[Acute appendicitis: a study on 118 patients].

One hundred eighteen patients with acute appendicitis operated on at Kawasaki Medical School Hospital during the 8-year period from Aug. 1, 1976 to Feb. 29, 1984 were reviewed. There were 78 children and 40 adults. The clinico-pathological types of acute appendicitis were: simple acute in 35 cases (29.7%); phlegmonous in 30 cases (25.4%); gangrenous in 24 cases (20.3%) and perforated in 29 cases (24.6%). Of the 78 children, 27 (34.6%) had perforated appendixes, whereas of the 40 adults only 2 (5.0%) showed perforated appendicitis. The initial symptoms were abdominal pain in 96.6%; nausea, vomiting or both in 33.1%, and fever in 11.9%. The physical findings on admission were abdominal tenderness in 99.2%, rebound tenderness in 52.5% and palpable mass in 17.8%. The mean body temperature on admission was 37.2 degrees C, and mean WBC count was 12,900/mm3. The roentgenography of the abdomen revealed the "sentinel loop sign" in 66.3%. In the patients under 15 years of age, the frequency of perforation seen in those underwent operation within 24 hours after onset and in those after 24 hours was 12.7% and 56.4%, respectively. From this result, it is advised that all the young patients suspected of having acute appendicitis should be admitted to the hospital promptly, and the diagnosis should be made within 24 hours.

Characteristics of ATPase activity and the subunit composition of myosin in the conduction system of bovine heart.

The ATPase activity, light chains and isoenzymes of myosin from specialized myocardial tissue (the A-V node, bundle of His, and right and left bundle branches) of bovine heart were compared with those of atrial and ventricular myosins. The order of Ca2+-activated ATPase activity was atrial greater than specialized myocardial tissue greater than ventricular myosin. SDS-polyacrylamide gel electrophoresis showed that myosin from the specialized myocardial tissue contained the light chains of both atrial and ventricular myosins. On the other hand, the specialized myocardial tissue contained one V3 isomyosin and showed no difference from ventricular myocardial tissue on pyrophosphate gel.

Analysis of proteins associated with chromosome condensation in baby hamster kidney cells.

To identify proteins concerned with chromosome condensation processes, we used a temperature-sensitive mutant, tsBN2 derived from BHK21, in which premature chromosome condensation occurred at high temperature. When the proteins synthesized in tsBN2 during the induction of premature chromosome condensation were analyzed by two-dimensional gel electrophoresis, we found that an acidic protein with a molecular weight of 35,000 was specifically associated with chromosome condensation. In the normal cell cycle, this protein was synthesized from the G2 through the M phase. The protein was located mainly in the chromosome fraction and was phosphorylated.

Substance P-like immunoreactivity in the pulp-dentine zone of human molar teeth demonstrated by indirect immunofluorescence.

Freshly-extracted human third molars were fixed in Zamboni fixative, demineralized with the mixture of EDTA and the fixative; substance P-like immunoreactivity (SPLI) was revealed by the indirect immunofluorescence technique of Coons. Substance P (SP) was observed in the pulp-dentine zone and the dental pulp. Some of SP-containing fibres ended at the odontoblast layer and did not reach the predentine; others terminated at the predentine surface or penetrated into the predentine. In the predentine, some of SP fibres accompanied odontoblast processes and ended near the mineralized dentine; others changed course transversely at various levels.

Ultrastructure of granular materials in rat incisor enamel organ at early maturation stage.

With scanning electron microscopy, globules were seen in inter-cellular spaces between ameloblasts with ruffled borders and papillary cells at early maturation stage. Numerous globules were located on longitudinal folds between parallel ridges on the sides of ameloblasts, but only a few in the papillary layer. In previously-scanned specimens sectioned for transmission electron microscopy, the globules were composed of finely granular material. Vacuoles containing material identical to extra-cellular granules were observed in intra-cellular portions of the ameloblasts and papillary cells.

Urometric evaluation of intramural ureter function with continual ureteral perfusion.

Urometry provides a valuable test of ureteral function wherein intraureteral pressure changes associated with ureteral peristaltic contraction are recorded via a fine catheter inserted through the bladder into the ureter. An attempt was made to evaluate the function of the intramural ureter by analysis of urometric wave pattern of resting pressure recorded during the phase of no ureteral peristaltic contraction with continual instillation of sterile water via the catheter at a constant rate. By this technique, the length of functional intramural ureter (FUimL) was estimated to be 10-15 mm and the resting pressure in the intramural ureter (Pr) was between 5 and 10 mmHg in patients with possibly normal intramural ureter (4 ureters). In cases of chronic dysuria with cystographic evidence of morphologic changes in the bladder and with no vesicoureteral reflux (VUR), the FUimL was 5 mm and the Pr 2-3 mmHg. The corresponding values were 5 mm and 1 mmHg with a ureter showing VUR. The data obtained indicated that the product of these parameters, FUimL X Pr, may reflect the function of intramural ureter. The method reported herein being procedurally simple requiring no particular apparatus provides a remarkably valuable test for the evaluation of function of intramural ureter which has been virtually impracticable.

[Effect of anabolic steroid on immune response].

Using lymphocyte, monocyte and eosinophil counts of the peripheral blood, PHA-blastoid transformation, immunoglobulin and beta 2-microglobulin, the influence of anabolic steroid on the immune reactivity of the host was dissected by administration of Deca-Durabolin ( nandrolone decanoate) to both tumor-bearing host and tumor-free host after operation for alimentary tract. The number of peripheral lymphocytes and monocytes, the PHA-blastoid transformation of peripheral lymphocytes and the IgG level were increased, and the beta 2-microglobulin level showed the tendency of decrease after the administration of Deca-Durabolin.

Histone H1 and H3 phosphorylation during premature chromosome condensation in a temperature-sensitive mutant (tsBN2) of baby hamster kidney cells.

The histone phosphorylations of temperature-sensitive mutant cells (tsBN2) were investigated during the induction of premature chromosome condensation (PCC). At the permissive temperature (33.5 degrees C), the histones of the cells were phosphorylated typically as in any other mammalian cell. However, at the nonpermissive temperature (40.5 degrees C), both histone H1 and H3 were phosphorylated extensively as in mitotic cells, and the increase in these phosphorylations throughout S to G2 phase was closely correlated to the frequency of cells showing PCC. The pattern of H1 subtype phosphorylations was quite similar, and the sites of H1 phosphorylation from PCC were the same as those from mitotic cells. Although the degree of phosphorylation was low, H1 and H3 phosphorylations were observed even in G1 phase at the nonpermissive temperature. The effects of metabolic inhibitors on the induction of PCC were parallel in H1 and H3 phosphorylations; actinomycin D failed to inhibit either PCC induction or these phosphorylations, whereas cyclohexamide did, completely inhibiting H3 phosphorylation.

Transformation of temperature-sensitive growth mutant of BHK21 cell line to wild-type phenotype with hamster and mouse DNA.

A temperature-sensitive (ts) mutant, tsBN2, which was derived from BHK21 and is defective in the regulatory mechanism for chromosome condensation, was transformed to the temperature-resistant (ts+) phenotype by means of DNA-mediated gene transfer with hamster and mouse DNA. Treatment of mouse DNA with the restriction enzymes EcoRI, HindIII, PstI and SalI, but not with XhoI, almost completely abolished the transforming activity. A fluctuation test, originally devised by Luria and Delbrück, was used to estimate the reversion and transformation frequencies of tsBN2 cultures.

Scanning electron microscopy of the papillary layer of the rat-incisor enamel organ.

The enamel organ isolated from the maturation zone was treated with hydrochloric acid to remove such extracellular materials as collagen fibrils and basal lamina covering the papillary layer. Two types of the ridges were classified on the basis of their morphology and distribution: (1) globular and irregularly arranged ridges at the periphery of the early maturation zone, (2) elliptical and parallel ridges in the central part of this zone and over the whole of the late maturation zone. The fine surface structures of the ridges were characterized by numerous interdigitating cell processes.
