Glucocorticoid hormone suppression of human neutrophil-mediated tumor cell cytostasis.

In the present study, we have investigated the effect of glucocorticoid hormones on neutrophil-mediated tumor cell cytostasis and found that hydrocortisone and a synthetic hormone, dexamethasone (Dex), inhibited cytostasis in the presence or absence of tumor necrosis factor-alpha. The effect of Dex was completely reversed by a glucocorticoid receptor antagonist, RU38486. To clarify the underlying mechanisms, we examined effects of Dex on the binding avidity of beta2 integrin on the neutrophil surface and how these might in turn affect neutrophil-to-tumor cell binding. Dex was found to inhibit these neutrophil properties, and RU38486 completely suppressed both forms of Dex inhibition. Taken together, our findings suggest that glucocorticoid hormone inhibition of neutrophil-mediated tumor cell cytostasis is at least partially due to a lowering of the ligand binding avidity of beta2 integrin on the neutrophil surface.

Microdeletion of the DAZ (deleted in azoospermia) gene or the YRRM (Y chromosome ribonucleic acid recognition motif) gene does not occur in patients with Klinefelter's syndrome with and without spermatogenesis.

OBJECTIVE: To evaluate the occurrence of microdeletions of the Y chromosome involving the DAZ and YRRM genes in patients with Klinefelter's syndrome with and without spermatogenesis. DESIGN: Controlled clinical study. SETTING: Yamagata University Hospital, Yamagata, Japan. PATIENT(S): Patients with Klinefelter's syndrome with spermatogenesis (n = 1) and without spermatogenesis (n = 20) and a control group of men of proven fertility (n = 10). INTERVENTION(S): Blood and semen sampling and testicular biopsy. MAIN OUTCOME MEASURE(S): Semen analysis, polymerase chain reaction amplification of 32 DNA loci on the long arm of the Y chromosome involving the DAZ (deleted in azoospermia) and YRRM (Y chromosome ribonucleic acid recognition motif) genes, and measurement of plasma FSH, LH, and testosterone levels. RESULT(S): No microdeletions of 32 loci were found in any of the patients with Klinefelter's syndrome, either with or without spermatogenesis. Plasma LH and FSH levels were abnormally high and testosterone levels were reduced in all the patients with Klinefelter's syndrome. American Society for Reproductive Medicine.) CONCLUSION(S): Severe impairment of spermatogenesis in patients with Klinefelter's syndrome is not caused by microdeletions of the Y chromosome involving the DAZ and YRRM genes.

Effects of estrogen and progesterone on urinary bladder in female rabbit: evaluation by quantitative morphometric analysis.

OBJECTIVES: To investigate possible effects of estrogen and/or progesterone on the histologic characteristics of female rabbit urinary bladders, we carried out quantitative morphometric analysis of the rabbit bladders. METHODS: Mature female rabbits were treated by ovariectomy with and without successive estrogen and/or progesterone administration. Area densities of the connective tissue (CT) and smooth muscle (SM) cells, the area of single SM cells, and the thickness of the bladder wall were determined by computer-assisted quantitative morphometric analysis. RESULTS: Six weeks after each treatment, ovariectomy alone resulted in a decrease in CT density of the bladder. Successive estrogen treatment increased the bladder wet weight and SM cell density within the bladder wall. Progesterone treatment reduced CT degradation in ovariectomized rabbits. Sex steroids did not significantly influence the area of each SM cell. There was no significant difference in histologic characteristics between the rabbits treated by estrogen alone and those treated by combination (estrogen and progesterone) therapy. CONCLUSIONS: Ovariectomy and successive hormonal replacement therapy resulted in morphologic changes within the rabbit urinary bladder. Cotreatment with progesterone did not significantly change the morphologic findings produced by estrogen treatment alone.

Relation between urethral elasticity and bladder outlet obstruction and histologic composition of the prostate in patients with benign prostatic hyperplasia.

OBJECTIVES: In benign prostatic hyperplasia (BPH), bladder outlet obstruction is caused by mechanical blockage resulting from an enlarged prostate, and by the increased tone of prostatic smooth muscle. In patients with obstructive BPH, the prostatic urethra urodynamically corresponds to the flow-controlling zone (FCZ). We sought to investigate the relation between the obstruction and elastic properties of the FCZ and the histologic composition of the prostate in symptomatic patients with BPH. METHODS: The grade of infravesical obstruction was classified according to preoperative pressure-flow data from 30 men with symptomatic BPH, and the elastic properties of the FCZ were evaluated as the mean elastance, which was calculated using the three-parameter model. The area densities of smooth muscle, fibrous tissue, epithelium, and lumen were determined by quantitative morphometry, using BPH tissue obtained by transurethral resection. RESULTS: The area density of smooth muscle had a negative correlation with mean elastance (p = -0.50, P <0.01), the linear passive urethral resistance relation (p = -0.43, P <0.05), and the group-specific urethral resistance factor (p = -0.39, P <0.05). No other histologic element was correlated with these variables. CONCLUSIONS: Our results suggest that urethral elasticity and bladder outlet obstruction grade are influenced by the relative content of smooth muscle within the prostate in patients with BPH.

Analysis of protooncogene c-erbB-2 in benign and malignant human prostate.

In this report, we characterized the c-erbB-2 gene and its product in prostatic cancer cells. Three prostatic cancer cell lines (PC3, DU145 and TSU-Pr1), one primary prostatic cancer and four benign prostatic hyperplasias (BPH) were studied. In reverse transcribed polymerase chain reaction, c-erbB-2 mRNA was demonstrated in all three cell lines and prostatic cancer tissues as well as BPH. The c-erbB-2 protein was expressed higher in prostatic cancer cells and tissues as compared with benign tissue by enzyme immunoassay, but it was not statistically significant. Immunohistochemical study, with the monoclonal antibody SV2-61gamma that recognizes the extracellular domain of c-erbB-2, showed that all the prostatic tissues and cells had reactivity. Antigenicity was mainly in the cytoplasm. Analysis of genomic DNA failed to disclose gene amplifications or rearrangements of c-erbB-2 in both prostatic cancer and BPH. The sequence of amplified c-erbB-2, which corresponds to transmembrane domain, disclosed wild type in all prostatic cancer cells. These results demonstrate that although the number is limited, c-erbB-2 gene and protein are expressed in prostatic cancers and benign prostates. In the previous studies on c-erbB-2 expression in prostatic tissue, mainly conducted by immunohistochemistry, its frequency varies among each study, ranging from less than 0% to 100%. Therefore, to evaluate the c-erbB-2 in prostatic tissue precisely, it is also necessary to detect mRNA of c-erbB-2 as demonstrated in our study.

Adrenal adenoma with foci of oncocytes in a patient with pre-Cushing's syndrome.

A case of unilateral adrenal adenoma with foci of oncocytes in a 64-year-old woman is reported. The tumour manifested clinically pre-Cushing's syndrome with autonomous steroidogenesis which was insufficient to cause typical features of Cushing's syndrome. Whether the pathogenesis of oncocytes and adenoma cells, which were thought to have some metabolic deficiency in the present tumour, are linked remains to be determined.

In vitro effect of amezinium on the rabbit urinary bladder: muscle strip and whole bladder evaluation.

Dose-response studies for norepinephrine and amezinium were obtained for isolated muscle strips and in vitro whole bladder preparations to investigate the effect of amezinium on the bladder. Specimens (strips from the bladder base and body, and the whole bladder) were obtained from mature female rabbits. By the muscle strip study, amezinium showed a weak contractile effect (up to 40% of spontaneous contraction) on strips from the bladder base. Muscle strips from the bladder body presented no response to amezinium. In the whole bladder preparation, administration of norepinephrine showed an increase in intravesical pressure in 4 out of 7 rabbits, whereas administration of amezinium had negligible effect on intravesical pressure. Administration of amezinium showed no influence on strips from the bladder body and the whole bladder preparation. Direct contractile response in muscle strips from the bladder base by amezinium appeared to be a favourable effect considering urinary storage.

Comparison of voiding disturbances in patients with LUTS and BPH at home and by episodic checks at an outpatient clinic.

To clarify the presence of voiding disturbances in patients with lower urinary tract symptoms (LUTS) and benign prostatic hyperplasia (BPH), measurements of the capacity at normal desire to void (CNDV), residual urine volume (RUV) and RUV rate at home and in a hospital were done. It has been found that (1) RUV and RUV rate in patients with BPH were markedly greater than that in the normal group and LUTS patients staying at home. Moreover, there was no difference in RUV and RUV rate between the normal group and the LUTS patients. (2) RUV and RUV rate in patients with LUTS and BPH decreased more markedly without changing CNDV when the patients stayed at home than when they underwent episodic checks in a hospital. In conclusion, frequent checks of LUTS and BPH in patients in their domestic environment can probably be utilized for management or determination of treatment.

The prognosis of stage A patients treated with the antiandrogen chlormadinone acetate.

To determine the efficacy of the antiandrogen chlormadinone acetate as a treatment option for stage A prostate cancer, a retrospective analysis of 111 patients who received chlormadinone acetate was done. Of 55 stage A1 patients, progression was seen in one patient (1.8%) 8 years after the diagnosis. Six out of 56 stage A2 patients (10.7%) showed disease progression at 1-7 years (mean 68 months) after the diagnosis. Progression rates associated with antiandrogen therapy for stage A1 and stage A2 patients were lower than those without treatment reported in the literature. It could be assumed that antiandrogen treatment with chlormadinone acetate inhibited the progression.

Long arm deletion of chromosome 10 in a boy with monorchidism.

We report on a boy with long-arm deletion of chromosome 10 and compare this case to 10 previously reported patients. He had right cryptorchidism and absence of the left testis, but the size of his penis was normal. Cytogenetic analysis of the case showed the deficiency of 10q26.1-26.3 and the presence of 10qter. Four of 10 previously reported patients had an intersex phenotype, and all others had combinations of cryptorchidism, micropenis and hypospadias. These facts indicate that the terminal of chromosome 10q is strongly associated with abnormal male development.

Effect of immobilization stress on testicular germ cell apoptosis in rats.

The influence of immobilization stress on testicular germ cell apoptosis was investigated in rats. A transient increase in serum corticosterone and a transient decrease in serum testosterone were observed during each period of immobilization stress. Twenty-four hours after the last immobilization session, the testicular weight and serum concentrations of corticosterone and testosterone were the same between the immobilization stress and control groups. However, the percentages of apoptotic tubules and apoptotic cells in the stress group were significantly higher than those in controls (P < 0.001). These facts suggest that immobilization stress can enhance testicular germ cell apoptosis in rats.

Multiple fixed drug eruption caused by iomeprol (Iomeron), a nonionic contrast medium.

Most cases of drug eruption caused by nonionic contrast media (NICM) reported to date have been of the erythema multiforme type. Herein we report the first case of multiple fixed drug eruption (FDE) caused by iomeprol (Iomeron(R)). A 67-year-old woman developed multiple pea-sized erythematous papules on the trunk and extremities 4 days after receiving 100 ml of iomeprol for a computed tomography examination. Some of the papules coalesced, forming 7 large plaques on the limbs. Six months later, the patient was mistakenly administered iomeprol again. On the following morning, erythematous plaques admixed with vesicles recurred at the same sites as during the previous episode. In both episodes, the lesions cleared leaving pigmentation that faded with 6 weeks. Both patch testing and an intradermal test with iomeprol on lesional pigmented skin were positive. The present case indicates that NICM may cause multiple FDE and that repeated administration of the causative agent may increase the severity of the eruption.

Relationship between urodynamic type of obstruction and histological component of the prostate in patients with benign prostatic hyperplasia.

OBJECTIVES: This study was designed to compare urodynamic types of bladder outlet obstruction (BOO) and histological architecture of the prostate in patients with benign prostatic hyperplasia (BPH). PATIENTS AND METHODS: Twenty-four patients with symptomatic BPH, who underwent transurethral resection of the prostate (TURP), were included in the study. Before surgery, a pressure-flow (P-F) study was performed in all patients. According to the classification by Spanberg et al., they were divided into 5 normal type, 6 low-compliant, 6 compressive and 7 constrictive obstruction. The area density and total area of smooth muscle, fibrous tissue, glandular epithelium and glandular lumen in BPH specimens were estimated using morphometric analysis. RESULTS: In all groups, the stromal component occupied about three fourths of BPH specimens, and fibrous tissue was the most prominent element. However, neither area density nor total volume of each histological component showed a significant difference among the four groups. CONCLUSION: The type of BOO in patients with BPH cannot be explained by histological architecture alone.

Clinical application of three-dimensional anisotropy contrast magnetic resonance axonography. Technical note.

The utility of three-dimensional anisotropy contrast (3DAC) magnetic resonance (MR) axonography, a method sensitive to neuronal fibers and their directionality, was investigated in the clinical setting using a 3-tesla MR imaging system based on a General Electric Signa platform. The study focused on healthy volunteers and patients with common structural central nervous system disorders, namely chronic infarction, brainstem cavernous hemangioma, supratentorial meningioma, and astrocytoma. Three orthogonal anisotropic diffusion-weighted images were first obtained. Three primary colors were each assigned to a diffusion-weighted image, respectively, and the images were subsequently combined into a single-color image in full-color spectrum (3DAC MR axonography image). Fiber-tract definition in the cerebral peduncle of the midbrain of healthy volunteers showed intersubject variation, with two general patterns recognized: dispersed (60% of cases) and compact (40% of cases). Pathological alterations in the fiber tracts were readily identified in cases involving wallerian degeneration of the pyramidal tract, as illustrated in the cases of chronic infarction. Displacement of major tracts, such as the medial lemniscus or corticospinal tract, as well as fiber directionality, was also easily recognized in cases of mass lesions. As an imaging method uniquely capable of providing information regarding axonal connectivity, 3DAC MR axonography appears to have promising potential for routine clinical application.

High-field, high-resolution MR imaging of the human indusium griseum.

The human indusium griseum (IG), the paired dorsal continuation of the hippocampus, was investigated with high-field (3.0T) MR imaging. The IG was clearly visible in 16 out of 20 healthy volunteers. The most common pattern was a single lateralized strip. The classical neuroanatomic pattern of paired symmetric strips along the midline was found in one case. The study clearly demonstrates that diminutive, hitherto overlooked structures such as the IG now can be readily investigated in vivo by non-invasive high-field MR imaging.

[MR analysis of brain function].

Magnetic resonance (MR) is a remarkably versatile technology applicable to various aspects of medical science. Currently, there are three categories of MR techniques available for probing human brain function in detail. The first category comprises the most widely utilized techniques which make use of the metabolic effects of brain activation, represented by BOLD functional magnetic resonance imaging (fMRI). The second category of techniques deals with apparent diffusion tensor in probing the cellular aspects of brain function and is represented by three dimensional anisotropy contrast (3DAC) axonograpy. The third category of techniques is the oldest and as yet the most underutilized. These techniques which include MR spectroscopy (MRS) and spectroscopic imaging (SI), are based on classical nuclear magnetic resonance (NMR) spectroscopy and are capable of providing biochemical information in vivo. Because of its inherently non-invasive nature, MR technologies are especially important for the field of pediatric neurology and developmental neuroscience.
