Macrophages in the urine in acute bacterial cystitis.

In the previous study in this series of studies concerning the role of macrophages in urinary tract infection, we attempted to detect macrophages in the urine of acute bacterial cystitis patients by nonspecific esterase staining of urinary sediment, however none of the leukocytes stained, probably because of cell damage caused by the urine and by centrifugation. In the present study, detection of macrophages in urine was again attempted, this time by prompt transfer of urinary leukocytes to culture medium after minimum centrifugation, 1 hr culture in a glass bottom chamber and non-specific staining of leukocytes adhering to glass. Macrophages in urine were detected by this method, and they comprised 5.9% of the adherent leukocytes, although macrophage spreading, which implies macrophage activation and is often seen in the early stage of nonbacterial prostatitis, was hardly ever observed. The percentages of adherent leukocytes were not correlated with urine osmolarity, probably because the effect of urine was minimized by prompt transfer of urinary leukocytes to culture medium after the urine samples had been collected. There have been quite few studies involving culture of urinary leukocytes in the past. Our simple techniques, such as prompt transfer of urinary leukocytes to culture medium after centrifuging with minimum gravity and for a minimum period of time, appear to be useful in the study of urinary leukocytes using other cells which appear in urinary tract infection, as well as cytokines and antibiotics, to clarify cellular mechanisms of defenses in urinary tract infection.

Effects of vasoactive intestinal contractor on voltage-activated Ca2+ currents in feline parasympathetic neurons.

Intracellular current-clamp and single-electrode voltage-clamp techniques were used to study in vitro action potentials and the action of vasoactive intestinal contractor (VIC; 0.03-1 microM) on the high-voltage-activated Ca2+ currents (ICa) of neurons in feline colonic parasympathetic ganglia. In the current-clamp recording mode, action potential amplitude was depressed by cobalt (1 mM) and omega-conotoxin (300 nM) or in nominally Ca(2+)-free Krebs solutions. In the single-electrode voltage-clamp recording mode, the ICa was isolated by blocking the voltage-gated Na+ current with tetrodotoxin (1-3 microM) and by Krebs solutions containing a low Na+ concentration. The voltage-activated K+ currents were blocked by intracellular injection of cesium through a recording electrode filled with 2 M CsCl and external application of tetraethylammonium (30-50 mM) and barium (2 mM). The Ca(2+)-dependent Cl- current was blocked by replacement of Ca2+ (2 mM) with equimolar barium. Anomalous rectification was blocked by external application of 2 mM cesium. The ICa was evoked by depolarizing step commands more positive than -40 mV from holding potentials ranging between -80 and -60 mV. ICa was depressed by cobalt (1 mM), cadmium (100 microM), and omega-conotoxin (500 nM) but not by nifedipine (10 microM), nicardipine (10 microM), and verapamil (10 microM). BAY K 8644 (3-10 microM) also did not affect the ICa. VIC (0.1-1 microM), one of the endothelin (ET) isopeptides, caused an inward current followed by an outward current. The VIC-induced inward and outward currents were associated with an increase and decrease in membrane conductance, respectively. VIC also caused an initial depression followed by a long-lasting augmentation of the ICa. ET-1, ET-2, and ET-3 equally mimicked the action of VIC on both holding current and ICa. These data suggest that VIC activates a receptor-operated channel and modulates the omega-conotoxin-sensitive voltage-activated Ca2+ channels through ETB receptor subtypes of neurons in feline colonic parasympathetic ganglia.

Neuropathologic changes in the gerbil brain after chronic hypoperfusion.

BACKGROUND AND PURPOSE: An animal model has been developed to elucidate the pathological changes in brain cytoskeletal proteins during chronic hypoperfusion. METHODS: Newly designed coiled clips were placed around both carotid arteries of Mongolian gerbils (n = 10) to cause stenosis without occlusion. Those gerbils showing impaired learning ability by the passive avoidance paradigm were killed for neuropathologic study after 12 weeks. RESULTS: The brains showed ventricular dilatation, cortical atrophy, and rarefaction of the white matter. Immunoreactivity to anti-microtubule-associated protein 2 antibody in the cerebral cortex and the hippocampus was diminished, indicating dendritic changes of neurons. In the thalamic axonal regions, staining with anti-neurofilament 200K protein antibody was increased, suggesting increased amounts of neurofilament proteins or increased phosphorylation of the protein. Increased immunoreactivity to anti-glial fibrillary acidic protein antibody was observed in a wedge-shaped configuration, corresponding to the border zone of perfusion by small vessels. CONCLUSIONS: These findings suggest that changes in the cytoskeletal proteins in dendrites, axons, and glial cells may cause neuronal death under conditions of chronic cerebral hypoperfusion.

Perfusion lung scintigraphy in primary pulmonary hypertension.

15 cases of primary pulmonary hypertension were classified into two groups by patterns of perfusion lung scintigraphy. Perfusion scintigrams showed multiple, small, ill-defined defects (mottled + ve) pattern in eight cases, and the remaining seven cases had a normal (mottled - ve) pattern. The mean pulmonary arterial pressure in patients with a mottled pattern (54 +/- 10 mmHg) was higher than in those with a normal pattern (42 +/- 9 mmHg, p < 0.05). There were no significant differences between the two groups in right ventricular ejection fraction, partial pressures of oxygen in the arterial blood or alveolo-arterial oxygen difference. All the patients with a mottled pattern died within 2 years following the lung scintigraphy. There was a significant difference in the survival curves between the two groups. Although our statistical analysis must be evaluated with caution because of small numbers of patients it is suggested that perfusion lung scintigraphy is useful in assessing the prognosis in primary pulmonary hypertension.

Continuous Fick cardiac output measurement during exercise by monitoring of mixed venous oxygen saturation and oxygen uptake.

To determine whether continuous Fick cardiac output measurement is applicable to exercise testing, cardiac output data obtained by the continuous Fick method (Qcf) during exercise were compared with data obtained by the thermodilution method (Qth). Seventeen patients with old myocardial infarction underwent a 1-min or 3-min incremental exercise test (protocols 1 and 2, respectively). During exercise, the oxygen consumption (VO2), arterial oxygen saturation (SaO2), and mixed venous oxygen saturation (SvO2) were monitored continuously. Qcf was calculated at 12-s intervals by the Fick equation. The SaO2 remained almost constant during exercise. The SvO2 showed four characteristic phases during exercise protocol 1. SvO2 values changed rapidly in phases 2 and 4, but only slightly during phase 3. In exercise protocol 2, SvO2 almost reached a steady-state by the end of each stage. The correlation between Qcf and Qth was good in protocol 1 (r = 0.86), except in phases 2 and 4, and was also good in protocol 2 (r = 0.80). We conclude that the continuous Fick method may be applicable for determining the cardiac output during exercise provided that the variation in SvO2 is slight.

Arterial supply of the maxillary teeth in the crab-eating monkey (Macaca fascicularis).

Superior dental rami diverging from arterial multi-resources of the crab-eating monkey were investigated by means of plastic corrosion castings. Correlations between the dental ramus/rami entering the apical foramen/foramina of each maxillary tooth were elucidated. Superior dental rami supplying the maxillary teeth diverged from the posterior superior alveolar, infraorbital, descending palatine, major palatine and sphenopalatine arteries as follows: The posterior superior alveolar gave rise to molar branches which supplied three molars and rarely an additional two premolars, to premolar branches and to a canine branch as its terminus. The infraorbital gave off canine branches. The descending palatine gave rise to a molar branch which supplied the lingual root of each molar. The major palatine gave rise to a premolar branch which supplied the lingual roots of two premolars and the 1st molar, and an incisive branch which supplied two incisors. The sphenopalatine supplied the lingual root of each molar and rarely the 1st premolar in addition. Two incisors were supplied by the major palatine (100%), the canine by the posterior superior alveolar (50%), this artery and the infraorbital (43.3%) or the infraorbital only (6.7%). Three roots of each premolar were supplied by the posterior superior alveolar, occasionally the lingual root (16.7%) and mesiobuccal root (3.3%) of the 1st premolar by the major palatine, the lingual root of the 2nd premolar by the major palatine (6.7%) and the sphenopalatine (3.3%). Two buccal roots of each molar were also supplied by the posterior superior alveolar (100%), occasionally the lingual root of the 1st molar by the descending palatine (6.7%), the major palatine (6.7%) or the sphenopalatine (3.3%), the lingual root of the 2nd molar by the descending palatine (30%), the sphenopalatine (10.0%) or the infraorbital (3.3%), the lingual root of the 3rd molar by the descending palatine (23.3%), the sphenopalatine (20.0%) or the infraorbital (3.3%). Superior dental rami diverging from the posterior superior alveolar artery were distributed to the molars, premolars and canine as principal arterial vessels of the crab-eating monkey. The descending palatine, sphenopalatine and the infraorbital arteries were supplementary to the above main arteries. Two incisors were only supplied by the major palatine but the anterior and middle superior alveolar were not observed.

Immunohistochemical study of superoxide dismutase in the olfactory mucosa.

We used an anti-copper, zinc-superoxide dismutase (Cu,Zn-SOD) antibody in an immunohistochemical investigation of the distribution of SOD in the mouse olfactory mucosa. We also investigated differential staining with this antibody in comparison with an antibody specific to carnosine, a marker of mature olfactory cells. Mouse olfactory mucosa were immunostained with the ABC method with anti-SOD and anti-carnosine antibodies. When anti-SOD antibody was used, the sustentacular cells were positive in the olfactory region, whereas the olfactory cells were negative. In the respiratory region, the columnar epithelial cells were positive, whereas goblet cells were negative. The olfactory cells, olfactory vesicles and axons showed positive staining with anti-carnosine antibody, whereas the sustentacular and basal cells were negative.

[Molecular genetic analysis of sporadic Alzheimer's disease].

The authors report the clinical findings and the results of molecular genetic analysis of 8 patients with sporadic Alzheimer's disease. Differential diagnosis was carried out on the basis of familial history, laboratory data, brain imaging analysis using CT, MRI and SPECT. According to the clinical stage criteria made by Cummings and Benson, 6 cases were in stage 1 and the remaining 2 in stage 2. Recently, it was reported that affected members from 6 Japanese kindreds with familial Alzheimer's disease (FAD) had missense mutation in exon 17 of the gene for beta/A4 amyloid precursor protein (APP). Amino acid substitution (Val-Ile) at codon 717 by this mutation was considered to be responsible for FAD in these kindreds. We used genomic DNA from 8 sporadic cases to determine whether the disease in these families is associated with an APP 717 mutation and the mutated codons, 102, 117, 129, 178, and 200, on the gene for proteinase-resistant prion protein (Prp) which causes transmissible dementia, Creuzfelt-Jacob disease (CJD) and Gerstmann-Sträussler syndrome (GSS). The results showed that there were no mutations on these genes in 8 patients. It would be necessary to analyze DNA from patient with sporadic Alzheimer's disease to examine the mutations found in the APP gene and Prp gene of heredity Alzheimer's disease patients.

[Characterization of primary and metastatic cell lines established from a patient with renal cell carcinoma].

We established four renal cell carcinoma (RCC) cell lines (HANKS), namely the primary tumor (HANKS-Pr), metastasized to the lung (HANKS-Lu), liver (HANKS-Li) and lymph node (HANKS-LN) derived from a patient with advanced RCC, and analyzed their characters. Each had an epithelial morphology and exhibited multilayering. These cell lines have been maintained for more than 36 months and over 100 in vitro passages. In karyotype analysis, the common aberration in the four cell lines was marker chromosome t (3;18) (p13;q21). In soft agar culture, HANKS-Pr showed the lowest growth. Furthermore, we found high level expression of major histocompatibility complex (MHC) class I antigen on HANKS-Pr and HANKS-Lu, and low expression of MHC class II antigen on four cell lines. HANKS-LN had transplantability in nude mice. We determined the different biological properties among HANKS cell lines stemming from the same origin.

New mutation of the myelin P0 gene in a pedigree of Charcot-Marie-Tooth neuropathy 1.

P0, the major structural protein of peripheral myelin, is a homophilic adhesion molecule with a single immunoglobulin (Ig) domain, which contains a single N-linked glycosylation site and two cysteines. We have previously reported four different mutations of the myelin P0 gene in four families of Charcot-Marie-Tooth neuropathy type 1 (CMT1). In this study we found a new mutation of the myelin P0 gene in a small family of CMT1. The affected persons had an A - to - G substitution of nucleotide 245 of the myelin P0 gene in one allele, leading to a cysteine substitution for tyrosine82 in the extracellular Ig-domain. An additional cysteine in the extracellular domain may form a disulfide bond and cause an inappropriate change in the tertiary structure of the functional Ig-domain of P0.

[A case of amylase producing lung cancer].

The patient was a 72-year-old man, who was admitted to our hospital because of cough. Chest X-rays showed a mass shadow in the right lower lung field. Amylase activities in serum and urine were extremely high. Amylase isozyme pattern identified salivary type amylase. Cytological examination of the sputum suggested adenocarcinoma. Amylase activities in serum and urine gradually decreased with the administration of chemotherapy. Afterwards, pleural effusion increased, and the amylase activity in pleural fluid was also extremely high. Pleural fluid also showed adenocarcinoma. Enzyme-labeled antibody method (PAP) on this specimen from pleural fluid proved that tumor cells were producing amylase ectopically.

[Radionuclide assessment of heart failure].

Radionuclide assessment of heart failure was reviewed in this paper. The measurement of left ventricular function has been established by using gated blood poor scan. In most cases, left ventricular ejection fraction was below 35% in heart failure. The therapeutic effect of beta-blocker was also evaluated by quantitative sector analysis from gated blood pool scan. In addition, the uptake of 111In-antimyosin Fab was observed at damaged myocardium in dilated cardiomyopathy. 123I-MIBG which reflects sympathetic nerve activity showed decreased uptake in heart failure. It was demonstrated that cardiac MIBG imaging had powerful prognostic value in patients with heart failure. In conclusion, radionuclide assessment will contribute to the evaluation of severity and prognosis of heart failure.

[Effect of UFT and CBDCA combined therapy on human laryngeal cancer transplanted into nude mice].

Nude mice transplanted with a human tumor of the larynx were treated by combination therapy with UFT and carboplatin (CBDCA) for a prolonged period (3 months). To compare the combination therapy with UFT or CBDCA treatment alone, the mice were divided into four groups; control group, group orally given UFT (20mg/kg, five times a week for 3 months), group intraperitoneally given ip CBDCA (50mg/kg, every 3 weeks for 3 months), and group given combination therapy with UFT and CBDCA. No antitumor effect was observed in the group received UFT or CBDCA alone, while a significant antitumor effect was noted in the combination therapy group, compared to the control as evidenced by DNA histogram, showing a decrease in atypical cell count. The results of this study suggest that the combination of UFT and CBDCA provides an efficacious chemotherapy for cancers of the head and neck.

Detection of postural cerebral hypoperfusion with technetium-99m-HMPAO brain SPECT in patients with cerebrovascular disease.

Seventeen of 19 patients (67 +/- 8 yr, 17 males and 2 females) had more than 75% unilateral stenosis or occlusion of the internal carotid or middle cerebral artery and two patients had carotid endarterectomy that previously had 90% stenosis of the internal carotid artery. They were studied during upright 99mTc-HMPAO brain SPECT. HMPA was injected immediately after arising from a supine position. Patients were classified into Group A (n = 10) with occlusion of the internal carotid or the middle cerebral artery or Group B (n = 9) with more than 75% unilateral stenosis of the internal carotid or the middle cerebral artery and with carotid endarterectomy. Additional cerebral blood flow perfusion abnormalities between upright and supine 99mTc-HMPAO brain SPECT were detected in seven patients in Group A and in only one patient in Group B. Semiquantitative analysis showed that the asymmetric ratios between upright and supine positions changed significantly in Group A from 0.82 +/- 0.15 to 0.89 +/- 0.10 (p < 0.01), but not in Group B, from 0.89 +/- 0.11 to 0.92 +/- 0.12 (ns). Additional perfusion abnormalities were relevant to occlusion of the internal carotid or middle cerebral artery indicated postural cerebral hypoperfusion. We conclude that upright 99mTc-HMPAO brain SPECT visualizes postural cerebral hypoperfusion possibly related to silent cerebral ischemia.

[Criteria for clinical evaluation of antibiotics in pediatrics. The purpose and process for establishing the criteria].

This paper describes the purpose and process for establishing "Criteria for Clinical Evaluation of Antibiotics in the Pediatric Field", which was reported in the Japanese Journal of Antibiotics Vol. 46, May, 1993. The Criteria Committee was organized in November 1991. Four meetings were held to establish the draft criteria. The criteria were applied to the evaluation of oral cephem S-1108 and parenteral cephem SCE-2787. When the criteria were compared with the conventional criteria, the results indicated that no difference was obtained in the efficacy rate as a whole, the sum of "Good" and "Excellent" cases, but there was a difference in the cases judged to be "Excellent". Partial alteration was made to the draft criteria and the Committee produced the final version of the criteria. However, the criteria are far from complete, so it will be subjected to further revision it accordance with future advance in chemotherapy.

[Relationship between reduced myocardial uptake of beta-methyl-p-(123I)- iodophenyl-pentadecanoic acid (123I-BMIPP) and regional diastolic ventricular dysfunction in patients with hypertrophic cardiomyopathy].

To assess whether regionally depressed myocardial uptake of beta-methyl-p-(123I)-iodophenyl-pentadecanoic acid (123I-BMIPP), is related to regional ventricular diastolic dysfunction in patients (pts) with hypertrophic cardiomyopathy (HCM), we carried out 123I-BMIPP myocardial SPECT and radionuclide ventriculography (RNV) at rest in 9 HCM pts (eight pts with asymmetric septal hypertrophy (ASH), and one patient with apical hypertrophy). The defects of 123I-BMIPP were semiquantitatively estimated by segmental analysis. Regional LV function was determined by sector analysis in RNV: regional peak filling rate (rPFR), regional time to peak filling rate (rTPFR), and early 1/3 filling rate (1/3 fil) were estimated in the LV septal and posterior sectors. Wall thickness was evaluated with M-mode echocardiography. As results, the defects of 123I-BMIPP myocardial SPECT, were found frequently in septal wall, but any defect was not recognized in all septum with significant hypertrophy. Also, the degree of septal hypertrophy was not significantly related to BMIPP defect score. However, the regional diastolic function (rPFR, rTPFR, 1/3 fil) in the septal lesions with 123I-BMIPP defects was decreased more than those without 123I-BMIPP defect in HCM. Furthermore, the relative regional diastolic dysfunction (the differences between rTPFR and 1/3 fil in the septum and in the posterior wall) was significantly related to BMIPP defect score. In conclusion, this results suggest that the decreased uptake of 123I-BMIPP is related to regional diastolic dysfunction more than wall hypertrophy.

[A case of radiation-induced chronic constrictive pericarditis developing 16 years after irradiation].

We reported a 51-yr-old female with radiation-induced chronic constrictive pericarditis. At age 29, she had received a mastectomy and postoperative irradiation because of left breast cancer. At age 45, she had syncope and was diagnosed with complete atrioventricular block and a pacemaker was implanted. At that time, pericardial thickening with effusion was noted. The following year, tricuspid regurgitation was noted. On catheter study, a dip and plateau pattern of the right ventricular pressure curve appeared. At age 50, tricuspid regurgitation worsened due to the lead wire of the pacemaker compressing the leaflet, and the pacemaker was reimplanted. However, the following year, she complained of general fatigue and dyspnea and was admitted to our hospital. On 67Ga study, diffuse accumulation in the cardiac region appeared. There was no perfusion defect detected in the left myocardium, but right myocardial damage was suspected by thallium study. In 99mTc-HSA RI angiography, right atrium dilatation appeared and a pericardial halo around the ventricles was seen. She underwent pericardectomy, tricuspid replacement and pacemaker reimplanted, but she died. On autopsy, pericardial thickening and adhesion, right myocardial fibrosis, the fibrotic change of the bundle branches were seen. We reported a case of radiation-induced constrictive pericarditis. Radionuclide studies were useful in diagnosing and following the patient.

[Diagnostic vitrectomy for chronic uveitis].

Uveitis is diagnosed by ophthalmologic and systemic examination, but it may be difficult to diagnose accurately in some chronic cases. We performed diagnostic and therapeutic vitrectomy for chronic uveitis in 6 eyes of 4 patients, and examined vitreous specimens with immunohistochemical staining. Visual acuity improved in all cases due to clearing up of the vitreous cells. One patient had sarcoidosis, one had B cell malignant lymphoma (ocular reticulum cell sarcoma), and two had reactive lymphoid hyperplasia. Vitrectomy for chronic uveitis is useful for diagnosis and improvement of vision, and we recommend immunohistological staining of vitreous specimens. Particularly, when intraocular lymphoma is suspected, vitrectomy must be performed early.
