Immunocytochemical localization of nicotinic acetylcholine receptor in the rat cerebellar cortex.

Although nicotinic acetylcholine receptors (nAChRs) have been reported to function in the cerebellar cortex, nicotinic acetylcholine receptor subtypes contributing to nicotinic currents in the cortex have not been well characterized at the subcellular level. This study deals with immunocytochemical localization of the nicotinic acetylcholine receptor alpha4 subunit using a monoclonal antibody against the alpha4 subunit. Alpha4-LI (alpha4-like immunoreactivity) was detected in the cell bodies of molecular, Purkinje cell and granular layers. In particular, the cell bodies of Purkinje cells were extensively immunostained. In Purkinje cells, alpha4-LI was found in perikarya mainly associated with rough endoplasmic reticulum, plasma membrane, and cytoplasmic matrix. At higher magnification, the immunoreaction product was densely localized along with somatic plasma membranes at the axo-somatic synapse and the plasma membranes at extrasynaptic regions of cell bodies. Alpha4-LI was also found in the axon terminals which form synapses with Purkinje cells. In the granular layer, somatic cell membranes of granular cells were immunostained. These morphological observations suggest that alpha4-containing nAChRs contribute nicotinic currents reported in Purkinje cells, and that presynaptic alpha4-containing nAChRs regulate the release of neurotransmitters on the axon terminals found near Purkinje cells.

Risk factors for sudden deafness: a case-control study.

In order to investigate risk factors for idiopathic sudden sensorineural hearing loss (sudden deafness), a case-control study was done in 109 patients with sudden deafness who visited our hospital between 1992 and 1994, with 109 controls matched to each patient by gender and age. Odds ratio (OR) and 95% confidence interval (CI) for smoking habits, drinking habits, dietary habits, environmental noise, past history of disease, sleeping hours, appetite, fatigue, incidence of common cold were obtained. Fatigue (OR: 3.28; 95% CI: 1.36-7.90) and loss of appetite (OR: 8:00; 95% CI: 1.00-64.0) elevated the risk for sudden deafness. Those who ate many fresh vegetables were at a decreased risk (OR: 0.48; 95% CI: 0.24-0.96 for light-colored vegetables, OR: 0.55; 95% CI: 0.30-1.02 for green-yellow vegetables). Personal histories of hypertension and thyroid disease, and susceptibility to colds appeared to be positively associated with the risk (0.05 < P < 0.10). Smoking habits, drinking habits and environmental noise had no significant association with sudden deafness. These results suggested that environmental factors, including diet, may be importantly involved in the genesis of sudden deafness.

A case of pleomorphic adenoma of the epiglottis. Bilateral vocal-cord paralysis after YAG laser surgery.

Pleomorphic adenoma of the larynx is a rare disorder, and until recently has been treated mainly by approaches involving pharyngotomy. We encountered a case of pleomorphic adenoma originating from the laryngeal surface of the epiglottis and removed it using a YAG laser through a suspension laryngoscope. This case was complicated by delayed-onset bilateral vocal-cord paralysis, the causes of which are also discussed.

A rare case of complete proximal epiphyses (so-called pseudoepiphyses) of the metacarpal and metatarsal bones in the human.

We report a rare case of complete proximal epiphyses (so-called pseudoepiphyses) of metacarpal and metatarsal bones in a human. These phenomena were seen in both the hands and feet of a seven-year-old girl undergoing medical examination for pain in the right foot. During medical treatment, X-ray photographs of the hands and feet were taken. Each metacarpal and metatarsal bone except for the first metacarpal bones had supernumerary epiphyses (complete pseudoepiphyses) at the nonepiphyseal end of these bones. This is a rare case even for an experienced orthopaedic surgeon.

p53 and bcl-2 expression and argyrophilic nucleolar organizer regions in patients with malignant maxillary sinus tumours.

To investigate the relation between the clinicopathological findings, and the survival of patients with malignant tumours of the maxillary sinus, and the tumour's biological parameters, we examined the expression of p53 and bcl-2 and the number of silver-stainable nucleolar organizer regions (AgNORs) in 23 paraffin-embedded specimens from primary tumours. Fifteen of 23 (65 per cent) maxillary tumours expressed p53 and two of 23 (nine per cent) tumours expressed the bcl-2 gene product. The mean number of AgNORs per nucleus was 5.2 +/- 2.4. There was no relationship between the expression of p53 or bcl-2 and the patient's clinical course. In contrast, there was a statistically significant difference in the survival of patients with a high (> 6) number of AgNORs and patients with a low (< 6) number of AgNORs. These data suggest that the number of AgNORs may be useful in evaluating the prognosis of patients with malignant maxillary sinus tumours.

Inner ear disorders due to pressure change.

We reviewed the records of 136 patients who had inner ear disorders including hearing loss and vertigo caused by pressure change. We divided them into three groups, according to the aetiology: group A, change in atmospheric pressure (diving, airplane travel, etc.); group B, rapid change in ear pressure in normal atmosphere (nose blowing, heavy lifting, etc.); and group C, blast injury. A flat initial audiogram was the most common type in groups A and B. In group C, high-tone hearing loss was the most common type of audiogram. These results correspond to findings previously reported in animal experiments. Exploratory tympanotomy was performed more than 12 days after the pressure change in 16 patients. Although the vertigo disappeared after surgery, hearing did not improve.

The highly conserved DAD1 protein involved in apoptosis is required for N-linked glycosylation.

BACKGROUND: The tsBN7 cell line is one of the temperature-sensitive mutants for cell proliferation which have been isolated from the BHK21 cell line derived from the golden hamster. It has a mutation in the DAD1 gene encoding a 12.5kDa highly conserved protein through evolution, and enters apoptosis at the restrictive temperature due to this mutation. RESULTS: DAD1 was recovered in light membrane fractions after differential centrifugation. It could not be released from the membrane, even by carbonate extraction, without a detergent. Upon digestion with proteinase K, both N and C terminal portions-but not the middle portions of DAD1- were released from the membrane. Thus, DAD1 appears to be an integral membrane protein in which both termini are located in the cytosol. DAD1 was localized in the endoplasmic reticulum. In accordance with a similarity to the yeast protein Ost2p, which is a subunit of the oligosaccharyltransferase, at the restrictive temperature, loss of DAD1 function caused a defect of N-linked glycosylation in tsBN7 cells resulting in apoptosis. However, tunicamycin, which is known to inhibit N-linked glycosylation did not induce apoptosis in either tsBN7 or BHK21 cells. CONCLUSION: tsBN7 cells have a defect in N-linked glycosylation caused by the loss of DAD1.

Purification of a factor from CSF in patient after SAH which induces the cytosolic free calcium elevation in vascular smooth muscle cells.

Cerebrospinal fluid from patients with aneurysmal subarachnoid hemorrhage induces the elevation of cytosolic free calcium [Ca2+]i in cultured vascular smooth muscle cells. We have purified a [Ca2+]i elevating factor from cerebrospinal fluid of a patient with subarachnoid hemorrhage due to aneurysm rupture. The calcium-elevating protein factor was purified to homogeneity by ammonium sulfate precipitation and a combination of Mono Q, Superose 12, and Mono S columns using liquid chromatography. Fifteen microgram of the purified protein was obtained from 340 mg of cerebrospinal fluid proteins and the molecular mass of the protein was estimated to be 81 kDa by sodium dodecyl sulfate polyacrylamide gel electrophoresis. It was also shown that the purified protein was cross-reactive with anti-human transferrin antibody. These results suggested that transferrin may be involved with the cerebral vasospasm after subarachnoid hemorrhage.

Urine viscosity after injections of iotrolan or iomeprol.

PURPOSE: Previous observations in rats caused us to speculate whether the injection of iotrolan, a nonionic dimeric contrast medium (CM), would increase urine viscosity enough to obstruct urine outflow in the collecting duct. MATERIAL AND METHODS: The urine viscosity in dogs was measured directly with a viscosimeter after injections of iotrolan or of iomeprol, a nonionic monomeric CM. RESULTS AND CONCLUSION: The injection of iotrolan increased urine viscosity considerably whereas iomeprol had little effect on this variable. The osmolality-dependent adverse reactions of CM have previously been emphasized but viscosity-dependent adverse reactions must also be considered when the CM is a polymer with a low osmolality.

Possible involvement of protein kinase C in the modulation of inotropic and chronotropic effects induced by ouabain in rat right atrial muscles.

Modulations of the inotropic and chronotropic effects of ouabain and protein kinase C (PKC) stimulation with phorbol esters in rat right atria were examined. Cumulative administration of ouabain (3-30 microM) caused a positive inotropic effect in a concentration-dependent manner, but did not produce a chronotropic effect. A single administration of ouabain (30 microM) also had similar effects: + 74.4 +/- 8.4% (n = 23, P < 0.01) in the contractile force and -0.7 +/- 1.3% (n = 23, P > 0.05) in the sinus rate. Addition of phorbol esters reinforced the ouabain-evoked positive inotropic effect: 26.5 +/- 8.9% (n = 6, P < 0.05) with 100 microM 4-beta-phorbol-12,13-dibutyrate (PDB), and 6.4 +/- 3.3% (n = 6, P > 0.05) with 100 microM 12-O-tetradecanoyl-phorbol-13-acetate (TPA). Simultaneously, the mixture of ouabain and phorbol ester raised the resting tension. Phorbol esters alone caused a positive inotropic effect (by about 21-27%). Non-PKC activating phorbol ester, 4-alpha-phorbol-12,13-didecanoate (PDD, 100 microM), did not have any effect. Pretreatment with the PKC inhibitor (staurosporine 100 microM) significantly decreased the ouabain-induced positive inotropic effect and caused a negative chronotropic effect, but H-7 (1-(5-isoquinolinylsulphonyl)-2-methylpiperazine dihydrochloride) (5 microM) had no effect. These results suggest that PKC stimulation may be involved in the ouabain-evoked responses in the right atria of rat as seen by increased cellular Ca2+ concentration (and Ca(2+)-sensitivity); thus the positive inotropic effect may not be due only to modulation of Na+/K+ pump activity.

Fas and Fas ligand interaction is necessary for human osteoblast apoptosis.

We investigated the cellular and humoral interactions between peripheral blood mononuclear cells (PBMCs) and human osteoblasts, leading to apoptosis of osteoblasts. Human osteoblastic cell line MG63 and human primary osteoblast-like cells obtained from biopsy specimens were used in this study. PBMCs were isolated from healthy donors and cultured with or without stimulation by recombinant interleukin-2 followed by 12-o-tetradecanoylphorbol 13-acetate with ionomycin. Fas was functionally expressed on MG63 and primary osteoblast-like cells. Activated PBMCs expressed Fas ligand (FasL) strongly on their surface and killed MG63 and primary osteoblast-like cells. Cultured supernatants of activated PBMCs also induced apoptotic cell death of MG63 and primary osteoblast-like cells. In contrast, both unstimulated PBMCs and cultured supernatants of unstimulated PBMCs did not induce apoptosis of these cells. Furthermore, the cytotoxic effect and induction of apoptosis against MG63 and primary osteoblast-like cells by activated PBMCs and cultured supernatants were inhibited significantly by human Fas chimeric protein. Our data showed that human osteoblasts expressed Fas fuctionally and both membrane-type and soluble form FasL from activated PBMCs induced apoptosis of these cells, providing the one possible mechanism of bone loss in inflammatory diseases such as rheumatoid arthritis.

Severity of neurological disorders in elderly residents of a farming town in Kyushu, Japan; prevalence of disability in activities of daily living.

Activities of daily living (ADL) were evaluated in 421 elderly people, aged 60-95, who resided in 27 randomly selected communities in the town of Mifune, Japan. The disability of ADL increased with age, and was particularly rapid after age 80. Most of the elderly were independent; the prevalence of dependent elderly ranged from 0.7% (in feeding) to 9.5% (in working). The prevalence of dependent elderly increased markedly after age 80 in all ADL items, especially those related to behavior such as ambulation, stair climbing, transfers, and working (i.e., employment, housekeeping, etc.). These results emphasize the importance of community health care of the elderly for preventing and treating neurological disorders which comprised the main cause of disabilities of behavioral ADL.

Hepatic damage influences the decay of nitroxide radicals in mice--an in vivo ESR study.

To determine the role of the liver in the elimination of free radicals from the body, the clearance rate (K) of nitroxide radicals (Tempol) at the hepatic domain was compared with that at the pelvic domain of live mice, using L-band ESR spectroscopy. The reduction of Tempol in biopsy specimens (liver tissue and femoral muscle) and blood obtained from Tempol-treated mice was also monitored using X-band ESR spectroscopy. Results indicated that the reduction of nitroxide radicals was delayed in both the liver and peripheral tissues when the liver was damaged. The decrease in both blood supply and reductants in the damaged liver might be involved in delaying the reduction in the whole body, because the liver can reduce the radicals supplied via the blood from the peripheral tissues, and the reductants such as reduced, glutathione in the peripheral tissues are supplied from the liver.

[Clinicopathological study of 54 cases of adenoid cystic carcinoma in the head and neck].

A retrospective analysis was performed on 54 patients with adenoid cystic carcinoma in the head and neck, treated from January 1971 to July 1997 in the Kurume University Hospital. The patients consisted of 35 females and 19 males, with a mean age of 59 years. The site of the lesion was the oral cavity in 13, nasal cavity in 11, submandibular gland in 8, parotid gland in 5, paranasal sinus in 3, nasopharynx in 3, oropharynx in 3, external auditory canal in 3, orbita in 2, lip in 1, lacrimal gland in 1 and trachea in 1. The over all 5-year, 10-year and 15-year survival rates as determined by the Kaplan-Meier method were 72%, 53%, and 46%, respectively. There was no relationship between survival rate and the site of the original lesion. According to the histologic criteria reported by Szanto et al., when more than 30% of the neoplasma showed a solid pattern, its treatment outcome appeared poor. In this study, the same tendency was seen. Fifteen patients died of the tumor. Ten died of distant metastasis and 5 died of local recurrence invading the cranium. No patient died of neck metastasis. Adenoid cystic carcinoma was thought to be radioresistant, but recently some authors have reported this tumor as radiosensitive. This study also showed that postoperative radiotherapy was effective, producing an increase in the survival rate.

[The effect of CO2 and apnea on cochlear and middle ear blood flow in guinea pigs].

The effect of 10% CO2 (in air) and apnea on cochlear blood flow (CBF) and middle ear blood flow (MEBF), capillary vessel diameters and blood pressure (BP) were investigated in guinea pigs. Intravital microscopic techniques (IVM) using video system, and laser Doppler flowmetry (LD) were used. MEBF was measured in the blood vessels of the middle ear mucosa over the cochlea and CBF was measured in the lateral wall vessels in the second or third cochlear turn. During 10% CO2 respiration for 10 min, the highest vessel diameter dilated about 11% in the middle ear and 5% in the cochlea. During 5 minutes apnea, the highest vessel diameter constricted about 30% in the middle ear and 5% in the cochlea. Elevation of PCO2 dilate blood vessels with constant PO2, but constriction of blood vessels as observed in extremely low PO2 range even if PCO2 was elevated. The ratio of the change in blood flow volume to the change in BP was obtained after the change in blood flow volume was calculated from blood flow velocity and blood vessel diameter. The mean change of ratio in MEBF was a 10% decrease with 10% CO2, 28% decrease with apnea in IVM. The mean change of ratio in CBF was a 44% increase with 10% CO2, 14% increase with apnea in IVM, 67% increase with 10% CO2, 42% increase with apnea, in LD. The change of CBF in LD was about 20% larger than in IVM. Our results showed that CBF belonging to vertebral artery system was more strongly maintained in association with autoregulation even under conditions of low PO2 or high PCO2 compared to MEBF in the external carotid artery system. It was suggested that blood flow behavior was significantly different between the middle ear and inner ear.

[The storage technology of blood, the latest advances--cryopreservation of red blood cells].

In the field of red blood cell cryopreservation, the latest research informations were mainly divided into two parts as follows: 1) The improvement of the frozen storage bag. 2) The long-term storage of the frozen and thawed red blood cells. That was based on the research for changing the film of frozen storage bags from polyvinyl chloride to polyolefin. In consequent, the broken ratio of the bags in the period of frozen storage was expected to reduced, immediately. And this was studied for establishment of the closed bag system and introduction of the mannitol-adenine-phosphate(MAP) solution. From these results, it was made clear that the frozen and thawed red blood cells could be stored in MAP solution at 4 degrees C for 3 weeks.

Cyclin D1 overexpression in primary hypopharyngeal carcinomas.

BACKGROUND: Hypopharyngeal squamous cell carcinomas (HPCS) are associated with an extremely poor prognosis. Generally, conventional clinicopathologic factors have only limited value as prognostic factors for this malignancy. It is therefore clinically important to identify new prognostic factors that accurately reflect the biologic aggressiveness of this malignancy. The amplification and overexpression of the cyclin D1 protooncogene have been reported in a variety of malignancies, and are thought to be related to tumor progression. Based on this phenomenon, the authors immunohistochemically evaluated overexpression of the cyclin D1 gene in 42 cases of primary HPCS. In addition, the immunohistochemical staining of the proliferation marker MIB-1 (Ki-67 antibody) was also performed. METHODS: Formalin fixed, paraffin embedded biopsy specimens obtained prior to treatment were examined. Cyclin D1 and Ki-67 were detected using monoclonal antibodies by means of the streptavidin-biotin method. The relationship between cyclin D1 overexpression and the stage, histologic grade, presence of lymph node metastases, proliferation index, and survival was then statistically analyzed. The correlation between the proliferation index, other clinicopathologic factors, and survival was also evaluated. RESULTS: Twenty-three (54.8%) HPCS specimens showed a 20% or greater immunoreactivity for cyclin D1. Cyclin D1 overexpression was related to cervical lymph node metastases (P = 0.037) but not to clinical stage, histologic grade, or the proliferation index. Cyclin D1 negative tumors were associated with a significantly better prognosis (P = 0.023), particularly in patients who underwent multimodality treatment. Finally, the MIB-1 labeling index showed no correlation with either the clinicopathologic parameters or overall survival. CONCLUSIONS: Based on these findings, cyclin D1 immunohistochemical staining is considered to be useful, not only as a prognostic factor for HPCS, but also as a means of determining the optimum treatment for each individual patient. Conversely, the MIB-1 labeling index appears to have no clinical significance in HPCS.
