Possible involvement of dopamine D-1 and D-2 receptors in diazepam-induced hyperphagia in rats.

Possible involvement of dopamine receptors in diazepam-induced (1 mg/kg, subcutaneous (sc] hyperphagia was studied in nondeprived rats. Pretreatment with the selective D-1 antagonist, SCH23390 (0.03 mg/kg, sc) inhibited diazepam-induced hyperphagia. In addition, pretreatment with the preferential D-2 antagonists, haloperidol (0.1 to 0.3 mg/kg, sc) and clebopride (0.1 to 0.3 mg/kg, sc) inhibited diazepam-induced hyperphagia in a dose-dependent manner. Pretreatment with co-administration of SCH23390 (0.1 mg/kg, sc) and clebopride (0.03 mg/kg, sc) completely inhibited this hyperphagia. The selective D-2 antagonist, sulpiride (40 mg/kg, sc) and the peripheral D-2 antagonist, domperidone (10 mg/kg, sc) did not affect diazepam-induced hyperphagia. However, sulpiride (10 micrograms, icv) or domperidone (2 micrograms, icv) administered centrally inhibited this hyperphagia. The highest dose of haloperidol (0.3 mg/kg, sc) or clebopride (0.3 mg/kg, sc) and higher doses of SCH23390 (0.01 and 0.03 mg/kg, sc) or SCH23390/clebopride (0.01/0.03 and 0.01/0.1 mg/kg, sc) tended to decrease spontaneous feeding in non-deprived rats. In addition, the highest dose of haloperidol, clebopride or SCH23390/clebopride inhibited spontaneous feeding in deprived rats. Interestingly, diazepam-induced hyperphagia was inhibited significantly by doses of haloperidol (0.1 mg/kg, sc), clebopride (0.1 mg/kg, sc) and SCH23390/clebopride (0.003/0.03 and 0.003/0.1 mg/kg, sc) which did not affect spontaneous feeding in non-deprived or deprived rats. Pretreatment with alpha-methyl-p-tyrosine (40 mg/kg, IP x 2, 6 and 2 h prior to diazepam administration) failed to inhibit this hyperphagia. Furthermore, pretreatment with a large dose of haloperidol (5 mg/kg, sc, 4 days before diazepam administration) augmented the sub-hyperphagic effect to diazepam (0.5 mg/kg, sc). Thus, these findings suggest that hyperphagia to diazepam is mediated in part by both dopamine D-1 and D-2 receptors in non-deprived rats.

Three cases of multiple myeloma developing into melphalan-related chronic myelomonocytic leukemia.

From 1980 to 1987, three cases of chronic myelomonocytic leukemia (CMML) were encountered among 68 cases of multiple myeloma who survived more than three years from the diagnosis. The incidence (4.8%) of secondary myelodysplastic syndrome (MDS) is almost identical to previous reports, but case reports of chronic myelomonocytic leukemia were rare. In Japan, there are few reports of multiple myeloma patients who later developed secondary MDS or acute myelogenous leukemia (AML). In our cases, none of the 31 patients treated with cyclophosphamide developed secondary MDS, while three of 37 patients treated with melphalan developed CMML. This difference is not statistically significant.

A patient with primary plasma cell leukemia accompanied by an extensive polypoid infiltration of the gastrointestinal tract.

A 37-year-old male presented with primary plasma cell leukemia (PCL) with kappa-type Bence Jones proteinuria (BJP) and polyposis of the stomach and colon. His plasma cell leukemia was not preceded by a pre-existing multiple myeloma, and presented the complication of polyposis in both stomach and colon. Biopsy of mucosal lesions revealed marked accumulation of atypical plasma cells with positive cytoplasmic kappa-chain. No amyloidosis was present. His disease responded remarkably well to intermittent melphalan and prednisolone. This case represents an uncommon combination of primary plasma cell leukemia and polypoid gastrointestinal lesions with plasma cell infiltration.

T-zone lymphoma in association with systemic lupus erythematosus.

Two patients with systemic lupus erythematosus (SLE) and T-zone lymphoma are described. On admission, both showed arthralgia, generalized lymphadenopathy, hypergammaglobulinemia and positive antinuclear antibody. Lymph node biopsies revealed diffuse infiltration of atypical T-lymphocytes in the expanded interfollicular area (T-zone), a finding characteristic for the T-zone lymphoma. Renal biopsy showed lupus nephritis and neoplastic lymphoid cell infiltration in the glomeruli of one patient, but only diffuse infiltration of neoplastic lymphoid cells and mature plasma cells were observed in the interstitium of the other patient. Both patients responded remarkably well to prednisolone (1 mg/kg/day).

[A case of T cell lymphoma of the lung].

A 70-year-old woman complained of cough, sputum and fever. Chest roentgenogram showed a mass-like lesion with pleural effusion in the left thorax. Bronchoscopy revealed a polypoid lesion at the orifice of left B10 and mucosal swelling of the left lower lobe bronchi. The biopsied material of the polypoid lesion was diagnosed as malignant lymphoma (diffuse small cell type by LSG classification) and the immunohistochemical examination showed monoclonal proliferation of T lymphocytes. The atypical lymphocytes which were positive for T lymphocyte surface marker were detected in the pleural effusion. There was no evidence of metastasis in any part of the body under clinical examinations, and the lung was thought to be a primary site of the lymphoma. The doubling time of tumor was estimated to be 134 days. Four-drug combination chemotherapy resulted in a marked decrease of the tumor size. Immunohistochemical study to determine the subpopulation of lymphoma cells are required for diagnosis of pulmonary lymphoma, because the biological characters of T cell lymphoma might differ from those of B cell lymphoma.

[Treatment of 61 patients with acute myelogenous leukemia at first relapse].

Treatment results in the 61 adult patients with AML in first relapse were analyzed to establish a better strategy for this group of patients. These patients received reinduction chemotherapy during 1979-1988. Complete remission (CR) was obtained in 57.4% of the cases, and the probability of survival and remaining in CR at five years was 11.9% and 17.9%, respectively. The longer duration of initial remission was favorable factor for achieving second CR. Type of reinduction regimens which were different from those used in the initial induction phase did not influence the second CR rate. The use of different consolidation regimen appeared to favorably affect the survival and probability of remaining in CR.

Comparative study of intravenous administration of Ringer's lactate, Ringer's acetate and 5% glucose containing these Ringer's solutions in human being.

The effects of the administration of Ringer's lactate (L) and Ringer's acetate (A) solution on blood biochemistry in human subjects operated for tympanoplasty under general anesthesia were investigated. And the feasibilities of the clinical use of Ringer's lactate (LD) and Ringer's acetate (AD) solution containing 5% glucose were also assessed. In all cases the rate of infusion was 500 ml for initial 20 min, and then 5 ml.hr(-1).kg(-1) B.W. for 3 hr and 10 min. There were significant increases in blood L- and D-lactate, pyruvate, and L-lactate/pyruvate ratio in L group. A significant increase in blood acetate but not lactate was found in A group. These metabolic changes were minimal and considered as clinically not significant. The urinary excretion of lactate, pyruvate, acetate and glucose were also negligible. In both LD and AD group, the higher blood concentrations of lactate, pyruvate, acetate and glucose were found than in L and A group. Urinary excretions of these metabolites were much higher in LD and AD group than in L and A group. So glucose containing Ringer's lactate or acetate solutions should be administered in appropriate amounts and rate not to induce clinically significant metabolic alterations.

FK506, a novel immunosuppressive agent, induces antigen-specific immunotolerance in active Heymann's nephritis and in the autologous phase of Masugi nephritis.

FK506 is a new drug which has potent immunosuppressive activity. We studied its immunosuppressive effects on active Heymann's nephritis and the autologous phase of Masugi nephritis. The induction of active Heymann's nephritis was completely suppressed by FK506 injected simultaneously with the antigen (day 1) and then daily for 14 days at a dose of 0.64 mg/kg per day or more. With a lower dosage of this agent, antibody production and immune deposits in the glomerular basement membrane occurred despite the suppression of proteinuria. Similar results were obtained in rats on other treatment schedules (1-7 days or day 8-14 days duration). Rats that were prevented from developing Heymann's nephritis or the autologous phase of nephrotoxic antiserum nephritis by FK506 treatment exhibited a suppressed immune response to a second immunization of the same antigen even 4 weeks after cessation of drug administration: however, they developed antibodies when inoculated with other antigens. Rat peripheral leucocyte counts and serum creatinin were not remarkably influenced by the administration of FK506. These results indicate that FK506 has potent immunosuppressive activity, and it is suggested that it is able to induce an antigen-specific immunotolerance.

[Efficacy of a 48-hour infusion of 5-fluorouracil in patients with stage IV gastric cancer after palliative gastrectomy].

Efficacy of a 48-hour infusion of 5-FU in patients with stage IV gastric cancer was investigated. A one-cycle regimen consisted of a 48-hour infusion of 5-FU (30 mg/kg/24 hours) every week for 6 weeks. Fourteen patients with stage IV gastric cancer who underwent absolutely non-curative gastrectomy, received 5-FU infusion postoperatively. Ten of the 14 received more than 2 cycles. Survival rates in these patients (group I) were evaluated by Kaplan-Meier method and compared with those of matched pair controls (group II) who received other anticancer agents. Side effects were generally mild. Nausea during infusion was observed in only 3 cases. The 50% survival period was 393 days in group I and 135 days in group II, respectively. The survival curve in group I was significantly higher than in group II (p = 0.05). From these results, this treatment is considered to be very effective.

Ultrastructure of multinucleated giant myeloma cells: report of one case.

The ultrastructure of multinucleated giant myeloma cells obtained from an IgA kappa myeloma patient was investigated. The characteristic findings were marked nucleocytoplasmic asynchrony, irregular nuclear indentation, branched mitochondria, and Golgi apparatus surrounded by nuclei. The patient died of renal failure only 6 months after diagnosis.

Bilateral tympanic cavity infiltration with effusion in a patient with acute myeloblastic leukemia.

A case of acute myeloblastic leukemia with bilateral tympanic cavity infiltration with effusion in addition to right facial and bilateral auditory nerve involvement is described. Tympanotomy demonstrated the presence of leukemic cells in the effusion of bilateral tympanic cavities. While repeated examinations of the cerebrospinal fluid using the cytocentrifuge failed to show any leukemic cells, it was suspected that the bilateral tympanic cavity infiltration was associated with meningeal involvement.

Clear cell carcinoma of the thyroid. A case report.

A rare case of clear cell carcinoma of the thyroid is presented herein. A 31-year-old male had a large soft tumor in the right lobe of the thyroid which showed a hot nodule on scintigram with 99mTc. A right hemithyroidectomy was thus performed after the preoperative diagnosis of a functioning tumor had been made. At surgery, a tiny nodule behind the tumor was found and the histopathological diagnoses of the large tumor and the tiny nodule were clear cell carcinoma and papillary carcinoma, respectively. The thyroglobulin stain was positive in the cytoplasm of many clear cells and scattered colloid. This patient has been well without any evidence of recurrence over the past five years following surgery. The biological characteristics of clear cell carcinoma of the thyroid are discussed in this report.

[Myeloid crisis of chronic myelogenous leukemia showing dramatic response to mithramycin and hydroxyurea combination].

After 4 years of chronic phase, a 22-year-old female with Ph1 (+) chronic myelogenous leukemia developed myelomonocytic crisis. On admission, her Hb was 9.9 g/dl, Plt 4.1 x 10(4)/microliters, WBC 138,000/microliters with 16.5% blasts. Bone marrow contained 38% blasts. She received a combination chemotherapy of mithramycin and hydroxyurea, as reported by Koller et al. Dose of mithramycin was reduced to 20 micrograms/kg. Following 1st and 2nd infusions of mithramycin, severe nasal bleeding was seen. Prednisolone 10 mg/day was given from the 3rd dose of mithramycin with apparent hemostatic effects. Calcium gluconate 3 g/day was administered concomitantly. Her disease responded promptly to this treatment and hematological remission was achieved.

Intraplatelet and urinary serotonin concentrations in systemic lupus erythematosus with reference to its clinical manifestations.

Intraplatelet and plasma serotonin (5-HT) levels were measured in 39 patients with systemic lupus erythematosus (SLE) and in 15 healthy subjects. Urinary 5-HT concentrations were also measured in 32 patients with SLE and 14 healthy subjects. A significantly lower intraplatelet 5-HT level was observed in SLE patients, especially in the active phase with hypocomplementemia (CH50 less than 30 U/ml), 0.17 +/- 0.09 pmol/10(5) platelets, than that in normal controls, 0.36 +/- 0.14 pmol/10(5) platelets (p less than 0.01). Active SLE showed a significantly higher urinary 5-HT concentration, 0.37 +/- 0.15 nmol/ml/mg Cr, than normal controls, 0.21 +/- 0.08 nmol/ml/mg Cr (p less than 0.01). Serial measurements of intraplatelet and urinary 5-HT levels in five patients with SLE revealed a significant correlation between clinical activity and intraplatelet and urinary 5-HT levels. Exogenous 5-HT uptake by platelets in vitro is not altered in patients with SLE. The mechanism of decrease of intraplatelet 5-HT and increase of urinary 5-HT in SLE may be explained by the continuous activation of the blood coagulation within the vessel.

[Result of surgical treatment to benign thyroid tumor].

Surgical results of 81 cases with various benign thyroid tumors were reviewed. All had undergone mainly hemi-lobectomy including isthmectomy and had been followed up for more than 2 years. Sixty-eight (84%) have been well without any complaint. However, 5 of 15 cases with adenomatous goiter (AG) again developed nodule(s) in the remnant thyroid and 1 of 37 cases with adenoma developed cancer in the remnant. In order to prevent recurrence in the cases with AG, total thyroidectomy may be required with the meticulous selection of the case. For the cases with AG which undergone hemilobectomy or lumpectomy, TSH suppression treatment may be also required after surgery. It is of caution that the nodule developed in the thyroid after surgery may be not always the same nodule as primary one.
