricular parameter. In conclusion, radionuclide assessment of right ventricular function should be valuable for the prognostic evaluation of DCM patients.

[Quantitative evaluation of myocardial viability using Tl-201 myocardial SPECT].

In order to evaluate myocardial viability, we developed a new method for quantification of redistributed area in Tl-201 myocardial SPECT, and it was applied to 71 patients with LAD lesion. Initial image was subtracted from delayed image, and redistributed area was displayed as an unfolded map (viability map). Extent and viability score, which correspond to extent and degree of the viable area, was also calculated. In the cases with prior myocardial infarction, viability score was widely distributed and extent score was smaller, while the cases with angina pectoris had larger extent score. Visual assessment of redistribution to the infarcted area was compared with % score, which was calculated as ratio to initial defect score. When the criteria of viability was defined as % viability score greater than 36 or % extent score greater than 16, quantitative evaluation agreed with visual assessment, and corresponded to clinical course. This method was considered to be effective for evaluation of extent and degree of myocardial viability, but further evaluation need to be done in comparison with reinjection, PET study, and so on, and usefulness in multi-vessel disease.

[Hemodynamic assessment before and after PTMC by using cardiac blood pool scintigraphy].

Rest and exercise cardiac blood pool scintigraphy with 99mTc-RBC were performed in 12 patients with symptomatic mitral stenosis before percutaneous transvenous mitral commissurotomy (PTMC) and at one week, one month after PTMC. Comparison of hemodynamic parameters before and one month after PTMC showed that LVEDV (97.4 +/- 8.9 vs. 111.4 +/- 12.1 ml; p less than 0.01) and diastolic indices (1/3FF: 23.6 +/- 6.5 vs. 30.9 +/- 4.1%; p less than 0.01) both increased, while there was no significant change in LVEF and RVEF. Comparison of symptom-limited bicycle exercise before and one month after PTMC showed that the duration of exercise time (7.6 +/- 1.6 vs. 8.6 +/- 1.4 min; p less than 0.02) and anaerobic threshold (10.6 +/- 3.0 vs. 15.4 +/- 3.2 ml/min/kg; p less than 0.01) increased. In comparison of hemodynamic response before and one month after PTMC, there were significant increases in cardiac output (CO) at the same workload (15 W: 3.9 +/- 0.7 vs. 4.8 +/- 0.8 l/min; p less than 0.05, 30 W: 3.9 +/- 0.7 vs. 5.2 +/- 1.2 l/min; p less than 0.05) despite similar CO at rest (3.5 +/- 0.8 vs. 3.9 +/- 0.7 l/min; NS). We conclude that cardiac blood pool scintigraphy is useful to evaluate improvement of hemodynamic parameters and exercise capacity after PTMC non-invasively.

[Clinical application of extracorporeal shock wave lithotripter (SD-3) to upper urinary tract stones].

The extracorporeal shock wave lithotripter (SD-3) was used on 30 patients with upper urinary tract calculi from December, 1988 to March, 1989. At one month after the treatment, 11 patients (36.7%) were free from stones and 9 (30.0%) had stone fragments than 5 mm in diameter, making an effective rate of treatment of 66.7%. Gross hematuria, skin ecchymosis, pain and fever were observed as side effects, but none of them were serious. We concluded that extracorporeal shock wave lithotripsy using SD-3 for upper urinary tract stones is an effective treatment without any serious side effects.

[The activity of etoposide (VP16) in combination chemotherapy against human bladder cancer cells in vitro].

The activity of Etoposide (VP16) in combination chemotherapy against four human transitional cell carcinoma cell lines of bladder (TCCaB) was determined by in vitro colony formation assay. Four anti-tumor agents (methotrexate: MTX, vinblastine: VBL, adriamycin: ADM, cisplatin: DDP) were used for combination chemotherapy with VP16. The ADM + VP16 combination exhibited a strong synergistic antitumor effect against the human TCCaBs compared with other combinations in this study. The combination chemotherapy of ADM + VP16 may be useful as a new chemotherapeutic regimen for advanced bladder cancer.

General pharmacology of the novel angiotensin converting enzyme inhibitor benazepril hydrochloride. Effects on cardiovascular, visceral and renal functions and on hemodynamics.

The effects of benazepril hydrochloride (CGS 14824 A, CAS 86541-74-4), a novel angiotensin I converting enzyme inhibitor, on cardiovascular, visceral and renal functions and on hemodynamics, were studied in various experimental animals. Even at a high dose of 100 mg/kg p.o. benazepirl hydrochloride had no influence on the respiration, heart rate and ECG of normotensive anesthetized cats and, except at higher doses, had little effect on the contractile tension of mammalian isolated atrium, ileum, trachea, stomach fundus strips, vas deferens or uterus. Benazepril hydrochloride even at a high dose of 100 mg/kg p.o. had little effect on spontaneous uterine motility, charcoal transportation and gastrointestinal tract motility. In addition, it did not cause gastric irritation, alter the secretion of gastric and biliary juices, and did not affect the tension of the nictitating membrane or the twitch tension of the gastrocnemius muscle in various experimental animals. Benazepril hydrochloride had no effect on the blood glucose and cholesterol levels in alloxan-induced diabetic rats but decreased the triglyceride and total cholesterol levels in normotensive rats at a dose of 30 mg/kg p.o. Benazepril hydrochloride at 3 mg/kg.day s.c. for 10 weeks caused a significant decrease in aortic atherosclerosis without reducing hypercholesterolemia in cholesterol-fed rabbits. Benazepril hydrochloride at a high dose of 100 mg/kg p.o. showed no effect on the urine volume and urinary excretion of electrolytes but decreased PSP excretion in normotensive rats. At a dose of 3 or 10 mg/kg.day p.o. for 4 weeks benazepril hydrochloride inhibited the increase in the excretion of urinary protein in DOCA/salt spontaneously hypertensive rats. It caused hemolysis at concentrations as high as 0.1-1% in rabbits, however, even at a high dose of 100 mg/kg p.o. it did not affect red blood cell fragility in rats, and, except at a high dose of 10(-4) g/ml, showed little effect on the platelet aggregation response induced by collagen or arachidonic acid in rabbits. From these results, benazepril hydrochloride is considered to be a safe and well-tolerated addition to the therapeutic armamentarium of cardiovascular drugs.

[Clinical evaluation of 99mTc-hexakis 2-methoxy isobutyl isonitrile (MIBI): multicenter phase III clinical trial].

Multicenter Phase III clinical trial on a new myocardial imaging agent, 99mTc-hexakis 2-methoxy isobutyl isonitrile (99mTc-MIBI), was carried out in 464 patients with various cardiac disorders. Both planar and tomographic images provided high-quality regional myocardial perfusion images which correlated well with 201TlCl images. The positive ratios of myocardial infarction and angina pectoris by the agent were as high as that of 201TlCl. In addition, ventricular function and wall motion could be assessed with first-pass and ECG gated studies. There was no serious adverse reaction which were attributed to 99mTc-MIBI in any of patients studied. A transient metallic taste was observed immediately after the injection in approximately 64% of the patients. Based on its safety and efficacy, the clinical usefulness of 99mTc-MIBI was observed in 562 (93.1%) of 604 injections. In conclusion, 99mTc-MIBI was useful as a imaging agent of myocardial blood flow.

[Usefulness and limitations of stress 201-thallium myocardial imaging in patients with Kawasaki disease].

Thallium-201 myocardial imaging was performed in 150 patients with a history of Kawasaki disease, aged 6 months to 16 years old. Forty-five patients were studied with ergometer exercise and 105 patients had dipyridamole administration. The findings of thallium imaging were compared with those of coronary angiography. Seventy-two cases with severe coronary stenosis (over 75%) were classified in 3 groups; 13 with the left anterior descending artery lesion, 31 with only the right coronary artery lesion and 28 with multi-vessel involvement. In 72 cases with severe coronary stenosis, the sensitivity of thallium imaging for detecting coronary stenotic lesions was 85%, 74% and 67% in the 3 groups, respectively. In 78 cases without severe coronary stenosis, 5 cases (6%) had persistent perfusion defects on thallium imaging, 3 of which were associated with dyskinesis of the left ventricle documented by cineangiography. They were considered to have damaged myocardium probably due to peripheral myocardial infarction or myocarditic problems. Thallium-201 myocardial imaging was proved to be a useful method to detect coronary stenosis, however its sensitivity for detecting lesions of the right coronary artery or multi-vessels was relatively low. This may be attributable to a significantly higher incidence of segmental stenosis (recanalization) with sufficient coronary flow and multiple coronary collateral vessels in patients with such lesions.

[Laboratory and clinical studies of cefpirome in pediatric field].

We have carried out laboratory and clinical studies on cefpirome (CPR, HR 810). The results are summarized as follows. CPR was given by 30-minute drip infusion to 3 children at a single dose of 20 mg/kg. After the 30-minute drip infusion, the mean peak serum level of CPR was 65.7 +/- 2.2 micrograms/ml at the end of infusion, and the mean half-life was 1.49 +/- 0.046 hours. The mean urinary excretion rate of CPR was 57.0 +/- 25.8% in the first 8 hours after the 30-minute drip infusion of 20 mg/kg. Treatment with CPR was made in 9 cases of pediatric bacterial infections; 1 case each of tonsillitis, pharyngitis, and bronchopneumonia, 4 cases of pneumonia, 2 cases of urinary tract infection. Results obtained were excellent in 6 cases, good in 3 cases. No significant side effects due to the drug were observed except one case of elevated GOT and GPT, and 3 cases of eosinophilia.

[Prognostic significance of scintigraphic silent myocardial ischemia detected by stress thallium scan in patients with recent myocardial infarction].

To evaluate the prognostic significance of scintigraphic silent myocardial ischemia (SMI) detected by stress thallium scan in patients with myocardial infarction (MI), we performed a retrospective investigation on cardiac events (CE) during a two-year follow-up period in 149 patients with MI within three months of onset (34 +/- 19 days). SMI was defined as asymptomatic redistribution (RD) in the infarcted area. The patients were divided into three groups based on results of stress thallium scan as follows: 50 patients with neither chest pain nor RD (Group A), 46 patients with SMI (Group B) and 53 symptomatic patients (Group C). In comparison of the incidence of CE, which included cardiac death, recurrent MI, chronic heart failure, angina pectoris, PTCA, CABG and severe ventricular arrhythmia (lown grade greater than or equal to 3) during two-year follow-up, Group C had significantly higher incidence of PTCA and CABG than Group B (p less than 0.01), but there was no significant difference of other CE between groups B and C except PTCA and CABG. In addition, Groups B and C had a significantly higher incidence of CE than Group A in cardiac event-free curves, but there was no significant difference for Groups B and C. We conclude that patients with SMI are associated with unfavorable prognosis as symptomatic patients and that these patients should undergo careful follow-up.

[Evaluation of left ventricular function using gated planar myocardial imaging with Tc-99m-MIBI].

A quantitative index of left ventricular wall motion obtained from ECG-gated planar myocardial images has been developed. Five normal controls and 39 patients with heart disease received an injection of Tc-99m-MIBI (550-740 MBq) at rest, and ECG-gated planar scintigraphy (LAO view) was performed 3 h later. Mean End-diastolic (ED) and end-systolic (ES) myocardial activities were measured using circumferential profile analysis and %count increase (CI) was determined according to the following formula; (ES count-ED count)/(ED count) x 100. The global %CI was compared with the EF obtained from contrast ventriculography (LVG; n = 29) and radionuclide ventriculography using Tc-99m-labelled RBC (RNV; n = 24). The regional %CI was compared with left ventricular wall motion assessed by LVG. The global %CI was correlated well with EF by LVG (r = 0.70) and EF by RNV (r = 0.75). The regional %CI significantly decreased in accordance with wall motion on LVG worsened at impaired region. In conclusion, gated perfusion imaging with Tc-99m-MIBI provides useful information on ventricular function in addition to myocardial perfusion.

[Cerebral perfusion scintigraphy in patients with cerebrovascular disease by using 99mTc-ECD: comparative study with 123I-IMP SPECT].

99mTc-ECD SPECT was performed in 19 patients with cerebrovascular disease, and location, extent, and severity of the lesions on 99mTc-ECD SPECT were compared with those on 123I-IMP SPECT. The initial brain uptake was 5.5 +/- 0.7% of the injected dose at 10 minutes after injection, 5.3 +/- 1.3% at 90 minutes, and clearance from the brain is slow. The distribution in the brain was changed, especially washout from the thalamus was slower than that from other regions. The count ratio of perfusion defect to normal area (D/N) on 99mTc-ECD SPECT was unchanged over the time, and had no significant differences from that on 123I-IMP SPECT. 99mTc-ECD SPECT was superior in detection of the lesion in the basal ganglia, and showed the images with superior spatial resolution due to physical characteristics of 99mTc. However, mild ischemic lesion and peri-infarct area was not clearly visualized, while 123I-IMP SPECT could demonstrate these lesions with better contrast.

[Treatment of superficial bladder tumor].

In period of April, 1982 to March, 1987 for 5 years, fifty-four patients with superficial bladder tumors were treated at the Department of Urology, Nippon Medical School. There were 42 males and 12 females with age ranged from 28 to 85 years old. All patient underwent TUR-Bt (trans urethral resection-bladder tumor) as an initial treatment. The rather long term follow-up study after initial treatment was 47 months in average. Recurrence rate at 2 years after TUR-Bt was 30%. The risk factors of recurrence were analysed in connection with sex, age, tumor number, histological grade and stage, and infiltration pattern. In conclusion, significantly higher recurrence rate were observed in G2 than G1 and G3, and in pT1b rather than pTa and pT1a. Other factors were not distinctly correlated with incidence of recurrence.
