[Tumor lysis syndrome at the induction therapy of the first remission in two cases of T-ALL].

The anti-tumor therapy followed by tumor lysis syndrome may cause the metabolic disorders including hyperkalemia, hyperphosphatemia and hyperuricemia. It should be known that it occurs frequently in lymphoproliferative diseases, especially in Burkitt's lymphoma. Two cases of T-ALL accompanied by this syndrome, from which the patients were recovered, at the induction therapy of the first complete remission are reported here. Case 1. A 28-year-old man received VP therapy under the diagnosis of T-ALL with massive hepatosplenomegaly and bilateral enlarged kidneys. During the therapy, metabolic disorders with both renal failure and ventricular tachycardia happened. They were resolved by certain series of treatments. The patient was brought to a complete remission with normal size of liver, spleen and kidneys. Case 2. A boy aged 15 having received the intrasubarachnoidal infusion of MTX and 1-Ad-VP therapy under the diagnosis of T-ALL accompanied by this syndrome which was improved by an appropriate treatment, and the patient was lead to the remission. The risk factors of this syndrome, such as 1-high drug sensitivity of the tumor; 2-renal dysfunction; 3-rapid cytokinetics of the tumor cell; 4-bigger size of the tumor, as well as the preventive treatment of this syndrome are reviewed.

Clinical usefulness of vinca alkaloid slow infusion in the treatment of chronic refractory idiopathic thrombocytopenic purpura: a multicenter cooperative study.

Thirty-eight patients with chronic refractory idiopathic thrombocytopenic purpura (ITP) were treated with weekly slow infusions of vincristine (0.02 to 0.04 mg/kg) or vinblastine (0.1 to 0.2 mg/kg). Twenty-two patients showed good to excellent responses after one to eight infusions. These responses were generally short, and lasted only in six patients after discontinuance of the therapy. The efficacy was comparable between vincristine and vinblastine. Neither the age, sex, duration of the disease, prior splenectomy nor combined use of adrenocortical steroids was likely to have influenced the therapeutic effect. Side effects such as peripheral neuropathy, alopecia, gastrointestinal symptoms and leukopenia occurred in 34 patients, and necessitated discontinuance of the therapy in eight patients. Slow infusions of vinca alkaloids can be an effective means of inducing platelet response in patients with chronic refractory ITP, but frequent side effects limit its clinical usefulness.

[Atrial septal defect with idiopathic thrombocytopenic purpura: a case of open-heart surgery].

A 40-year-old woman of atrial septal defect associated with idiopathic thrombocytopenic purpura (ITP) was successfully operated upon under cardio-pulmonary bypass. Steroids had been given preoperatively, resulting in an increase of the platelet count from 0.7 X 10(4)/mm3 to 20.6 X 10(4)/mm3. Steroids could be withdrawn before the operation. No bleeding tendency was encountered during and after surgery. This is the 6th case of open-heart surgery associated with ITP in the literature.

[Antibacterial activity of local anesthetics].

In order to study the antibacterial activity of local anesthetics quantitatively, we procured their minimum inhibitory concentration (MIC), killing curves and postantibiotic effect (PAE), using the standard colony of Staphylococcus aureus ATCC 25923, Staphylococcus epidermidis ATCC 14990 and Pseudomonas aeruginosa NCTC 10490. Both bupivacaine and lidocaine had bactericidal activity at a clinical concentration. MIC of the former was lower than that of the latter, and it means that bupivacaine has a greater antibacterial activity than lidocaine. At the same concentration, the commercial solutions, such as Xylocaine and Marcain, which contain preservatives, showed a greater antibacterial activity than the pure anesthetic solutions which contain no preservatives. However, the preservatives had no bactericidal activity, but weak bacteriostatic activity.

Radioprotective effects of dimethyl sulfoxide in golden hamster embryo cells exposed to gamma rays at 77 K. II. Protection from lethal, chromosomal, and DNA damage.

Golden hamster embryo cells were exposed to 137Cs gamma rays in the presence or absence of dimethyl sulfoxide at both 310 and 77 K. Dimethyl sulfoxide gave significant protection against cell killing at both 310 and 77 K. The extent of radioprotection with 1.28 M dimethyl sulfoxide at 77 K was 85-89% of the lethal effects observed in the absence of dimethyl sulfoxide at 310 K; the dose-modifying factor was 5.7. Dimethyl sulfoxide also exerted protected against gamma-ray-induced DNA single-strand breaks and chromosomal aberrations with a maximum protection of 80-100% at a dimethyl sulfoxide concentration of 1.28 M at 77 K. At 77 K, H atoms, ion holes, and electrons can migrate through frozen cells but OH radicals cannot diffuse. Thus the protective effects of dimethyl sulfoxide against cell killing, chromosomal aberrations, and DNA single-strand breaks at 77 K may be due to the scavenging of H atoms or other ions, rather than OH radicals.

[Phase I study of YNK01 (1-beta-D-arabinofuranosylcytosine-5'-stearylphosphate)].

Phase I study of YNK01 (1-beta-D-arabinofuranosylcytosine-5'-stearylphosphate), a derivative of cytosine arabinoside (Ara-C), was conducted by cooperative study groups between January 1986 and June 1987. The dosage for the single and 5-day oral administration ranged 50 to 1,200 and 100 to 900 mg/body/day, respectively. The main adverse effects were myelo-suppression and gastrointestinal toxicities such as nausea, vomiting, anorexia and diarrhea. In the single administration, the maximum tolerated dose (MTD) could not be determined, however, in the 5-day schedule MTD was considered to be 700 to 900 mg/body/day, and the dose limiting factor to be thrombocytopenia. After the single administration (800 mg/body/day) of YNK01, the plasma concentration of Ara-C, an active metabolite of YNK01, was 3.43 ng/ml (Cmax) at 24 hrs. and 0.82 ng/ml at 72 hrs. During the 5-day administration for 300 mg/body/day and 500 mg/body/day, the Ara-C concentration changed from 2.3 to 4.1 ng/ml, and from 1.5 to 11.9 ng/ml respectively, and sustained almost the same concentration as to during the administration period until the 2nd day after the completion of the administration.

[The study on descriptive epidemiology of peptic ulcer in Hokkaido].

We previously reported that the reported that the ratio of gastric ulcer to duodenal ulcer in Hokkadio was higher than that in other area of Japan. This time, we analysed peptic ulcer cases of five hospitals in Hokkaido on the point of geographical and pathological distribution. When we divided these hospitals into two groups (in Sapporo city and other provincial areas) and calculated the sex and age adjusted ratio of gastric ulcer to duodenal ulcer in each group, we found the difference between the two groups (in Sapporo city: 2.26, other provincial area: 5.86) and thought the feature of high ratio the feature of the provincial hospitals. This difference in Hokkaido may be due to the environmental factors for peptic ulcer or the biased sampling of the study. The further study is needed to reveal the geographical and pathological distribution of this disease.

Amino acid substitutions determining G3m(g5).

Amino acid substitutions specific for allotypic G3m(g5) marker were studied by sequence analysis of the C-terminal BrCN peptides of myeloma protein Ba [G3m(g5+)] and Bu [G3m(g5-)]. The results indicate that arginine and tyrosine at position 435 and 436 are responsible for the specificity. Two substitutions of IgG3 Bu [G3m(b1, b3)] and IgG3 Jir [G3m(b3,s,t)] [Matsumoto et al.: J. Immun. 131: 1865, 1983], in the same positions are arginine-phenylalanine and histidine-tyrosine, respectively. Affinity chromatographic data of modified IgG3 proteins show that the tyrosine residue at position 436 associated with phenylalanine at position 124 of protein A-B fragment plays a role in the interaction. The differences in the yields between IgG3s carrying various haplotypes on Protein A-Sepharose affinity chromatography [Ito et al.: Proc. Japan Acad. 56B: 226, 1980] are also explained through this chromatography and also by the configuration of the residues in tertiary structure [Deisenhofer: Biochemistry 20: 2361, 1980].

[Protective effect of calcium channel blockers on the liver against halothane hepatitis in rats].

This study examined whether nifedipine or flunarizine, calcium channel blockers, could protect the liver against halothane hepatotoxicity in rats. Six groups of 102 enzyme-induced male Sprague-Dawley rats were studied. Group N1 and N2 rats received nifedipine 20 micrograms and 100 micrograms.kg-1.day-1, and group F1 and F2 rats received flunarizine 100 micrograms and 1mg.kg-1.day-1 in their drinking water for 24 hours prior to the experiment. They were then anesthetized with N2-O2-halothane 0.5% under the condition of hypoxia (FIO2 0.12) for two hours. Animals in group H were given clear water, and then anesthetized with halothane and hypoxia. Animals in group C were given clear water and exposed to hypoxia alone. Twenty-four hours after anesthesia, all animals were decapitated and their liver specimens were stained with H&E for the examination by light microscopy. Eighty percent of rats in group H showed centrilobular degeneration, but administration of nifedipine or flunarizine reduced the incidence significantly. Furthermore, histological score graded from 0 to 3, according to the degree of centrilobular degeneration was also significantly reduced by administration of nifedipine or flunarizine. These results suggest that oral administration of nifedipine or flunarizine could prevent the incidence of halothane-induced hepatotoxicity in enzyme-induced rats, probably by preventing the increase in cytosolic calcium concentration.

[A case of aprindine-induced pneumonitis].

A 73-year-old man had been treated with Aprindine because of paroxysmal atrial fibrillation. On July 13, 1987, five months after the commencement of aprindine administration, he developed dyspnea and low grade fever. His chest X-ray revealed multiple infiltrative shadows in both lung fields. He was treated by various antibiotics, but the infiltrative shadows increased. BALF showed increased percentage of lymphocytes and a decrease in the OKT4/T8 ratio, and the histological findings of TBLB carried out on August 6, 1987, showed alveolitis with Masson bodies. The lymphocyte stimulation test by drugs was positive only for aprindine. After cessation of Aprindine administration, his complaints and laboratory data improved, but his abnormal shadow on chest X-ray did not diminish completely. Open lung biopsy was performed for differential diagnosis of BOOP, on Sep. 14, 1987. The histopathology of specimens of the lung was compatible with drug-induced pneumonitis. The administration of 30 mg of prednisolone was started on Oct. 14, 1987, and the dosage was decreased gradually. The abnormal shadow on chest X-ray improved. To our knowledge, there has been no reported case of Aprindine-induced pneumonitis, and this could be the first report.

[A case of adrenal myelolipoma with colonic cancer and gallstones].

A 47-year-old female, referred to Kushiro Rohsai Hospital because of upper abdominal pain on January 10, 1986, was found to have a gallbladder stone on X-ray examination of her abdomen. On admission, the occult blood found in her stool was strongly positive and a barium enema and a colonoscopy showed an elevated lesion in the caecum. Subsequently, a diagnosis of a colonic cancer was made after a biopsy. Sonography and CT scans revealed a gallbladder stone and a right suprarenal mass 4 cm in diameter. At surgery, this gallbladder stone was found, as was a Borrman II type tumor in the caecum. The surface of an adrenal mass was hard and smooth and a cut of this surface showed a yellowish gray color. Microscopic examination revealed adrenal cortical tissue beneath the capsule with a mixture of hematopoietic and adipose tissue. These findings were compatible with myelolipoma. Postoperatively, a bone marrow aspiration was performed, but no definite abnormality was detected.

[Three cases of pulmonary atypical mycobacteriosis associated with lung cancer and adult T-cell leukemia].

Three cases of pulmonary atypical mycobacteriosis (AM) were reported. Two cases were associated with lung cancer in which the diagnosis of malignancy was difficult and delayed by the coexistence of AM. The third was a case of adult T-cell leukemia (ATL) which manifested during the course of AM. In case 1 (73 years, male) and case 2 (86 years, male), chest roentgenogram abnormalities as well as clinical symptoms were considered to be caused by mycobacteriosis because of positive smear of acid-fast bacilli in sputa on admission. Therefore it took four months and three months respectively for final diagnosis of lung cancer. The autopsy of case 1 revealed a poorly differentiated adenocarcinoma with coexisting foci of squamous cell carcinoma in right lower lung, and granulomatous inflammations with caseating necroses in right mid and lower lungs. M. avium complex was cultured from sputum on admission, and also a high titer of HTLV-I antibody was demonstrated. In case 2 malignant cells were detected in sputa (class V), however his general condition did not allow an aggressive anticancer chemotherapy and he died of malignancy with complication of thromboangiitis obliterans on right lower leg. Case 3 was a 76-year-old male who had been diagnosed as lung AM for more than two years. His chest radiography showed bilateral infiltrative shadows with frequent positive cultures of M. avium complex (more than 100 colonies) from sputum. A generalized lymphadenopathy including right hilar lymph node on chest X-ray film was followed by the presence of atypical lymphocytes in peripheral blood and the elevation of HTLV-I antibody in serum. Four months later he died with hypercalcemia and renal failure in spite of chemotherapy (CPM + VCR + ADR + PLS). The above cases suggest that AM as well as tuberculosis should be considered when pulmonary infiltrates were observed in malignant patients, especially in patients with retrovirus infections.

[Transcatheter arterial embolization for postpartum hemorrhage].

A case of postpartum hemorrhage controlled by transcatheter arterial embolization is reported. The determination of the exact location of the bleeding vessel in postpartum hemorrhage may be difficult using angiography. Selective embolization is effective but the selection of the artery to embolize (internal iliac versus uterine artery) is still controversial.

[Clinical study of reaccumulation and persistence of chronic subdural hematoma; setting an Ommaya reservoir].

We treated 57 cases of chronic subdural hematoma by aspiration via the Ommaya reservoir following burr hole opening and irrigation of the hematoma in order to reduce the reaccumulation and persistence of the hematoma. An Ommaya reservoir was placed on the following patients: (1) the volume of hematoma was more than 100 ml and/or (2) expansion of the brain after evacuation of the hematoma was poor. However in spite of these techniques there were 6 cases (10.5%) of recurrence and 21 cases (36.8%) with persistence of the hematoma cavity for longer than 2 months after the first operation. The recurrence was due to occlusion of the Ommaya reservoir by massive rebleeding into the hematoma cavity in cases with a large subdural hematoma and/or an immature external membrane. Persistence of the hematoma cavity for longer than 2 months after the first operation was due to minor rebleeding and CSF inflow into the hematoma cavity through the damaged inner membrane in cases of acute onset and/or or coexisting intracranial disease (brain atrophy, cerebral infarction and subdural effusion). The most suitable cases for application of this technique were those with poor expansion of the brain after evacuation of the hematoma, or those with a mature external membrane and a medium-size hematoma.

Possible disorder of B-cell-related surveillance and malignant lymphoma of the thyroid.

Malignant lymphoma of the thyroid has been shown to be of B-cell origin. To clarify its genetic origin, we have investigated HLA-A, B, C, and DR antigens, and the immunoglobulin G (IgG) heavy-chain allotype, Gm, in such patients. There was no correlation between the occurrence of malignant lymphoma of the thyroid and the HLA antigens tested, but patients with this lymphoma had significantly lower frequency of the Gm1,21 haplotype than healthy persons. These results suggested a disorder of surveillance related to B-cells may be involved in the pathogenesis of malignant lymphoma of the thyroid.
