Oral kinesthesia in patients with Duchenne muscular dystrophy.

In trials on normal subjects and patients with Duchenne muscular dystrophy (DMD), interdental dimension discrimination (IDD) was tested by assessing the ability of subjects to discriminate between pairs of sticks of different dimensions held between the upper and lower teeth. The IDD ability of the DMD patients was significantly inferior to that of the normal subjects. Further, DMD patients tended to overestimate the dimension of the first stick of each pair even more than did normal subjects. Vibration applied to the mandible seemed to lessen such inaccuracies of oral kinesthesia in the DMD patients. These results are compatible with the idea that muscle receptors, especially muscle spindles in jaw closing muscles, are mainly responsible for IDD.

In vitro synthesis and posttranslational uptake of cytochrome c into isolated mitochondria: role of a specific addressing signal in the apocytochrome.

Administration of the thyroid hormone 3,3,5'-triiodo-L-thyronine (T3) to rats leads to a marked increase in hepatic levels of mRNA for cytochrome c. Messenger RNA prepared from the free polysomes of T3-treated rats directed the in vitro synthesis of a polypeptide which only differed in amino acid sequence from mature cytochrome c in that it contained an NH2-terminal methionine. The in vitro product was incorporated specifically into purified rat liver mitochondria and became inaccessible to added trypsin when the mitochondria were added after translation was completed. Horse heart apocytochrome c, but not the holocytochrome, could compete with the in vitro synthesized polypeptide for its uptake into mitochondria. This suggests that the primary structural features of apocytochrome c, which serve as an addressing signal for mitochondria, are masked after the acquisition of heme and that this process occurs in the mitochondria. The addressing signal seems to be contained in a specific segment of the cytochrome polypeptide because only one fragment generated by CNBr cleavage of horse apocytochrome c, extending from residue 66 to the carboxy end of the molecule, could compete with the in vitro product for its transfer into mitochondria.

Effect of oral administration of lysozyme or digested bacterial cell walls on immunostimulation in guinea pigs.

The effect of oral administration of bacteriolytic enzymes and enzymatically digested bacterial cell walls on immunostimulation in guinea pigs was studied. Guinea pigs were given lysozyme or pronase or both orally for a period of 8 days, and on day 7 they were primed with hepatitis B surface antigen. Circulating antibody titers to the antigen in the enzyme-treated group were significantly higher (four to six times, P less than 0.05) than those in nontreated control groups on day 16 after immunization. Stimulation of cellular immunity in the group receiving both lysozyme and pronase simultaneously was significantly increased compared with the group receiving only one of them. The humoral immune response was enhanced by oral administration of enzymatically digested cell walls isolated from Bifidobacterium longum. The result suggested that intestinal bacteria might be solubilized by oral administration of bacteriolytic enzymes and that the absorbable fragment of peptidoglycan released from the bacterial cell walls might be responsible for the enhanced host immune responses.

Proprioceptive input from the jaw muscles and its influence on lapping, chewing, and posture.

Experiments are described which define criteria for identifying fusimotor axons discharging in filaments of the masseter nerve in lightly anaesthetized cats. During reflex movements of the jaw two patterns of discharge were observed in different fusimotor fibres. One type, called "sustained," fired at a fairly constant increased rate. The other, called "modulated," fired at high frequencies during the extrafusal muscle contraction. Evidence from spindle primary and secondary recordings in similar experiments strongly suggests that the "sustained" type were dynamic fusimotor fibres and the "modulated" type were static fusimotor fibres. New spindle recordings in normal unanaesthetized cats indicate that the modulated pattern of static fusimotor discharge also occurs in these conditions. Its effect is to reduce the tendency for spindle afferents to become silent during muscle shortening. A proposal is made that the static fusimotor discharge in cyclic movements may represent a temporal "template" for the intended movement as directed by the central pattern generator.

On-line control of circulating blood volume.

We developed an experimental method which enabled continuous determination of the circulatory blood volume, when the blood volume was subjected to alterations. The blood volume was modified by a servo system to control transfer of the blood between an external reservoir and the cardiovascular system. Possible application of our method to the analysis of circulatory responses to blood volume changes is discussed.

Transvascular fluid shift after blood volume modification in relation to compliances of the total vascular bed and interstitial fluid space.

The transvascular fluid shift after blood volume modification was analyzed by means of continuous blood volume determination and stimulation analysis. On splenectomized dogs, 15% of the circulating blood was withdrawn and retransfused, while the circulating blood volume, hematocrit and colloid osmotic pressure were continuously monitored. Based on the results, the transvascular fluid shift was stimulated by a two-compartment model. The compliance of the intravascular and interstitial fluid spaces as well as the transvascular filtration coefficient was determined on the whole body, simultaneously. No statistically significant differences were found between any of the values for blood withdrawal and retransfusion. The compliance of the vascular space, 4.62 +/- 0.82 ml/mmHg . kg of body weight, was almost identical with that of the interstitial fluid space, 4.75 +/- 1.52 ml/mmHg . kg of body weight. The whole body capillary filtration coefficient was 0.74 +/- 0.39 ml/min . mmHg . kg of body weight, which is within the range of the reported values of the capillary filtration coefficient for muscle and intestine. It is suggested that the rapid transvascular fluid shift after blood volume modification is provided with the fact that the compliances of intravascular and interstitial fluid spaces are almost identical.

[Bilateral internal carotid artery thrombosis (author's transl)].

Nineteen patients with bilateral internal carotid artery thrombosis were analysed with respect to long term clinical course from first onset to admission. The patients comprised 19.3% of all patients with occlusive cerebral disease treated in past three years at our clinic. In reviewing the symptoms and signs associated with bilateral carotid occlusions and the clinical course, several patterns became evident and in this report the patients are divided into 3 types: Apoplectic type, those who had severe neurological deficit acutely after occlusion, whose angiography showed impaired or absent collateral circulation, and whose CT scan demonstrated bilateral diffuse low density areas; Progressing type, those whose neurological signs and symptoms grew progressively serious, whose angiography showed moderate or adequate retrograde filling with collateral circulation through posterior cerebral arteries and whose CT scan demonstrated low density lesions in the bilateral frontal lobes; TIA . RIND repeated type, those who had transient ischemic attacks or reversible ischemic neurological deficits repeatedly, whose angiography showed good normograde filling of bilateral internal carotid artery with collateral circulation through posterior communicating arteries and whose CT scan demonstrated no abnormal low density areas except findings of brain atrophy.

Biopsy of the breast.

A study on surgical biopsy for treatment of breast cancer was done by sending out questionnaires to membership institutions of the Japan Mammary Cancer Society. The problems related to diagnostic biopsy of the breast mainly concern the following two points; the first is the methodology, incisional or excisional, the second, whether delay between biopsy and radical mastectomy influences survival. The prognostic features in patients with excisional biopsy were significantly superior. The prognostic features in patients with excisional biopsy were significantly superior. As for the lapse of time between biopsy and radical mastectomy, 14 days were considered safe, and under conditions such as T2 or n0, this maximum was 7 days.
