Pancreatic hormone changes in infantile obstructive jaundice.

Metabolic disturbances of pancreatic hormones in obstructive jaundice in infancy were evaluated experimentally and clinically. In our experimental study, using young rats, the level of plasma insulin (IRI) gradually increased after ligation of the common bile duct. These levels were a little lower than those in the non-treated controls. The level of plasma glucagon (IRG) increased remarkably 4 weeks after ligation of the common bile duct. Clinically, there were no significant differences in the levels of IRI and IRG among normal controls and cases of neonatal hepatitis and congenital biliary atresia (CBA). In CBA patients, these levels can be correlated with the progression of hepatic fibrosis; an increase in IRG and a decrease in the IRI/IRG mol ratio was noticed in patients with grade III of hepatic fibrosis. These results indicate that, in obstructive jaundice in infancy, the more severe the hepatic damage due to obstructive jaundice, the higher the level of plasma glucagon concentration will rise.

[Establishment of an inbred strain of LEC (Long Evans Cinnamon) rats with spontaneous hepatitis].

At the Center for Experimental Plants and Animals, Hokkaido University, two inbred strains, Long Evans Cinnamon (LEC) and Long Evans Agouti (LEA), which were selected for coat colour, were isolated from a closed colony of Long Evans rats. While the two inbred strains were maintained by sibmating, only LEC rats, over 24-generation, spontaneously developed acute hepatitis with sudden appearance of systemic jaundice at around four months after birth. The frequency of acute hepatitis was 80 to 90% and the disease in nearly 80% of these rats were progressive and they died within two weeks after onset, with their clinical course and histopathological findings similar to those of human fulminant hepatitis. LEC rats with spontaneous hepatitis had strong-conversion of urine-bilirubin, ultimate increase of blood-bilirubin and abnormal increase of serum-transaminases (GOT, GPT; GOT greater than GPT). Histopathological findings of the livers in the rats with acute hepatitis showed spotty necrosis and abnormal hepatocytes containing giant bizarre nuclei and in the rats with fulminant-type hepatitis showed central or coagulated necrosis and marked infiltration of inflammatory cells. Serum levels of albumin in LEC rats before being affected by hepatitis were low compared with those of LEA rats and especially characteristic fact was that cellulose-acetate electrophoresis could not reveal gamma-globulin fraction in LEC rats of 6-week and 12-week old, which will be a clue to analyze the etiology of hepatitis in LEC rats.

[Development of a new therapeutic method in hepatocellular carcinoma using a culture system--the intensifying effect of human epidermal growth factor (hEGF) on the antitumor action of antitumor agents].

The culture system of hepatocellular carcinoma (HCC) has been established and the effect of hepatotrophic factors on these cell lines have been studied. During this study the effect of the human epidermal growth factor intensifying anti-tumor action of anti-tumor agents will be evaluated. HCC cell lines (C-HC-4, C-HC-20) show logarithmic growth in the serum-free medium. These growth curves after administration of 1 x 10(-12- -8) M of hEGF were measured, and the existence of the hEGF receptors was evaluated using the ABC method. The growth curve of HC-4 transplanted into nude mice was measured in 4 groups; the control group, the hEGF group, the gamma Interferon (gamma IFN) group, and the hEGF + gamma IFN group. The hEGF enhanced the cell growth of C-HC-4 and C-HC-20, and EGF receptors were confirmed immunohistologically. In the hEGF+ gamma IFN group, growth inhibition seems to be most important. The hEGF may intensify the anti-tumor action of the anti-tumor agents.

[Experimental analysis of postoperative early recurrence of liver cancer].

The author suspected that the high incidence of early recurrence after macroscopically curative operation in human liver cancers correlated with the production of liver regeneration factor which was induced following partial hepatectomy (PH). The author therefore analyzed whether PH enhanced the growth of liver cancers or not, and the relevant mechanism involved, using rats subcutaneously injected with hepatocellular carcinoma (KDH-8, AH-66) cells. Primarily, it proved that PH significantly enhanced the growth of liver cancers injected in rats. The effect of this enhancement of liver cancer growth appeared as an abrupt increase in tumor volume within 24 hours following PH, which fact was supported by the mitotic indices of the hepatocellular carcinoma (KDH-8) cells. However PH did not affect rats injected with mammary carcinoma (SST-2) cells without estrogen receptor (E2R) or fibrosarcoma (KMT-75) cells. Secondly, based on this result, the author tried to analyze the mechanism of enhanced growth of liver cancers following PH, from the standpoints of; changes in postoperative immunity, expression of cytosol E2R in liver cancer cells or liver regeneration factor, using KDH-8 cells. The changes in postoperative immunity (NK-activity and Blastogenesis) did not correlate with the changes in liver cancer growth. Although serum estradiol (E2) increased significantly after PH, E2R was not detected in the KDH-8 cells used in this experiment. Serum was obtained from healthy rats 24 hours after PH, and 20 mg of serum, as calculated from total protein, was eluted into 50 fractions by high liquid chromatography (column; TSK G3000 SW). When the author examined which fractions stimulated both the growth of primarily cultivated hepatocytes and KDH-8 cells, only the fraction Fr. 30, the molecular weight of which was about 100 Kd, enhanced both. Furthermore, the author performed an in vivo assay to determine the number of days needed for tumor appearance: PHs were carried out 2 months, 5 days and 1 day before, at the same day of, and 1 day and 5 days after KDH-8 cell (500 cells/100 microliters, sc) inoculation. The author also noticed from these in vivo tests that PHs which were performed 1 day before, at the same day of, and 1 day and 5 days after the KDH-8 cell inoculation enhanced significantly the growth of liver cancers.

[Pathogenesis of spontaneous hepatitis in an inbred strain of LEC (Long Evans cinnamon) rats].

We have reported an inbred strain LEC rats with high frequency of spontaneous hepatitis. In this paper, we study a possible involvement of hepatitis virus, hereditary background, and correlation between serum IgG level and onset of hepatitis in LEC rats. Neither electron microscopy nor indirect immunofluorescent test could detect hepatitis virus article or antigen. Furthermore, we could not succeed in inducing hepatitis of LEA and WKA/H rats by injecting the serum, plasma and liver homogenates of the affected IEC rats, nor in promoting/developing hepatitis of LEC and LEA rats under the immunosuppressed condition by administration of steroid hormone. When we made a mating between LEC and non-hepatitis rats to investigate the hereditary mode, F1s had no hepatitis, but F2s and backcrosses developed hepatitis. We observed, in particular, the lower the serum IgG level was, the higher the rate of developing hepatitis was. From these results, we speculate that the involvement of hepatitis virus is not likely, but that a genetic mutation might cause low level of IgG and be also correlated with development of hepatitis.

[Combined immunodeficiency in LEC (Long Evans Cinnamon) rats with spontaneous hepatitis].

We investigated LEC rats immunopathologically which spontaneously developed hepatitis to find out the genesis, in comparison with non-hepatitis LEA (Long Evans Agouti) rats. 1) Wet weights of the spleen and thymus of 6-week old LEC rats were significantly lighter than those of LEA rats of the same age. 2) Serum IgG (Immunoglobulin G) in LEC rats remained markedly low after the age of two months and IgG antibody formation to SRBC (Sheep Red Blood Cell) as detected by plaque assay was also significantly suppressed. On the other hand, IgM antibody formation to SRBC was significantly suppressed through serum IgM level in LEC rats was normal or rather increased. 3) Blastogenic responses of spleen cells to PHA and Con A were much more suppressed in LEC rats than in LEA rats. 4) Cytostatic activity of intraperitoneal macrophages against tumor cells was more evident in LEC rats than in LEA rats, but there was no difference in NK (natural killer) activity between the two rat strains. From these results, it is speculated that spontaneously hepatitis-developing LEC rats possess T and B cell deficiency (combined immunodeficiency) and that the increase of macrophage and NK cell activities are linked to the genesis of developing hepatitis.

Diagnosis of arteriovenous malformation of the pancreas by color Doppler ultrasonography.

Arteriovenous malformation (AVM) of the pancreas is a rare disease, and once this silent disease develops portal hypertension it is too difficult for the physician to successfully control the disease. AVM is usually diagnosed by invasive diagnostic modalities, and noninvasive modalities should be developed for its diagnosis. We have experienced two cases of AVM of the pancreas, which were located at the pancreatic head and diagnosed by color Doppler ultrasonography (Doppler US). The two AVM lesions were detected as a mosaic pattern, and the lesions were connected to the main portal vein on Doppler US; the pulsatile waves were also observed in the mosaic lesions by the analysis of the Doppler spectrum. We determined the two patients suffered from congenital AVM of the pancreas, since they had no abnormal findings on past and physical histories, laboratory data, and other images. One patient refused surgery, and another underwent pancreatoduodenectomy. Histology of the resected specimen demonstrated the lesion was AVM of the pancreas. These cases support that the present modality is useful for the diagnosis of the pancreatic AVM as a noninvasive procedure, and we discuss the usefulness of noninvasive Doppler US which shows not only static but also hemodynamic informations.

Endoscopic prediction of tumor depth of gastric carcinoma for assessing the indication of its limited resection.

Limited surgery for an early gastric carcinoma is advocated, since certain carcinomas have no nodal involvement. However, the endoscopic accuracy of distinguishing each cancer-depth has not been detailed from the standpoint of limited surgery. We retrospectively reviewed a total of 2,628 patients to assess the diagnostic accuracy of their endoscopic infiltration-depth with the nature of the tumors. Endoscopic distinction of early from advanced carcinomas was satisfactory with a reliability of 86.5%, sensitivity of 87.1%, and specificity of 85.9%. In the 1,354 early gastric carcinomas the microscopic infiltration-depth was significantly related to macroscopic appearance, histologic differentiation and tumor size. Accompanying ulcer or scar significantly suggested that the carcinoma had spread vertically and horizontally. Macroscopically elevated and differentiated carcinomas without ulcer are usually limited to the mucosa, and undifferentiated and/or ulcer-positive carcinomas are more invasive than predicted by most present clinical standards. Endoscopically differential diagnosis of the infiltration-depth of gastric carcinomas is reliable, and the indication for limited surgery can be endoscopically determined in many individual patients.

Blunt liver trauma: a new concept for classification of liver trauma based on vessel injury.

We reviewed a consecutive 143 cases of blunt liver trauma, 43 children and 100 adults, to clarify the pathosis of liver trauma and to reasonably select treatment, non-operative or operative management. The primary determinant in the prognosis of liver trauma was clearly hemorrhage due to vessel injuries and its main treatment method was hemostasis. We developed a new concept for the classification of blunt liver trauma based on vessel injuries: I = subcapsular Glissonian vessel injuries, II = transcapsular Glissonian vessel injuries and III = in-/out-flow vessel injuries and 3 basic types were divided into 2 subtypes, respectively. Distribution of liver trauma according to the present classification was as follows: type I = 23% in child cases and 22% in adult cases, type II = 65% and 64% and type III = 12% and 14%, respectively. The main management was non-operative for type I, minor or major surgery for type II, and extended surgery for type III. The analysis of dead cases showed the following: I = 0% in child cases and 6% in adult, II = 60% and 55%; III = 40% and 39%, respectively. The first direct cause was hemorrhage: 60% in child cases and 49% in adult. The present classification was applicable for both child and adult patients, and presented a selection of management for liver trauma, suggesting the prognosis.

Surgery and outcome of alveolar echinococcosis of the liver: historical comparison of mass screening systems in Japan.

The prognosis of patients with alveolar echinococcosis of the liver (AEL) is excellent when the lesions were completely resected, suggesting the significance of mass screening systems. We investigated resectability and prognosis of the disease with historical comparison of screening systems. The patients were classified into three groups: Group A (n = 63) detected by the current screening system, enzyme-linked immunosorbent assay and ultrasonography, since December 1984; Group B (n = 39), detected by a serological screening system performed between 1974 and 1984; and Group C (n = 64), non-screened patients accidentally discovered. The lesions at an early stage were most frequently found in group A, and the complete resection was performed at 74.6% in A, 41.0% in B, and 18.8% in C (p < 0.0008). Five-year survival rate was 100% in A, 74.0% in B, and 69.5% in C, with a significant intergroup difference (p < 0.05). The current system facilitates early diagnosis of the disease, contributing to its resectability and to a better outcome.
