[Effects of chemo-endocrine therapy on DMBA-induced rat mammary carcinoma].

The effect of chemo-endocrine therapy (5-FU plus TAM) on 7,12-DMBA-induced rat mammary carcinomas was studied in terms of its antitumor effects and changes in hormone receptors (ER and PgR). The study detected no difference in antitumor effects between combined administration of 5-FU and TAM, and either agent alone. The ER and PgR contents markedly decreased following chemo-endocrine therapy and endocrine therapy (TAM), but no significant decrease was achieved by chemotherapy (5-FU) alone. These results suggest that, in the combined administration of 5-FU and TAM, TAM was more effective than 5-FU. The expected advantage from chemo-endocrine therapy was not observed.

Flexible bronchoscopy for foreign body in airway.

The flexible bronchoscopy has not yet been popular in the foreign body in the airway, especially in children. We performed the flexible bronchoscopy for 27 patients with foreign bodies in the airway including 22 patients who had not been removed foreign bodies by the rigid bronchoscopy. Nineteen of 27 patients were children. The success rate of flexible bronchoscopy was 81% (22/27 cases) in all patients, 77% (17/22 cases) in patients failed by the figid bronchoscopy, and 82% (9/11 cases) in young children of 2 years old or less. For the removal of foreign bodies under the flexible bronchoscope, a biopsy forceps and a Fogarty balloon catheter were effective. The flexible bronchoscope is useful for foreign bodies in the airway, even in young children.

[Morphological study on open bite patients].

The purpose of the present study is to investigate the morphological features of open bite patients who have also disharmony of anteroposterior relations between maxilla and mandible. Lateral head films of 40 adult females with open bite were used. The samples were classified into three groups by ANB angle. Three groups were as follows. 1. Type I open bite (12 subjects, 1 degree less than or equal to ANB less than or equal to 4.5 degrees). 2. Type II open bite (13 subjects, ANB less than 4.5 degrees). 3. Type III open bite (15 subjects, ANB less than 1 degree) The normal samples was the longitudinal data of females in our department. Skeletal and dental variables were calculated and compared norm versus type I open bite, type I open bite versus type II open bite and type I open bite versus type III open bite, respectively. The following results were obtained. 1. Disharmony of the vertical dimensions in type I open bite was mainly due to long anterior lower facial height which was accompanied by large gonial angle as compared with norm. 2. There were no difference in anteroposterior position of the maxilla and nasal floor in all three open bite groups. 3. There were no difference in anterior and posterior facial height between type I and type II open bite. However, type II open bite showed the backward rotation of the mandible accompanied by over eruption of upper first molars. 4. Type III open bite showed longer anterior lower facial height which was caused by excessive mandibular growth than type I open bite. 5. Upper incisors showed labial inclination as compared with norm in all three open bite groups. However, lower incisors showed labial inclination in type II open bite and lingual inclination in type III open bite as compared with type I open bite. As a result, the inter-incisal angle was small in type II open bite and large in type III open bite as compared with type I open bite. These results suggested that the dentoalveolar factors were large in type II open bite and the skeletal factors were large in type III open bite as compared with type I open bite.

[Time course of blood velocity changes and clinical symptoms related to cerebral vasospasm and prognosis after aneurysmal surgery].

Cerebral vasospasm is a major complication associated with subarachnoid hemorrhage. In spite of extensive research, the pathogenesis of vasospasm remains obscure, and clinical management has so far been extremely difficult. For the evaluation of the efficacy of any treatment and the timing of operation, the development and resolution of the arterial narrowing should be monitored. It is important to know the correlation between the change in flow velocity and the course of clinical symptoms. In this study, the blood flow velocity in the different arteries was measured at short intervals (at least every third day) with transcranial Doppler ultrasonography, in eighteen patients with ruptured cerebral aneurysm. Flow velocity changes began to accelerate from 3 days after surgery, and reached the maximum value between 7 and 10 days, with normalization occurring within the following 2 weeks. The changes showed a significant relationship to the severity of vasospasm, the side of the operative approach, and the administration of calcium antagonist. Based on our results, we categorized the flow velocity changes of vasospasm for clinical practice as follows. (1) A slight acceleration of up to 120 cm/s in MCA and 90 cm/s in ACA was defined as within the normal range. In this state vasospasm was never observed angiographically. (2) Flow velocity changes between 120 and 140 cm/s were a subcritical state. These changes were seen in vasospastic patients who did not develop ischemic deficits. (3) Flow velocity changes of over 140 cm/s in MCA and 100 cm/s in ACA were critical vasospasm, since all of the patients who developed ischemic symptoms were in this group. Velocity changes higher than 170 cm/s in MCA and 130 cm/s in ACA seem to indicate a critical condition with a tendency to develop into brain infarction. The comparison between the time course of the flow velocity changes and clinical status showed that in symptomatic vasospasm the increase in velocity occurred before the manifestation of clinical symptoms. The transcranial Doppler measurements help to identify those patients who have a high risk of developing neurological deficits due to vasospasm, and help to select those who would benefit from preventive treatment in asymptomatic stage.

[Alteration of phenotype, morphology and drug sensitivity in colon cancer cell lines under various culture conditions].

Thirteen colorectal cancer cell lines were established from 120 fresh colon cancer specimens. Expression of colon cancer-related antigens and blood group-related antigens on these cell lines were evaluated by Mixed Hemadsorption test (MHA) and Immune Adherence test (IA). The results were compared with the results of immunohistological analysis of the antigen expression in sections of the original tissues, and deletions of several blood group-related antigens in cell lines were observed. Colon cancer cell line HT-29, cultured in different lots of FCS or in different types of serum-free medium showed various antigen expression, suggesting the effect of culture medium on phenotypic expression. In glucose-free medium, HT-29 cells showed the characteristic "dome formation" which is typical of enterocytic differentiation forming brush borders in such cells. HT-29 cells cultured on bovine corneal endothelial extracellular matrix (BCE-ECM) coated plates showed similar enterocytic changes under phase-contrast microscope. Also, irradiated HT-29 cells cultured under 8-Azaguanine containing medium retained their strong resistance to 8-Azaguanine. These results suggest that these cancer cell lines might alter their phenotype, morphology, differentiation, growth pattern and drug sensitivity depending on the culture environment.

[Problems of minor surgery for the early breast cancer--from the clinicopathological study].

In 183 mastectomy specimens, performed for invasive or noninvasive carcinomas, histologic study was performed to assess the local extension of carcinoma. A wide carcinomatous extension, reached up to more than 4.5 cm from the margin of tumor, was found in 21 of 87 cases (24%) of breast cancer with tumor size less than 2 cm. There was 3 cases, having double palpable tumors in the same breast, no carcinomatous continuity between the 2 foci. The results have supported a concept of multicentric development of the cancer in the same breast. Carcinomatous involvement of the nipple and/or areola was found in 44 of 141 specimens (31%). These findings indicate that the possibility of leaving a carcinomatous residue if a partial mastectomy was performed.

[The results of experimental study of six-hour heart-lung preservation by autoperfusion method--its evaluation of optimal conditions and lung function after preservation].

Up to date, it has been reported that the maintenance of ideal function of the preserved lungs were much more difficult than that of the hearts in heart-lung preservation. In this communication the authors have reported the results of experimental study for optimal conditions for preserving better function of the lungs by autoperfusion method by means of heart-lung preparation using 43 dogs. In this study the conditions of the preservation were fixed as following: perfusing blood temperature 29 degrees C, blood flow 30 ml/kg/min., FiO2 30%, FiCO2 5%, tidal volume 15 ml/kg, ventilation rate 10/min., and PEEP 5 cmH2O. Glucose-Insulin-Potassium (0.03 gm., 0.05 U., 0.02 mEq/kg/hr. respectively) were administered continuously by an infusion pump. The results showed that extravascular lung water contents after 6 hours of preservation was 0.79 (mean) +/- 0.01 (SD), which was increased only 1% over than the control group: 0.78 +/- 0.01. There was no significant difference of static lung compliance in two groups: the preserved group was 0.47 +/- 0.02 ml/gm.cmH2O compared to 0.51 +/- 0.06 in the control group. These results suggest that the autoperfusion method on our preserving conditions seems to be very promising and very effective to keep much better condition of the lungs in heart-lung preservation.

[The use of free rectus abdominis myocutaneous flap to close the empyema space with alveolar fistula--a case report].

A 59-year-old man, who had had right middle and lower lobectomy for pulmonary tuberculosis, admitted for the treatment of empyema with fistula. Closure of empyema space with free rectus abdominis myocutaneous flap was performed following open window thoracotomy and thoracoplasty. As he previously underwent two major operation, lobectomy by posterolateral approach and gastrectomy for gastric ulcer, free rectus abdominis flap was chosen instead of omental flap or latissimus dorsi myocutaneous flap. Postoperative CT film showed that this flap was filled up in all interstices of the empyema cavity. The pedicle vessels to this flap are large enough to provide long stalks, so microsurgical anastomosis can be accomplished safely. The use of free rectus abdominis myocutaneous flap is one of a useful maneuver for chronic empyema with fistula.

[Comparison of the binding nature of mibolerone to androgen receptor of human prostate with that of R1881].

The authors examined the binding nature of mibolerone in cytosols of hypertrophic prostates from 15 patients to androgen receptor using Dextran-charcoal assay, analyzed it by the method of Scatchard, and compared it with that of R1881. The addition of triamcinolone acetonide into the incubation medium induced specific single binding of mibolerone to androgen receptor with high affinity as well as R1881. The receptor contents obtained with mibolerone were higher than those of R1881, and both of them correlated well. The dissociation constants of both ligands showed good correlation and no significant differences. Mibolerone seems to be as suitable a ligand as R1881 for measuring the androgen receptor.

Mucinous carcinoma of the breast in Japan. A prognostic analysis based on morphologic features.

In 175 women with mucinous carcinoma (MC) of the breast, the morphologic features, the clinicopathological features (age, tumor size, and nodal status) and prognoses were investigated. They were divided into two types, those with "unmixed type" (n = 140) showing no component of invasive ductal carcinoma (IDC) and "mixed type" (n = 35) including an IDC component. The unmixed type showed less frequent (P less than 0.01) nodal involvement and a higher degree (P less than 0.005) of extracellular mucus production than the mixed type. The presence of nodal metastases showed no correlation with the degree of mucus production. The 10-year survival rate of the unmixed type (90.4%) was better (P less than 0.001) than in the mixed type (66.0%). In the unmixed type, a higher level of mucus production showed trends toward a better prognosis, younger age and smaller tumor size (P less than 0.01). In the mixed type, the degree of mucus production showed no significant correlation with either the age, tumor size, or prognosis. It is more important for the prognosis of patients with MC to have no IDC component than to show abundant mucus within the tumor.

Rapidly growing mature teratoma of the mediastinum: do sex hormones affect growth of the tumor?

The case of a 12-year-old girl with an anterior mediastinal tumor is reported. A mass grew rapidly and occupied the anterior mediastinum after menarche. Resected material was diagnosed as a mature teratoma. Both estrogen receptors and progesterone receptors were detected in the resected tissue. The titer of estrogen receptors was 2-11.3 femtomoles per milligram of protein and that of progesterone receptors was 9.5-15.7 fmol/mg protein. Based on these data, it was surmised that sex hormones may affect the growth of the tumor in this case. The change in sex hormones after menarche may have played a role in the rapid growth of this tumor.

Prognostic value of hormone receptors in breast cancer.

The aim of this study was to establish the role of estrogen receptor (ER) and progesterone receptor (PgR) as prognostic indicators for early recurrence and survival. In all, among breast cancer patients, 166 patients who had undergone radical or extended radical mastectomy were studied. These patients were treated with adjuvant chemotherapy alone for 2-3 years after surgery. No patients had adjuvant endocrine therapy. Local recurrence and/or distant metastases were treated by endocrine therapy and/or chemotherapy. The relapse-free interval was not different between the ER-positive and ER-negative patients. The postrelapse survival curve was significantly different between the two groups. There was no significant difference in the relapse-free interval and the postrelapse survival curve between the PgR-positive and PgR-negative patients. These results suggest that ER is a good predictor of the response to endocrine therapy given after relapse, but not of early recurrence.

Wide variation of myoglobin contents in gizzard smooth muscles of various avian species.

We determined myoglobin contents of gizzards (muscular stomach) and breast muscles in 34 avian species by a modification of Reynafarje's spectrophotometric procedure. The birds were apparently differentiated into two groups in respect of the gizzard, one with a high myoglobin content (7.74 +/- 1.81 mg/g muscle) and the other with a low (1.54 +/- 0.41 mg/g). In the former group of 15 species all but one were herbivorous, and all but one were carnivorous or else omnivorous in the latter group of 19 species. The myoglobin level was considered to closely correlate with mechanical performance and therefore oxygen demands of the gizzards. It might also be relevant to a circulatory situation during the tonic contractions of this organs.

Is the deterioration of liver viability due to hepatic warm ischemia or reinflow of pooled-portal blood in intermittent portal triad cross-clamping?

The effects of hepatic warm ischemia and portal pooling on the viability of the liver were investigated with respect to hepatic energy metabolism by performing intermittent portal triad cross-clamping (Pringle's maneuver) on dogs with or without portosystemic shunt. The dogs were divided into two groups of five: Group 1, non-shunt group, underwent Pringle's maneuver performed for 30 min and declamping for 30 min, a process that was repeated five times; and Group 2, shunt group, underwent the same procedure as Group 1, except for portosystemic shunt using a heparinized hydrophilic catheter between the splenic and jugular veins. The shunt was opened during Pringle's maneuver and was closed immediately at declamping. In the non-shunt group, portal pooling increased and systemic blood pressure decreased when Pringle's maneuver was performed, but in the shunt group portal and systemic blood pressures remained within the normal range. In the non-shunt group, the initial velocity of arterial blood ketone body ratio (KBR) recovery after each declamping significantly (P less than 0.01) decreased from 0.122 +/- 0.016 (per min) after the first declamping to 0.028 +/- 0.017 (per min) after the fifth declamping. Hepatic energy charge [= (ATP + 1/2 ADP)/(ATP + ADP + AMP)] decreased from 0.840 +/- 0.003 before ischemia to 0.749 +/- 0.003 30 min after the fifth declamping (P less than 0.001). The concentrations of lactate and total amino acids in arterial blood increased. On the other hand, in the shunt group, the initial velocity of KBR recovery and hepatic energy charge showed little change even after the fifth declamping (0.081 +/- 0.016 per min and 0.851 +/- 0.009, respectively). The concentrations of lactate and total amino acids showed almost no increase. The impairment of hepatic energy metabolism by intermittent portal triad cross-clamping is mainly due to reinflow of pooled-portal blood to the previously ischemic liver, rather than hepatic warm ischemia. The KBR may be useful for determining the degree of impairment of hepatic energy metabolism.
