[Clinical study on cefmenoxime (CMX) in urology].

Basic and clinical studies were made on Cefmenoxime (CMX), a new cephalosporin antibiotic, and the following results were obtained. The serum concentration of CMX was examined in four healthy adults after administration of 250 mg of CMX by intramuscular injection, intravenous injection and one-hour intravenous drip infusion (cross over). In the case of intramuscular injection, the peak value of 5.9 micrograms/ml was obtained 30 minutes after administration, and the half-life in serum was 1.41 hours. In the case of intravenous drip infusion, injection, a concentration value of 11.1 micrograms/ml on the average was obtained after 15 minutes of administration, and the half-life in serum was 1.26 hours. In the case of intravenous drip infusion, the concentration was 12.4 micrograms/ml upon completion of drip infusion, and CMX disappeared from serum at a half-life of 0.94 hour. The urinary recovery up to 6 hours was from 60 to 70% in each The efficacy rate of this preparation was 100% for 4 cases of acute simple cystitis. The efficacy rate of CMX was 70% for 10 cases of complicated urinary tract infection; the 3 cases in which CMX was not effective were patients with a residual catheter and Pseudomonas persisting or appearing as superinfection. It was noted that Serratia, which was resistant to the conventional cephalosporin antibiotics, became negative. No subjective side effects due to the administration of this preparation were observed. As for abnormal laboratory findings, a slight and transient rise in transaminases was observed in one case. On the basis of the above-mentioned results, it was concluded that CMX is an effective preparation for the treatment of urinary tract infections.

[The effect of preoperative embolization of Wilms' tumor].

The case was a 2-year and 11-month-old boy with Wilms' tumor. Preoperative embolization using gelatin sponge was performed after diagnosis of aortography. Immediately after the treatment, tumor size and vascularity were reduced remarkably. Nephrectomy was carried out without remarkable bleeding and the tumor was removed easily.

[Histopathological studies on bladder carcinoma. 2: Role of local lymph follicle and immunoblast cells].

The relationship between the cellular stromal reaction and systemic immunity was examined in 34 cases of bladder tumors. To determine the cellular stromal reaction, we examined the local lymph follicles such as peritumoral lymphocyte accumulation, and the immunoblast cells having abundant, basophilic cytoplasm with large vesicular nuclei which contained 1-3 large and irregular nucleoli. To measure systemic immunity, lymphocytes, T cells, B cells, IgG-Fc receptor T cells, and lymphocyte blast formation ratio with PHA were examined. The survival rate was higher in the group with a strong stromal reaction. The distribution of the immunoblast cells showed a tendency to accumulate in the paracortical area of the local lymph follicles. Examination of the depth of tumor infiltration and grade revealed many local lymph follicles and immunoblast cells in pT 2 and G 2 groups. The groups in which lymph follicles and/or immunoblast cells were positive showed a tendency of high lymphocyte blastformation ratio with PHA accompanied by a low percentage of IgG-Fc receptor T cells. These findings suggest that local lymph follicles and immunoblast cells may be a manifestation of the systemic immunity acting as a tumor defense mechanism.

The fine structure of the synovial membrane of the knee joint in rats with special reference to regional differences.

The synovial membranes (SM) in the infrapatellar fat pad (fat pad) and the suprapatellar recessus (recessus) of the rat knee joint were studied by transmission electron microscopy. The architecture of microvasculature in these two regions was also examined by scanning electron microscopy, using the resin casting method. The SM of the rat did not exhibit any species-specific variance in basic structure, however, there were apparent differences in ultrastructure between the two regions; i.e., synovial cells and capillary networks in the fat pad were more tightly packed than those in the recessus. In addition, the occurrence of fenestrated capillaries was higher in frequency in the fat pad than in the recessus. In the experiments using horseradish peroxidase (MW, 40,000) as a tracer material, it was strongly suggested that the SM in the fat pad mediated a more rapid and massive exchange of synovial fluid between the joint cavity and the bloodstream than the SM in the recessus, in which the fenestrated capillaries played a most important role.

Studies of the nonspecific cellular immune response in patients with urinary bladder carcinoma. I. PHA-induced lymphocyte transformation.

The nonspecific cellular immune response, as measured by phytohemagglutinin (PHA)-induced blastoid transformation of peripheral lymphocytes in autologous serum, was evaluated in 119 patients with bladder carcinoma and in 21 control subjects. Blastoid transformation was determined by quantifying 3H-thymidine incorporation (counts per minute). The rate of blastogenesis of lymphocytes was shown as the stimulation index which was calculated by dividing the number of counts per minute with PHA by that without PHA. The stimulation index of the control subjects was 29.9 +/- 6.6 (+/- SD); that of the experimental group was 13.3 +/- 7 (significantly lower reactivity). There was a converse correlation between the stimulation index and the grade and stage of tumor. In some cases the change in cellular immunity was compared before and after surgery.

Studies of the nonspecific cellular immune response in patients with urinary bladder carcinoma. II. Immunodepressive effect of sera from bladder carcinoma.

In vitro reactivity of normal lymphocytes in various sera was determined by quantitation of phytohemagglutinin (PHA)-induced tritiated thymidine incorporation rate (counts per minute). Sera were obtained from normal subjects, fetal calf, and patients with bladder carcinoma. The rate of lymphocyte blastogenesis was expressed as a stimulation index (SI) that was calculated by dividing the number of counts per minute with PHA by that without PHA. The effect of sera on blastogenesis of normal lymphocytes was expressed as percent change (%SI) per SI with fetal calf serum. The SI of normal lymphocytes was similar in autologous serum (26.7 +/- 11.6), fetal calf serum (25.4 +/- 11.8), and heat-inactivated homologous normal AB serum (28.8 +/- 11.4). The SI of normal lymphocytes in sera from patients with bladder carcinoma was significantly lower than in the other two sera (14.0 +/- 7.9). The serum inhibitory effect was significantly associated with the stage and grade of tumor and was diminished after removal of the tumor. The SI of lymphocytes from patients with certain other malignant diseases was significantly lower in autologous serum than in fetal calf serum or homologous normal AB serum. The SI of the lymphocytes from patients with prostatic carcinoma and embryonal cell carcinoma was also low in sera from patients with bladder carcinoma as well as in autologous sera. It is likely that the depressive effect of sera from patients with bladder carcinoma was not specific to bladder carcinoma.

[Clinical study of carfecillin in chronic urinary tract infection (author's transl)].

1. Ten patients with chronic urinary tract infection by Pseudomonas aeruginosa or Proteus species were treated with carfecillin. Excellent response was seen in one case, fairly good in 5 cases and clinical effective rate was 60%. 2. The recurrence suppression effect of carfecillin was examined in two patients suffering from complicated urinary tract infection. In both cases, recurrence was not found when carfecillin was administered for a week after the effective pretreatment of carbenicillin, but found when administered for two weeks. 3. Side effects were observed in 3 cases (itching, tinnitus, diarrhea).
