[A case of cerebral arteriovenous malformation with accidental migration of a microcatheter during endovascular procedure].

This is a report of a case of cerebral arteriovenous malformation with accidental breakaway of a microcatheter during endovascular embolization. A 25-year-old female suddenly suffered severe headache. CT scan demonstrated intraventricular hemorrhage, and the angiogram revealed right parieto-occipital AVM. We performed endovascular embolization using a flow-directed microcatheter (1.5F BALT MAGIC catheter) 2 weeks after onset, and the microcatheter broke accidentally and a fragment of the soft tip migrated to the distal posterior cerebral artery during the procedure. Although we tried to retrieve the catheter using several retrieval devices, we failed. Two days after the procedure we undertook surgical treatment. The migrated catheter was recognized in a small feeding artery which was stretched tightly, so retrieval was very risky. Using hemoclips for trapping the part of the catheter which remained in the feeder, we were able to retrieve it by cutting the feeder. Flow-directed microcatheters are very usefull in endovascular procedures, but it is necessary to handle them carefully in accord with sufficient knowledge of their use and handling methods.

Risk factors for left ventricular hypertrophy in chronic hemodialysis patients.

Left ventricular hypertrophy is reported to be common in chronic hemodialysis patients, and also to increase the risk for mortality in chronic hemodialysis patients. An echocardiographical and clinical study was carried out to investigate the risk factors for left ventricular hypertrophy in 151 non-diabetic chronic hemodialysis patients without valvular diseases or myocardial infarction in two hemodialysis units in Fukuoka, Japan. The left ventricular mass index (LVMI) correlated positively to age, systolic blood pressure and interdialysis weight gain while it correlated negatively to the duration of hemodialysis therapy and hematocrit. Resorting to a multivariate analysis, the LVMI was found to positively correlate to age and the systolic blood pressure while it correlated negatively with the duration of hemodialysis therapy and the hematocrit level. These findings suggest that hypertension and anemia may thus be independent risk factors for left ventricular hypertrophy.

[Occupational dangers to health of nursing staffs in hemodialysis units].

Since hemodialysis patients are at high risk for blood-borne viral infection such as hepatitis B and C virus infection, nursing staffs of the hemodialysis units have a significant occupational exposure to blood-borne virus infection. Furthermore, they are in danger of contacting occupational low back-pain because they have to take care of patients in a half-rising position. In addition, they may also suffer from burned-out syndrome because they have to look after all the chronic renal failure patients as well as run machines during the hemodialysis procedure. In this paper, we describe occupational danger to health of nursing staffs working in hemodialysis units.

[The association of self-rated health and mortality--a 7-year follow-up study of a Japanese community].

This study aims to examine whether self-rated health is an independent predictor of cause-specific mortality even after the influence by a variety of factors in relation to mortality reported by previous studies have been excluded. This study included randomly selected 4,259 inhabitants (1,827 men and 2,432 women) in Y city, Fukuoka prefecture, Japan, aged from 30 to 79 in 1987. These subjects were surveyed in 1987 by self-administered questionnaires regarding self-rated health as well as life habits, and followed up for their vital status and underlying causes of death. After excluding the individuals who were lost to follow up or who didn't respond to the question on self-rated health, 4,046 individuals were analysed to see the relationship between self-rated health and cause-specific mortality by Cox proportional hazard models, controlling for sex, age, smoking, BMI, medical care use and ADL. It was shown that relative risks for all causes, cancer, circulatory disease and other causes among the unhealthy group were 2.95 (95% CI: 1.93-4.50), 2.96 (1.53-5.73), 2.32 (0.86-6.26) and 4.09 (2.12-7.89), relatively. In the analyses of the subgroup (subjects without diseases in 1987 or subjects excluding deceased cases within first 3 years after follow-up), to avoid selection bias, the association between self-rated health and mortality was substantially similar to the results obtained in the former analysis, even the association was weakened. Even after excluding both of the subjects with diseases in 1987 and the subjects who died in the first 3 years after follow-up, self-rated health could be associated with mortality from all causes (RR = 1.89, 95%CI; 0.91-3.94). From the results it is suggested that self-rated health itself can be the independent predictor of mortality.

[A sero-epidemiological study on atrophic gastritis and Helicobacter pylori infection].

The aim of this study is to clarify the relationship between the incidence of atrophic gastritis and Helicobacter pylori infection. A case control study was conducted in 1995. Cases and controls were selected among those who took an annual health examination in a rural area of Fukuoka Prefecture. Cases are those who were not diagnosed as having atrophic gastritis in 1991 or 1992 but were diagnosed as having atrophic gastritis in 1995 by the examination of serum pepsinogens. Controls are those who were not diagnosed as having atrophic gastritis between 1991, or 1992 and 1995. Helicobacter pylori infection was evaluated by anti Helicobacter pylori IgG in 1995. The odds ratio for atrophic gastritis was calculated using Logistic regression analysis. The odds ratio of age and sex was not significant, but the odds ratio of Helicobacter pylori infection was significantly elevated (5.2, 95% confidence interval: 2.08-13.17). As for the influence of sex difference on the incidence of atrophic gastritis by Helicobacter pylori infection, the odds ratio among males was not significant (1.4, 95% confidence interval: 0.30-6.10), but the odds ratio among females was significantly high (9.8, 95% confidence interval: 2.82-34.11). These results suggest that Helicobacter pylori infection is associated with the incidence of atrophic gastritis. There may be a risk difference between males and females in the incidence of atrophic gastritis caused by Helicobacter pylori infection.

Postoperative complications after pneumonectomy for treatment of lung cancer: multivariate analysis.

The charts of 62 patients with primary lung cancer who underwent a pneumonectomy at our department from 1979 through 1992 were reviewed for the evaluation of postoperative morbidity and mortality. The 30-day mortality was 3/62 or 4.8%. Postoperative complication occurred in 37 of 62 patients (60%). The most common complication was a supraventricular tachyarrythmia. A major complication, which was defined as one necessitating re-thoracotomy or one which caused death, occurred in 19 patients (31%). We analyzed 43 perioperative variables for their predictive value of postoperative morbidity and mortality. Univariate analysis indicated that an elevated serum LDH, low predicted forced vital capacity, low predicted forced expiratory volume in 1 sec (FEV1) were significantly associated with the occurrence of a major complication. A multivariate logistic regression model indicated that a high LDH level, a low predicted FEV1 and no extubation following surgery were associated independently with a postoperative major complication. Since only the complete removal of a tumor offers a chance for cure for the treatment of non-small cell lung cancer, it is sometimes necessary to perform a pneumonectomy for these high-risk patients. Patients identified as being at high risk of a major complication should be candidates for intensive preoperative evaluation and perioperative care.

Matrix metalloproteinase-9 secretion by human pituitary adenomas detected by cell immunoblot analysis.

Twenty-two pituitary adenomas were examined on the secretion of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) using a cell immunoblot assay, and discussed regarding an association between cavernous sinus invasion and the secretion of these proteins. The cell immunoblot assay, a kind of immunoblot procedure, is able to detect proteins at the single cell level and to detect the incidence of tumour cells secreting the target proteins in the total tumour cell population. The incidence of tumour cells secreting MMP-9 was significantly higher in invasive adenomas than in noninvasive ones. On the other hand, TIMP-1 secretion was not detected in any adenomas in this study. This result suggested that MMP-9 secretion, and especially the number of MMP-9-secreting cells, may be associated with cavernous sinus invasion of pituitary adenomas.

Activities of an occupational health organization in Japan, in special reference to services for small- and medium-scale enterprises.

This paper reports the activities of occupational health organizations in Japan, taking Nishinihon Occupational Health Service Center, one of the largest scale occupational health service providers, as an example. The organizations, at the request of enterprises, provide occupational health services on a profit basis especially for small- and medium-scale enterprises which lack the human resources and/or facilities to meet legal requirements on occupational health. The main services include: (1) providing various types of medical examinations for workers, (2) measuring work environment, (3) occupational physicians and occupational health nurses providing a comprehensive service. Among these services, legally required health examinations and work environment measurements are well served even for small-scale enterprises. Less effective are the health promotion and occupational health service by the occupational health staffs. In the future, these service will be better provided by a newly developed network system to support occupational heath activities in small-scale enterprises.

[Occupational and lifestyle factors related to musculoskeletal and fatigue symptoms among middle-aged female workers in a frozen food processing factory].

A survey was conducted on musculoskeletal and fatigue symptoms related to individual and work-related risk factors among middle-aged female workers in a frozen food processing factory by using a self-administered questionnaire. Proportions of workers who frequently experienced, during the last one month, stiff muscle or pain in the neck-shoulder, back and lower limbs were 32.9%, 26.8% and 15.9%, respectively. The proportion for fatigue symptom was 30.5%. Factors related to fatigue were: short duration of employment [odds ratio (OR) 1.8, 95% confidence interval (CI) 1.1-2.9], light body weight [OR 0.6, CI 0.3-1.1], long house work [OR 1.7, CI 0.9-3.3], short sleep hours [OR 1.6, CI 1.0-2.5], walking to and from work [OR 0.5, CI 0.2-1.0], those related to back pain were: high body height [OR 2.4, CI 0.8-7.4], light body weight [OR 0.4, CI 0.2-0.9], those related to lower limb symptom were: working height below hip height [OR 2.3, CI 1.2-4.4], working height above shoulder height [OR 3.0, CI 1.5-5.7], high body height [OR 3.4, CI 1.2-10.1] and low body height [OR 2.4, CI 0.8-7.4]. Workers' subjective evaluation of height of work table was significantly related to the body height. These results indicate that it would be necessary to improve ergonomic aspects of work considering individual characteristics as well as to advise workers on their lifestyle.

Proliferating cell nuclear antigen may be superior to argyrophilic nucleolar organizer regions in predicting shortened survival of patients with non-small cell lung cancer.

We examined proliferating cell nuclear antigen (PCNA) in 102 patients with surgically treated non-small cell lung cancer (NSCLC). PCNA labelling index (LI) tended to be higher in tumours of higher stages than those of early stages, in squamous cell carcinomas than adenocarcinomas, or in poorly differentiated adenocarcinoma than in well-differentiated. A positive correlation was observed between the PCNA LI and argyrophilic nucleolar organizer regions (Ag-NOR) count which we previously examined (r = 0.31, P = 0.002). In survival analysis of 79 patients who died of lung cancer, only age, stage and PCNA LI were found to be significant prognostic factors on multivariate analysis among seven potential prognostic factors including sex, age, year of operation, histological type, stage, Ag-NOR count, and PCNA LI. We conclude that PCNA may be superior to Ag-NOR in predicting shortened survival of patients with non-small cell lung cancer. PCNA staining can be performed with ease and it may be applied in a clinical laboratory on a routine basis to help predict prognosis of NSCLC.

Multiple channels for occupational health services to small-scale enterprises in Japan.

This study was performed to clarify the characteristics of multiple channels for occupational health services (OHS) to small-scale enterprises (SSEs) in Japan employing less than 50 workers by reviewing relevant research papers. OHS were provided to SSEs by both government organizations and non-government organizations. Government organizations included the Japanese Industrial Safety & Health Association, regional occupational health centres, prefectural occupational health promotion centres, and other health centres. Non-government organizations were parent companies, occupational health organizations, hospitals, medical associations, trade associations and health insurance societies. The quality of OHS in terms of provision of health personnel and the nature of their services differed greatly between the organizations. Health care programmes were more popular than management of the work environment or working practices in OHS to SSEs. Few organizations provided comprehensive OHS to SSEs. At present, parent companies and occupational health organizations appear to provide the best OHS available to SSEs in Japan.

Nucleolar organizer regions are independently associated with a shortened survival in patients with non-small cell lung cancer.

We examined nucleolar organizer regions (Ag-NOR) in 102 patients with non-small cell lung cancer (NSCLC). Ag-NOR counts were generally higher in tumours from more advanced stages, or in poorly differentiated adenocarcinomas. Six potential-prognostic indicators included sex, age, year of operation, histology, stage and Ag-NOR count. Stage and Ag-NOR counts had a significant prognostic value in the univariate analysis for 79 patients who died of their lung cancer. In the multivariate analysis, high Ag-NOR counts as well as disease stage were independently associated with a poor prognosis. Furthermore we divided patients into three groups according to their Ag-NOR counts and found that in patients with stage I and II disease higher Ag-NOR counts were associated with shortened survival. This trend did not continue in patients with stage III and IV disease.

[A population based survey of attendance at screenings in urban areas].

To clarify the present situation of screenings in urban areas, a population based survey on attendance at screenings was conducted in Kitakyushu City. Self-administered mailed questionnaires were sent to three thousand randomly selected subjects 40 years of age or over living in Kitakyushu City. The results are summarized as follows: 1. Considerable differences were observed in the number of people who were offered screenings at work sites for those between the ages of 40-59 compared to those 60 years of age or over, and between full-time employees and part-time or self-employed workers. 2. Among people who obtained stomach examinations, middle age men mainly received cancer screenings at work sites, while women or older men were mainly examined when they went to clinics or hospitals when abdominal discomforts occurred. Among women who received uterine examinations, all age groups mainly received cancer screenings at city administered screenings at clinics or hospitals, and rates decreased as age increased. 3. Among people who did not obtain stomach examinations, many middle age people wanted to be examined for stomach cancer but were unable to, while older people did not think it necessary. In the case of uterine cancer screening, middle age women did not want to receive it, while older women did not think it necessary. 4. For those people who are not offered cancer screenings at workplaces, it is assumed that those who have a positive attitude toward screening will obtain it, if offered, at sites conducted by the local government.(ABSTRACT TRUNCATED AT 250 WORDS)

[A compartment model of carcinogenesis in lung cancer].

A compartment model of carcinogenesis which describes separately the process of smoking-related lung cancer and that of smoking-unrelated lung cancer is presented. This model is well fitted to the equation representing the lung cancer incidence rate of the British physicians' cohort. The compartment model is shown to agree with the frozen type of incidence curve among ex-smokers. This model is biologically plausible in the context of the mechanism of carcinogenesis. For planning public health policies in a community and for health education based on smoking data, we calculated the change of risk after the cessation of smoking.

Prospective study of diabetes mellitus and liver cancer in Japan.

BACKGROUND: Previous studies have identified the association between diabetes mellitus and liver cancer. However, the detail of this association is still unclear, in terms of confounding factors, the trend according to the duration of diabetes, and the interaction between diseases associated with the liver cancer and this association. The purpose of the present study was to examine the association between diabetes and liver cancer in view of the trend and the interaction. METHODS: The baseline survey was conducted during the period 1986-1989 among the general population of Fukuoka Prefecture, Japan (15 417 persons aged 30-79 years). The respondents were assessed for history of diabetes, age at which they had had diabetes, and other covariates by means of a baseline questionnaire. A total of 7308 persons aged 40-79 years were retrieved for the main analysis and 4902 persons for a subcohort from which the information on history of diseases associated with liver cancer were obtained. The relative risks (RRs) and their 95% confidence intervals (CIs) were estimated using the Cox proportional hazards model. RESULTS: After adjustment for smoking, alcohol and the diseases associated with liver cancer, the RR for liver cancer was 2.06 (95% CI=1.01-4.19). Diabetes increased the risk of liver cancer in persons with hepatitis and/or cirrhosis (RR=2.90, 95% CI=1.13-7.41). However, the RR of diabetes for liver cancer was 1.35 (95% CI=0.41-4.43) in persons without hepatitis and cirrhosis. The trend according to the duration of diabetes was not seen. CONCLUSIONS: A significant association between diabetes and liver cancer was observed. Moreover, this association was modified by hepatitis and cirrhosis.

Evaluations of p53 immunoreactivity, nucleolar organizer regions, and proliferating cell nuclear antigen in non-small cell lung carcinoma.

We examined p53 protein expression, proliferating cell nuclear antigen (PCNA), and argyrophilic nuclear organizer regions (AgNOR), in 102 patients with surgically-treated non-small cell lung cancer (NSCLC). p53 positive cases with DO-1 were defined when more than 10% of the tumor cell nuclei were stained. Mean AgNOR count and PCNA LI were 2.80 and 40.7 and there were no significant differences of AgNOR count and PCNA LI between p53 positive and negative cases. We assessed the relationship between the p53 immunoreactivity and various clinical or pathological parameters. p53 positive rate of stage III disease (46.3%) was significantly higher than that of stage II disease (28.6%). The p53 positive rate of squamous cell carcinoma (42.1%) tended to be higher than that of adenocarcinoma (33.9%). In the survival curves of patients with NSCLC according to the p53 immunoreactivity, there was no significant difference between p53 positive and negative cases. Eight potential prognostic parameters (p53 immunoreactivity, AgNOR count, PCNA LI, sex, age, year of operation, histology, and stage) were also estimated, using univariate and multivariate analysis. In univariate analysis, PCNA LI and AgNOR count, and stage were significantly related to shortened survival. In multivariate analysis, PCNA LI, Age, and stage were independently associated with shortened survival of NSCLC patients. PCNA staining may be more useful than p53 and AgNOR staining in assessing the aggressiveness of surgically-treated NSCLC, although the most useful clinical prognostic parameter should be achieved by the combined analysis of several prognostic indicators.

N-acetylation polymorphism in patients with lung cancer and its association with p53 gene mutation.

We determined the polymorphic N-acetyltransferase 2 (NAT2) genotype of 124 patients with non-small-cell lung cancer (NSCLC) and 376 control subjects using a PCR-based assay. The slow acetylator genotype was present in 17 (14%) of 124 NSCLC patients and in 40 (11%) of 376 control subjects. The relative risk of slow acetylators compared with rapid acetylators in patients with adenocarcinoma was 2.01 (p = 0.05). This trend was more marked when the analysis was confined to patients under the age of 65 (relative risk, 2.7; p = 0.03). No such trend was identified in patients with squamous cell carcinoma. We also determined the incidence of p53 gene mutations to investigate the possibility of a link between this gene mutation and N-acetylation polymorphism. There was no significant association between p53 gene mutations and NAT2 polymorphism in the overall NSCLC group. However, the incidence of p53 mutations in adenocarcinoma patients under the age of 65 who had the slow acetylator genotype was 63% compared with 38% in patients with the rapid or intermediate acetylator genotype. These findings suggest that the slow acetylator phenotype may be linked to the impaired metabolism of a carcinogen that predisposes individuals to p53 gene mutations, and thus may be associated with an increased risk of pulmonary adenocarcinoma.
