Pulmonary blood volume and pulmonary extravascular water volume in men.

In order to estimate the pulmonary blood volume between the pulmonary artery trunk and the left atrium (PBV PAT-LA), simultaneously with the pulmonary extravascular water volume (PEWV) in the chronically stable cardiac patients, we employed the double indicator dilution method using heat as a diffusible indicator and indocyanine green as a non-diffusible indicator. The PEWV was obtained as the extravascular lung thermal volume estimated in the aortic root (LTVAo). In the group of hemodynamically normal patients, in spite of ischemic heart disease (G-N, n = 22), the mean pulmonary artery wedge (PAW) pressure was 9.3 +/- 3.9 mmHg (mean +/- SD), PBVPAT-LA was 8.95 +/- 1.71 ml/kg, LTVAo was 5.71 +/- 1.37 ml/kg and PBVPAT-LA/LTVAo ratio was 1.64 +/- 0.44. In the group of hemodynamically slightly compromised patients with mitral stenosis (G-MS, n = 13), the mean PAW pressure was 14.2 +/- 2.6 mmHg, PBVPAT-LA was 11.12 +/- 2.86 ml/kg, LTVAo was 5.68 +/- 1.41 ml/kg and PBVPAT-LA/LTVAo ratio was 2.02 +/- 0.58. Between the two groups, LTVAo was not statistically significant, whereas the mean PAW pressure, PBVPAT-LA and PBVPAT-LA/LTVAo ratio were all significantly increased in G-MS (p less than 0.05). Neither PBVPAT-LA nor LTVAo correlated with the mean PAW pressure in both groups. From PBVPAT-LA/LTVAo ratio, fluid volume in the intravascular space was greater than that in the extravascular space in both groups. From LTVAo, PEWV in G-MS was identical with that in G-N, in spite of the elevated mean PAW pressure. Therefore, even in the patients with mitral stenosis, the interstitium in the lung is kept "dry" under conditions of the mean PAW pressure below 20 mmHg. The safety factors that prevent pulmonary edema, as evidenced in animal studies, seem to operate effectively in man.

[Results of registration of gynecologic malignant neoplasms in Shizuoka Prefecture--evaluation for one and a half years].

To eliminate death by cancer, it would be necessary for us to register newly developed cancer in certain districts. We have registered all gynecologic malignant neoplasms in Shizuoka Prefecture, Japan, since October 1982 using a microcomputer. Analysis for one and a half years was performed as follows. Gynecologic malignancies are as detailed below: cervical cancer 304 cases, corpus cancer 58 cases, ovarian cancer 51 cases, vaginal cancer 4 cases, vulvar cancer 5 cases. A study of the correlation between detected malignancies and gynecologic examinations, particularly concerning cytologic check-up, revealed the importance of cytology in early cancer detection.

[Hyperthermia in cancer therapy].

Hyperthermia is being re-evaluated and currently used in the treatment of malignant tumor. There have been numerous in vitro and in vivo experimental data presented which confirm the selective heat damage on malignant cells. This review will attempt to summarize the information on experimental and clinical reports concerning hyperthermia. The results of extracorporeally induced hyperthermia of our series are also mentioned.

Allergic contact urticaria caused by raw prawns and shrimps: three cases.

Contact urticaria from raw prawns and shrimps was suspected in three housewives with hand eczema who visited our clinic. A practice skin test in which patients stripped the shells from raw prawns and shrimps was positive, but an intradermal skin test with an allergen extracted from a mixture of prawns and shrimps was negative. Results of skin tests with fresh extracts of raw prawns and shrimps were positive. The RAST with a paper disc method with use of a raw prawn and shrimp as antigens was also positive in these three patients. It is evident from the results obtained that some common substances causing a local urticaria exist in raw prawns and shrimps.

Simultaneous masking between two sinusoidal gratings measured in terms of temporal sensitivity.

Effects of high-contrast simultaneous masking gratings on the temporal sensitivity of signal gratings were measured as a function of masking spatial frequency, where temporal sensitivity was defined as the reciprocal of the minimum exposure duration required for a signal to be detected. The sensitivity of signal gratings was shown to be facilitated when signal frequencies and masking frequencies were separated by more than about 2 octaves but inhibited when the two frequencies were not separated this much. The results were quantitatively different from the results obtained in terms of contrast sensitivity. Psychometric functions on grating detection were shown to become steeper when the temporal sensitivity was enhanced and less steep when it was decreased.

Coagulant and fibrinolytic activities in the leukemic cell lysates.

We attempted to examine procoagulant activity (PCA), X activator activity (XAA) and plasminogen activator activity (PlgAA) of various leukemic cell lysates: 17 acute myelocytic leukemias (AML), 4 acute promyelocytic leukemias (APL), 9 acute myelomonocytic leukemias (AMMoL), 7 chronic myelocytic leukemias (CML), 4 CML with blastic crisis, 7 T cell acute lymphocytic leukemias (ALL), 8 adult T cell leukemias (ATL), 8 null cell ALL, 6 B cell lymphocytic leukemias. Among those 70 cases, 4 APL, 4 AMMoL and 5 AML were associated with overt disseminated intravascular coagulation (DIC) and 5 T cell ALL, 7 ATL and 2 null cell ALL were associated with hypofibrinogenemia not adapted for DIC. The sample used was the lysate of 10(7) cells. PCA was measured by recalcification time of normal plasma with the cell lysate, XAA and PlgAA was measured by chromogenic substrate. APL and AML, especially those associated with overt DIC, had high PCA, and lymphocytic leukemia generally had low PCA in comparison with normal controls. Total PCA (PCA multiplied by cell count/microliter) was remarkably increased in DIC and mildly increased in ALL with hypofibrinogenemia. The change in XAA and total XAA (XAA multiplied by cell count/microliter) was not remarkable in any leukemia except for T cell ALL and null cell ALL with hypofibrinogenemia. PlgAA was high in lymphocytic leukemias with hypofibrinogenemia, APL and AMMoL with DIC. Total PlgAA (PlgAA multiplied by cell count/microliter) was high especially in T cell ALL and null cell ALL with hypofibrinogenemia. Thus it is probable that PCA is the most important factor causing DIC in myelogenous leukemia and that PlgAA is the most important factor causing hypofibrinogenemia in lymphocytic leukemia. The measurement of these activities in the leukemic cells is valuable in prediction and prevention of the hemostatic disorder in leukemia.
