Evaluation of lymphocyte proliferative responses to casein hydrolysate formula in cow's milk-sensitive children: response of peripheral blood mononuclear cells to hydrolysate formula.

The proliferative responses of peripheral blood mononuclear cells (PBMCs) to cow's milk proteins and a casein hydrolysate formula were investigated in 10 cow's milk-sensitive patients. The casein hydrolysate formula reduced the symptoms in seven children but not in three. The proliferative responses of PBMCs to cow's milk proteins and the formula were higher in the three patients whose symptoms were not reduced by casein hydrolysate formula. However, the proliferative responses of PBMCs to hydrolysate formula were lower than those to cow's milk proteins in patients whose symptoms were reduced by casein hydrolysate formula. These results suggest that an elimination diet with casein hydrolysate formula may not be as effective in some patients with cow's milk allergy whose PBMCs proliferate to casein hydrolysate formula.

Tranexamic acid reduces blood loss after cardiopulmonary bypass.

To evaluate the effect of tranexamic acid (TA) on blood loss after cardiopulmonary bypass (CPB), 157 patients who underwent elective valve replacement operations were studied, with one group of 90 patients receiving tranexamic acid (Group TA) and 67 patients serving as the control group (Group N). In group TA, 50 mg/kg of tranexamic acid was administered just before and after CPB, and every 90 minutes during CPB. The activated coagulation time was maintained at more than 450 seconds during CPB in both groups. There was no significant difference in the CPB time between the groups (163 +/- 32 min in group N and 152 +/- 38 min in group TA:NS). The time required for hemostasis was shortened in group TA, which resulted in a shorter operation time (6.7 +/- 1.5 hrs vs 6.0 +/- 1.5 hrs in group N and group TA, respectively: p = 0.006). The amount of chest tube drainage within 12 hours after surgery was significantly reduced (225 +/- 129 ml vs. 180 +/- 118 ml in group N and group TA, respectively: p = 0.026). The chest tube was able to be removed earlier in group TA, and the total blood loss was significantly smaller in group TA (402 +/- 292 ml) than in group N (631 +/- 609 ml; p = 0.004). The authors thus conclude that antifibrinolytic therapy during CPB with tranexamic acid reduces postoperative blood loss, and shortens the operation time due to an improvement in hemostasis.

Evaluation of lymphocyte proliferative responses to food antigens with regard to age and food-specific IgE antibodies in food-sensitive atopic dermatitis.

Peripheral blood mononuclear cells (PBMCs) from patients with atopic dermatitis (AD), selected for sensitivity to hen's egg or cow's milk, showed significant proliferation in response to food antigens, ovalbumin or bovine serum albumin compared with PBMCs from nonatopic control subjects. The proliferative responses of PBMCs to food antigens were evaluated with regard to age and specific IgE antibodies to food antigens in food-sensitive patients with AD. There were no differences in proliferation between each age group, and the proliferation in patients with AD was significantly higher than in age-matched control subjects. There was no relationship between proliferative responses of PBMCs and specific IgE antibodies to food antigens in patients with AD. These results suggest that the responsiveness of lymphocytes to food antigens is independent of age and food-specific IgE antibodies in food-sensitive patients with AD.

Medial antebrachial cutaneous nerve conduction in true neurogenic thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) and its diagnosis have been topics of controversy, fueled in part by the lack of an objective test for early diagnosis. True neurogenic TOS results in diagnostic EMG findings only in its later stages. We describe a series of patients in whom the diagnosis is supported by changes in the medial antebrachial cutaneous (MAC) nerve at or before the development of these typical EMG findings in median and ulnar nerves.

Hepatic sinusoid endothelial dysfunction plays a role in hyperbilirubinemia in patients following implantation of an LVAD.

To clarify the mechanism of hyperbilirubinemia in the setting of a left ventricular assist device (LVAD), the change in hepatocellular function, hepatic sinusoid endothelial microcirculation, and inflammatory response before and after LVAD implantation were evaluated. Eight consecutive patients underwent the placement of an LVAD, and serum levels of total bilirubin (TB), transaminases [alanine transaminase (ALT), aspartate transaminase (AST)], interleukin (IL-6, IL-8), and hyaluronic acid (HA), an indicator of hepatic sinusoidal circulation, were measured before and after LVAD implantation. The TB of all patients increased significantly in the first post operative week (p < 0.05 vs. pre-operatively). In five patients, the elevated TB (4.6 +/- 4.1 mg/dl) returned to pre-operative levels (2.7 +/- 2.0 mg/dl) by the 14th post operative day (Group R), but in the other three patients who died of multiple organ failure, the level of TB increased to 39.9 +/- 16.4 mg/dl (Group A). Levels of HA and IL-8 had good correlation with the level of TB (HA: r = 0.60, p < 0.05; IL-8: r = 0.55, p < 0.05). However, AST, ALT, and IL-6 were not related to changes in TB. These results suggest that hepatic sinusoid endothelial dysfunction and inflammatory reaction may play a significant role in hepatic failure in patients following implantation of an LVAD.

Intrasplenic autograft of testicular tissue in rats.

Intrasplenic heterotransplantation of infantile testis is a conventional method to produce testicular tumors in adult castrated rodents. To avoid possible immunological interference in the tumorigenic process in allografted testis, the parenchyma of testis of Wistar strain rats aged 7 weeks was injected into the spleen of the same rat after bilateral gonadectomy. In 17 of 20 animals testicular neoplasms, resembling gonadal stromal cell tumors in human, developed from the intrasplenic gonadal tissue in the 14th month following transplantation. This autografting technique is feasible to induce testicular neoplasms in granulosa cell tumor category in high incidence, and thus deserves further attention.

Intrasplenic grafting of ovarian tissue containing 7, 12-dimethylbenz[a]anthracene.

To produce malignant granulosa cell tumors, ovarian tissue containing 7,12-dimethylbenz[a]anthracene (DMBA) was autografted in the spleen of 20 castrated rats. Adenocarcinoma developed in 8 rats within 6 months from the transplantation. Granulosa cell tumor arose from the intrasplenic graft, in 6 rats at the 14th month. The neoplastic cells were diffusely arranged in sheet showing focal luteinization. In 3 cases extrasplenic spreads were found on the peritoneal surface. Since malignant ovarian tumors seldom develop by intrasplenic grafting alone, the DMBA treatment may contribute to the occurrence of malignant granulosa cell tumor.

Experimental and clinical evaluation of cisplatin-containing microspheres as intraperitoneal chemotherapy for ovarian cancer.

BACKGROUND: We aimed to evaluate the in vitro and in vivo effects of poly [L-lactic acid] microsphere containing cisplatin (CDDP-MS) for intraperitoneal (i.p.) chemotherapy for ovarian cancer. METHODS: We initially examined the in vitro and in vivo profile of cisplatin release from the CDDP-MS, then this drug delivery system was evaluated in 15 patients. RESULTS: The in vitro study showed that cisplatin was released constantly over a 3-week period. Rats in the CDDP-MS group had a significantly lower peak serum concentration of platinum compared with rats in the aqueous cisplatin solution (CDDP-S) group; the serum concentration of platinum showed a gradual decline. The ascitic fluid concentration of platinum also gradually decreased in the CDDP-MS group. We treated 15 patients with recurrent ovarian cancer with CDDP-MS containing 200 mg of cisplatin (n = 5) or CDDP-S containing 100 mg of cisplatin (n = 10) administered i.p. The peak serum and ascites concentrations of platinum were lower immediately after administration of CDDP-MS than after administration of CDDP-S, but increased over time in the CDDP-MS group, reflecting the slow-release effect of CDDP-MS. Grade 1 to 2 leukopenia and/or neutropenia occurred in 2 of 5 patients. No thrombocytopenia or renal or neurologic toxicity was observed; CONCLUSION: These findings indicate that the i.p. administration of CDDP-MS increased the dose intensity of cisplatin and appeared to be safe and effective for the treatment of ovarian cancer.
