[Total cystectomy for urinary bladder cancer: clinicopathological study of 95 cases].

Total cystectomy was performed on 95 patients with primary urinary bladder cancer between 1973 and 1983. Histopathological and prognostic studies were reviewed according to the general rules for clinical and pathological studies on bladder cancer. The cancer histological type were transitional cell carcinoma in 87 cases, squamous cell carcinoma in 5 cases, adenocarcinoma in 2 cases, and undifferentiated carcinoma in 1 case. The overall 5-year actuarial survival rate was 36.0%. As for the growth pattern of the bladder cancer, the 5-year survival rates for the patients with papillary non-invasive type (PNT), papillary invasive type (PIT), and non-papillary invasive type (NIT) were 100%, 25.8% and 34.8% respectively. As for the stage, the 5-year survival rates for the patients with pTa, pT1, pT2, pT3a, pT3b, and pT4 were 81.8%, 64.7%, 40.1%, 30.5%, 22.6% and 6.7% respectively. Of 87 patients with transitional cell carcinoma, the 5-year survival rates for the patients with grade 1, grade 2 and grade 3 were 100%, 43.0% and 32.1% respectively. Intramural lymphatic invasion and vascular invasion and intramural histopathological mode of spread were significant indicators of prognosis.

[A case of renal oncocytoma].

A case of renal oncocytoma is reported. A 56-year-old woman was admitted with the complaint of an abdominal mass. Right transabdominal nephrectomy was performed on January 23, 1984. The resected kidney weighed 262 g and contained a well-demarcated 5 X 5 X 4 cm tumor in the middle portion. The cross section of the tumor was tan-brown. Light microscopic examination disclosed that the greater part of the tumor was composed of cells with abundant, and finely granular eosinophilic cytoplasm and with moderate nuclear pleomorphism. Electron microscopic examination confirmed that the cytoplasm had numerous mitochondria and few other organelles. According to these findings, the tumor was diagnosed as renal oncocytoma. Furthermore, it is noteworthy that in this case there were tubular arrangement of the cells resembling proximal renal tubules in a focal area and a group of cells with considerable nuclear atypia and hyperchromatism in another area. The patient has been well without any local recurrence or distant metastasis ten months after the operation. In addition, fourteen cases of renal oncocytoma in Japan, including the present case, are reviewed.

[Nine cases of bladder cancer occurring in occupational dye users].

Workers in the dye manufacturing industry have a high risk of urinary bladder cancer. There may also be a high relative risk of bladder cancer in occupational dye users. Nine occupational dye users were found to have bladder cancer. The period of engaging with dye work ranged from 5 to 40 years. Seven patients had bladder cancer and the other 2 patients had lesions both in the bladder and in the renal pelvis. Histopathology of all cases was transitional cell carcinoma. Three cases were classified into grade 1 and 6 cases into grade 2. One patient died of bladder cancer. The other 8 patients are alive. Four cases recurred after TUR therapy. It is necessary to establish systemic mass screening examination of occupational dye users for the early diagnosis of bladder cancer.

Pulmonary macrophage: a major source of lipoprotein lipase in the lung.

The lipase released by heparin infusion into perfused rat lungs is shown to be identical with lipoprotein lipase on the basis of the following characteristics: 1) it required plasma for full activity; 2) it was largely inhibited by 1 M NaCl; and 3) it bound to a heparin-Sepharose gel and eluted with buffer containing 1.5 M NaCl. Macrophages prepared from rat lungs released a considerable amount of lipoprotein lipase, with characteristics similar to those of the lipoprotein lipase released from heparin-perfused rat lungs. Pulmonary surfactant-producing type II pneumocytes did not cause a significant release of lipoprotein lipase. No apparent lipoprotein lipase activities were detected in the conditioned medium of other lung cells from which macrophages had been selectively removed.

Wide variations of plasma triglyceride concentrations in guinea pigs.

Guinea pigs have varying plasma triglyceride concentrations ranging from 28 to 1392 mg/dl, with relatively uniform plasma cholesterol and phospholipid levels. To understand why the animals exhibit such wide variations of plasma triglyceride concentrations, we have explored the triglyceride hydrolyzing system by measuring tissue lipoprotein lipase activities and plasma activator for the enzyme. Lipoprotein lipase activities of epididymal adipose tissue of these animals were 759 +/- 117 (mean +/- SE) n moles FFA X min-1 X g wet tissue-1, markedly low compared with those of rats. There were no relationships between plasma triglyceride concentrations and tissue lipase activities. Plasma activator for lipoprotein lipase was lacking in this animal. Guinea pigs with ascorbic acid deficiency for 2 weeks also showed marked variations of plasma triglyceride concentrations, without any changes in tissue lipoprotein lipase activities. Low adipose tissue lipoprotein lipase activities with deficient plasma activator for the enzyme suggest that the lipoprotein lipase-mediated triglyceride degradation could be impaired in this animal, and this may account for the marked variation of plasma triglyceride concentrations.

The excretion of dimethadione in pure pancreatic juice and bile in postoperative patients.

The excretion of dimethadione (DMO) in pure pancreatic juice and bile was studied in postoperative patients undergoing external drainage of pancreatic juice and in those who had undergone percutaneous transhepatic cholangiodrainage. During and after oral administration of trimethadione, which is the precursor of DMO, pancreatic DMO concentration closely paralleled plasma DMO concentration. Pancreatic juice/plasma concentration ratio for DMO exceeded 1.0. In a patient with a large pancreatic cyst communicating with the main pancreatic duct, pancreatic DMO concentration after single rapid injection of secretin inversely correlated with the flow rate, while its output depended directly on the rate. Biliary DMO output after secretin injection was closely dependent on the flow rate, but was extremely low. The present results are essentially compatible with the findings previously observed in dogs.

Plasma lipoproteins in streptozotocin-diabetic rats, after Triton WR-1339 treatment.

In an attempt to examine whether the VLDL secreted by diabetic rats may be different from that by normal rats, plasma obtained after Triton WR-1339 treatment was analyzed. After Triton treatment, a marked increase in plasma triglyceride levels with lesser increases in cholesterol and phospholipids was observed in both diabetic and control rats. The degree of such increases was almost equal between two groups of rats. The large increases in plasma lipid levels presumably reflect the increase in plasma VLDL fraction. The lipid composition of VLDL of diabetic rats was very similar to that of normal controls. The LDL fraction of diabetic rats was low in its contents of both cholesterol and phospholipids compared with that of normal controls. The composition of IDL and HDL in diabetic rats was similar to that of controls. From these data, we suggest that the VLDL of diabetic rats secreted under basal fasting condition is not different in its lipid composition as well as in amounts from that of controls.

Reciprocal changes of plasma apo AI and apo E levels in streptozotocin-induced diabetic rats.

Amounts of plasma lipids, apolipoprotein AI (apo AI) and apolipoprotein E (apo E) were measured in streptozotocin-induced diabetic rats. Plasma triglyceride and cholesterol levels of diabetic rats were not significantly different from those of control rats. Plasma apo AI levels of diabetic rats were significantly higher than those of control rats (78.2 +/- 29.3 vs 27.2 +/- 3.4 mg/dl, P less than 0.001), while plasma apo E levels of diabetic rats were significantly lower than those of control rats (4.2 +/- 1.0 vs 13.9 +/- 5.3 mg/dl, P less than 0.001). Insulin treatment (12U/day) of diabetic rats decreased plasma apo AI levels significantly (treated: 32.8 +/- 3.4, untreated: 48.7 +/- 6.2, control: 28.5 +/- 2.4 mg/dl) and normalized plasma apo E levels (treated: 16.1 +/- 1.7, untreated: 5.4 +/- 0.7, control: 15.8 +/- 1.3). Insulin injection (4U/day) to normal rats did not cause any changes in both plasma apo AI and apo E levels. The data indicate that diabetes is not always accompanied by hyperlipidemia, however this inevitably carries apoprotein abnormalities characterized by the high plasma apo AI and low apo E levels, which are reversible with insulin treatment. The changes in the levels of plasma apo AI and apo E could be related to the development of atherosclerosis in diabetes.

Effects of plasma infusion on plasma lipids, apoproteins and plasma enzyme activities in familial lecithin: cholesterol acyltransferase deficiency.

The siblings presented here are the third family found in Japan with familial LCAT deficiency. Their post-heparin plasma lipoprotein lipase and hepatic triglyceride lipase activities were measured selectively by an immunochemical method. Plasma triglyceride levels were elevated, and post-heparin plasma lipoprotein lipase was decreased only in a patient with nephropathy, while hepatic triglyceride lipase activities were within reference limits in both patients. The plasma concentrations of apo A-I, apo A-II, and apo B were reduced in both patients. On the other hand, the plasma concentration of apo E was markedly increased. Enzyme replacement therapy by plasma transfusion in the propositus resulted in marked improvement of deranged compositions of triglyceride-rich lipoproteins. Also, improvement of the plasma apo E concentration was demonstrated, while the improvement of post-heparin lipase did not occur. These results suggest that LCAT may play an important physiological role in triglyceride metabolism as well as in cholesterol metabolism.

[Therapeutic effect of ceftizoxime on severe infectious complications in blood disorders. Tohkai Research Group on Infections in Hematopoietic Disorders].

Ceftizoxime (CZX) was given by intravenous injection in daily doses of 2-8 g to 103 patients with severe infections complicating hematopoietic disorders. The clinical effect was evaluated in 95 of the 103 patients. The causative organisms were identified in 22 patients but were unknown in the remaining 73. Infected sites were the respiratory tract, urinary tract, soft tissue, and blood. The overall effectiveness rate (inclusive of marked and moderate) was 61.1% (58/95). The effectiveness rate was 63.6% (14/22) in patients in whom the causative organisms were identified and 60.3% (44/73) in patients in whom the causative organisms could not be identified and 60.0% (18/30) in 30 patients with less than 100 neutrophils per mm3 before treatment. No side effects were noted except drug fever in 1 patient. Abnormal hepatic function was seen in 6 patients but was not attributed to the drug in any case. The results indicate that CZX is a safe and useful antibiotic for the treatment of severe infectious complications in hematopoietic disorders.

Sixteen adult patients with acute leukemia treated by chemotherapy, total body irradiation and allogeneic marrow transplantation.

Since 1976, 16 adult patients with acute leukemia have been treated by chemotherapy, total body irradiation (TBI) and allogeneic bone marrow transplantation (BMT) in the medical school hospital and the satellite hospitals of Nagoya University. The first group of 10 patients were given marrow grafts at the time of leukemic relapse and the second group of six patients were given the grafts in the period of remission of their disease. For the first group (ALL/ANLL 2:8, age (median) 33, M/F 8:2), HLA-identical donor cells (25 x 10(7)/kg [median]) were infused after the patients were conditioned with NSC D 245382 (ACNU) or daunorubicin, cyclophosphamide (CY) and a single shot of 1000 rad of TBI. For the second group (ALL/ANLL 4:2, age (median) 20, M/F 5:1), HLA-identical donor cells (22 x 10(7)/kg [median]) were infused after the patients were conditioned with CY and fractionated (250 rad x 4) TBI. All the patients were isolated in a laminar air flow room (LAF) after gut and skin decontamination. Engraftment of donor cells was confirmed in 15 out of the 16 patients. Febrile periods in LAF and the days required for platelet transfusion were prolonged in the first group. All the patients in the first group died within 12-214 days after BMT because of interstitial pneumonitis (7 patients) or bacterial infection (3 patients). On the other hand, five out of six patients in the second group are alive 84-540 days after BMT. For the surviving patients, the complications of chronic graft versus host disease, viral infections, tuberculosis, hepatitis, hemorrhagic cystitis and recurrence of leukemia are now the problems. It can be stated that the patient's clinical condition at the time of BMT is one of the most essential factors for the success of BMT although the effects of other variables, such a change in the conditioning regimens of the supportive care, must also be carefully analyzed.

[An evaluation of antimicrobial removal device to isolate bacteria from bacteremic patients with hematological disorders].

Antibiotics contained in blood specimens often inhibit bacterial growth in culture media. Recently, the antimicrobial removal device (ARD) containing resins to absorb antibiotics has been made available. To evaluate the effectiveness of the ARD, we investigated how much the antibiotics were removed by the ARD. The ARD method was compared to the conventional culturing method in isolating organisms from blood specimens of patients with hematological disorders receiving antimicrobial agents. The antibiotics, including cefotiam, cefsulodin, cefmenoxime, cefazolin and sulbenicillin, were proved to be almost completely removed by the ARD. Bacteria were detected only by use of the ARD in the blood cultures from 2 of 21 blood specimens (11 patients) entered in the study, while all of the blood cultures were negative by the conventional method. These isolated bacteria were S. aureus and P. aeruginosa, of which septicemias were cured by intensive antibiotic therapy for these bacteria. It is suggested that pretreatment with the ARD makes detection of bacteria easier in blood from the patients receiving antibiotics. Literatures were reviewed concerning improvement and shortened time for isolation of organisms by using the ARD.

[Pelvic recurrence of bladder carcinoma following total cystectomy].

The patients who undergo total cystectomy for high stage bladder carcinoma have poor prognosis because of local extension or distant metastases. In the case of recurrent lesions after total cystectomy, radiotherapy and chemotherapy are generally selected. In some cases of recurrent tumors localized within the pelvic cavity and with no distant metastases, however, surgical resection of the tumor is effective in local disease control combined with radio- and chemotherapy. Surgical treatment is not only effective in reducing the cell number in a bulky tumor, but also improves clinical symptoms and conditions such as pain, bleeding, rectal irritability, intestinal obstruction and so on. We present here three patients who underwent extensive operation for pelvic recurrence of bladder carcinoma following total cystectomy. Two of these cases died of progressive disease, nine months and five months after the extensive operations. The third patient died of acute pericarditis thirteen months after the second operation. Autopsy revealed that the surgical treatment was effective in controlling the local recurrence in the third case. Furthermore, some problems regarding pelvic recurrence and surgical treatment are discussed.
