Spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein.

BACKGROUND: The significance of preserving the spleen has recently been shown. However, there are few reports of spleen-preserving distal pancreatectomy for lesions of the body and tail of the pancreas, and this procedure is not generally performed. The aim of this study was to clarify the technique and indications for spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein for benign lesions of the distal pancreas. METHODS: Four patients (two each with an endocrine tumor and a cystic lesion) underwent the procedure. We also performed the spleen-preserving Puestow's procedure with removal of the tail of the pancreas in a 22-year-old male patient with familial chronic pancreatitis. An important technique is to remove the splenic vein from the pancreas from the body of the pancreas toward the spleen and to remove the splenic artery from the pancreas in the other direction. The splenic vein is identified behind the pancreas and within the thin connective tissue membrane, which is cut longitudinally above the splenic vein. It is important to remove the splenic vein from the pancreas from the body of the pancreas toward the spleen, because it is very difficult to remove it in the other direction. There are many branches from the splenic vein on both sides; these branches should be carefully ligated and cut. The pancreas is removed from the splenic artery from the spleen toward the head of the pancreas. This procedure is much easier than removal of the pancreas from the vein. RESULTS: The postoperative course was uneventful in four of the five patients. A pancreatic fistula remained in one patient for several weeks. The mean and standard deviation of the operative blood loss, the duration of the operation, and the postoperative hospital stay in the four uneventful cases, excluding the one in which Puestow's procedure was performed, were 600 +/- 479 ml, 290 +/- 48 minutes, and 40 +/- 21 days, respectively. No severe complications occurred in any of the five patients. CONCLUSIONS: Spleen-preserving distal pancreatectomy with conservation of the splenic artery and vein is easy and safe and should be performed for benign lesions of the distal pancreas.

Surgical resection for small hepatocellular carcinoma.

BACKGROUND: Surgical resection for hepatocellular carcinoma (HCC) can be curative in selected patients, particularly in those with a solitary small HCC (s-sHCC; 2 cm or less in diameter). However, even these patients often have a risk of tumor recurrence or death from underlying liver dysfunction. Therefore it is important to determine which clinicopathologic features are related to the long-term prognosis after resection of s-sHCC. METHODS: Fifty patients with s-sHCC underwent partial hepatectomy at our department between 1977 and 1992. Six (12%) died of liver failure in hospital after operation. Eight clinicopathologic features were examined in the remaining 44 patients with regard to their long-term prognosis by use of univariate and multivariate analyses. RESULTS: The 1-, 3-, and 5-year survival rates were 90%, 75%, and 53%, respectively. The corresponding disease-free survival rates were 80%, 53%, and 30%, respectively. None of the following parameters was significantly related to survival rate or disease-free survival rate: presence of vascular invasion or capsular formation, the distance of free surgical margin (1 cm or more or not), serum alpha-fetoprotein level, positive hepatitis B surface antigen, and preoperative transarterial embolization. Complicated liver function was the only significant factor related to survival rate and disease-free survival rate. CONCLUSIONS: A good hepatic reserve is an important factor in treating patients with s-sHCC by surgical resection, even for a long-term prognosis. Liver transplantation should be considered for patients with severe cirrhosis and s-sHCC, even though a curative resection might be possible.

Effects of growth hormone and insulin-like growth factor 1 (IGF-1 ) on hepatic IGF-1-mRNA, plasma IGF-1 and nitrogen excretion in gastrectomized rats with liver cirrhosis.

The growth hormone (GH)-insulin-like growth factor 1 (IGF-1) axis is impaired in liver cirrhosis. We determined the effects of GH and IGF-1 treatments in gastrectomized rats with thioacetamide-induced cirrhosis. GH did not increase hepatic IGF-1-mRNA, plasma IGF-1 or the tissue, i.e. gastrocnemius muscle IGF-1 level. IGF-1 administration increased plasma IGF-1 without increasing hepatic IGF-1-mRNA. GH and IGF-1 independently decreased postoperative urinary nitrogen excretion. We conclude that both GH and IGF-1 improve postoperative nitrogen metabolism. Furthermore, GH may exert its anabolic effects directly and/or via actions mediated by IGF-1 production, other than in the liver and in the skeletal muscle, in the setting of cirrhosis.

Linear and angular measurements of the mandible during maximal mouth opening.

PURPOSE: There are numerous reports of the translatory movement of the condyle in maximal mouth opening; however, the rotatory movement is less well understood. The purpose of this study was to investigate the rotatory condylar movement and the factors associated with it in normal subjects. PATIENTS AND METHODS: To study the role of facial morphology in maximal mouth opening, linear and angular measurements of the mandible in both the closed and maximal open positions were made by means of cephalometry. Dental students, 29 men and 31 women with no functional disorders of the masticatory system, were investigated. RESULTS: When the mouth was fully opened, the linear measurement of condylar movement was 20.5 +/- 4.0 mm in men and 18.1 +/- 2.5 mm in women, and the angular measurements of the rotation of the mandibular ramus were 39.1 +/- 5.9 degrees in men and 36.3 +/- 4.3 degrees in women. These differences were statistically significant (P < .01). CONCLUSIONS: During condylar movement and rotation of the mandible, mandibular length and the inclination of the mandibular ramus in the former, and the inclination of the mandibular ramus, the mandibular angle and the position of the condyle in the latter, were important factors.

E-cadherin is involved in the intrahepatic metastasis of hepatocellular carcinoma.

In human hepatocellular carcinoma (HCC), the liver is the major target organ of metastasis, which is known as intrahepatic metastasis. To analyze the mechanism of this metastasis, we established two sublines from the human HCC cell line Li7. Subline Li7HM produced multiple liver metastasis, whereas subline Li7NM never did so after intrasplenic injection into nude mice. Two-dimensional gel electrophoresis and immunoblot analysis showed that only Li7NM expressed vimentin and lacked E-cadherin expression, indicating that this clone had undergone epithelial-mesenchymal transition. We then transfected mouse E-cadherin complementary DNA into Li7NM cells and found that the transfectant cells (EM16.21B.3) formed liver metastasis (8/16 mice) after intrasplenic injection and liver tumors (11/13 mice) after intrahepatic injection, whereas the control cell line formed no tumors. These results suggest that E-cadherin plays an important role in the process of intrahepatic metastasis of HCC.

Activities of an occupational health organization in Japan, in special reference to services for small- and medium-scale enterprises.

This paper reports the activities of occupational health organizations in Japan, taking Nishinihon Occupational Health Service Center, one of the largest scale occupational health service providers, as an example. The organizations, at the request of enterprises, provide occupational health services on a profit basis especially for small- and medium-scale enterprises which lack the human resources and/or facilities to meet legal requirements on occupational health. The main services include: (1) providing various types of medical examinations for workers, (2) measuring work environment, (3) occupational physicians and occupational health nurses providing a comprehensive service. Among these services, legally required health examinations and work environment measurements are well served even for small-scale enterprises. Less effective are the health promotion and occupational health service by the occupational health staffs. In the future, these service will be better provided by a newly developed network system to support occupational heath activities in small-scale enterprises.

Beliefs and attitudes towards AIDS policies and educational programs among Japanese businesses.

This study was undertaken to clarify the beliefs and attitudes of Japanese businesses towards AIDS. Questionnaires were mailed to 1,655 Japanese companies listed on the Tokyo Stock Exchange in June 1993, and the response rate was 38% (n = 620). About 60% of the responding companies had already implemented or were developing AIDS measures. Only 7% of the companies had an AIDS policy, and 5% of the companies were developing one. Eighty-five per cent of the companies perceived AIDS education as necessary, while 53% of the companies replied that HIV testing was unnecessary. As reasons for providing AIDS education, avoiding discrimination and anxiety in the workplace were frequently cited. Ninety-five per cent of the companies wanted guidelines or manuals for AIDS to be developed by the Ministry of Labour, or to have training courses for personnel in charge of education or counselling provided by the Ministry. Although AIDS measures are currently unsatisfactory in Japanese businesses, they are expected to develop effective measures, considering their high recognition of the need for AIDS education.

Lysyl-tRNA synthetase from Bacillus stearothermophilus. Purification, and fluorometric and kinetic analysis of the binding of substrates, L-lysine and ATP.

Lysyl-tRNA synthetase [L-lysine:tRNA(Lys)ligase (AMP forming); EC 6.1.1.6] was purified from Bacillus stearothermophilus NCA1503 approximately 1,100-fold to homogeneity in PAGE. The enzyme is a homodimer of M(r) 57,700 x 2. The molar absorption coefficient, epsilon, at 280 nm is 71,600 M-1.cm-1 at pH8.0. Enzyme activity in the tRNA aminoacylation reaction and the ATP-PPi exchange reaction increases up to 50 degrees C at pH 8.0, but is lost completely at 70 degrees C. The pH-optima of the two reactions are 8.3 at 37 degrees C. In the tRNA aminoacylation reaction, the Km values for L-lysine and ATP are 16.4 and 23.2 muM, respectively, and in the ATP-PPi exchange reaction, the Km values for L-lysine and ATP are 23.6 and 65.1 muM, respectively at 37 degrees C, pH 8.0. Interaction of either L-lysine or ATP with the enzyme has been investigated by using as a probe the ligand-induced quenching of protein fluorescence and by equilibrium dialysis. These static analyses, as well as the kinetic analysis of the L-lysine dependent ATP-PPi exchange reaction indicate that the binding mode of L-lysine and ATP to the enzyme is sequential ordered (L-lysine first). The interaction of lysine analogues with the enzyme has also been investigated.

Treatment of multiple early gastric cancer.

To investigate the treatment of multiple early gastric cancer, 82 cases were compared with 829 single early gastric cancers. Univariate analyses with respect to eight clinicopathological factors--age, sex, family history of gastric cancer, macroscopic appearance, histologic type, depth of tumor invasion, tumor location, and lymph node metastasis--were performed. Age, male sex, elevated and differentiated-type tumors, frequent occurrence in the lower third, and mucosal cancers were correlated significantly with multiple early gastric cancer. However, there was no significant difference in the frequency of node involvement. Multiple early gastric cancer, limited to the mucosal layer, was not associated with node involvement. Therefore, endoscopic mucosal resection may be feasible for the treatment of multiple early gastric cancer when there is no evidence of submucosal invasion in any of the lesions and none exceed 2.0 cm in diameter. Upon examination of the long-term results for patients with multiple early gastric cancer, two (3.0%, 2/66) had died of recurrence due to hematogenous spread, and one (1.9%, 1/52) had developed cancer of the remnant stomach. Other primary malignancies were observed in 12 patients (18.2%, 12/66). In particular, lung cancer was the major neoplasm occurring after gastrectomy. These results suggest the importance of systemic surveillance for the detection of other malignancies as well as cancer of the remnant stomach and recurrence after gastrectomy for multiple early gastric cancer.

Route of nutritional supply influences local, systemic, and remote organ responses to intraperitoneal bacterial challenge.

OBJECTIVE: The authors' aim was to investigate whether antecedent nutritional routes influence immune responses after surgical insult. SUMMARY BACKGROUND DATA: Total parenteral nutrition (TPN) may influence host responses to infection. To the best of the authors' knowledge, however, no study has focused on the mechanisms underlying the influence of nutritional route on local, systemic, and remote organ (lung) responses after surgical insult. METHODS: Sixty-eight rats were divided into TPN and total enteral nutrition (TEN) groups. The two groups received identical nutrients for 7 days and were then challenged intraperitoneally with 3 x 10(8) Escherichia coli. In the first experiment, the rats were observed for survival. In the second experiment, the rats were killed before (0 hours) challenge or 2 or 6 hours after challenge. Peritoneal exudative cells (PEC) and bronchoalveolar cells (BALC) were harvested and cultured in vitro. Colony-forming units of bacteria in the peritoneal lavage fluid (PLF) were determined. Tumor necrosis factor (TNF), interleukin-1 alpha (IL-1 alpha), interferon-gamma (IFN-gamma) levels in serum, PLF, bronchoalveolar lavage fluid (BALF), and cell culture supernatants were measured. RESULTS: The 48-hour survival rate was higher in TEN than in TPN rats. Local immunity was depressed in the TPN group. Bacterial colony counts in PLF were significantly higher in the TPN group than in the TEN group after challenge. The number of PECs was significantly lower, and at 2 hours, local cytokine (TNF and IL-1 alpha) responses were diminished in the TPN group compared with the TEN group at 2 hours. The number of PECs showed a significant positive correlation with levels of local cytokines in the TEN group but not in the TPN group. Elevation of local IFN-gamma was significant from 0 to 6 hours in the TEN group but not in the TPN group. In vitro production of TNF by PEC was impaired in the TPN rats before challenge. Remote organ (lung) responses were suppressed in the TPN group. The number of BALCs and the TNF levels in BALF declined significantly between 0 and 2 hours in the TEN group but not in the TPN group. Interferon-gamma levels in BALF were higher in the TEN group than in the TPN group at 2 hours. Systemic cytokine responses were disturbed in the TPN group. Production of systemic TNF was greater, but the IFN-gamma response was diminished in the TPN group compared with the TEN group after intraperitoneal bacterial challenge. CONCLUSION: Local, systemic, and remote organ (lung) immune responses to intraperitoneal bacterial challenge are suppressed in TPN-treated animals, leading to poor survival after challenge. Enteral nutrition before surgical insult may enhance host immune responses after the insult as compared to parenteral nutrition.

Effect of oral administration of protease inhibitor on pancreatic exocrine function in WBN/Kob rats with chronic pancreatitis.

The effect of oral administration of protease inhibitor (camostat) on pancreatic morphology and exocrine function (conscious rat model) was investigated using WBN/Kob rats with spontaneous chronic pancreatitis. In nontreated WBN/Kob rats (2-12 months of age), pancreatic fibrosis and parenchymal destruction compatible with human chronic pancreatitis appeared at 3 months and advanced with each month. Pancreatic secretion was markedly impaired at all ages. In WBN/Kob rats fed diets containing camostat (from 2-3 or 4-5 months of age), the pancreas was hypertrophic and did not show any histological appearances compatible with chronic pancreatitis, and moreover, exocrine function was thoroughly restored with increased plasma cholecystokinin concentrations. Oral administration of protease inhibitor has both preventive and therapeutic effects on pancreatic lesions and dysfunction in an animal model of chronic pancreatitis, probably via endogenous cholecystokinin release.

Growth hormone and insulin-like growth factor I augment Escherichia coli-killing activity of murine peritoneal exudative cells.

Effects of growth hormone (GH) and insulin-like growth factor (IGF)-I on Escherichia coli-killing activity of murine peritoneal exudative cells (PECs) were investigated. Plasma from the mice, injected subcutaneously with saline, GH (4.8 mg/kg/day), or IGF-I(24 mg/kg/day) for 6 days, was mixed with E. coli and pooled murine PECs. Plasma from GH- and IGF-I-treated mice modestly but significantly augmented the E. coli-killing activity of PECs, as compared with that from saline controls. Plasma from IGF-I-treated mice also enhanced PEC interleukin 1 production. In the next experiment, PECs preincubated with medium, GH (10-1000 ng/mL), or IGF-I (50-5000 ng/mL) for 3 h were investigated for E. coli-killing activity. Preincubation of PECs with all concentrations of GH and IGF-I significantly enhanced the E. coli-killing activity of PECs, as compared with the medium control. These results indicate that GH and IGF-I enhance phagocytosis and the E. coli-killing activity of PECs, via a modestly increased plasma capacity to support these activities, as well as by a strong direct action.

Histologic classification of endoscopically removed flat colorectal polyps: a multicentric study.

A total of 594 flat colorectal polyps, removed at endoscopy, were histologically classified into non-neoplastic (n = 49) and neoplastic (n = 545) polyps. Non-neoplastic polyps were subdivided into metaplastic (n = 45) and hyperplastic (n = 4), whereas neoplastic polyps were subdivided into adenomas (n = 481), intramucosal carcinomas (n = 28) and invasive adenocarcinomas (n = 36). Several adenoma phenotypes were discerned: tubular (n = 375), serrated (n = 59), villous (n = 39), mixed (n = 7) and fenestrated (n = 1). Intramucosal carcinomas were subdivided into tubular (n = 26) and serrated (n = 2), and invasive adenocarcinomas into tubular (n = 32), serrated (n = 3) and fenestrated (n = 1). The microscopic characteristics of each histologic phenotype described in this communication are defined and illustrated.

The relationship between maximal jaw opening and size of skeleton: a cephalometric study.

The purpose of this study was to investigate the correlation between maximal mouth opening and size of the skeleton. The subjects were dental students at the Dental School of the Health Sciences University of Hokkaido with no functional disorders of the masticatory system. The stature and maximal mouth opening were recorded in 29 males and 31 females. These subjects agreed to have a lateral cephalogram taken at the closed and maximal mouth opening positions. The correlations between the maximal mouth opening and the body height, mandibular length and mandibular angle were significant. The correlation between the maximal mouth opening and the movement of the condyle was also significant.

Functional stability of porcine hepatocyte spheroids in various culture systems under 100% porcine and human plasma conditions.

To select an immobilization method suitable for bioartificial liver (BAL) modules utilizing porcine hepatocyte spheroids, functional activities were compared in various systems in 100% porcine and human plasma together with a synthesized medium. The spheroids, continuously suspended in rotating dishes or entrapped in collagen (CN) gel, expressed approximately two times higher ammonium detoxification abilities over conventional monolayers during 8 days of direct contact with 100% human or porcine plasma with a standardized inoculum cell number. No significant deterioration was observed in the abilities as compared with that in a synthesized medium. Although the cell number gradually decreased in rotational culture, the abilities per cells remaining on Day 10 were two times higher than in the CN-gel entrapped spheroids in all the media examined, presumably due to the diffusion limitation by the gel. Thus, in utilizing porcine hepatocyte spheroids in BAL modules, immobilization allowing direct contact of spheroids with perfused patient plasma was concluded to be possible and suitable.

Zonal adult Hirschsprung's disease.

BACKGROUND: Hirschsprung's disease is a congenital disorder which is rare in adulthood. In typical cases the aganglionosis involves mainly the rectum or rectosigmoid colon and the lesion starts from the anal valve. Zonal segmental aganglionosis is a very rare type even in children. PATIENT: A 54 year old women with zonal segmental aganglionosis had an aganglionic segment 18 cm in length located in the rectosigmoid colon with an 8 cm long normal appearing rectum and dilated proximal colon. Resection of the stenotic segment with end to end anastomosis was performed. CONCLUSION: The functional result was excellent five years after the operation.

Growth hormone and the immune response to bacterial infection.

Growth hormone (GH) and insulin-like growth factor 1 (IGF-1), especially the former, have immunoregulatory effects in addition to anabolic effects. The hormones may act to protect the host from lethal bacterial infection by promoting the maturation of myeloid cells, stimulating phagocyte migration, priming phagocytes for the production of superoxide anions and cytokines, and enhancing opsonic activity. GH administration may be beneficial for the prevention, as well as treatment, of severe sepsis in critical illness.
