[Analysis of the results of mycological examination of skin samples from 1997 to 1998].

Mycological findings of 2,447 skin specimens were analyzed. The aim of the study was to estimate the methodology for mycological examination, frequency of particular fungal species isolation and localization on the skin. Fungi were found in 591 specimens: 53.64% microscopically and isolated in cultures, 31.47% only microscopically and 14.89% only isolated in cultures. Total number of isolated strains was 405 (356 strains grew on Sabouraud agar with antibiotics and 272 on mycobiotic agar). The most frequently isolated species were: Trichophyton mentagrophytes (27.9%), Trichophyton rubrum (20%) and Candida albicans (9.14%). The most frequent localizations on skin where fungi had been found were foot (41.47%), toe-nail (22.06%) and finger-nail (10.88%). Sabouraud agar is more reliable for isolation of dermatophytes and mycobiotic agar for Candida species. Usage of both media for primoisolation provides better and faster identification of fungi causing dermatomycoses. Comparing our with other studies, the patterns of fungal appearance in our population had changed and the number of nail specimens, where fungi were found had increased.

[Delayed-pressure urticaria--analysis of 28 patients].

Twenty eight patients (18 men and 10 women) with delayed pressure induced urticaria were analyzed. The average age was 35.5 years, and the illness lasted 1-20 years, 4.07 years on average. The spontaneous cessation of illness came in 8 patients after approximately 5.75 years. In 23 patients there was also a classical chronic urticaria. C3, C4, alpha 1 antitrypsin, alpha 2 macroglobulin, immunoglobulin IgG, IgA, IgM and IgE were determined for 15 patients and findings were within normal limits apart from the moderate increase of IgE in 2 patients. Fifteen patients were examined for intestinal parasites and in 3 patients they were found. Also, a pathogenetic mechanism of illness was considered: the role of histamine, proteinase inhibitors, mediators of the delayed allergic urticaria.

[Clinical and laboratory studies of cholinergic urticaria].

The clinical and laboratory study of 80 patients (75 males, 5 females) with cholinergic urticaria (CHU), treated from 1971 to 1996 is presented. The mean age at CHU occurrence was 22.3 years (10-50). The disease lasted at the time of examination from 3 months to 14 years, on an average 2 years and 11 months. Among 30 studied patients, the disease ceased after 1-3 in 5 patients, and it lasted from 3 to 24 years, on an average 9 years, in the remaining 25 patients. Exercise-induced anaphylaxis occurred in 8 patients and simultaneously cold urticaria occurred in 7 patients. The increase of histamine level in blood, following the exercise and/or heat test was found in 12 examined patients. The higher increase was noticed after the exercise, than after the heat test, which was well correlated with the severity of clinical features and the duration of skin manifestations. Repeated heat and/or cold test, performed at the same day, showed the weaker or no reaction at all. The role of the sweat, acetylcholine and histamine in CHU occurrence was discussed.

[Contact thermometry of lesions in alopecia areata].

The contact thermometry has been performed at the center of lesions, at the periphery and at the adjoining healthy skin in 10 patients with alopecia areata (AA) of the scalp. The lowest temperature was recorded in the center of lesions (approximately 33.55 degrees C), slightly higher in the periphery (approximately 33.71 degrees C) and the highest in the adjoining healthy skin (approximately 34.01 degrees C). The authors suggest the skin temperature to be the result of the degree of vascularization. The obtained results were examined in reference to the results published elsewhere on the light and electronic microscopy of AA lesions where the changes in capillaries were found. Based on all the above, the authors consider the topical use of drugs that improve the peripheral circulation to be justified in the treatment of AA.

[Angiokeratoma circumscriptum].

We have presented a 12-year-old boy with angiokeratoma circumscriptum on his left knee. The lesion first appeared at the age of eight as a dark brown nodus with the verrucous surface. It bled easily even after mild trauma this being a principal reason for the hospital admission and surgical excision in toto. Pathohistological examination confirmed the clinical diagnosis. Differential diagnosis, clinical and pathohistological findings, as well as therapy of the disease have been discussed in some detail.
