[A case of systemic lupus erythematosus (SLE) developing pan-dysautonomia].

A 43-year-old woman who had been diagnosed as primary Sjögren's syndrome since 1986 developed severe constipation, urinary retention, dizziness at standing and polyarthralgia in February, 1990. Laboratory tests revealed proteinuria, hypocomplementemia and high titer of anti-DNA antibody. Diagnosis of SLE was made and she was admitted to our hospital on April 2, 1990. Physical examination on admission showed that she also had asymmetric pupils, impairment of sweating, orthostatic hypotension, neurogenic bladder, gastro-intestinal dysmotility and the diminution of R-R interval variability during deep breathing on the electrocardiogram. These findings suggested that she had pan-dysautonomia but there were no signs of motor and sensory disturbance. Because other diseases such as diabetes mellitus and amyloidosis which induced dysautonomia could be ruled out, her pan-dysautonomia seemed to be due to SLE. After the treatment with steroid pulse therapy, most of her dysautonomia improved rapidly. However, some of the disturbance had persisted for a long time. Pan-dysautonomia has been rarely reported as a complication of SLE, and high dose of steroid therapy at the early stage should be considered.

[Systemic lupus erythematosus associated with benign intracranial hypertension: a case report].

A case of systemic lupus erythematosus (SLE) with benign intracranial hypertension (BIH) is reported. A 41-year-old male with a history of SLE starting in 1982 was admitted to our hospital in December 1989 because of headache and vertigo. Laboratory examinations on admission showed proteinuria, mild anemia, and positive antinuclear and anti-Sm antibodies. No abnormal findings except high pressure of 350 mmH2O were observed in his cerebrospinal fluid (CSF). Fundoscopic examinations showed marked bilateral papilledema and retinal bleeding. Brain CT, MRI and angiography revealed diffuse brain edema without space occupying lesion and cerebrovascular diseases. Because there were no diseases such as endocrinological disorders, severe anemia, and no history of the administration of drugs which might cause intracranial hypertension, the diagnosis of BIH was made. Subsequently, he was treated with intravenous methylprednisolone therapy and osmotic diuretics and his clinical symptoms and pressure of CSF gradually improved. The decrease of CSF adsorption was observed with RI cisternography in our case. Psychosis, seizures and meningitis are common CNS manifestations in SLE patients. But BIH is very rare and its cause is unclear. Only 17 cases of SLE with BIH have been reported. The pathogenesis and treatment of BIH in SLE patients were discussed in this paper.

[Cisplatin etoposide versus cisplatin/etoposide/ifosfamide combination chemotherapy in small-cell lung cancer: a multicenter randomized controlled study. Hokkaido Study Group of Treatment for Small-Cell Lung Cancer].

Ninety-two patients with small-cell lung cancer (SCLC) were randomized to receive cisplatin (80 mg/m2, day 1)/etoposide (100 mg/m2, day 1, 3, 5) (PE) or cisplatin (80 mg/m2, day 1)/etoposide (100 mg/m2 1, 3, 5)/ifosfamide (2 g/m2, day 1, 2, 3) (PEI) combination chemotherapy. After 2 courses of chemotherapy, patients with limited disease (LD) received chest irradiation with 40-50 Gr. Of the 89 patients evaluable, the overall response rate was 77.8% in PE therapy, and 73.7% in PEI therapy (NS). The complete response rate (CR) of PE v PEI was 13.9% v 21.2% for all patients (NS), 22.2% v 30.4% for LD. Median survival times of PE v PEI for all patients were 55 weeks v 56 weeks (NS). The 2-year survival rate of PE v PEI was 15.4% v 16.5% for all patients (NS). There was no difference of the duration of response in cases with CR or PR between PE and PEI therapy. Leucopenia occurred more often after PEI than after PE therapy (p less than 0.01). These results suggest that PEI is not superior to PE chemotherapy in SCLC.

[A case of rheumatoid arthritis developing pemphigus-like skin lesion during treatment with bucillamine].

Bucillamine is a useful medication for treatment of rheumatoid arthritis (RA) but some patients develop side effects from it. Here, we report a patient with RA developing a pemphigus-like skin lesion during treatment with bucillamine. A 55 year old woman with RA (stage III, class II) had been treated with bucillamine in our hospital since September 1988. Her symptoms of RA had gradually improved after administration of bucillamine but the generalized skin rash with itching developed in June 1989. Skin biopsy revealed spongiosis, the infiltration of lymphocytes in the epidermis and inter-cellular deposition of IgG. These findings were consistent with the histological change of pemphigus. Since symptoms of the skin disappeared two months later after the discontinuation of bucillamine. We considered that her pemphigus-like lesion was induced by this drug. D-penicillamine is one of the drugs which induce pemphigus. Though the mechanisms of this side effect have not been clear, it is thought that autoantibody induced by D-penicillamine could be one of the cause of pemphigus. Because the chemical structure of bucillamine is similar to that of D-penicillamine, the autoimmune mechanisms may also play a role in the onset of the pemphigus-like lesion in this case.

[A phase II study of CPT-11, a camptothecin derivative, in patients with primary lung cancer. CPT-11 Cooperative Study Group].

A Phase II study of CPT-11, a new camptothecin, was performed in patients with primary lung cancer. Patients with previously untreated non-small cell carcinomas (group A), or previously treated non-small cell carcinomas (group B), and with small cell carcinomas (group C), were enrolled in this study. CPT-11 was given at a dose of 100 mg/m2 i.v. infusion once a week for three weeks or more. Out of 153 patients enrolled, 128 (A: 67; B: 26; C: 35) were assessed to be evaluable for response by an extramural review committee. Response rates were 34.3% (23/67) for A, 0% (0/26) for B and 37.1% (13/35) for C. The response rate was 50% for previously untreated patients (4/8), and 33.3% for previously treated patients (9/27) including 2 complete responses in the group C. Major toxicities were leukopenia, nausea/vomiting, diarrhea, anorexia and alopecia. Leukopenia and diarrhea were considered to be dose limiting toxicities, but they were reversible. It was, however, suggested that some patients should be monitored carefully for severe reactions and delay in recovery. The results showed that CPT-11 was highly effective against non-small cell and small cell carcinomas of the lung.

[Thermography of collagen diseases with Raynaud's phenomenon].

In forty-four patients with collagen diseases accompanied with Raynaud's phenomenon(systemic lupus erythematosus(SLE)10, progressive systemic sclerosis(PSS)15, primary Sjögren's syndrome(PSJS)18 and mixed connective tissue disease(MCTD)1), we analyzed the thermography of fingers after thermal stimulation with cold water(15 degrees C, 30 seconds). In normal controls, the temperature of the distal points of fingers was higher and recovered more quickly than that of the proximal points(proximal interphalangeal joint; PIP). But in the patients with Raynaud's phenomenon, the temperature of distal point of fingers was lower and recovered more slowly than that of proximal points. Because the difference of the temperature between the distal and the proximal points (DP)is a good parameter to distinguish these two thermographic patterns, we analysed DP. DP is less influenced by room temperature than the temperature of fingers and the recovery temperature after thermal stimulation which have been used as indicators of Raynaud's phenomenon. DP in the normal controls was significantly higher than that in the patients. DP in PSS was lower than that in SLE and PSJS, and statistically associated with the markers of disease activities, such as erythrocyte sedimentation rate, titers of anti-RNP antibody and diffusing capacity of lung. These results show that thermographic pattern and the value of DP are the useful indicator of severity of Raynaud's phenomenon.

[Pneumoperitoneum without perforation of the gastrointestinal tract in a patient with systemic lupus erythematosus].

Pneumoperitoneum often occurs after the perforation of the gastrointestinal tract. However, pneumoperitoneum without the perforation has been reported as one of the complications of collagen diseases, the cause of which is usually the rupture of pneumatosis cystoides intestinalis (PCI). PCI is sometimes observed in the patients with scleroderma and mixed connective tissue disease but rarely in the patients with systemic lupus erythematosus. We reported here a case of systemic lupus erythematosus developed the pneumoperitoneum without the perforation of gastrointestinal tract. A 51-year-old female who had been diagnosed as systemic lupus erythematosus and taken steroid for 12 years, visited our hospital because of general malaise. She had no abdominal symptoms but the roentgenographic examinations revealed the pneumoperitoneum. The laparotomy was performed and there were no findings of the perforation of the gastrointestinal tract. Because PCI is hardly recognized macroscopically after the rupture and the pneumoperitoneum due to PCI is often asymptomatic, we considered the cause of the pneumoperitoneum in this case was the rupture of PCI. The mechanisms of the formation of PCI in patients with collagen diseases were also discussed in this paper.

Conformation and immunogenicity of engineered repeating segment of the circumsporozoite surface protein of Plasmodium falciparum.

Peptide vaccines based on units of the immunodominant tetrapeptide repeats, Asn-Ala-Asn-Pro and Asn-Val-Asp-Pro, of the circumsporozoite surface protein of the parasite Plasmodium falciparum are presently being developed as potential malaria vaccines. The N-terminal fusion of a hydrophobic protein to units of the tetrapeptide repeat affected the immunogenicity and conformational stability of the peptide, and also induced a secondary structure in the peptide. Peptide antigenicity, as well as conformational stability, was significantly increased.

Inter-relationships among anticipatory EMG activity, Hoffmann reflex amplitude and EMG reaction time during voluntary standing movement.

In order to understand the process of executing a voluntary standing movement, the parameters latency (AEA-LT), duration (AEA-DUR) and amplitude (AEA-AMP) of the anticipatory electromyographic (EMG) activity (AEA) in the tibialis anterior muscle, Hoffmann (H) reflex amplitude in the soleus muscle (Sol) prior to the onset of EMG activity in that muscle, and EMG reaction time (EMG-RT) were measured during heel raising from the standing position. The following results were obtained: the three parameters of AEA correlated with EMG-RT in each subject; the average values for all nine normal subjects were r = 0.856 for AEA-DUR, r = 0.448 for AEA-LT and r = -0.215 for AEA-AMP; for the group the mean value of AEA-DUR correlated significantly with that of EMG-RT (r = 0.983, P less than 0.01), while no such significant correlation was observed for AEA-LT; the average value of the AEA-DUR in three slower EMG-RT performers (SLOW-PFM) was significantly longer (P less than 0.05) than that in three faster ones (FAST-PFM), while no significant difference in the AEA-LT was observed; and lastly the total area of the anticipatory suppression of the Sol H reflex amplitude in the SLOW-PFM was greater than that in the FAST-PFM. These results suggest that AEA-DUR, representing postural responses, rather than AEA-LT, reflecting cognitive processes, may have had a close link with EMG-RT, and that the increased suppression in Sol H reflex amplitude originated from the increased anticipatory postural requirement, thus bringing about the EMG-RT delay.

Gametocytogenesis induction by ammonium compounds in cultured Plasmodium falciparum.

Asexual parasites of a strain that seldom or never produce gametocytes in in vitro culture began gametocytogenesis after 24 h treatment with RPMI 1640 medium containing concanavalin A (final concentration, 10 micrograms ml-1) and ammonium carbonate (final concentration, 15 mM ml-1) or ammonium bicarbonate (final concentration, 15 mM ml-1). Gametocytogenesis was consistently observed from the 3rd day after the treatment. Concanavalin A enhanced gametocytogenesis induction by ammonium carbonate or ammonium bicarbonate, although concanavalin A does not itself have gametocytogenesis induction activity. Whereas no gametocytogenesis was observed after addition of concanavalin A and ammonium acetate (final concentration, 5-25 mM ml-1) or ammonium chloride (final concentration, 5-15 mM-1). Addition of ammonium compounds resulted in decrease of parasitemia, regardless of gametocytogenesis.

Increase in glucose-stimulated insulin release and insulin biosynthesis in isolated pancreatic islets from D-galactosamine-treated rats.

Insulin secretion in response to glucose, glucose-stimulated insulin biosynthesis and insulin content was studied in pancreatic islets freshly isolated from male Wistar rats (150-200 g) with galactosamine-induced hepatitis. Animals were sacrificed by decapitation 3, 6, 12 and 24 hours after a single intraperitoneal injection of 500 mg/kg of galactosamine. Isolated islets prepared by collagenase method were perifused in Swim's medium with 20 mM glucose at 37 degrees C up to 30 minutes. Samples were taken at 2-10 min intervals for insulin assay. Insulin biosynthesis was assessed by the incorporation of [3H]-leucine into immunoprecipitable products (insulin and proinsulin) in pancreatic islets after 120 min incubation with 20 mM glucose. Glucose-stimulated insulin secretion was significantly increased at 6, 12 and 24 hours following the administration of galactosamine compared to control. The rate of insulin biosynthesis was stimulated to 170, 138 and 185% of control level 3, 6 and 12 hours after galactosamine-treatment, respectively. Significant increase in insulin content of islets was found 24 hours after galactosamine treatment, following the increased insulin biosynthesis. The present results indicate that pancreatic B cell function is activated in early stage of acute liver injury.

Bacterial aetiology of infantile diarrhoea in Papua New Guinea.

Bacterial examination of stools of children with diarrhoea was carried out at Port Moresby, Papua New Guinea, where little information is available concerning the causative bacteria of diarrhoea. Shigellae, enterotoxigenic Escherichia coli, Bacillus cereus, Staphylococcus aureus, Campylobacter jejuni, and Aeromonas hydrophila were isolated with the decreasing frequency in that order. Among these, enterotoxigenic E. coli and C. jejuni were isolated for the first time in this country. Although Vibrio parahaemolyticus was recovered from sea water, no cases suffering from this organism were found.

Dynamic microstructure and hydration of peroxidized membrane of rat cardiac mitochondria and effects of adriamycin.

Nanosecond time-resolved fluorometry of diphenyl hexatriene, DPH, fluorescence was used to study the effects of lipid peroxidation caused by NADH or adriamycin treatment on the dynamic microstructure of mitochondrial membranes from rat myocardium. Isolated mitochondria were incubated with NADH, FeCl3, and ADP, or with adriamycin. Parameters for microdynamics were calculated from the fluorescence intensity and anisotropy decay curves for DPH fluorescence. Peroxidized lipids were measured as malondialdehyde (MDA) resulting from the thiobarbiturate reaction. As peroxidized lipids accumulated, the membrane viscosity increased and the wobbling angle of the phospholipids decreased. The structural changes induced in unsaturated phospholipids by peroxidation probably increased the friction of neighboring phospholipids and restricted the range of their wobbling motion. The fluorescence intensity and fluorescence lifetimes decreased significantly when MDA was higher than 10 nmol/mg protein. These alterations in the behavior of DPH fluorescence strongly suggest that the hydration of the phospholipid layer of the mitochondria is occurring as a consequence of lipid peroxidation, since the fluorophore, DPH, is hydrophobic and its fluorescence is known to be quenched by increasing the dielectric constant of the surrounding media. The present results provide experimental supports to the hypothesis of membrane hydration induced by lipid peroxidation.

[HLA-DR positive T cells and CD45R positive CD4 cells in primary Sjögren's syndrome].

In 55 patients with primary Sjögren's syndrome (PSJS), we studied on peripheral HLA-DR positive T cells, suppressor inducer (CD4 + CD45R) cell and helper inducer (CD4 + CDw29) cells by 2-color analysis using flow cytometry system. As shown in systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), progressive systemic sclerosis and myositis, the rate of pheripheral HLA-DR positive T cells in PSJS (5.9% +/- 3.9) was significantly higher than in normal (1.6% +/- 1.1). The rate of CD4 +/- CD45R + cells in PSJS (12.8% +/- 6.1) and SLE (12.8% +/- 8.2) were significantly lower than in normal (16.6% +/- 3.9). With respect to the disease pattern of PSJS, the increase of HLA-DR positive T cell and decrease of CD4 + CD45R cell were recognized in patients with leucopenia, high titer of anti-RNP antibody and/or abnormal findings of sialography, and the decrease of CD4 + CD45R + cell was not recognized in patients with kerato-conjunctivitis sicca alone. In either SLE or RA, there were no correlations between the rate of T cell subsets (HLA-DR positive T cells and CD4 + CD45R cells) and the degree of salivary gland damage. T cell subsets in PSJS were useful indicators of disease severity and it is also useful for classifying PSJS into several subtypes. To analyze T cell surface markers in PSJS is very important for investigation of functions of lymphocytes in this disease.

[Neuro-psychiatric symptom associated with primary Sjögren's syndrome].

Neuro-psychiatric complications of primary Sjögren's syndrome (SjS) were studied on their frequency and characteristics of clinical signs in 150 patients of our clinic. Clinical symptoms and laboratory examinations were compared in the patients group complicated with neuro-psychiatric symptoms and the non-complicated group. Neuro-psychiatric symptoms (neuro SjS) were complicated with 63 patients (42.0%). Central nervous system (CNS) involvement were occurred in 47 patients (31.3%). In this group (CNS SjS), psychiatric complications were observed in 43 patients (28.7%), consisted with 29 cases of neurosis, 7 cases of depression, 6 cases of presenile dementia, and others. Nine patients (6.0%) in this group had neurologic involvement that were 3 cases of aseptic meningitis, 3 cases of neurogenic bladder, and each case of multiple infarction, multiple sclerosis, and others. These 9 patients showed more frequently positive results for rheumatoid factor, anti-SSA antibody, leucopenia, and lymphopenia, so this subgroup might be consisted of a small group which was resemble to systemic lupus erythematosus. Cranial nerve involvement, mainly injured in trigeminal nerve, and peripheral nerve involvement were complicated with 5 patients (3.3%) and 24 patients (16.0%), respectively. In neuro-psychiatric complicated group, mean age was older and dry mouth were more frequent (p less than 0.05) than non-complicated group. In psychiatric complicated group, mean age was older (p less than 0.01) and dry mouth were more frequent (p less than 0.05). In any groups of neuro-psychiatric complications, the frequency of any autoantibodies was not statistically different from non-complicated group. But, in the group of cranial nerve involvement, anti-RNP antibody was 60.0% positive (in non-complicated group, 17.2%: p less than 0.05). In CNS SjS group, magnetic resonance imaging (MRI) abnormalities were occurred in 8 of 13 patients. High intensity lesions in white matter on the T2-weighted examinations was characteristic, which showed lacnar infarctions or demyelinated lesions in MRI abnormalities. In CNS SjS, corticosteroid or immunosuppressant was not always necessary, because this complication was self-limited or enough controlled by minor-tranquilizer or anti-depressant. The character of onset in neuro SjS was chronic and neuro SjS was insidiously developing disease. And more studies including treatment on neuro SjS were needed.

Gametocytogenesis induction in cultured Plasmodium falciparum and further development of the gametocytes to ookinetes in prolonged culture.

Gametocytogenesis induction medium (RPMI-FSC reactive medium), prepared with a mixture of the culture supernatant from anti-Plasmodium falciparum antibody-producing hybridoma cells and hybridoma cell lysate, consistently induced gametocytogenesis in cultured P. falciparum. Gametocytogenesis was induced in three strains (0662, FCB 1, and FCR-3) and one clone (R.FCR-3) as has previously been described in two other strains (Ono et al. 1986). Both caffeine (2 mM/ml) and calmodulin (5 micrograms/ml) enhanced the effects of RPMI-FSC reactive medium. Furthermore, gametocytes that appeared after treatment with RPMI-FSC reactive medium continued their development to ookinete formation in candle jar culture with Waymouth's medium.
